ABSTRACT

@#AIM: To observe the therapeutic effects of different dosage regimens of conbercept on clinical significant diabetic macular edema(CSME).<p>METHODS: Totally 65 patients with diabetes and CSME who were admitted to the Ophthalmology Department at Ganzhou People's Hospital between January 2019 and January 2020 were selected as the research subjects, and divided into observation group(<i>n</i>=33, conbercept 5+PRN regimen)and control group(<i>n</i>=32, conbercept 3+PRN regimen)using random number table method. Visual acuity test and optical coherence tomography(OCT)were carried out at 1, 2, 3, 6, 9, 12mo after treatment. Changes in the best corrected visual acuity(BCVA,LogMAR)and central macular thickness(CMT)were compared between the two groups. The mean injection times and complications in the two groups were recorded.<p>RESULTS: The BCVA was improved and CMT thinned in the two groups at 1, 2, 3, 6, 9, 12mo after treatment(<i>P</i><0.05). There was no statistically significant difference in BCVA and CMT between the two groups before treatment, and at 1, 2, 3, 6, 9, 12mo after treatment(<i>P</i>>0.05). The mean injection times in observation group was more than that in the control group \〖(5.81±0.54)times <i>vs </i>(4.19±0.41)times\〗(<i>P</i><0.05). In the early stage of postoperative follow-up, there were 30 times(23%)and 15 times(22%)of subconjunctival hemorrhage in observation group and control group, respectively(<i>P</i>>0.05). No other severe complications were observed in the two groups.<p>CONCLUSION:Both conbercept 3+PRN and conbercept 5+PRN are effective in the treatment of patients with diabetes and CSME. Both can significantly improve the patients' visual acuity and reduce CMT, with equivalent effect. The latter may increase the injection times but its adverse reactions are tolerable. In addition, it is safe and reliable.

ABSTRACT

Objective:The study aimed to evaluate the effect of systemic-pulmonary shunt(SPS) on the infants with tetralogy of Fallot(TOF) with hypoplasia pulmonary arteries.Methods:Among 949 infants less than three-year-old who underwent surgical intervention between January 2010 and December 2015, 853 infants underwent one-stage primary repair(group Ⅰ), whereas 96 infants underwent SPS(group Ⅱ). Among them, 546 were males and 403 were females. The average age and average weight at operation were 13 months and 9.4 kg, respectively. 20.8% infants had clinical symptoms before operation. Perioperative and follow-up parameters were assessed including age, weight, sex, transcutaneous oxygen saturation(SPO 2), preoperative clinic symptom, ratio of McGoon, NAKATA index, Z score of pulmonary annulus, left ventricular end diastolic diameter(LVEDD). Results:There were 28(3.0%) early postoperative deaths in the whole group. And no significant difference in mortality was found between group Ⅰ(2.9%) and group Ⅱ(3.1%). Compared with group Ⅰ, age and weight were significantly lower in patients in group Ⅱ[(0.95±0.56)years old vs.(1.09±0.59)years old] and[(8.6±1.8)kg vs.(9.5±2.1)kg], respectively. Patients in group Ⅱ had a lower SPO 2(0.75±0.09 vs. 0.85±0.09) and was more prone to appear hypoxic spell(30.2% vs. 19.7%) than patients in group Ⅰ. Patients in group Ⅱ had a more poor development in pulmonary artery. The ratio of McGoon, NAKATA index and Z score of pulmonary annulus in group Ⅱ were significantly less than that in group Ⅰ[1.4±0.3 vs. 1.9±0.5, (124±43)mm 2/m 2 vs.(222±88)mm 2/m 2 and-4.3±2.6 vs.-2.3±2.1], respectively. There was no difference of extubation time bewteen two groups. SPO 2 rose significantly from 0.75 to 0.83 after SPS. And after SPS, the diameter of main pulmonary artery, the ratio of McGoon, NAKATA index and Z score of pulmonary annulus increased significantly from 6.4 mm, 1.42, 126.4 mm 2/m 2 and -4.3 to 9.2 mm, 1.83, 212.6 mm 2/m 2 and -2.4, respectively. Moreover, LVEDD also increased significantly from 21.2 mm to 24.5 mm after SPS. Conclusion:SPS is safe and effective for little infants with anoxic spell and hypoplasia pulmonary arteries. SPS can improve oxygenation significantly and delay the age for radical surgery. SPS also promote the growth of the pulmonary arteries, pulmonary valve annulus and left ventricular, and it facilitates the preservation of pulmonary valve annulus and reduce the rate of transannular patching in a portion of infants.

