ABSTRACT
The rol genes on pRiA4 plasmid of Agrobacterium rhizogenes are potent genes that promote secondary metabolism. Molecular breeding of Atropa belladonna can be conducted by introducing rol genes to increase tropane alkaloids (TAs) content in A. belladonna. In this study, the rolB gene was overexpressed in A. belladonna plants to study the effect of rolB gene on the biosynthesis of TAs. The phenotype, TAs content and expression levels of key enzyme genes in the pathway of TAs biosynthesis of transgenic A. belladonna were analyzed. The results showed that transgenic A. belladonna had developed root system, enlarged leaves, increased leaf fresh weight, deepened leaf color, enlarged flowers, changed flower shape, reduced pistil height and decreased pollen vitality. The content of TAs in the stems of transgenic A. belladonna was significantly higher than that of the control, and the contents of scopolamine, anisodamine, hyoscyamine can reach 2.11-2.91, 1.23-2.37 and 4.88-5.20 times of the control, respectively. Compared with the control group, the expressions of key enzymes putrescine N-methyltransferase (PMT), type III polyketide synthase (PYKS), tropinone reductase I (TRI), aromatic amino acid aminotransferase 4 (ArAT4), UDP-glycosyltransferase 1 (UGT1) and hyoscyamine 6-β-hydroxylase (H6H) in the TAs biosynthesis pathway were up-regulated, and the expression of tropinone reductase II (TRII) as a metabolic shunting gene was down-regulated. The results indicated that rolB gene enhanced TAs synthesis ability in roots and accumulation in stems of A. belladonna by enhancing metabolic flow of TAs synthesis pathway and weakening the metabolic shunt of competing pathway. This study laid a foundation for molecular breeding of A. belladonna with high-yield TAs content using rolB gene.

@#Abstract: Objective To investigate a poisoning incident caused by eating eight treasure congee, and establish liquid chromatography (LC)-mass spectrometry (MS)/MS screening method of 28 alkaloids to provide references for disposal of similar poisoning incidents. Methods LC-MS/MS was used for screening 28 alkaloids in the urine, eight treasure congee and food raw material, and the detected alkaloids were quantified. Samples were extracted with 0.4% formic acid aqueous solution and separated by a Acquity UPLC BEH C18 column (1.7 μm, 100 × 2.1 mm). Acetonitrile-0.2% formic acid aqueous solution was used as the mobile phase and gradient elution was adopted. The ionization mode was electrospray positive ionization mode, and the detection method was multi-reaction monitoring (MRM). Analytes were quantified with the external standard method. Results In the concentration range of 0-100 ng/mL, the linear correlation coefficient r were greater than 0.999 for 28 alkaloids. The recovery of 28 alkaloids in urine sample ranged from 63.0% to 105.0%, and the relative standard deviations (RSDs) were between 5.8% and 8.6%. The recovery of 28 alkaloids in eight treasure congee sample ranged from 72.0% to 109.0%, and the RSDs were between 6.3% and 9.7%. The recovery of 28 alkaloids in semen sesami nigrum sample ranged from 60.0% to 95.0%, and the RSDs were between 4.8% and 8.2%. Hyoscyamine (2 380.0 ng/mL), scopliamine (3.6 ng/mL) and rac-anisodamine (4.7 ng/mL) were detected in the patient's urine. Hyoscyamine (63.3 μg/g), scopliamine (5.7 μg/g) and rac-anisodamine (2.1 μg/g) were detected in eight treasure congee. Hyoscyamine (901.0 μg/g), scopliamine (80.0 μg/g) and rac-anisodamine (30.1 μg/g) were detected in the seed of Datura stramonium L. The ratio of scopliamine and hyoscyamine in the seed of D. stramonium was 1∶11, which complies with the characteristics of D. stramonium L. In urine sample, the proportion of scopliamine and rac-anisodamine was 0.15% and 0.20%, and hyoscyamine accounted for 99.65%. Conclusion Seed morphology, the content range and proportion of three alkaloids are all in accord with the characteristics of D. stramonium. Combined with the clinical symptoms of atropine poisoning, it can be deduced that this incident is a family food poisoning caused by accidental consumption of seed of D. stramonium L. The method can provide technical support for the clinical diagnosis and treatment of alkaloid poisoning patients, and also provide a basis for emergency detection and disposal of alkaloid poisoning events.

