ABSTRACT

Objective:To investigate the correlation between serum adiponectin (APN) and inflammatory markers, nutritional status and severe acute exacerbation in late stage in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Methods:One hundred and sixty-three male patients with AECOPD from July 2017 to April 2019 in Wusong Hospital of Baoshan District, Shanghai City were selected. The patients were divided into low APN group (serum APN ≤ 102.38 μg/L, 97 cases) and high APN group (serum APN>102.38 μg/L, 66 cases). The interleukin (IL)-6, IL-8, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), hemoglobin, albumin, prealbumin, body mass index (BMI) and number of severe acute exacerbation in 1 year′ follow-up were compared between 2 groups, and the correlation between APN and inflammatory markers, nutritional status and number of severe acute exacerbation were analyzed.Results:The BMI, hemoglobin, prealbumin and albumin in low APN group were significantly higher than those in high APN group: (23.20 ± 3.26) kg/m 2 vs. (20.77 ± 3.78) kg/m 2, (133.23 ± 17.13) g/L vs. (125.68 ± 20.83) g/L, (185.36 ± 57.60) g/L vs. (148.83 ± 64.79) g/L, (37.09 ± 4.06) g/L vs. (33.77 ± 5.86) g/L, and there were statistical differences ( P<0.01 or<0.05); there were no statistical differences in total protein, IL-6, IL-8, CRP and ESR between 2 groups ( P>0.05). APN showed negative correlation with BMI, hemoglobin, albumin and prealbumin ( r = -0.42, -0.28, -0.33 and -0.31; P<0.01); there was no correlation with IL-6, IL-8, ESR and CRP ( P>0.05). The rate of 4 times of severe acute exacerbation in 1 year′ follow-up in low APN group was significantly lower than that in high APN group: 8.25% (8/97) vs. 22.73% (15/66), and there was statistical difference ( χ2 = 6.79, P<0.05). Conclusions:The AECOPD patients with low APN have better nutritional status and less exacerbation. Serum APN can be used as a predictive index of severe acute exacerbation in AECOPD patients

ABSTRACT

Objective To explore the relationship between serum pepsinogen (PG), PGⅡ, PG ratio (PGR), gastrin 17 (G17) and gastric cancer, and to provide the basis for the early diagnosis and differential diagnosis of gastric cancer. Methods Two hundred and forty-eight cases of gastric disease diagnosed with gastroscopy in Baoji Central Hospital from December 2015 to February 2017 were selected as study subjects. According to the results of histopathology, the patients were divided into 4 groups: gastric cancer group (47 cases), chronic atrophic gastritis group (52 cases), chronic non-atrophic gastritis group (81 cases), gastric ulcer group (68 cases). Meanwhile, 50 cases healthy people were enrolled as the control group. The levels of serum PGⅠ, PGⅡ and G17 were measured by enzyme linked immunosorbent assay (ELISA). Results The levels of PGⅠ and PGR in gastric cancer group were lower than those in other groups [PGⅠ: (43±7) vs. (47±7), (69±14), (75±17), (112±22) μg/L; PGR: 5.6±0.5 vs. 10.3±2.6, 10.5±2.5, 11.9±2.7, 14.6± 3.5], and there was a significant difference (PGⅠ: F= 58.42, P = 0.000; PGR: F= 6.15, P = 0.034). The level of serum G17 in gastric cancer group was significantly higher than that in other groups [(43.8±4.3) vs. (22.4±3.6), (10.7±2.1), (13.2±2.4), (5.8±1.3) pmol/L, F= 43.22, P = 0.000]; The PGⅠ, PGR and G17 combined detection of sensitivity(88.6 %), specificity(97.5 %), area under the curve(0.986) was significantly higher than that of single detection(78.3 %, 89.0 %, 88.4 %; 72.9 %, 87.0 %, 73.2 %; 0.848, 0.912, 0.923, respectively; χ2= 7.86, 6.42, 9.10; P = 0.019, 0.044, 0.012). The regression equation provided a basis for confirmation or exclusion of gastric cancer. Conclusions Serum PGⅠ, PGR, G17 and gastric cancer has a good correlation. Combined detection of serum PGⅠ, PGR and G17 could be used for the diagnosis and differential diagnosis of gastric cancer.

