ABSTRACT

Objective To investigate the effect of bilateral knee osteoarthritis (KOA) ondynamic balance ability of ankle strategy in aged women. Methods The dynamic balance ability tester was used to test the balance score, the rotation speed, the maximum rotation speed, and the percentage of the target ball's residence time in each area of KOA patients (KOA group)and the general elderly (control group), and a comparative analysis between groups was conducted. Results The balance score of KOA group was lower than that of control group; the dynamic balance control ability of KOA group in the horizontal direction was basically the same as that of control group, but the dynamic balance control ability of KOA group in the vertical direction was weaker than that of control group.Bilateral KOA reduced dynamic balance ability of ankle strategy in the aged women.It could not affect the left-right symmetric balance ability of the aged women, but it would reduce its forward-backward symmetrical balance ability. Conclusions Bilateral KOA aged women may be more likely to fall forward or backward, while not easy to fall laterally. For elderly female patients with bilateral KOA, methods such as strengthening ankle joint strength, proprioception and responsiveness can be used to prevent falls that may be caused by reduced dynamic balance ability, especially falls in the forward and backward directions.

ABSTRACT

Objective To determine the genotype and clustering characteristics of multidrug-resistant (MDR) mycobacterium tuberculosis in Wuhan City. Methods A total of 149 mycobacterium tuberculosis strains from MDR tuberculosis (MDR-TB) patients were genotyped by DTM-PCR, mycobacterial interspersed repetitive unit–variable number tandem repeat typing (MIRU-VNTR) and sequencing of drug-resistant associated genes. The Hunter-Gaston discriminatory index (HGDI), clustering rate and recent minimum estimated infection rate were calculated. Results The cumulative HGDI of all MIRU loci was 0.9944. Gene locus with the highest HGDI(0.6467)was QUB11b. Combing MIRU-VNTR and DTM-PCR, 111 unique genotypes and 11 clustered genotypes(38 strains)from 149 isolates were identified. The clustering rate was 25.50%, and the recent minimum infection rate was estimated to be 18.12%. After further genotyping by sequencing of drug-resistant associated genes, the results showed that 17 (44.74%) strains in each cluster shared the same mutation sites, while the other strains had different mutation sites. Conclusion The combination of DTM-PCR, 24 locus MIRU-VNTR typing and drug resistance gene sequencing demonstrates a relatively high discriminatory power, which is suitable for the genotyping of multi drug resistant mycobacterium tuberculosis in Wuhan. The epidemic of MDR-TB in Wuhan is mainly caused by reactivation, and the recent transmission rate is not high.

ABSTRACT

Objective To construct an early warning model of fall risk for the elderly based on six kinds gait parameters. Methods A digital field was used to collect parameters from six kinds of gait for the elderly with or without the history of falls, and the binomial logistic regression analysis was used to establish a regression equation for predicting the fall risks in the elderly, and an early warning model was constructed. Results The regression equations constructed according to the parameters from six kinds of gait were statistically significant. The overall correct rate was predicted from high to low: walking forward with closed eyes (97.1%), walking backward with open eyes (92.9%), walking backward with closed eyes (88.6%), walking forward with open eyes (87.1%), turning head up and down with open eyes (85.7%), turning head left and right with open eyes(82.9%). The constructed early warning model for fall risk of the elderly mainly included five steps, namely, judgment, test, extraction, calculation and early warning, which was suitable for gait testing and evaluation of the elderly in the laboratory. Conclusions Parameters from six kinds of gait could predict the fall risk of the elderly. Among them, walking forward with closed eyes was best to predict the fall risk in the elderly. The established early warning model of fall risk for the elderly could be used to predict the fall risk of 65-75 year old people within one year, which could provide early warning based on the probability of falling, playing a positive effect on preventing falls in the elderly.

ABSTRACT

Alzheimer's disease (AD) is a chronic central neurodegenerative disease. The pathological features of AD are the extracellular deposition of senile plaques formed by amyloid-β oligomers (AβOs) and the intracellular accumulation of neurofibrillary tangles formed by hyperphosphorylated tau protein. In this paper, an in vitro pathological model of AD based on neuronal network chip and its real-time dynamic analysis were presented. The hippocampal neuronal network was cultured on the microelectrode array (MEA) chip and induced by AβOs as an AD model to simultaneously record two firing patterns from the interneurons and pyramidal neurons. The spatial firing patterns mapping and cross-correlation between channels were performed to validate the degeneration of neuronal network connectivity. This biosensor enabled the detection of the AβOs toxicity responses, and the identification of connectivity and interactions between neuronal networks, which can be a novel technique in the research of AD pathological model .

