ABSTRACT

ABSTRACT The desperate attempt to improve mortality, morbidity, quality of life and patient-reported outcomes in patients on hemodialysis has led to multiple attempts to improve the different modes, frequencies, and durations of dialysis sessions in the last few decades. Nothing has been more appealing than the combination of diffusion and convection in the form of hemodiafiltration. Despite the concrete evidence of better clearance of middle weight molecules and better hemodynamic stability, tangible evidence to support the universal adoption is still at a distance. Survival benefits seen in selected groups who are likely to tolerate hemodiafiltration with better vascular access and with lower comorbid burden, need to be extended to real life dialysis patients who are older than the population studied and have significantly higher comorbid burden. Technical demands of initiation hemodiafiltration, the associated costs, and the incremental benefits targeted, along with patient-reported outcomes, need to be explored further before recommending hemodiafiltration as the mode of choice.


RESUMO A tentativa desesperada de melhorar a mortalidade, morbidade, qualidade de vida e desfechos relatados pelos pacientes em indivíduos em hemodiálise levou a diversas tentativas de aprimorar os diferentes modos, frequências e durações das sessões de diálise nas últimas décadas. Nada foi mais atrativo do que a combinação de difusão e convecção na forma de hemodiafiltração. Apesar das evidências concretas de melhor depuração de moléculas de peso médio e melhor estabilidade hemodinâmica, evidências tangíveis para apoiar a adoção universal ainda estão distantes. Os benefícios de sobrevida observados em grupos selecionados que provavelmente toleram a hemodiafiltração com melhor acesso vascular e com menor carga de comorbidades precisam ser estendidos aos pacientes reais em diálise, que são mais velhos do que a população estudada e apresentam uma carga de comorbidades significativamente maior. As exigências técnicas do início da hemodiafiltração, os custos associados e os benefícios incrementais almejados, juntamente com os desfechos relatados pelos pacientes, precisam ser melhor explorados antes de se recomendar a hemodiafiltração como o modo de escolha.

ABSTRACT

Objective To investigate the hepatic hemodynamic characteristics of cirrhotic patients with splenectomy using iodine map of dual-source computed tomography(DSCT).Methods Twenty-four cirrhotic patients with splenectomy were selected as a study group,41 cirrhotic patients without splenectomy as a cirrhosis group and other 32 patients with normal liver as a control group.The iodine concentration(IC)in hepatic arterial and venous phases was measured on the iodine map,and the arterial iodine fraction(AIF)and portal venous iodine concentration(PVIC)were calculated.Receiver operating characteristic(ROC)curves were plotted and the area under the curve(AUC)was recorded to evaluate the diagnostic efficacy of each parameter using the DeLong test.Results IC in arterial phase and AIF were significantly higher,and IC in venous phase and PVIC were significantly lower in study group(P<0.05).The AUC values of the four parameters between study group and cirrhosis group were 0.735,0.992,0.943,and 0.994,respectively.Conclusion DSCT iodine map is helpful for clinical quantitative assessment of hepatic hemodynamic characteristics in cirrhotic patients with splenectomy,and the PVIC has optimal independent diagnostic performance.

Objetivo: identificar a qualidade e confiabilidade das informações sobre a pressão arterial invasiva abordadas em vídeos do YouTube. Método: pesquisa descritiva e exploratória com vídeos do YouTube sobre pressão arterial invasiva, identificados em busca única no dia 23 de fevereiro de 2023. Os vídeos foram organizados em uma lista de reprodução e submetidos à análise utilizando a ferramenta Discern Questionnaire para a qualidade educacional do vídeo e a Journal of American Medical Association Benchmarks para a confiabilidade e qualidade da informação e realizada análise estatística descritiva simples. Resultados: elencou-se 62 vídeos publicados há dois anos, dos quais 93,55% apresentaram qualidade educacional dos vídeos ruim, 6,45% moderada, demonstrando 70,97% baixa confiabilidade das informações e 32,25% citaram informações incompatíveis com a ciência. Conclusão: os vídeos encontrados na plataforma do YouTube apresentaram lacunas relacionados ao conteúdo de pressão arterial invasiva demonstrando não ser uma fonte confiável de respaldo para aprendizado do enfermeiro.