ABSTRACT

Objective To study the inhibitive effects of matrine on the formation of glaucoma filtration bleb scar.Methods A total of 40 New Zealand white rabbit were chosen and randomly divided into four groups(group A,B,C and D).Both eyes in four groups were treated with glaucoma filtration surgery.The cotton pieces under the sclera flap were made:1.0 g · L-1 matrine cotton piece in group A,0.4 g · L-1 matrine cotton piece in group B,0.2 mg · L-1 mitomycin cotton piece in group C and PBS cotton piece in group D.The intraocular pressure,filtering bleb,anterior chamber reaction and complications were observed at preoperative and postoperative 1 day,3 days,7 days,14 days,28 days in all groups.Two rabbits selected randomly from every group were killed at 1 day,3 days,7 days,14 days and 28 days after surgery.Tissue was harvested from the bleb area and made into pathological section,immunestaining and Masson trichromatic dyeing were used to observe the inflammatory cells,collagen,fibroblasts,the change of muscle fibroblasts and new collagen fibers under light microscope.The cornea was observed at 7 days under transmission electron microscope for drug toxic reaction,Results At postoperative 7 days,14 days,28 days,there were statistical difference in IOP between group A,B,C and group D group (P < 0.05),and there were statistical difference between group A and group B (P < 0.05).At postoperative 7 days,14 days,28 days,there were statistical difference in fibroblasts counting between group A,B,C and group D group (P <0.01),and there were statistical differences between group A and group B (P < 0.05).Conclusion Matrine can inhibit fibroblasts hyperplasia,reduce the scar of filtration area and has no side effects on eye tissues.

ABSTRACT

Objective To observe the clinical effects of eyehd reconstruction by regional flap combined with xenogeneic acellular dermal matrix transplantation for eyelid defect after malignant tumor excision.Methods Thirty-five cases (35 eyes) in our hospital were selected as the objects.Among them,basal cell carcinoma was 21 cases,meibomian gland carcinoma was 13 cases,squamous cell carcinoma was 1 cases;12 cases of upper eyelid and 23 case of lower eyelid were involved.All patients were subjected to intraoperative frozen,and the incision margin was determined according to the frozen results.After resection of the tumor,the eyelid had full-thickness defects in different degrees.The xenogenic acellular dermal matrix was used to replace the conjunctival tarsal tissue,and the adjacent flap or transposition flap was used to repair the eyelid defect according to the size of skin defect.The healing of flap,oral repair film,eyelid closure and adhesion were observed.Results After half a year follow-up,the acellular dermal matrix were completely dissolved by crawling the conjunctival epithelium covering,the flap healed with no flap necrosis.28 patients recovered well after operation without hypophasis and entropion,ectropion.4 patients had mild hypophasis,and there was no case of exposure keratitis.3 patients were with mild symblepharon.Conclusion The acellular dermal matrix can replace tarsal conjunctival tissue,which combined with regional flap has good clinical curative effect for eyelid defect after malignant tumor excision.This treatment can reduce the pain of patients who take oral mucosa and avoid the reoperation of eyelid reconstruction.

ABSTRACT

Objective To investigate the efficiency and safety of self-made Neiyixiao decoction combined with leuprorelin in patients with endometriosis (EMs). Methods A total of 82 patients with EMs were randomly recruited into a treatment group and a control group, with 41 patients in each group. The treatment group was treated with self-made Neiyixiao decoction combined with leuprorelin, and the control group was treated with leuprorelin only. The serum levels of CA125 and RANTES were detected with enzyme-linked immunosorbent assay. The symptoms and signs scores for pelvic endometriosis were evaluated before and after the treatment. Results The symptoms scores (0.4 ± 0.2 vs. 0.6 ± 0.3;t=3.552, P=0.001) and the signs scores (0.8 ± 0.4 vs. 1.1 ± 0.6;t=2.664, P=0.009) for pelvic endometriosis after the treatment in the treatment group were significantly lower than those in the control group. The serum levels of CA125 (26.4 ± 11.9 U/ml vs. 33.9 ± 12.8 U/ml; t=2.012, P=0.048) and RANTES (34.3 ± 14.1 ng/L vs. 43.7 ± 16.8 ng/L;t=2.744, P=0.008) after the treatment in the treatment group were significantly lower than those in the control group. The total effective rate in the treatment group was significantly higher than that in the control group (90.2%vs. 73.2%;χ2=3.998, P=0.046), and the rate of adverse reaction significantly lower (7.3%vs. 24.4%;χ2=4.479, P=0.034). The recurrence rate in the treatment group was significantly lower than that in the control group (16.2%vs. 46.7%;χ2=5.954, P=0.015) during 12 months of follow-up. Conclusion Self-made Neiyixiao decoction combined with leuprorelin can decrease serum levels of CA125 and RANTES, alleviate symptoms and signs, and its efficiency and safety are superior to leuprorelin alone in patients with endometriosis.