ABSTRACT

Objective:To investigate the effect of anterior or posterior debridement on infections of the lumbar vertebral fusion with cage and pedical screw system.Methods:A total of 10 cases (3 males and 7 females, age 49.80±13.29 years) with infections in the lumbar fusion device were admitted from January 2013 to December 2019. The cases were diagnosed with deep infections after the preview surgery at 10.80±13.24 months, including 10 cases with 12 cages infections and 8 cases with screw system infections. 7 cases were done with debridement and removal of the cages via the anterior approach. And another 3 cases underwent posterior debridement and removal of lumbar fusion cages. The changes in laboratory examination such as WBC count, ESR, CRP, visual analogue scale (VAS) and Oswestry Disability Index (ODI) score were compared preoperatively, 2 months and 6 months postoperatively.Results:No patients were lost in the follow-up. The average follow-up time periods is 16.30±5.10 months (range 9-24 months). There was no significant difference in WBC count between 2 and 6 months after surgery compared with preoperation ( F=0.855, P=0.436). The ESR, CRP, VAS and ODI scores decreased in 2 months and 6 months after sugery, which has significant differences compared preoperation ( P<0.05). The abnormal signals in the lambar vertebral showed a gradual fading trend in the postoperative MRI. The completely fading time was 5.00±1.33 months (range 3-7 months). One-stage bone grafting was performed in 6 cases with 8 spaces via anterior approach, 5 cases with 7 spaces showed the intervertebral fusion after postoperative with 6.80±2.28 months (range 4-10 months), 1 case wirh 1 space was not fused. One-stage bone grafting was performed in 2 cases via posterior approach: 1 case was fused after postoperative and the other patient was not, which due to bone graft area infection. Postoperative bone graft displacement occurred with 1 case and 1 case with L 5 nerve root pulling injury during the operation. Conclusion:The fusion cage can be removed by debridement via posterior approach, but it is difficult to done the completely debridement, which the main reason is the obstruction of the posterior nerve and bone structure. Posterior approach also have risks of infection recurrence and nerve root injury. Cage removal via anterior approach was relatively easy and debridement was thoroughly, which has the risk of injury of important adjacent vessels.

ABSTRACT

Objective:To investigate the early effect of modified posterior tibialis muscle transfer in treating foot drop and varus deformity caused by common peroneal nerve injury.Methods:A retrospective case series study was conducted to analyze the clinical data of 6 patients with foot drop of common peroneal nerve palsy and varus deformity admitted to Beijing Jishuitan Hospital from December 2017 to October 2019, including 4 males and 2 females, aged 33-48 years [(39.5±6.0)years]. The left side was involved in 4 patients and the right side in 2 patients. All patients underwent posterior tibial muscle transfer. The insertion of posterior tibial muscle was reconstructed in the fourth metatarsal with the tendon allograft to correct foot drop and varus deformity. The range of motion of active ankle dorsiflexion and plantar flexion and range of motion of varus and valgus of foot were measured before operation and at the last follow-up. The isokinetic torque peak value of ankle dorsiflexion and plantar flexion at 60°/s angular velocity and peak isokinetic torque of foot varus and valgus at 60°/s angular velocity were measured before operation and at the last follow-up. The postoperative foot imaging evaluation was performed at the last follow-up, including Meary angle, calcaneal projection and diameter of the fourth metatarsal. The American Orthopaedic Foot & Ankle Association (AOFAS) ankle-hindfoot score was used to evaluate the ankle function.Results:All patients were followed up for 6-17 months [10(6, 15)months]. At the last follow-up, the range of motion of dorsiflexion of the ankle [6(0, 10)°] and foot valgus [3(0, 5)°] were significantly improved compared with the preoperative level [-31(-33, -28)°, -10(-12, -8)°] ( P<0.05); the ankle dorsiflexion torque [(7.7±0.8)Nm] and foot valgus torque [(7.2±0.7)Nm] were significantly improved compared with the preoperative level [(0.0±0.0)Nm, (2.1±0.6)Nm]at 60°/s angular velocity ( P<0.01). Imaging examination showed no acquired flat foot deformity and deformation of the fourth metatarsal insertion. The AOFAS ankle-hindfoot score increased from 50-73 points [(61.3±8.4)points] before operation to 75-97 points [(86.8±7.2)points] at the last follow-up ( P<0.01). Conclusion:For foot drop and varus caused by common peroneal nerve injury, modified posterior tibialis muscle transfer can increase the torque of ankle dorsiflexion and valgus, improve the ankle range of motion, and accelerate the ankle function recovery.

ABSTRACT

Objective:To investigate the vitamin D level of pulmonary tuberculosis patients in Qinghai Province, and to explore the correlation between vitamin D level and pulmonary tuberculosis.Methods:From May to September 2020, 208 bacterial confirmed pulmonary tuberculosis patients who were admitted to The 4th People′s Hospital of Qinghai Province were enrolled as the pulmonary tuberculosis group, and 129 healthy people who underwent physical examination during the same period were enrolled as the healthy control group. Independent sample t test and chi-square test were used for statistical analysis. Results:The deficiency rate of vitamin D was 11.06%(23/208) in the pulmonary tuberculosis group, which was higher than that (3.10%(4/129)) in the healthy control group, and the difference was statistically significant ( χ2=6.840, P=0.009). The vitamin D level was (56.84±20.03) μg/L in the pulmonary tuberculosis group, which was lower than that ((67.39±17.07) μg/L) in the healthy control group, and the difference was statistically significant ( t=5.154, P<0.01). The vitamin D levels were not different between the newly treated ((56.66±20.02) μg/L)) and retreated pulmonary tuberculosis patients ((59.11±20.81) μg/L) ( t=0.468, P=0.650). The vitamin D level of simple pulmonary tuberculosis patients ((57.82±20.01) μg/L) was higher than that of pulmonary tuberculosis patients combined with other diseases ((48.08±18.46) μg/L), and the difference was statistically significant ( t=2.132, P=0.034). Conclusion:Pulmonary tuberculosis is associated with decreased vitamin D levels, and patients with pulmonary tuberculosis are more likely to suffer from decreased or deficient vitamin D, which suggests clinicians considering the vitamin D status when treating pulmonary tuberculosis patients.