Objective: to identify the quality and reliability of information on invasive blood pressure covered in YouTube videos. Method: descriptive and exploratory research with YouTube videos on invasive blood pressure, identified in a single search on February 23, 2023. The videos were organized into a playlist and subjected to analysis using the Discern Questionnaire tool for the educational quality of the video and the Journal of American Medical Association Benchmarks for reliability and quality of information and performed simple descriptive statistical analysis. Results: 62 videos published two years ago were listed, of which 93.55% presented poor educational quality, 6.45% moderate, 70.97% demonstrated low reliability of information and 32.25% cited information incompatible with science. Conclusion: The videos found on the You Tube platform presented gaps related to the content of invasive blood pressure, demonstrating that it is not a reliable source of support for nurses' learning.

Resultados: Se enumeraron 62 videos publicados hace dos años, de los cuales el 93,55% presentó mala calidad educativa, el 6,45% moderada, el 70,97% demostró baja confiabilidad de la información y el 32,25% citó información incompatible con la ciencia. Conclusión: Los videos encontrados en la plataforma You Tube presentaron lagunas relacionadas al contenido de presión arterial invasiva, demostrando que no es una fuente confiable de apoyo para el aprendizaje de los enfermeros.

ABSTRACT

Abstract Perioperative Goal-Directed Therapy (PGDT) has significantly showed to decrease complications and risk of death in high-risk patients according to numerous meta-analyses. The main goal of PGDT is to individualize the therapy with fluids, inotropes, and vasopressors, during and after surgery, according to patients' needs in order to prevent organic dysfunction development. In this opinion paper we aimed to focus a discussion on possible alternatives to invasive hemodynamic monitoring in low resource settings.

ABSTRACT

Resumen: Las hormonas tiroideas forman parte fundamental del mantenimiento de la homeostasia, se encuentra particularmente relacionado con la función cardiovascular. Los estados distiroideos clínicos o subclínicos pueden comprometer este sistema en forma significativa durante los procedimientos quirúrgicos. Existen múltiples fármacos que pueden modificar la patología tiroidea en mayor o menor medida, disminuyendo el riesgo de complicaciones en la eventualidad de una cirugía. La utilización de anestesia general, ya sea balanceada o total endovenosa, se ha convertido en el estándar de oro, por la menor tasa de complicaciones asociadas. Durante el período perioperatorio se debe mantener un monitoreo estricto de la función cardiovascular para detectar alteraciones en forma temprana e iniciar las correcciones necesarias.


Abstract: Thyroid hormones are a fundamental part of the maintenance of homeostasis, it is particularly related to cardiovascular function. Clinical or subclinical dysthyroid states can significantly compromise this system during surgical procedures. There are multiple drugs that can modify the thyroid pathology to a greater or lesser extent, reducing the risk of complications in the event of surgery. The use of general anesthesia, whether balanced or total intravenous, has become the Gold standard, due to the lower rate of associated complications. During the perioperative period, strict monitoring of cardiovascular function must be maintained to detect alterations early and initiate the necessary corrections.