A 30-year-old female patient with idiopathic sudden hearing loss was treated with anisodamine hydrobromide injection.After 10 minutes of the intravenous injection of this drug,the patient suffered from hypoptyalism and taste disorder for two days.After the second medication,the patient's taste recovery time was significantly shortened by holding the ice water in mouth compared to the first time.Hypoptyalism and taste disorder were considered to be the adverse drug reaction caused by anisodamine hydrobromide injection.After withdrawal the suspected drug,the patient did not experience any further discomfort since.Although taste disorder is not life-threatening,it directly affect the patient's medication experience and quality of life,clinical awareness and attention of the adverse reactions should be increased.

Hair roots induced by Agrobacterium rhizogene produce higher levels of secondary metabolites than non-induced plants, and the enhanced metabolic capacity is driven by the rol gene. We hypothesized that rol genes can be utilized to improve the biosynthesis of tropane alkaloids (TAs) in Atropa belladonna. In this study, the rolC gene from Agrobacterium rhizogene pRiA4 plasmid, driven by a CaMV35S promoter, was overexpressed in A. belladonna. The phenotypes, TAs content and transcriptional expression of key genes in TAs biosynthesis were analyzed in transgenic A. belladonna plants. Results show that transgenic A. belladonna exhibited a well-developed root system, male sterility, higher stamen column length than pistil, early flowering, internode shortening, smaller but more flowers, increased axillary buds and lateral buds, decreased apical dominance, and long and narrow leaves as compared to wild-type plants. Transgenic A. belladonna produced more TAs than wild-type plants, with the content of hyoscyamine, anisodamine and scopolamine reaching 2.58, 3.59 and 15.77-fold that of the control group, respectively. The gene expression of putrescine N-methyltransferase (PMT), tropinone reductase I (TRⅠ) and hyoscyamine 6-β-hydroxylase (H6H), key enzymes in TAs biosynthesis, were up-regulated compared with the control group. The above results indicate that the rolC gene enhances TAs biosynthesis in A. belladonna by up-regulating the expression of key enzymes in the TAs biosynthesis pathway, laying a foundation for genetic manipulation of A. belladonna to increase TAs content by increasing rolC gene expression.

Objective@#This study aimed to examine the effects of microcirculatory dysfunction and 654-1 intervention after cardiopulmonary resuscitation on myocardial injury.@*Methods@#Landrace pigs were divided into a sham operation group (S group, n= 6), ventricular fibrillation control group (VF-C group, n= 8) and 654-1 intervention group (VF-I group, n= 8). Hemodynamics was recorded at baseline, at recovery of spontaneous circulation (ROSC), and 1 h, 2 h, 4 h and 6 h thereafter. Sidestream dark field (SDF) technology was used to evaluate and monitor the microcirculation flow index, total vessel density, perfusion vessel ratio, De-Backer score, and perfusion vessel density in animal viscera at various time points.@*Results@#After administration of 654-1 at 1.5 h post-ROSC, the hemodynamics in the VF-I group, as compared with the VF-C group, was significantly improved. The visceral microcirculation detected by SDF was also significantly improved in the VF-I group. As observed through electron microscopy, significantly less myocardial tissue injury was present in the VF-I group than the VF-C group.@*Conclusion@#Administration of 654-1 inhibited excessive inflammatory by improving the state of visceral microcirculation.