ABSTRACT

Objective:To evaluate the safety and clinical effect of gene therapy for Leber hereditary optic neuropathy (LHON).Methods:A multi-center prospective non-randomized controlled trial was conducted.Eighty eyes of 40 LHON patients with mitochondrial DNA 11778 mutation were enrolled in Taihe Hospital from December 2017 to February 2018.Intravitreal injection of recombinant adeno associated virus 2-NADH dehydrogenase 4 (rAAV2- ND4) was carried out in the unilateral eye with worse visual acuity or the right eye (if the visual acuity of both eyes was equal) of each subject as the treated group and the fellow eyes as the untreated group.The best corrected visual acuity (BCVA) was detected using a standard logarithmic chart and intraocular pressure (IOP) was measured with a non-contact tonometer before treatment and 1, 3, 6, 12 months after treatment.The manifestations of the ocular anterior segment and fundus were examined by slit lamp microscopy and color photography.The changes of visual acuity and IOP before and after gene therapy were compared, and complications were evaluated between the treated group and the untreated group.The effective rate defined as visual acuity improved ≥0.3 LogMAR at the end of follow-up was assessed.This study adhered to the Declaration of Helsinki and the study protocol was approved by an Ethics Committee of Taihe Hospital (No.201807). Written informed consent was obtained from each subject prior to any medical examination and treatment. Results:The visual acuity improved 6 eyes in the treated group and 4 eyes in the untreated group, and 13 patients showed bilateral improvement.The visual acuity improvement ≥0.3 LogMAR in 23 patients with the effective rate 57.5%.The BCVA was (1.51±0.62) LogMAR and (1.62±0.58) LogMAR at the end of following-up in the untreated group and treated group, respectively, which were significantly higher than (1.75±0.46) LogMAR and (1.83±0.47) LogMAR before treatment (both at P<0.01), and no significant difference was found in BCVA between the two groups ( Fgroup=0.084, P=0.772). There was no significant difference in IOP between the two groups before and after treatment ( Fgroup=0.557, P=0.575; Ftime=2.314, P=0.106). No serious complications were found in all subjects during following-up. Conclusions:rAAV2- ND4 gene therapy is safe and effective for LHON, and binocular vision can be improved by monocular intravitreal injection of rAAV2- ND4 gene.

Objective:To analyze the relationship between physical appearance perfectionism, negative body self, fear of negative evaluation and social appearance anxiety in female patients with psoriasis.Methods:A total of 251 female patients with psoriasis in Xinhua Hospital Affiliated to Medical College of Shanghai Jiaotong University from June 2019 to June 2020 were selected as the subjects. Physical Appearance Perfectionism Scale (PPAS), Social Appearance Anxiety Scale (SAAS), Brief Fear of Negative Evaluation Scale (BFNES) and Negetive Physical Self Scale (Npss) were used in the survey. The correlation of negative body self and negative evaluation fear between the appearance perfectionism and social appearance anxiety of female psoriasis patients was analyzed.Results:The score of physical appearance perfectionism was 49.42±5.32, the score of social appearance anxiety was 58.72±6.24, the score of fear of negative evaluation was 30.59±7.21 and the score of negative physical self was 29.21±6.24. Pearson correlation analysis showed that there was a significant positive correlation among physical appearance perfectionism, social appearance anxiety, fear of negative evaluation and negative physical self ( r value was 0.402-0.488, P<0.01). The mediating effect of negative physical self between physical appearance perfectionism and social appearance anxiety was 0.164 (95% CI 0.082-0.227), the mediating effect of negative body self between physical appearance perfectionism and social appearance anxiety was 0.152 (95% CI 0.079-0.214) and the chain mediating effect of fear of negative evaluation and negative physical self was 0.067 (95% CI 0.025-0.108). Conclusions:Female patients with psoriasis have a high level of physical appearance perfectionism, which can directly affect their social anxiety, while fear of negative evaluation and negative physical self indirectly affect the correlation between appearance perfectionism and social appearance anxiety. Psychological intervention measures should be taken to reduce their psychological burden on their own appearance defects and carry out social anxiety and fear.