ABSTRACT

Background: Subarachnoid block has been used for intracavitary radiotherapy (ICRT) for carcinoma cervix, but the literature on the appropriate dose of local anesthetic required to achieve the desired effect is lacking. We compared two different intrathecal doses of 0.5% hyperbaric bupivacaine (1.2 and 1.5 mL) for providing optimal surgical conditions and readiness to discharge in patients undergoing ICRT for carcinoma cervix. Materials and Methods: This prospective double?blind study was done in 80 patients undergoing ICRT. The patients were randomized into two groups (Groups I and II) to receive 1.2 and 1.5 mL of intrathecal hyperbaric bupivacaine, respectively, for ICRT. The level of sensory achieved, the patient satisfaction score, radiation oncologist score, time to L5 regression, and time to motor recovery (walking unaided) were assessed. Statistical Analysis Used: The data were analyzed using SPSS 20 for Windows (IBM, Chicago, IL, USA). Results: The time taken for the block to regress to L5 (Group I: 134.6 � 32.4 minutes vs. Group II: 143.2 � 43.0 minutes, P = 0.31) were comparable. However, the mean time for walking unaided (Group I: 220.87 � 47.12 minutes versus Group II: 247.00 � 49.83 minutes, P = 0.032) after the subarachnoid block was significantly less in Group I. The patient satisfaction with the procedure and overall satisfaction of the radiation oncologists regarding the operating condition were comparable in both the groups. Conclusions: Hence, a 1.2 mL dose of intrathecal hyperbaric bupivacaine for ICRT provides optimal surgical conditions with hemodynamic stability and ensures the early discharge of the patient.

Abstract Objective Acute kidney injury (AKI) in the neonatal period is associated with worst outcomes as increased mortality and increased length of hospital stay. Very low birth weight (VLBW) newborns are at higher risk for developing several other conditions that are associated with worst outcomes. Understanding the risk factors for AKI may help to prevent this condition and improve neonatal care for this population. Methods This retrospective cohort study included 155 very low birth weight newborns admitted between 2015 and 2017. The authors compared the newborns who developed neonatal AKI with the non-AKI group and analyzed the main risk factors for developing AKI in the population. The authors also performed an analysis of the main outcomes defined as the duration of mechanical ventilation, length of stay, and death. Results From the cohort, a total of 61 (39.4%) patients had AKI. The main risk factors associated with Neonatal AKI were necrotizing enterocolitis (aOR 7.61 [1.69 - 34.37]; p = 0.008), neonatal sepsis (aOR 2.91 [1.17 - 7.24], p = 0.021), and hemodynamic instability (aOR 2.99 [1.35 - 6.64]; p = 0.007). Neonatal AKI was also associated with an increase in the duration of mechanical ventilation in 9.4 days (p = 0.026) and in an increase in mortality 4 times (p = 0.009), after adjusting for the other variables. Conclusion The present results highlight the importance of minimizing sepsis and necrotizing enterocolitis, as well as the importance of identifying hemodynamic instability, to prevent AKI and diminish the burden of morbimortality in VLBW newborns.

ABSTRACT

Objective: To study the correlation of objective parameters for diagnosing shock viz., perfusion index (PI), plethysmography variability index (PVI) and serum lactate (SL) with invasive blood pressure in late preterm and term infants with shock. Methods: Prospective observational study (diagnostic test) conducted at the neonatal intensive care unit of Kanchi Kamakoti CHILDS Trust Hospital, Chennai between June, 2018 and May, 2020. Term and late preterm neonates with shock were included in the study. PI, PVI, SL, SpO2 and heart rate were monitored. PI, PVI and SLL were recorded at 0,12, 24 and 72 hours of onset of shock. All the babies were followed up till discharge or death. Results: Total 78 neonates were enrolled in the study. At 0 hour, SL and PVI had negative correlation (P =0.002 and P=0.003) while PI had a weak-to-moderate positive correlation (P=0.002) with invasive blood pressure. SL ?4.65 had a sensitivity of 75% and specificity of 75.8%, and PI <0.455 had a sensitivity of 65%, and specificity of 58.6% for predicting invasive hypotension. PVI ?23.5 had a sensitivity of 90% and specificity of 63.8% in predicting invasive hypotension. Conclusion: PI has moderate positive correlation while SL and PVI have moderate negative correlation with invasive blood pressure. The cutoff values of SL ?4.65, PI <0.45 and PVI ?23.5 can predict invasive hypotension with good sensitivity and negative predictive value.