Objective:To investigate whether anisodamine can regulate the ratio of helper T helper cells/regulatory T cells (Th17/Treg) and its protective effect on animals after resuscitation.Methods:Twenty-four Beijing white minipigs were randomly divided into sham operation group (Sham group), resuscitation and normal saline group (SA group), and resuscitation and anisodamine hydrobromide group (AH group), with 8 pigs in each group. In SA group and AH group, ventricular fibrillation was induced by continuous stimulation with intraventricular electrodes for 8 minutes and then resuscitated to establish ischemia/reperfusion (I/R) model. In SA group, after cardiopulmonary resuscitation (CPR), only normal saline was intravenously infused, while in AH group, normal saline and anisodamine hydrobromide were given intravenously at the same time point. Hemodynamic indexes, arterial blood gas analysis indexes, interleukins (IL-17, IL-10) levels in venous blood and IL-17/IL-10 ratio were recorded at 6 different time points: baseline, immediately after return of spontaneous circulation (ROSC), 1 hour, 2 hours, 4 hours and 6 hours after ROSC. The animals were sacrificed at 6 hours after ROSC, and intestinal lymphatic tissues were taken to observe pathological changes under light microscope. At the same time, the levels of IL-17 and IL-10 in intestinal lymphatic tissue were measured (the ratio of IL-17/IL-10 represents the ratio of Th17/Treg cytokines) to evaluate the immune status of the resuscitated animals. The bacterial translocations of different groups were evaluated by culturing intestinal lymphoid tissue.Results:With the extension of ROSC time, the levels of IL-17 in venous blood and the IL-17/IL-10 ratio in pig blood samples continued to decrease, while the levels of IL-10 continued to increase. From 2 hours after ROSC, the IL-17/IL-10 ratio in AH group was significantly higher than that in SA group continued until at 6 hours after ROSC (0.79±0.05 vs. 0.49±0.08, P < 0.05). Light microscopy showed that the number and size of lymph nodules in the cortex of intestinal lymphatic tissue were less in AH group, compared with SA group. Compared with Sham group, the levels of IL-17 and IL-17/IL-10 ratio also decreased in intestinal lymphatic tissue at 6 hours after ROSC [IL-17 (ng/L): 155.23±0.92, 178.76±7.25 vs. 209.21±19.82, IL-17/IL-10 ratio: 1.43±0.13, 1.92±0.18 vs. 3.30±0.31, all P < 0.05], and IL-10 increased significantly (ng/L: 109.85±11.60, 93.55±81.83 vs. 63.45±0.62, all P < 0.05); IL-17/IL-10 ratio in AH group was significantly higher than that in SA group (1.92±0.18 vs. 1.43±0.13, P < 0.05). Tissue culture indicated the intestinal bacterial translocation after resuscitation, suggesting that the animals had immunosuppression and the increased risk of intestinal secondary infection after resuscitation. Compared with SA group, the risk of bacterial translocation was lower than that in AH group [62.5% (5/8) vs. 87.5% (7/8), P < 0.05]. Conclusions:Anisodamine plays an immunomodulatory role by affecting the balance of Th17/Treg cytokines in resuscitated animals, so as to reduce the risk of intestinal secondary infection and has an organ protective effect.

Abstract Intestinal ischemia/reperfusion (I/R) causes barrier impairment and bacterial influx. This study explored the protective effects of anisodamine hydrobromide (AH) on intestinal I/R injury caused by cardiopulmonary resuscitation (CPR) after cardiac arrest (CA). After successful CPR, minipigs were randomly divided into two groups (n = 8): saline and AH (4 mg/kg), and then treated with saline or AH via central venous injection, respectively. The same procedures without ventricular fibrillation initiation were conducted in the Sham group (n = 8). Levels of interferon gamma (IFN-γ) and interleukin 4 (IL-4) were measured at different time points (0, 0.5, 1, 2, 4, and 6 h) in serum and 6 h in gut associated lymphoid tissues (GALTs) after the return of spontaneous circulation (ROSC) to evaluate changes in the proportion of T-helper type 1 (Th1) and T-helper type 2 (Th2). Moreover, the positive culture rates of GALTs were examined to evaluate bacterial translocation. AH treatment markedly alleviated aberrant arterial blood gas and hemodynamics as well as intestinal macroscopic and morphological changes after CPR. Moreover, AH treatment significantly increased IFN-γ and decreased IL-4 in both serum and GALTs. Furthermore, AH treatment dramatically decreased positive bacterial growth in GALTs. AH treatment mitigated immunosuppression caused by intestinal I/R and protected the intestinal immune barrier against bacterial translocation, thereby reducing the risk of secondary intestinal infection