OBJECTIVE:To in vestigate the effects of quercetin (Que)on the expressio n of angiotensin Ⅱ(AngⅡ)-induced myocardial contractile proteins of primary rats through angiotensin-converting enzyme 2-angiotensin-(1-7)-Mas (ACE2-Ang- (1-7)-Mas)axis. METHODS :Cardiac tissue of rats aged 1-2 d were collected ,and primary cardiomyocytes were isolated and cultured. The gene silencing model of cardiomyocytes ACE2 was constructed. Experiments were divided into 12 groups. Among them,AngⅡ group,AngⅡ+ small interference RNA (siRNA)group,and Ang Ⅱ+ A 779 group were the model groups ;AngⅡ+ losartan group was positive control group ;AngⅡ+Que40 group,AngⅡ+Que80 group,AngⅡ+siRNA+Que40 group,AngⅡ+ siRNA+Que80 group,AngⅡ+A779+Que40 group and Ang Ⅱ+A779+Que80 group were the experimental groups ;blank group and siRNA group were set up. Ang Ⅱ concentration was 1×10-6 mol/L;siRNA final concentration was 50 nmol/L;Que concentration was 40 and 80 μmol/L;A779(Mas receptor inhibitor )concentration was 1 μmol/L;losartan concentration was 1×10-4 mol/L. mRNA and protein expression of ACE 2,Ang-(1-7) and Mas in primary cardiomyocytes were detected ;the expressions of myocardial contractile proteins were also determined ,such as Na +/Ca2+ exchange channel (NCX),calcium pump (SERCA2a), phosphoprotein (PLB). RESULTS :Compared with Ang Ⅱ group,mRNA expression of Mas was increased significantly in Ang Ⅱ + Que 80 group (P<0.05);mRNA expression of ACE2 and Mas were increased significantly in Ang Ⅱ + CZ0210-01) losartan group (P<0.05). Compared with Ang Ⅱ group,the 851136165@qq.com protein expression of ACE 2 and Ang- (1-7) were increased significantly in Ang Ⅱ+ Que 40 group(P<0.05);compared with Ang Ⅱ + siRNA group ,the protein expression of Ang-(1-7)were increased significantly in Ang Ⅱ+ siRNA+Que 40 group(P<0.05);compared with Ang Ⅱ+A779 group,the protein expression of Ang- (1-7)were increased significantly in Ang Ⅱ+A779+ Que 40 group(P<0.05). Compared with Ang Ⅱ group,the protein expression of NCX was decreased in Ang Ⅱ+Que40 group(P<0.05),protein expression of NCX was reduced in Ang Ⅱ+ losartan group (P<0.05);compared with Ang Ⅱ+A779 group,the protein expression of NCX was decreased in Ang Ⅱ+A779+ Que80 group (P<0.05). CONCLUSIONS :Que improves the expression of Ang Ⅱ -induced ACE 2-Ang-(1-7)-Mas axis in cardiomyocyte model to some extent ,so as to regulate myocardial contractile protein.

ABSTRACT

Prostate cancer is an epithelial malignant tumor that occurs in the prostate. It is the most common cancer among men in the world. Its mortality rate is second only to lung cancer, which has seriously affected men's healthy life. Prostate cancer is not easy to be detected in the early stage. Most patients are already in the advanced stage and metastasized when they are diagnosed. At this time, radical surgery is not suitable for treatment. Advanced prostate cancer is mainly treated with endocrine therapy, but after a median time of 14-30 months, the patient will develop hormone-resistant prostate cancer and enter the advanced stage. Advanced hormone-resistant prostate cancer is treated with radiotherapy and chemotherapy, which, however, are not ideal for treating advanced prostate cancer. They can also cause serious side effects such as frequent urination, dysuria, urinary incontinence, rectal bleeding, loss of libido, hot flashes, and impotence, as well as generate multi-drug resistance. Therefore, it′s urgent to find a safe, effective and low-toxic anti-tumor drug. With the deepening of the research on the pharmacological effects of peptides, it has been proved that peptides have important application value in anti-fatigue, anti-aging, anti-inflammatory, antibacterial and anti-tumor. The anti-tumor peptides have attracted extensive attention in the field of anti-tumor research due to their high specificity, high bioavailability, good stability, low toxicity, low molecular weight, and easy synthesis. Pharmacological experiments have shown that polypeptides exert their anti-tumor effect mainly by inducing cell apoptosis, causing cell cycle arrest, inhibiting angiogenesis, and regulating immune function. This article summarized the related literature of peptides in recent years, the sources of peptides, the mechanisms and applications of peptides against prostate cancer for experimental research and clinical reference.