ABSTRACT

Background: Advanced age is associated with the physiologic process of aging and decreased functional reserve of organs. These patients are often frail and have several comorbid conditions. The physiological changes in the aging cardiovascular system affect the arterial and venous vasculature, myocardium, and autonomic nervous system, making the older person more prone to cardiovascular instability. In addition to the physiological changes, the cardiovascular status of the older person tends to be compromised by associated pathological conditions that are more common with increasing age. This study aimed to analyze cardiovascular challenges in geriatric anesthesia after induction of GA. Material & Methods: This prospective study was conducted at the Department of Anesthesia, Combined Military Hospital (CMH), Dhaka, Bangladesh. The study was carried out from April 2016 to September 2016. A total of 60 subjects were selected as per inclusion criteria. Subjects were divided into 2 groups to compare the parameters among the non-elderly and elderly respondents, Group A (age ?60 years) and Group B (age >60 years). Statistical analysis of the results was obtained by using Statistical Packages for Social Sciences (SPSS-17) software. Results: In this study, 30 (50%) patients belonged to group A with ?60 years of age and 30 (50%) patients belonged to group B with >60 years of age. Among the respondents 20 were male and 10 were female in group A, and 23 were male and 7 were female in group B. Regarding pre-existing cardiovascular co-morbid conditions, most patients (5, 16.67%) suffered from hypertension, followed by atherosclerosis (2, 6.67%) and 1 patient (3.33%) had cardiac arrhythmias in group A. Most of the patients (7, 23.33%) suffered from hypertension followed by congestive heart failure (6, 20.0%), 5 patients (16.67%) had atherosclerosis, 5 patients (16.67%) had coronary artery disease, 4 patients (13.33%) went through cardiac arrhythmias and the rest 3 (10.0%) patients suffered from aortic stenosis. The mean difference of SBP between the groups 20 minutes before and after induction of general anesthesia showed a statistically significant difference at 5,10,15 and 20th-minute follow-ups. (P=<0.05). The mean difference of DBP between the groups 20 minutes before and after giving general anesthesia showed a statistically significant difference at 5, 10, 15, and 20th-minute follow-up. (P=<0.05) The mean difference in heart rate between the groups 20 minutes before and after induction of general anesthesia showed statistically significant differences at 10, 15, and 20th minute follow up. (P=<0.05). Conclusion: T This study revealed that elderly patients suffered the most from hypertension followed by heart failure and coronary artery diseases. These pre-existing cardiovascular diseases along with hemodynamic instability during the surgical procedure after general anesthesia pointed towards a great challenge. Thus, care of the elderly patient requires thorough preoperative assessment and planning regarding both anesthetic and surgical techniques and the involvement of a multidisciplinary clinical team knowledgeable about and interested in the management of the elderly surgical patient.

Background: Laryngoscopy and endotracheal intubation are usually associated with tachycardia and hypertension. Pre-administration of melatonin has anxiolytic and sedative property which can reduce the tachycardia and hypertension during the surgical procedures. Aims and Objectives: The present study aimed to evaluate the melatonin effect on hemodynamic changes during laryngoscopy and intubation. Materials and Methods: This prospective study was done in the department of anesthesia, Sri Jayadeva Institute of Cardiovascular Sciences and Research, Bengaluru, Karnataka. Total 80 patients were included in the study based on the inclusion and exclusion criteria. Patients were divided into two groups. Group-A treated with placebo and Group-B treated with melatonin (6 mg) and demographic, clinical, and hemodynamic parameters were recorded. The data were analyzed with unpaired t-test with the use SPSS (20.0) version software. Results: Comparison of number and percentage of age, gender, and blood groups between the Group-I and Group-II not showed any significant difference. Group-I and Group-II mean age, height, and weight not showed any significant difference. Mean heart rate, systolic blood pressure, diastolic blood pressure, and mean arterial pressure were compared between the Group-I and Group-II at basal, during, after 1, 3, 5, and 10 min showed significant difference. Conclusion: Pre-administration melatonin showed significant reduction of hemodynamic changes compared to placebo group.