Objective To investigate the effect of anisodamine combined with blood purification on pulmonary fibrosis and levels of serum matrix metalloproteinase-9 (MMP-9) and matrix metalloproteinase 1 (TIMP1) in patients with paraquat poisoning.Methods From March 2013 to February 2017,84 patients with paraquat poisoning admitted to our hospital were enrolled in the observation group (anisodamine +blood purification) or control group (blood purification),42 cases in each group.The curative effect,indexes related to pulmonary fibrosis,serum MMP-9 and TIMP1 levels were compared between the two groups.The incidence of acute lung injury/acute respiratory distress syndrome (ALI/ARDS) and intensive care uni (ICU) admission time were recorded.Results There was significant difference (P < 0.05) in the total effective rate (88.1% vs 69.0%) and the high-resolution computed tomography (CT) score after 7 days of treatment (8.4 ± 0.9 vs 12.3 ± 1.4) between the observation group and the control group.At the same time,the serum levels of hyaluronic acid and type Ⅳ collagen in the two groups showed an upward trend with significant difference (P < 0.05).After 1 day of treatment,the serum levels of MMP-9 and TIMP1 in the observation group reached a peak,then gradually decreased,with significant difference (P <0.05).The incidence of ALI/ARDS in the observation group and the control group (11.9% vs 31.0%)and the duration of ICU treatment [(10.2 ± 3.2) d vs (13.2 ± 2.8) d].Conclusions For patients with acute paraquat poisoning,the combination of anisodamine and blood purification can down-regulate the expressions of MMP-9 and TIMP1 and delay the progression of pulmonary fibrosis,thus improving the clinical therapeutic effect.

Objective@#To assess the clinical effect of anisodamine combined with gabapentin on painful diabetic neuropathy(PDN).@*Methods@#From June 2013 to February 2017, 120 patients with PDN in the Fifth People's Hospital of Datong were assigned into anisodamine group, gabapentin group and combined group according to the digital table, with 40 cases in each group, the patients were treated with anisodamine, gabapentin and combination therapy respectively.The abnormal foot plantar pressure, MCV and SCV of median nerve and common peroneus nerve, VAS and total effective rate of three groups were compared.@*Results@#After treatment, the indicators of the combined group were improved more significantly than those of the anisodamine group and gabapentin group in abnormal foot plantar pressure[(6.10±1.66) vs.(10.80±2.64) vs.(6.37±1.44), F=14.602, P<0.01], total effective rate (92.5% vs.62.2% vs.75.0%, χ2=10.155, P=0.006), MCV[(50.33±5.54)m/s vs.(41.12±4.47)m/s vs.(42.32±4.40)m/s, F=9.404, P=0.001], and SCV of median nerve[(48.51±6.19)m/s vs.(41.81±5.72)m/s vs.(41.76±7.17)m/s, F=3.728, P=0.037], MCV of common peroneus nerve[(40.60±5.69)m/s vs.(32.04±4.47)m/s vs.(33.52±7.76)m/s, F=5.614, P=0.009]and SCV of common peroneus nerve[(42.72±4.97)m/s vs.(36.21±6.16)m/s vs.(35.45±5.54)m/s, F=4.265, P=0.025]. After treatment, the VAS scores of the three groups decreased, which were (4.49±1.61)points, (5.82±1.58)points, (6.37±1.44)points, respectively, the difference was statistically significant(F=14.602, P=0.000), and the decrease was more significant in the combined group.@*Conclusion@#Compared with the anisodamine group and gabapentin group, the combined treatment is more effective on PDN.

ABSTRACT
Objective: To establish a fingerprint of Hyoscyamus Extract Tablets (HET) by high performance liquid chromatography and determine the content of atropine, anisodamine, scopolamine, and anisodine. Methods: The separation was performed on a GL Sciences Inc C18 (250 mm × 4.6 mm, 5 μm) column with gradient elution of methanol and 0.05% phosphoric acid solution. The detection wavelength was set at 218 nm, the flow rate was 1.0 mL/min, and the column temperature was 30℃. Similarity evaluation system for chromatographic fingerprint of TCM was used for establishing the reference fingerprint and similarity evaluation, and clustering analysis was carried out by SPSS 19.0. Results: There were 14 marked common peaks, and the similarity of S1-S4, S11 and the control fingerprint was over 0.98, others were above 0.995. It was shown that these 12 batches of samples were of good similarity. Cluster analysis results showed that the S1-S6 were grouped into one class, and the remaining samples were grouped into one class. The results of assay showed that alkaloids in different batches of HET varied a lot in contents but kept consistent in compositions. Conclusion: The method could well display the chemical information of HET. It has simple operation and good repeatability, which provides more comprehensive and scientific data for quality evaluation of HET.