ABSTRACT

Objective:To systematically sort out and summarize the medication rules of clinical prescriptions for coronary heart disease with heart failure of Qi deficiency and blood stasis syndrome,and to provide reference for selecting prescriptions and medications for the treatment of coronary heart disease (CHD) with traditional Chinese medicine (TCM). Method:All relevant literature concerning the treatment of CHD with compound TCM prescriptions for Qi deficiency and blood statis syndrome from 2000 to 2020 were retrieved from the China National Knowledge Network (CNKI),WanFang database (WanFang),and VIP journal database (VIP),and the names of prescriptions and drug components were extracted,followed by the frequency of drug use and drug category. Association rules of high-frequency drugs were analyzed by SPSS 18.0,and systematic clustering analysis was conducted by SPSS 21.0. Result:Finally,41 qualified literature articles covering 35 prescriptons and 66 drugs were included in the study. The total frequency of the drugs was 433 times. Among them,there were a total of 25 traditional Chinese medicines with a frequency of ≥5 times. The top 3 frequently used Chinese medicines were Astragali Radix (8.8%),Salviae Miltiorrhizae Radix et Rhizoma (7.2%),Chuanxiong Rhizoma (5.8%). A total of 15 types of drugs were involved,among which tonic drugs (31.4%),blood promoting and blood stasis drugs (28.2%),and hydration and dampening drugs (7.6%) were used most frequently. The association rule analysis of traditional Chinese medicines with frequency of ≥ 5 showed that there were 13 pairs of binomial associations in 25 traditional Chinese medicines,with Astragali Radix in combination with Salviae Miltiorrhizae Radix et Rhizoma,Chuanxiong Rhizoma,Carthami Flos,et al. There were 8 groups of three associations,with Astragali Radix,Salviae Miltiorrhizae Radix et Rhizoma,and Chuanxiong Rhizoma as the main combinations. A systematic clustering analysis showed that the clustering effect was best when the 25 traditional Chinese medicines were clustered into 5 categories. Conclusion:The treatment of coronary heart disease with heart failure of Qi deficiency and blood stasis syndrome is based on replenishing qi and activating blood circulation,supplemented by warming yang,diuresis,and phlegm-resolving drugs,which can enhance the clinical efficacy. Two basic prescriptions of Buyang Huanwutang and Si junzitang are extracted. Other combinations of prescriptions and drugs can provide references for the clinical treatment of coronary heart disease with heart failure.

Objective:The chemical constituents in guarana (<italic>Paullinia cupana</italic> dried seeds) were systematically analyzed to provide a basis for further research, development and utilization of this plant. Method:The contents of crude protein, crude fat, crude polysaccharide and crude fiber in guarana were determined according to national standards and related documents, and the chemical constituents of guarana was qualitatively analyzed by ultra performance liquid chromatography-quadrupole-time-of-flight mass spectrometry (UPLC-Q-TOF-MS), ACQUITY UPLC-HSS-T3 column (2.1 mm×100 mm, 1.8 μm) was used with 0.1% formic acid aqueous solution (A)-0.1% formic acid acetonitrile solution (B) as mobile phase for gradient elution (0-5 min, 2%-10%B; 5-6 min, 10%-20%B; 6-9 min, 20%-30%B; 9-9.5 min, 30%-35%B; 9.5-10.5 min, 35%-45%B; 10.5~13 min, 45%-55%B; 13-15 min, 55%-80%B; 15-19 min, 80%-98%B; 19-20 min, 98%B; 20-20.3 min, 98%-2%B; 20.3-23 min, 2%B), the electrospray ionization (ESI) was used for detection in positive and negative ion modes, the scanning range was <italic>m</italic>/<italic>z</italic> 50-1 500, and the structure was identified according to the relative molecular weight and fragment information combined with database matching and comparison of reference substances. Result:The contents crude protein, crude fat, crude polysaccharide and crude fiber in guarana were (0.63±0.03)%, (2.73±0.09)%, (3.23±0.12)% and (8.89±0.59)%, respectively. A total of 42 chemical constituents in guarana were identified by UPLC-Q-TOF-MS, including 3 methylxanthines, 2 nucleosides, 1 amino acid, 3 organic acids, 33 flavonoids, 3 (<italic>L</italic>-tryptophan, epigallocatechin gallate, daidzein) of which were first discovered in guarana. Conclusion:Guarana is rich in nutrients and has good potential to be developed as a functional food. UPLC-Q-TOF-MS technique provides a simple, rapid and accurate method for the identification of chemical constituents in guarana. Methylxanthines and proanthocyanidins are the main chemical constituents of guarana, which is meaningful for quality evaluation and material basis of guarana.

Objective:To explore the correlations of the severity of heart failure related to coronary heart disease arising from Qi deficiency and blood stasis with cardiac function indexes, energy metabolism indexes, coagulation function indexes, and inflammatory factors, in order to provide a scientific basis for further research on the biological foundation of this disease. Method:Two hundred patients with heart failure related to coronary heart disease of Qi deficiency and blood stasis syndrome were collected and then classified into mild, moderate and severe groups according to their scores of Qi deficiency and blood stasis syndrome. Meanwhile, 40 healthy persons confirmed by physical examination during the same period were included into the control group. Such biological indexes as cardiac function indexes, energy metabolism indexes, coagulation function indexes, and inflammatory factors were determined in patients of each group for comparison. Then the Spearman rank correlation analysis was conducted to figure out the correlations between differential indexes and the severity of Qi deficiency and blood stasis syndrome, followed by the determination of risk factors for the severity of Qi deficiency and blood stasis syndrome by ordered logistic regression analysis. Result:The cardiac function indexes, energy metabolism indexes, coagulation function indexes, and inflammatory factors in patients with heart failure related to coronary heart disease arising from Qi deficiency and blood stasis varied significantly. There were significant statistical differences in the levels of N-terminal pro-B-type natriuretic peptide (NT-ProBNP), 6-minute walk test (6MWT), heart-type fatty acid-binging protein (H-FABP), prothrombin time (PT), activated partial thromboplastin time (APTT), tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>), and nitric oxide (NO) among the mild, moderate, and severe groups (<italic>P</italic><0.05). The severity of Qi deficiency and blood stasis syndrome was positively correlated with NT-ProBNP (<italic>r</italic>=0.144), PT (<italic>r</italic>=0.173), and APTT (<italic>r</italic>=0.144), but negatively with 6MWT (<italic>r</italic>=-0.287). The 6MWT[odds ratio(OR)=0.995, 95% confidence interval(CI) 0.991-0.998),<italic>P</italic><0.01] and APTT(OR=1.088,95%CI 1.021-1.157,<italic>P</italic><0.01) were independent risk factors affecting the severity of heart failure related to coronary heart disease arising from Qi deficiency and blood stasis. Conclusion:The severity of heart failure related to coronary heart disease of Qi deficiency and blood stasis syndrome is closely related to NT-ProBNP, 6MWT, H-FABP, PT, APTT, TNF-<italic>α</italic>, and NO. Moreover, 6MWT and APTT can be used as independent risk factors to evaluate the severity of patients with heart failure related to coronary heart disease due to Qi deficiency and blood stasis.