ABSTRACT

Background: Intense sympathetic activity is linked to laryngoscopy and endotracheal intubation, which could lead to intraoperative problems. We undertook this study to compare the effects of preoperative nebulized Fentanyl and Dexmedetomidine on hemodynamic response to laryngoscopy and endotracheal intubation, taking advantage of their high bioavailability and better absorption through nasal mucosa. Aim and Objectives: The objectives of the study were (i) to compare the effect of preoperative nebulization on the hemodynamic response to laryngoscopy and intubation; and (ii) to assess intraoperative requirement of anesthetic agents. Materials and Methods: This prospective, randomized, and comparative study was conducted among 100 American Society of Anesthesiologists (ASA) I, II patients (of either gender) undergoing elective surgeries and requiring tracheal intubation, were randomized in two groups. Group A was given Fentanyl Nebulization (2 ?g/kg in 4 ml of 0.9% saline) and Group B was given Dexmedetomidine nebulization (1 ?g/kg in 4 ml of 0.9% saline) 10 min before anesthesia induction. Hemodynamic parameters were noted before and immediately after induction, 1 min, 5 min and 10 min after intubation. The main goal was to assess how Fentanyl and dexmedetomidine nebulization affect the laryngoscopy and intubation-induced stress response. The secondary outcome was to assess the intraoperative requirement of anesthetic agents, observe adverse effects of study drug and sedation score. Results: Dexmedetomidine nebulization was found to be more effective in blunting rise in heart rate post laryngoscopy compared to Fentanyl Nebulization (P < 0.0001) as well as in MAP after 10 min of intubation (P < 0.0001). Requirement of propofol was seen to be significantly reduced in Group B compared to Group A (P < 0.05). Sedation scores were significantly higher in Group B (P < 0.05). No evidence of side effects was observed in any group. Conclusion: Nebulisation of dexmedetomidine was found to be more effective in attenuation of stress response of laryngoscopy and intubation compared to nebulisation of fentanyl, with stable intraoperative hemodynamic and no significant side effects.

ABSTRACT Cardiac output is an essential determinant of oxygen delivery, although unreliably measured on clinical examination and routine monitoring. Unfortunately, cardiac output monitoring is rarely performed in pediatric critical care medicine, with a limited availability of accurate methods for children. Herein, we report two pediatric cases in which noninvasive pulse-wave transit time-based cardiac output monitoring (esCCO, Nihon Kohden, Tokyo, Japan) was used. The esCCO system calculates cardiac output continuously by using the negative correlation between stroke volume and pulse wave transit time and requires only electrocardiogram monitoring, noninvasive blood pressure, and pulse oximetry signals. Before starting its use, esCCO should be calibrated, which can be done using patient information (gender, age, height, and body weight) or entering cardiac output values obtained by other methods. In both cases, when calibrations were performed using patient information, the agreement between esCCO and echocardiographic measurements was poor. However, after calibration with transthoracic echocardiography, the cardiac output values obtained by both methods remained similar after 2 hours and 18 hours. The results indicate that the esCCO system is suitable for use in children; however, further studies are needed to optimize its algorithm and determine its accuracy, precision, and trend in children.


RESUMO O débito cardíaco é um determinante importante do fornecimento de oxigênio, embora a sua mensuração seja realizada de forma pouco confiável no exame clínico e no monitoramento de rotina. Infelizmente, o monitoramento do débito cardíaco raramente é realizado na medicina intensiva pediátrica, com disponibilidade limitada de métodos precisos para crianças. Relatamos aqui dois casos pediátricos nos quais utilizouse o monitoramento não invasivo do débito cardíaco por meio da análise do tempo de trânsito de ondas de pulso (esCCO, Nihon Kohden, Tóquio, Japão). O sistema esCCO calcula o débito cardíaco continuamente pela correlação negativa entre o volume sistólico e o tempo de trânsito de ondas de pulso e requer apenas o monitoramento por eletrocardiograma, pressão arterial não invasiva e sinais de oximetria de pulso. Antes de iniciar seu uso, o esCCO deve ser calibrado, o que pode ser feito com informações do paciente (sexo, idade, altura e peso corporal) ou informando os valores do débito cardíaco obtidos mediante outros métodos. Em ambos os casos, quando as calibragens foram realizadas com informações do paciente, a concordância entre o débito cardíaco contínuo estimado e as medidas ecocardiográficas foi insatisfatória. Entretanto, após a calibragem com ecocardiografia transtorácica, os valores do débito cardíaco obtidos pelos dois métodos permaneceram semelhantes após 2 horas e 18 horas. Os resultados indicam que o sistema esCCO pode ser útil em crianças; entretanto, são necessários mais estudos para otimizar seu algoritmo e determinar sua exatidão, precisão e tendência em crianças.