Objective To investigate the effect of anisodamine on air repair of intussusception in children,to improve the success rate of air enema reduction in children with acute intussusception.Methods Based on the clinical symptoms and ultrasound diagnosis,582 children who were diagnosed with intussusception and needed air enema reduction were divided into two groups according to the modified method of operation.Children in the control group were not injected 654-2 before the air enema;the experimental group received intramuscular injection of 654-2 before air enema.The success rate and total success rate of the first reduction were observed and compared between the two groups.Results In the control group,229 cases was successfully repaired,and the success rate was 83.9%;17 cases were successfully reset by two times after intramuscular injection of 654-2;the overall success rate was 90.1%.In the experimental group, 295 cases were successfully repaired for the first time,and the success rate was 95.5%;3 cases were successfully reset by two times;the overall success rate was 96.4%.The successful rate and the total success rate of the experimental group were higher than those of the control group.χ2=22.71,P<0.001,was statistically significant.Conclusion Air enema before intramuscular injection of 654-2 is helpful to improve the success rate of air enema reduction in children with acute intussusception.

Methods 200 cases with normal delivery of anisodamine combined with epidural analgesia in our hospital from March 2014 to June 2016 were randomly selected as the experimental group, and 200 cases without users were selected as the control group. The time of first stage of labor and the amount of postpartum hemorrhage were observed. Results There was significant difference in the first time of labor between the two groups (P<0.01), but there was no significant difference of postpartum hemorrhage volume between the two groups. Conclusion Anisodamine hydrochloride combined with epidural analgesia can obviously shorten the first stage of labor, without any side effects such as postpartum hemorrhage.

Objective To study the effect of the combination of pathidine hydrochloride and anisodamine on the intraoperative effiacy of transhepatic biliary drainage and stent implantation done on the patient with advanced malignant obstructive jaundice.Methods 100 cases of percutaneous transhepatic catheterizde drainage (PTCD) and percutaneous transhepatic biliary stenting (PTBS) done for advanced malignant obstructive jaundice were divided into control group and experimental group,50 cases in each group.In experimental group,100 mg pethidine hydrochloride and 10 mg anisodamine were injected intramuscularly 30 min before operation,while in control group,no analgesic or antispasmodic were used before and during operation.The blood pressure and heart rate were observed,the incidence rate of biliary-cardiac reflex and operating time were recorded,while the scale of pain felt by patient was evaluated with visual analogue scale.Results Compared to the control group,the patients in experimental group have more stable vital signs,feel more comfortable,suffering shorter operative time,less incidence rate of biliary cardiac reflex and less pain by using pethidine hydrochloride and anisodamine.Conclusion Application of pathidine hydrochloride and anisodamine during the transhepatic biliary drainage and stent implantation done on the patient with advanced malignant obstructive jaundice may release pain,reduce operating time.It is safe,at lower cost and can be used as routine medicine before PTCD or PTBS.