ABSTRACT

Objective To investigate the improving efficacy of hospital -family bed nursing model on pa-tients with post -stroke depression.Methods 80 patients with post -stroke depression were randomly divided into two groups,40 cases in each group.In -hospital comprehensive rehabilitation intervention was applied in the control group,while comprehensive rehabilitation intervention of hospital -family bed was conducted in the observation group.Hamilton Depression Scale (HAMD)was adopted to evaluate the depressive condition before and after inter-vention.Non -conditional logistic regression analysis was applied to analyze the correlation between the patients'age, sex,scores before intervention,hospital -family bed nursing model and the improvement level of HAMD scores. Results The HAMD scores after nursing intervention in observation group and control group were significantly decreased [(control group from (29.57 ±5.69)points to (19.22 ±7.21)points,t =10.329,P =0.000;observation group from (28.60 ±4.12)points to (12.02 ±5.81)points,t =14.302,P =0.000)].After intervention,HAMD score in the observation group was significantly lower than the control group (t =0.786,P =0.000).The effective rate of the observation group was higher than the control group(97.5% vs.70.0%,χ2 =11.114,P =0.001).The results of multivariate logistic regression analysis showed scores before intervention (95%CI:0.788 -0.995,P =0.040)and hospital -family bed nursing model (95%CI:2.501 -23.309,P =0.000)were the independent factors of the reduction of HAMD score.Conclusion Rehabilitation nursing using hospital -family bed nursing model has a markedly effect on patients with post -stroke depression and it deserves the clinical expansion.

ABSTRACT

Objective To evaluate the efficacy of low-dose and long-term clarithromycin in treatment of chronic rhinosinusitis with positive bacterial biofilms.Methods Seventy-seven patients with chronic rhinosinusitis and positive bacterial biofilms underwent functional endoscopic sinusitis surgery between January 2008 and December 2009 in department of otolaryngology of Jinhua Central Hospital.Fortytwo patients were treated with clarithromycin 250 mg/d orally for at least 3 months (trial group).At the end of 3 months patients received endoscopic examination,for patients with mucosal epithelialization the clarithromycin therapy was stopped; for those with mucosal edema,granulation or polyoid formations the clarithromycin treatment was continued,but no longer than 5 months.Thirty-five patients in control group were not given clarithromycin.All patients were assessed by visual analogue scale (VAS) and nasal endoscopic examination (Lund-kennedy score) 6 months after surgery,the results were compared between two groups.Results Two patients in the trial group did not complete the protocol.VAS score and Lundkennedy score in trial group were 8.0 ± 0.8 and 3.6 ± 1.3 at 6 month after surgery respectively,while those in the control group were 7.3 ± 0.7 and 4.5 ± 1.7,and the differences were of statistical significance (t =5.311 and-3.187,P < 0.01).Conclusion Low-dose and long-term oral administration of clarithromycin has positive impact on the prognosis of bacterial biofilms positive patients with chronic rhinosinusitis.