Objective Although goal-directed fluid therapy (GDFT) has been proven to be effective in reducing the incidence of postoperative complications, the underlying mechanisms remain unknown. The aim of this study was to examine the mediating role of intraoperative hemodynamic lability in the association between GDFT and the incidence of postoperative complications. We further tested the role of this mediation effect using mean arterial pressure, a hemodynamic indicator. Methods This secondary analysis used the dataset of a completed nonrandomized controlled study to investigate the effect of GDFT on the incidence of postoperative complications in patients undergoing posterior spine arthrodesis. We used a simple mediation model to test whether there was a mediation effect of average real variability between the association of GDFT and postoperative complications. We conducted mediation analysis using the mediation package in R (version 3.1.2), based on 5,000 bootstrapped samples, adjusting for covariates. Results Among the 300 patients in the study, 40% (120/300) developed postoperative complications within 30 days. GDFT was associated with fewer 30-day postoperative complications after adjustment for confounders (odds ratio: 0.460, 95% CI: 0.278, 0.761; P = 0.003). The total effect of GDFT on postoperative complications was -0.18 (95% CI: -0.28, -0.07; P < 0.01). The average causal mediation effect was -0.08 (95% CI: -0.15, -0.04; P < 0.01). The average direct effect was -0.09 (95% CI: -0.20, 0.03; P = 0.17). The proportion mediated was 49.9% (95% CI: 18.3%, 140.0%). Conclusions The intraoperative blood pressure lability mediates the relationship between GDFT and the incidence of postoperative complications. Future research is needed to clarify whether actively reducing intraoperative blood pressure lability can prevent postoperative complications.

ABSTRACT

Objective To analyze the risk factors and build a clinical prediction model for hemodynamic depression (HD) after carotid artery stenting (CAS). Methods A total of 116 patients who received CAS in the Department of Vascular Surgery,Drum Tower Clinical College of Nanjing Medical University and the Department of Vascular Surgery,the Affiliated Suqian First People's Hospital of Nanjing Medical University from January 1,2016 to January 1,2022 were included in this study.The patients were assigned into a HD group and a non-HD group.The clinical baseline data and vascular disease characteristics of each group were collected,and multivariate Logistic regression was employed to identify the independent predictors of HD after CAS and build a clinical prediction model.The receiver operating characteristic (ROC) curve was drawn,and the area under the ROC curve (AUC) was calculated to evaluate the predictive performance of the model. Results The HD group had lower proportions of diabetes (P=0.014) and smoking (P=0.037) and higher proportions of hypertension (P=0.031),bilateral CAS (P=0.018),calcified plaque (P=0.001),eccentric plaque (P=0.003),and the distance<1 cm from the minimum lumen level to the carotid bifurcation (P=0.009) than the non-HD group.The age,sex,coronary heart disease,symptomatic carotid artery stenosis,degree of stenosis,and length of lesions had no statistically significant differences between the HD group and the non-HD group (all P>0.05).Based on the above predictive factors,a clinical prediction model was established,which showed the AUC of 0.807 and the 95% CI of 0.730-0.885 (P<0.001).The model demonstrated the sensitivity of 62.7% and the specificity of 87.7% when the best cut-off value of the model score reached 12.5 points. Conclusions Diabetes,smoking,calcified plaque,eccentric plaque,and the distance<1 cm from the minimum lumen level to the carotid bifurcation are independent predictors of HD after CAS.The clinical prediction model built based on the above factors has good performance in predicting the occurrence of HD after CAS.