ABSTRACT

Objective:To observe the clinical effect of compound Guizhencao granule (CGG) on grade 1 hypertension patients with dampness heat and blood stasis syndrome and its influence on related biological indicators and safety indexes. Method:A randomized,double-blind,placebo-controlled clinical trial design was used. 80 subjects who met the inclusion criteria were randomly divided into the treatment group (40 cases) and control group (40 cases). On the basis of health education,patients in the treatment group received 6.5 g CGG,twice daily,for four weeks. Patients in the control group received CGG simulant in a similar dosing scheme. The 24-hour ambulatory blood pressure monitoring(24 h ABPM),traditional Chinese medcine(TCM) syndrome score,angiotensin Ⅱ (AngⅡ),endothelin-1 (ET-1),homocysteine(Hcy) and safety indexes were observed. Result:Compared with that before treatment, systolic blood pressure (SBP) and diastolic blood pressure (DBP) of the consulting room in the treatment group were significantly lower (<italic>P</italic><0.01), and there was no significant difference in the control group;The daytime DBP and 24 h DBP in the control group decreased significantly (<italic>P</italic><0.01), and the 24 h SBP, 24 h DBP, daytime SBP, daytime DBP, nighttime SBP and nighttime DBP in the treatment group decreased significantly (<italic>P</italic><0.01). The total effective rate of 24 h ABPM on the nighttime blood pressure was 57.14% (20/35) in the treatment group, which was significantly higher than 28.57% (10/35) in the control group (<italic>Z</italic>=-2.310, <italic>P</italic><0.05); The total effective rate of daytime blood pressure and 24-hour blood pressure treatment group increased, but the difference was not statistically significant. The TCM syndrome score of two groups was significantly lower than that of the control group before treatment (<italic>P</italic>< 0.05, <italic>P</italic>< 0.01), and that of the treatment group was significantly lower than that of the control group after treatment (<italic>P</italic><0.01). The total effective rate of TCM syndrome score in the treatment group was 51.43% (18/35), which was significantly higher than 28.57% (10/35) in the control group (<italic>χ</italic><sup>2</sup>= 9.973, <italic>P</italic><0.05). Compared with that before treatment, the levels of ET-1 and Hcy in the control group decreased significantly (<italic>P</italic><0.05), and the levels of Ang Ⅱ, ET-1 and Hcy in the treatment group decreased significantly (<italic>P</italic><0.01); Compared with the control group after treatment, the levels of Ang Ⅱ and ET-1 in the treatment group decreased significantly (<italic>P</italic> < 0.01). Conclusion:CGG is safe and effective in reducing the blood pressure level,improving the TCM syndrome score,and regulating related biological indicators of patients with Grade 1 hypertension.

@# Objective    To systematically evaluate the expression of programmed cell death receptor 1 (PD-1) and programmed cell death ligand 1 (PD-L1) in esophageal squamous cell carcinoma and its relationship with prognosis. Methods    The literature from PubMed, EMbase, The Cochrane Library, Web of Science, CNKI and Wanfang data from inception to February 22, 2020 was searched by computer. Data were extracted and the quality of literature was evaluated using RevMan 5.3 software for meta-analysis. Egger's and Begg's tests were used to evaluate publication bias, and Stata 15.1 software was used for sensitivity analysis. Results     A total of 16 articles were included, and there were 3 378 patients with esophageal squamous cell carcinoma. The methodological index for nonrandomized studies (MINORS) scores were all 12 points and above. The meta-analysis results showed that the positive expression rates of PD-1 and PD-L1 in tumor cells were 37.8% (190/504) and 41.7% (1 407/3 378), respectively. The positive expression of PD-L1 in tumor immune infiltrating cells was 41.7% (412/987). The overall survival (OS) of the tumor cell with high PD-L1 expression was lower than that with low PD-LI expression (HR=1.30, 95%CI 1.01-1.69, P=0.04). The OS of the tumor immune infiltrating cell with high PD-L1 expression was significantly higher than that with low PD-LI expression (HR=0.65, 95%CI 0.53-0.80, P<0.000 1). Conclusion    PD-L1 has a high expression rate in esophageal squamous cell carcinoma and is an important factor for the prognosis of esophageal squamous cell carcinoma.