ABSTRACT

Objective To evaluate the effects of body position and cuffed endotracheal tube (cETT) position on the intracuff pressure during tonsillectomy in the pediatric patients.Methods Sixty pediatric patients,aged 3-12 yr,scheduled for elective tonsillectomy under general anesthesia,were divided into group A and group B using a random number table,with 30 patients in each group.The cETTs were placed on one side of the blade of Davis retractor in group A.The cETTs were placed between the blade of Davis retractor and the body of tongue in group B.The intracuff pressure was recorded when the head was in the neutral position (T1),after changing to the head back position (T2) and when the retractor was placed for operation (T3).Results The intracuff pressure was significantly higher at T2 than at T1 in the two groups (P<0.01).The intracuff pressure was significantly higher at T3 in group A and lower at T3 in group B than at T2 (P<0.01).Compared with group A,the intracuff pressure was significantly decreased at T3 (P<0.01),and no significant change was found in the intracuff pressure at T1,2 in group B (P>0.05).Conclusion The intracuff pressure is affected by body position and cETT position during tonsillectomy in the pediatric patients,and routine monitoring of the intracuff pressure is recommended.

ABSTRACT

Objective To investigate the diagnostic value of low-frequency repetitive nerve stimulation (RNS) in patients with ocular myasthenia gravis(MG).Methods The clinical data of the low-frequency RNS of 54 in-patients with ocular MG were analyzed.Results The positive rate of low-frequency RNS was 53.7% in 54 patients.The abnormal rate of the facial nerve and the proximal limb nerve was significantly higher than that of low limb distal nerve.The differences were statistically significant(x2 =6.783,P =0.009;x2 =4.167,P =0.041).In all the examined nerves,the abnormal rate of right facial nerve was the highest(46.3%),and the lowest on the ulnar nerve(11.1%).The abnormal rate was statistically significant in all these nerves (x2 =21.995,P =0.001).Conclusion Low-frequency RNS plays an important role in the diagnosis of ocular MG,and the facial nerve should be the first choice in this examination.

ABSTRACT

Objective To investigate the value of nerve conduction velocity detection in early diagnosis of peripheral neuropathy in type 2 diabetes.Methods Motor nerve conduction velocity (MCV)and sensory nerve con-duction velocity (SCV)for 94 patients were detected who was the first time diagnosed type 2 diabetes in our hospital from May 2014 to May 2015.The statistic results of nerve conduction velocity were conducted and then compared with clinical diagnosis to analyze the diagnosis rate for diabetic peripheral neuropathy (DPN).Results The abnormal rate of lower limb nerve conduction velocity was higher in the examined patients,which in the SCV of superficial peroneal nerve was especially highest(69.1%).The diagnostic rate of nerve conduction velocity in DPN was 71.3%,and the clinical diagnosis rate was 33.0%.The difference was statistically significant between the two diagnostic methods (χ2 =8.195,P =0.004).Conclusion Nerve conduction velocity detection has important value for the early diagno-sis of DPN.Patients for the first time diagnosed with type 2 diabetes should be carried out this detection to improve the diagnosis rate of DPN.

ABSTRACT

Objective To explore effects of doctor-nurse integrated care management model on fast track (FT) perioperative management of colorectal cancer patients.Methods Between January 2014 and September 2014,112 colorectal cancer patients (innovation group) underwent doctor-nurse integrated care and FT management in West China Hospital were analyzed retrospectively;who were compared with 96 patients (control group) underwent FT management from the same period of 2011.Early postoperative rehabilitation indicators,length of hospital stay,30d readmission rates,total hospitalization costs and patients satisfaction were compared.Results Innovation group of patients for the first time of post-operative flatus,defecation,oral feeding,ambulation,gastric and drainage tubes removal time were all earlier than those in control group,P <0.05,while the difference of urinary catheter tube removal time between the two groups was not statistically significant,P > 0.05;the length of hospital stay in innovation group(6.05±2.05) d was significantly shorter than that in control group (7.85±3.22) d,P < 0.05;preoperative hospital stay in innovation group was (1.35±0.45) d,shorter than that in control group (2.56±0.63)d,P < 0.05;the patients satisfaction points in innovation group was (94.t2±3.12) points,which was higher than that in control group (86.36±5.51) points,t=2.589,P <0.05.Conclusion Doctor-nurse integrated care management model can improve postoperative rehabilitation effects,can reduce average hospital stay,total hospitalization costs and improve patients satisfaction.