ABSTRACT

Objective:To explore the effect of quality improvement of hemodynamic monitoring on fluid overload (FO) and outcome in newborns with septic shock.Methods:Non-invasive cardiac output monitoring and functional cardiac ultrasound quality improvement program was started during January 2020 in our hospital. Neonates with septic shock admitted before and after the program were retrospectively analyzed. From January 2018 to December 2019 was pre-improvement period when fluid resuscitation was routinely performed and vasoactive drugs was selected empirically. From January 2020 to December 2021 was post-improvement period when fluid resuscitation and/or use and adjustment of vasoactive drugs were guided by hemodynamic parameters. The 24 h, 48 h, 72 h FO, duration of invasive respiratory support, vasoactive-inotropic score, septic shock score, incidences of complications and all-cause mortality were compared between the two groups.Results:A total of 284 eligible cases were enrolled, including 136 cases in pre-improvement group and 148 cases in post-improvement group. Post-improvement group had significantly lower gestational age (GA), birth weight (BW) and body weight at disease onset than pre-improvement group ( P<0.05). Incidences of 48 h and 72 h FO, fluid resuscitation volume within 72 h, pulmonary hemorrhage and periventricular leukomalacia (PVL) were significantly lower in the post-improvement group ( P<0.05). No significant differences existed in 24 h FO, other complications and all-cause mortality between the two groups ( P>0.05). No significant differences existed in GA and BW for neonates with pulmonary hemorrhage and PVL between the two groups ( P>0.05). Conclusions:Quality improvement of hemodynamic monitoring can effectively improve FO and reduce the incidences of pulmonary hemorrhage and PVL.

ABSTRACT

Objective:To investigate the relationship between uterine artery blood flow parameters detected by ultrasound examination during early pregnancy and pregnancy outcomes in pregnant women who had a history of recurrent spontaneous abortion.Methods:A total of 100 pregnant women with a history of recurrent spontaneous abortion who were at 6-10 weeks of gestation and received treatment in Zhuji Maternal and Child Health Hospital from June 2020 to June 2022 were included in the study group. Using a case-control study method, 100 normal pregnant women who were at 6-10 weeks of gestation who concurrently underwent physical examination were selected and included in the control group. Both groups of pregnant women were tested for uterine artery blood flow parameters using color Doppler ultrasound diagnostic equipment, including peak systolic/diastolic velocity (S/D), resistance index (RI), and pulsatility index (PI). The occurrence of early miscarriage was monitored in both groups. The value of uterine artery blood flow parameters in the prediction of adverse pregnancy outcomes and the correlation between uterine artery blood flow parameters and adverse pregnancy outcomes were analyzed. Multivariate logistic regression was performed to analyze the relationship between uterine artery blood flow parameters and adverse pregnancy outcomes.Results:S/D, RI, and PI in the study group were (3.87 ± 0.43), (0.93 ± 0.16), and (1.73 ± 0.41), respectively, which were significantly higher than (2.54 ± 0.37), (0.64 ± 0.14), and (1.24 ± 0.35) in the control group ( t = 23.44, 13.64, 9.09, all P < 0.05). The early abortion rate in the study group was 57.0% (57/100), which was significantly higher than 2.0% (2/100) in the control group ( χ2 = 72.73, P < 0.05). The receiver operating characteristic curve analysis showed that S/D sensitivity and specificity in the prediction of adverse pregnancy outcomes were 79.1% and 70.2%, respectively, RI sensitivity and specificity were 88.4% and 84.2%, respectively, and PI sensitivity and specificity were 65.1% and 75.4%, respectively. Pearson analysis results showed that S/D, RI, and PI were positively linearly correlated with adverse pregnancy outcomes ( r = 0.587, 0.648, 0.613, all P < 0.05). Conclusion:Uterine artery blood flow parameters S/D, RI, and PI are abnormally high in pregnant women with a history of recurrent spontaneous abortion in the early stage of pregnancy. These parameters are closely associated with adverse pregnancy outcomes and are the independent risk factors of adverse pregnancy outcomes.