Objective To analyze the thyroid hormone levels before and after treatment in the patients with chronic obstructive pulmonary disease in acute exacerbation stage (AECOPD) with respiratory failure.Methods In 40 cases of elderly AECOPD patients with respiratory failure,the thyroid hormone levels before and after the treatment were measured.At the same time,the thyroid hormone levels in another 40 agematched healthy volunteers were determined.Results In elderly AECOPD patients,serum T3,FT3,T4 and FT4were significantly lower than that of the control group:T3 ( [ 1.23 ± 0.25 ] nmol/L vs.[ 1.70 ± 0.29 ] nmol/L,t =3.97,P<0.01),FT3( [3.27 ±0.59] pmol/L vs.[4.48 ±0.95] pmol/L,t =6.09,P <0.01),T4 ([ 109.3 ±17.73 ] nmol/L vs.[ 116.01 ± 18.72 ] nmol/L,t =6.94,P <0.01 ),FT4( [ 15.11 ±2.37] pmol/L vs.[ 17.62 ±0.35 ] pmol/L,t =7.23,P < 0.01 ) ; While in the elderly AECOPD patients with respiratory failure,the T3,FT3,T4 and FT4 were lower than those patients without respiratory failure:T3 ( [ 1.08 ± 0.10 ] nmol/L vs.[ 1.35 ±0.26 ] nmol/L,t =4.02,P < 0.01 ),FT3 ( [ 2.89 ± 0.41 ] pmol/L vs.[ 3.59 ± 0.53 ] pmol/L,t =4.58,P <0.01 ),T4 ([96.54 ± 14.34] nmol/L vs.[ 115.20 ± 26.10] nmol/L,t =2.71,P < 0.01 ),FT4 ( [ 14.05 ±2.05 ] pmol/L vs.[ 17.11 ± 1.55 ] pmol/L,t =5.37,P < 0.01 ).In elderly AECOPD patients,the post-treatment T3,FT3 and T4 levels were elevated compared with that before treatment.The difference was statistically significant:T3 ( [ 1.43 ± 0.29 ] nmol/L vs.[ 1.25 ± 0.25 ] nmol/L,t =2.93,P < 0.01 ),FT3 ( [ 3.61 ± 0.49 ]pmol/L vs.[3.26 ± 0.60] pmol/L,t =2.73,P < 0.01 ),T4 ( [ 114.31 ± 10.99] nmol/L vs.[ 110.28 ± 16.91 ]nmol/L,t =4.58.P <0.01).Conclusion In elderly AECOPD patients,the T3,FT3,T4 and FT4 levels are reduced and those in AECOPD patients with respiratory failure were decreased even more.Supportive treatment is able to enhance the thyroid hormone levels and improve the patients' condition.Therefore,the level of thyroid hormone in elderly AECOPD patients is useful to predict the patient's condition and prognosis.

ABSTRACT

Both the sleep apnea scale of sleep disorders questionnaire (SA-SDQ) and the Epworth sleepiness scale (ESS) were used to evaluate 25 patients with snoring and 125 patients with obstructive sleep apnea hypoventilation syndrome ( OSAHS), and scores were calculated and compared to the results of polysomnography ( PSG). Results showed that there was significant difference in scores of ESS and SA-SDQ, apnea hyponea index (AHI) and the minimal saturation of blood oxygen (LSpO2) at night between patients with OSAHS and those with snoring (P < 0.05). Score of both ESS and SA-SDQ significantly correlated with AHI ( r = 0. 78 and r = 0. 66, respectively, P < 0. 01 ) , i. e. , more severe of the condition, higher the scores. Score of ESS is highly consistent with clinical presentations in patients with OSAHS and can be used as preliminary screening for its diagnosis, however, use of SA-SDQ score is limited in diagnosing OSAHS due to its lower consistency.

ABSTRACT

To synthesize salbutamol immunogen and develop an enzyme immunoassay (ELISA), a new salbutamol immunogen was synthesized using 4-aminobenzoic acid as a linker to connect hapten with carrier protein. An enzyme immunoassay based on the antibody prepared was developed and applied to detect salbutamol residue spiked in swine liver. An unusual coating antigen, clenbuterol-ovalbumin (OVA) conjugate instead of salbutamol-OVA conjugate, was used in the immunoassay and the results were discussed based on the structures of related compounds. The antibodies showed high sensitivity in the heterologous assay when using clenbuterol-OVA as a coating antigen, with an IC50 value of 8.97 ng mL(-1) toward salbutamol. The antibodies prepared showed high cross-reactivity with clenbuterol (107%) and were promising for the simultaneous determination of salbutamol and clenbuterol residues in food and food products. Recovery rates from the salbutamol-spiked swine liver samples were in the range of 70%-99%, while the intra-assay and inter-assay coefficients of variation were <13.3% and <14.3%, respectively. In summary, the antibodies of salbutamol have been successfully prepared. Sensitive and stable analysis for the detection of salbutamol residues in swine liver was obtained based on the competitive ELISA methods developed in this study.

