ABSTRACT

OBJECTIVE To evaluate the medication adherence of patients with hypertension in medication consultation clinics, and to analyze its influencing factors. METHODS The data of 389 patients who visited the medication consultation clinics of our hospital from June 2021 to June 2023 were collected. Univariate and multivariate Logistic regression analysis were used to analyze the related factors affecting medication adherence of hypertensive patients or those receiving different types of drugs. RESULTS Among 389 patients with hypertension, 302 cases (77.63%) had good adherence. Multivariate Logistic analysis showed that higher education level [corrected OR=2.25, 95%CI (1.29, 3.93), P=0.004] was positively correlated with medication adherence, average blood pressure level [corrected OR=0.19, 95%CI (0.10, 0.37), P<0.001], without complication [corrected OR=0.47, 95%CI(0.26,0.84),P=0.010] and antihypertensive drug regimen being free dose combination [corrected OR=0.27,95%CI(0.15, 0.47), P<0.001] were negatively correlated with adherence. Results of univariate Logistic regression analysis showed that patients who used β-receptor blocking agents [OR=1.65,95%CI(1.06,2.57),P=0.027], calcium channel blockers [OR=2.13,95%CI(1.33, 3.42),P=0.002] and agents acting on the renin-angiotensin system [OR=2.04,95%CI(1.29,3.22),P=0.002] had good medication adherence. CONCLUSIONS The medication adherence of hypertension patients needs to be improved. Hypertension patients with higher education level, lower average blood pressure level, complications and fixed-dose combination regimen and those who use agents acting on the renin-angiotensin system, calcium channel blockers and β-receptor blocking agents may have better medication adherence.

ABSTRACT

Objective To comparatively study the displacement of surrounding structures of jugular foramen (JF) via far lateral approach under middle cranial flexion supine and oblique supine,and to provide the anatomical basis for the protection of these relative structures during choosing regional surgical approaches.Methods Forty-one healthy adults were scanned by PHILIPS Brilliance 64-slice CT according to the conditions:1 mm thick bone window under the middle cranial flexion supine position and oblique supine positions,continuous axial scanning,and being ranged from horizontal canal to C3 levels bony landmarks.The scanned images were inputted Philips Extended Brilliance Workspace CT reconstruction of multi-plane (MPR) and the data were measured.Results In the middle cranial flexion supine position,the distances from transverse process atlas to the ipsilateral mastoid tip,tip of the styloid process,the midpoint of the outer edge in occipital condyle were (20.99±4.18) mm,(20.49±5.47) mm,and (22.035±4.27) mm.In the supine oblique position,the distances from transverse process atlasto the ipsilateral mastoid tip,tip of the styloid process,the midpoint of the outer edge in occipital condyle were (22.795±3.29) mm,(22.315±5.48) mm and (23.025±3.91) mm.Significant differences were noted in these three distances between the two positions (P<0.05).Conclusion In the JF surgical approach,supine oblique can change the relative spatial relationship of correlative structures,increase the exposure of the jugular vein area,therefore,protect the facial nerve,vertebral artery and posterior extracranial structures,and reduce the damage to surrounding structural tissues and the complications.

ABSTRACT

Objective To investigate the association between arterial compliance and fundus oculi arteriosclerosis in elderly people. Methods A total of 73 elderly subjects,including 61 elderly patients with fundus oculi arteriosclerosis and 12 healthy volunteers were recruited into the study.Capacitive arterial compliance(C1)and oscillatory arterial compliance(C2)were measured by DO2020 Cardiovascular Profiling Instrument.Moreover,fundus photographs were measured using Canon figure fundus camera. Results There were significant differences in systolic blood pressure(SBP),diastolic blood pressure(DBP),mean artery pressure,pulse pressure,capacitive arterial compliance (C1),and oscillatory arterial compliance(C2)between patients with fundus oculi arteriosclerosis and healthy volunteers(all P<0.05).In multiple regression analysis,the grade of fundus oculi arteriosclerosis was significantly correlated with SBP,DBP,mean artery pressure and pulse pressure (r=0.357,0.261,0.342,0.280,all P<0.05).Moreover,there were significant correlations between the grade of fundus oculi arteriosclerosis and Cl(r=-0.281,P<:0.05),and C2(r=-0.308,P<0.01). Conclusions These findings underscore the efficacy of C1 and C2 in identifying patients with fundus oculi arteriosclerosis.