OBJECTIVE@#To establish a risk assessment and prediction system for early osteonecrosis of the femoral head (ONFH) in order to predict the collapse risk.@*METHODS@#The risk assessment system for early necrosis and collapse of femoral head was established based on the combination of Steinberg stage, ABC typing and the proportion of the proximal sclerotic rim. Firstly, Steinberg stage system was applied. ABC typing was applied to predict risk in stage I, type C was risk free, type B was low risk, type A and type BC were medium risk, type A-C and type AB were high risk. The classification of proximal sclerotic rim was first applied when the Steinberg stage was Ⅱ-Ⅲ, and type 2 was expected to be low risk. If the classification of proximal sclerotic rimwas type 1, then the ABC typing was applied, type C was risk-free, type B was low risk, type A and type BC were medium risk, type A-C and type AB were high risk. According to this prediction system, the collapse risk of femoral head in 188 cases(301 hips) were predicted by retrospective analysis. All the hips were enrolled at the out-patient department of orthopedic in Guang'anmen Hospital attached to China Academy of Chinese Medical Science. The consistency of the prediction results of three doctors and one doctor at different times were evaluated.@*RESULTS@#Among them, 136 cases were male, 52 were female. 75 cases were single hip, 113 were double hip. The age of the patients wa 19 to 64(42.61±12.07) years. The natural course of disease was 0.33 to 5.00(3.62±1.93) years. 206 hips in 301 hips had collapsed, with a collapse rate of 68.44%. In the risk-free group, none hip had collapsed, with a collapse rate of 0%. In the low-risk group, 9 hip in 91 hips had collapsed, with a collapse rate of 9.89%. In the medium-risk group, 12 hip in 19 hips had collapsed, with a collapse rate of 63.16%. And in the high risk group, 185 hips in 190 hips had collapsed, with a collapse rate of 97.37%. They were significantly differences in their collapse rate (@*CONCLUSION@#The risk assessment and prediction system for early ONFH selects different methods to predict the risk of collapse according to the imaging characteristics of different stages, which is combines with the comprehensive assessment of multiple risk factors. The system is applicable to a wide range, simple operation and convenient for clinical application.

ABSTRACT

In order to clarify the influence of acute hypobaric hypoxia on the bile acids of the rat small intestine, we used ultra-high performance liquid chromatography-tandem mass spectrometry (UHPLC-MS/MS) to identify bile acids in the contents of the small intestine from untreated and acute hypobaric hypoxia-treated rats. Thirty-nine bile acids were detected; PCA and OPLS-DA analysis revealed marked differences in the composition of bile acids between the untreated and the acute hypobaric hypoxia groups. Bile acids were screened with VIP > 1, |log2FC| ≥ 1, P < 0.05, and a total of 7 bile acids with significant differences in content between the two groups were obtained, including 5 conjugated bile acids, 2 unconjugated bile acids; in addition, the content of conjugated bile acids has risen in the treated group. This study demonstrated the influence of high-altitude hypoxic environment on bile acid composition and metabolism in rats. All the animal experiments in this study were approved by the 940th Hospital Ethics Committee (approval No: 2020KYLL012).

Objective:To observe the changes of vision and visual evoked potentials (VEP) in patients with Leber hereditary optic neuropathy (LHON) before and after gene therapy.Methods:A retrospective cohort study. From December 2017 to October 2018, 35 cases of 70 eyes of m11778G.A/MT-ND4 mutant LHON patients who were diagnosed in the Tongji Hospital of Huazhong University of Science and Technology and received gene therapy were included in the study. There were 30 males (87.71%) and 5 females (12.29%), with the mean age of 23.31±6.72 years. The gene therapy method was intravitreal injection of rAAV2-ND4 (recombinant adeno-associated virus carrying NADH-ubiquinone oxidoreductase subunit 4 gene) into one eye. The eye with poor visual acuity was chosen as the injection eye. If both eyes had the same visual acuity, the right eye was designated as the injection eye. Seventy eyes were divided into the injected eye group and the non-injected eye group, in which were both 35 eyes. The best corrected visual acuity (BCVA) and pattern VEP (PVEP) examinations were performed in the injected eye group and the non-injected eye group before treatment (baseline), 1, 3, and 6 months after injection. Compare the changes of BCVA and PVEP between the injected eye group and the non-injected eye group at baseline, 1 month, 3 months, and 6 months after injection. Independent sample t test, paired sample t test or two independent sample nonparametric test were performed to compare the two groups. Results:Compared with baseline, 1, 3, and 6 months after treatment, the BCVA of the injected eye group ( t=3.530, 4.962, 5.281; P=0.001, 0.000, 0.000) and the non-injected eye group ( t=3.288, 2.620, 2.252; P=0.002, 0.013, 0.031) increased, and the difference was statistically significant; there was no statistically significant difference between VEP IT ( tinjected eye group=-0.158, 1.046, -1.134; Pinjected eye group = 0.875, 0.303, 0.190; tnon-injected eye group=0.773, -0.607, -0.944; Pnon-injected eye group = 0.445, 0.548, 0.352) and VEP A ( Zinjected eye group=-0.504, -0.934, -1.065; Pinjected eye group = 0.614, 0.351, 0.287; Znon-injected eye group=-0.521, -0.115, -0.491; Pnon-injected eye group = 0.602, 0.909, 0.623). Conclusion:After gene therapy, the visual acuity of the injected and non-injected eyes of LHON patients improved; PVEP did not change significantly, and remained stable compared with baseline.