@#AIM:To investigate the effect and local influence of atropine and anisodamine eye drops on adolescent pseudomyopia. <p>METHODS:Totally 110 cases of juvenile pseudomyopia were randomly divided into two groups, the control group was given 10g/L atropine sulfate eye gel, and the observation group was treated with 5g/L raceanisodamine eye drops. The efficacy of two methods, the changes of axial length and intraocular pressure before and after treatment, and the incidence of adverse reactions were compared. <p>RESULTS: There was no significant difference in cure rate between the two groups(<i>χ</i><sup>2</sup>=0.533, <i>P</i>=0.465), but the effective rate of observation group was significantly better than the control group(<i>χ</i><sup>2</sup>=3.907, <i>P</i>=0.048). Compared with the same group before treatment, the length of the axial length of the two groups increased in different degrees,and the increase value of the observation group was significantly higher than that of the control group, the difference was statistically significant(<i>P</i><0.05), however, there was no significant difference between the two groups after treatment(<i>P</i>>0.05). The intraocular pressure of the two groups was significantly lower than that of the same group before treatment, and the difference between the two groups after treatments was not statistically significant(<i>P</i> >0.05). The incidence of adverse reactions in the observation group was significantly lower than that in the control group(<i>χ</i><sup>2</sup>=18.939, <i>P</i><0.05). <p>CONCLUSION: Anisodamine eye drops in the treatment of juvenile pseudomyopia has obvious curative effect, its efficacy and safety are better than atropine eye gel.
ABSTRACT
OBJECTIVE:To prepare the Compound anisodamine and neostigmine sustained-release tablet and study the in vitro drug release behavior. METHODS:Using raceanisodamine and neostigmine methyl sulfate as main medicines,hydroxypropyl meth-yl cellulose as sustained release skeleton material,magnesium stearate as the lubricant,polyvinyl pyrrolidone as the adhesive,pre-gelatinized starch as the thinner,microcrystalline cellulose as the disintegrant and filler,Compound anisodamine and neostigmine sustained-release tablet was prepared by wet granulation method and direct compression method. The in vitro cumulative release rate within 12 h of the 2 main medicines was detected by HPLC method. RESULTS:Compound anisodamine and neostigmine sus-tained-release tablet was successfully prepared,and the in vitro release was basically completed within 12 h,with accumulative re-lease rate of 91.3% for anisodamine and 96.5% for neostigmine. CONCLUSIONS:Compound anisodamine and neostigmine sus-tained-release tablet that can cumulatively release for 12 h is successfully prepared.
ABSTRACT
Objective To explore the interventional effect of two different methods in the prevention of chemotherapy phlebitis caused by 5-fluorouracil.Methods Eighty cases of patients treated with cisplatin+5-fluorouracil (PF) chemotherapy in the First Affiliated Hospital of Guangxi Medical University from May 2015 to July 2016 were selected and divided into the observation group and control group according to admission time,40 cases in each group.The observation group was smeared with 2% anisodamine along the direction of vein puncture after peripheral venous puncture success,while the control group was smeared with mucopolysaccharide polysulphate emulsion paste.The occurrence of phlebitis and degree of local pain after intervention were compared between the two groups.The blood biochemical indexes and inflammatory factors levels before and after intervention were compared too.Results After the intervention,the distribution of phlebitis level and local pain degree had statistical difference between the two groups (Z=2.57,2.53;P=0.01);the observation group was superior to the control group in the aspects of phlebitis lesion degree and local pain alleviation;WBC,MPV and PDW in the observation group had statistical difference between before and after intervention;WBC,PLT and PCT in the control group had statistical difference between before and after intervention (P0.05).Conclusion 2% anisodamine can effectively prevent and alleviate the occurrence of 5-FU caused chemotherapy phlebitis and local pain.At the same time,PLT can provide a basis for preventing chemotherapy phlebitis.
ABSTRACT
OBJECTIVE:To investigate the effects of anisodamine combined with oxytocin on labor process of lying-in women with prolonged active phase in the first labor process and related indicators of newborns. METHODS:216 lying-in women with pro-longed active phase in the first labor process were randomly divided into control group(108 cases)and observation group(108 cas-es). All lying-in women were artificially ruptured,based on it,control group was given 2.5 u Oxytocin for injection,adding into 500 ml 5% Glucose injection by intravenous infusion,the drip rate was appropriately adjusted based on contractions;observation group was additionally given 10 mg raceanisodamine hydrochloride,adding into 10 ml 5% Glucose injection by intravenous infu-sion by 5 min slow intravenous injection. The fetal heart was warded to closely observe the labor process during medication. The cervical maturity,labor process time,delivery mode and postpartum situation of lying-in women,Apgar score,body mass and inci-dence of adverse reactions of newborn in 2 groups were observed. RESULTS:The lying-in women cases with no less than 9 cervi-cal maturity,natural delivery rate and 5 min newborn Apgar score in observation group were significantly higher than control group, lying-in women cases with 7-9 scores and less than 7 scores,cesarean section rate,perineal/ straight cut rate and 24 h postpartum hemorrhage were significantly lower than control group,the second,the third and total labor process were significantly shorter than control group,the differences were statistically significant(P0.05). CONCLUSIONS:Anisodamine combined with oxytocin can accelerate the cervical dilation of lying-in women with pro-longed active phase,shorten labor process,reduce cesarean section rate and improve prognosis,it did not affect the body mass of newborns,with good safety.
