ABSTRACT

For patients with type 2 diabetes,early intensive insulin therapy can quickly correct hyperglycemia,improve high glucose toxicity,reduce insulin resistance,protect and even reverse partial residual β-cell function and prevent or delay the occurrence of chronic complications.Therefore,this method is more and more widely used in clinical practice.For the patients receiving intensive insulin therapy,it is of great significance to identify the patients who are prone to recurrence as early as possible through the influencing factors (especially the predictive indicators before treatment) and take timely intervention measures to improve the treatment rate of diabetes mellitus and the rate of glycemic control.

ABSTRACT

Objective To observe the changes of serum leptin(LP) levels and serum tumor necrosis tactor-alpha(TNF?) concentrations and to explore the relationships with fasting serum insulin levels and congestive heart failure(CHF).Methods Serum leptin and inslin concentrations were measured by radioimmunoassay and tumor necrosis factor-alpha by ELISA.Body mass index(BMI),mean arterial blood pressure(MBP),fasting serum insulin(FINS),LP and TNF-alpha levels were determined in 40 patients with CHF and in 20 healthy controls matched for gender,age,BMI and MBP.The correlations between leptin and other parameters were analyzed.Results ⑴The fasting serum LP and FINS concentrations of patients with NYHA grade Ⅲ and NYHA grade Ⅳ were significantly lower as compared with the controls(P0 05).⑵The TNF alpha was higher in heart failure patients that in healthy controls(P

ABSTRACT

Objective To investigate the effects of rosiglitazone (RSG) on the levels of serum HbA1c, interleukin-6(IL-6) and high sensitive C-reactive protein (hsCRP) in type 2 diabetic patients who were inadequately controlled only with insulin therapy. Methods 40 patients with type 2 diabetes mellitus (T2DM), whose blood glucose was poorly controlled (HbA1c≥7 5%) after 4 weeks of standardized insulin therapy, were additionally given 4mg daily PSG. The levels of serum HbA1c, IL-6, hsCRP, total cholesterol (Tch), triglyceride (TG), low-density lipoprotein cholesterol (LDL-c) and high-density lipoprotein cholesterol (HDL-c) were measured before and after 12 weeks treatment with RSG. hsCRP was measured by high-sensitivity immunoturbidimetric method, and IL-6 by enzyme-linked immunoadsorbent assay (ELISA). Results ⑴ RSG 4mg daily significantly improved glycemic control, treatment with RSG 4 mg plus insulin resulted in a mean 1 59% reduction of HbA1c from baseline (P