Congenital bile duct cysts, which is a kind of malformation of bile duct, will lead to the inflammation in the bile duct system for a long time. Therefore, patients with cholelithiasis and cyst canceration have a risk. Surgical operation is the only treatment option for the congenital bile duct cysts patients. Resection of the cysts and complete " biliary-pancreatic duct shunt" are the key points of the standardized treatment of congenital bile duct cysts. The non-standard surgical treatment will bring the postoperative complications such as the stricture of choledochojejunostomy, the remnant cysts and even the canceration of cysts, which will seriously affect the quality of life and threaten the health of patients. Based on the retrospective study of congenital bile duct cysts patients in Eastern Hepatobiliary Hospital, the author proposed a new classification system according to the pathological and anatomical characteristics of congenital bile duct cysts. The congenital bile duct cysts can be divided into three regions and five types, i. e. localized type and diffuse type of extrahepatic bile duct cysts of hilar, trunk and terminal type; central type; and intrahepatic bile duct cysts of limited and diffuse type. It is our hope that this typing system will accurately guide the design and implementation of surgical treatment plans for congenital bile duct cysts and reduce the risk of long-term postoperative complications for patients.

22q11.2 deletion syndrome (22q11.2DS) is the second leading chromosomal cause of congenital heart disease (CHD) after Down syndrome.In the past few decades, great progress has been made in describing the anatomical types, potential heredity, and pathogenesis of CHD complicated with 22q11.2DS in terms of improving the diagnosis and treatment of these diseases.However, cardiac malformation is still the leading cause of death in 22q11.2 DS.Further studies are needed to elucidate the genetic mechanism behind high phenotypic variability of CHD and to explore the multidisciplinary treatment for the improvement of the prognosis.22q11.2 gene detection in patients suffering from CHD may provide useful insights into early diagnosis and multidisciplinary treatment.

ABSTRACT

Objective:To investigate the clinical characteristics, treatment effect and prognosis of children with nearly diploid neuroblastoma (NB).Methods:A retrospective analysis of the general clinical characteristics (including age, Gender, risk grouping, location of primary tumor, etc.), laboratory test results, treatment and recent prognosis of NB children with nearly diploidy in bone marrow chromosomes by G-banding technology who admitted to Beijing Children′s Hospital, Capital Medical University from January 2015 to December 2018. Kaplan- Meier method was adopted to calculate survival rate.Univariate analysis was performed using Log- Rank test, and multivariate analysis was conducted with Cox regression model. Results:A total of 43 patients, including 27 males and 16 females, with diagnosis were included, with 14 cases in the hypodiploid group and 29 cases in the hyperdiploid group, and the median age was 35.5 months.The 43 children were all in the high-risk group of International Neuroblastoma Staging System(INSS)-Ⅳ.The primary tumors were mainly located in the retroperitoneal adrenal region (83.7%, 36/43 cases). The largest diameter of the tumors was more than 10 cm (53.5%, 23/43 cases), and often accompanied by 2 or more metastases at the time of consultation.In terms of chromosome karyotype and chromosome karyotype of 14 children in the hypodiploid group was 41-45, the most common karyotype was 45 chromosomes[9 cases(64.3%)]. Among 29 children in the hyperdiploid group of the 47 chromosome karyotypes, 11 cases were common (37.9%). Tumor markers were as follows: neuron enolase (NSE) increased in 41 cases children (95.3%) at first diagnosis, and 25 cases (58.1%)> 370 μg/L; 42 cases (97.7%)had lactate dehydrogenase (LDH). The LDH of children in the hypodiploid group was all> 500 U/L, with 1 case was> 10 000 U/L.Nine cases (20.9%) of MYCN gene were detected by fluorescence in situ hybridization (FISH). Treatment and prognosis: the total course of chemotherapy for 43 patients was 1-12, 19(44.2%) patients received autologous stem cell transplantation, 21 patients (46.5%) received postoperative or autologous radiotherapy or metaiodobenzylguanidine treatment, 28 children developed or relapsed with a median duration of 13.8 months, and 15 cases (34.9%) died.The median follow-up time of the 14 children in the hypodiploid group was 14.9 months (2-38 months), 12 cases progressed or relapsed, and 7 died.The median follow-up of 29 children in the hyperdiploid group was 20.0 months (8.1-51.6 months), with 16 patients progressed or relapsed and 8 cases died. Kaplan- Meier survival analysis illustrated that the 3-year projected event free survival (EFS) rate of 43 children was 18.4%, of which 17.1% were in the hypodiploid group and 29.8% in the hyperdiploid group. Conclusions:Preliminary analysis reveals that children with nearly diploid NB are mostly in the stage Ⅳ high-risk group over the age of 18 months, and 2 or more metastases at the time of consultation.The 3-year estimated EFS of 43 children was 18.4%, and the prognosis was worse in the hypodiploid group.