ABSTRACT

Objective: To investigate the changes of peripheral cardiotrophin-1 (CT-1) and brain natriuretic peptide (BNP) concentrations in patients undergoing off-pump coronary artery bypass grafting (OPCABC), and the clinical significance of peripheral CT-1 and BNP in cardiac surgery thereof. Methods:Fifty elective OPCABG patients were included in the study. The concentrations of CT-l and BNP were measured before surgery, and at 6 hours, 72 hours, 1 week and 10 months after the operation respectively. The clinical data of the heart function were also collected in patients . Results:(1 )The peripheral CT-1 levels were significantly higher at 10 months after OPCABC than those before operation. (2 )The concentration of peripheral BNP increased at 6 hours after operation. The peak level was found at 72 hours, and remained higher level until the 1 week after operation. The BNP concentration was returned to the normal level at 10 months after OPCABG. (3 )The baseline levels of CT-1 and BNP were both positively correlated with NYHA rank and the diameter of the left ventricular end-diastolic dimension (LVEED), but negatively correlated with left ventricular ejection fraction (LVEF). However, no correlations were found between CT-1 and BNP and NYHA rank, LVEF or LVEED at 10 months after OPCAB. Conclusion:The perioperative blood BNP levels were increased significantly by OPCABG, while perioperative level of CT-1 increased slowly compared with that of BNP. In cardiac surgery,perioperative concentrations of CT-1 and BNP may reflect the cardiac function in patients before OPCABG. The further studies are warranted to assess the prognosis.

ABSTRACT

ObjectiveTo know about the correlation analysis of the physical and mental status of organ transplanted patients' family members during the patients' rehabilitation period and the long term survival of the transplanted organs.MethodsA total of 600 organ transplanted patients' family members were investigated by Zung self-rating anxiety scale (SAS) and simple coping style questionnaire.The results underwent subsequent analysis.ResultsAnxious state of organ transplanted patients' family members was more severe than that of domestic norm (P<0.01).There were some differences in terms of anxious state of family members with different genders,education backgrounds,income and the resource of medical expenditure (P<0.05).Family members most took positive coping styles,whereas fewer adopted negative coping styles (P<0.01).Positive coping styles were negatively correlated with the anxiety of family members (P<0.01) and positively correlated with long term survival of transplanted organ(P<0.05).However,negative coping styles of family members were significantly positively correlated with their anxiety (P<0.05) and were negatively correlated with long term survival of transplanted organ.ConclusionsThe anxiety generally exists in organ transplanted patients' family members.There are differences in terms of anxious state among family members of different genders,different education backgrounds,income or with the resource of medical expenditure.The more they adopt positive coping styles,the lower anxiety level they show and the longer the transplanted organ survive.Conversely,the more they adopt negative coping styles,the higher anxious level they show and the shorter the transplanted organ survive.

ABSTRACT

Using the large scale sequencing, a novel human cDNA of 1,645 bp was screened from the cDNA library of human fetal brain. The cDNA contains an ORF encoding a 296-aa protein with a calculated molecular weight of 34.0 KD. Compared with that of current sequence databases, the putative protein was found to have 36% homology with protein-disulfide isomerase (PDI). So this cDNA was named PDI like (PDI-L) gene. Multiple tissue Northern blot analysis shows that PDI-L cDNA is expressed in heart, brain, liver, kidneys and so on. The ORF fragment of PDI-L cDNA was inserted into reconstructed pBV220 expression vehicle and its predicted expressive protein was obtained.

ABSTRACT

AIM: To study the activation effects of maize pollen polysaccharides(PPM) on human thoracic cavity macrophage (hTM). METHODS: Activities of lactate dehydrogenase(LDH) and acid phosphatase (ACP) in the hTM were detected by automatic biochemical analyzer, and the level of tumor necrosis factor alpha(TNF-?) and interleukin-6(IL-6) in the hTM was analyzed by radioimmunoassay, after hTM were cultured with 0.312,0.625, 1.250, 2.500, 5.000 mg/mL PPM-RPMI 1640 for 24 and 48 hours in vitro. RESULTS: The activities of LDH and ACP increased in the hTM induced by PPM, and the levels of TNF-? and IL-6 in the hTM induced by PPM increased markedly too. And the induced expression effect of TNF-? and IL-6 is associated with the concentration of PPM, and time for PPM inducing. CONCLUSION: PPM can induce cytokines secretion in hTM,and activate hTM in vitro.