Abstract Background: Endotracheal intubation (ETI), which is the gold standard in coronary artery bypass grafting (CABG), may cause myocardial ischaemia by disturbing the balance between haemodynamic changes and oxygen supply and consumption of the myocardium as a result of sympathetic stimulation. In this study, we aimed to compare two different videolaryngoscopes (C-MAC and Airtraq) in the hemodynamic response to ETI. Methods: Fifty ASA II-III CABG surgery patients were randomly assigned to C-MAC or Airtraq. The hemodynamic data included arterial blood pressure [systolic (SAP), diastolic (DAP) and mean (MAP)] and heart rate (HR) and were recorded at six different points in time: before laryngoscopy-T1, during laryngoscopy-T2, immediately after intubation-T3, and 3 (T4), 5 (T5) and 10 (T6) minutes after intubation. Intraoperative complications were recorded. Patients were questioned about postoperative complications 2 and 24hours following extubation. Results: The hemodynamic response to ETI was significantly greater with C-MAC. The increase in HR started with the laryngoscopy procedure, whereas increases in SAP, DAP, and MAP started immediately after ETI (p = 0.024; p = 0.012; p = 0.030; p = 0.009, respectively). In group analyses, T1-T2, T2-T3 and T1-T3 comparisons did not show any significant differences in HR with Airtraq. However, with C-MAC, HR after intubation increased significantly compared to the pre-laryngoscopy values (T1-T3) (p = 0.004). The duration of laryngoscopy was significantly reduced with C-MAC (p < 0.001), but the duration of intubation and total intubation were similar (p = 0.36; p = 0.79). Conclusions: Compared to C-MAC, the hemodynamic response to ETI was less with Airtraq. Thus, Airtraq may be preferred in CABG patients for ETI.

Introducción: la monitorización hemodinámica constituye un conjunto de técnicas y parámetros que permiten valo rar si la función cardiovascular es la adecuada para mantener la perfusión y la oxigenación tisular que permita sa tisfacer las demandas metabólicas del organismo, valorar el estado y el comportamiento del sistema cardiovascular, orientando sobre la mejor estrategia terapéutica. La presente revisión busca proporcionar una descripción general e integrada de las diferentes técnicas de monitorización, así como aspectos fisiológicos relevantes para su entendi miento y empleo terapéutico. La monitorización hemodinámica acompañada de un adecuado conocimiento de la fisiología cardiovascular permite determinar el estado del sistema cardiovascular, la condición hemodinámica del paciente y la estrategia terapéutica requerida. Su interpretación debe partir de la integración y la correlación de diversos parámetros hemodinámicos.


Introduction: hemodynamic monitoring is a set of techniques and parameters that allow evaluating whether cardio vascular function is adequate to maintain tissue perfusion and oxygenation to satisfy metabolic demands of the or ganism, assess the condition and behavior of the cardiovascular system, providing guidance on the best therapeutic strategy. This review seeks to provide a general and integrated description of the different monitoring techniques, as well as physiological aspects relevant to their understanding and therapeutic use. Hemodynamic monitoring accompanied by an adequate knowledge of cardiovascular physiology allows to determine the state of the cardiovascular system, hemodynamic condition of the patient and therapeutic strategy required, its interpretation must start from the integration and correlation of different hemodynamic parameters.


Introdução: a monitorização hemodinâmica constitui um conjunto de técnicas e parâmetros que permitem avaliar se a função cardiovascular é adequada para manter a perfusão e oxigenação tecidual que permite satisfazer as exi gências metabólicas do organismo, avaliar o estado e comportamento do sistema cardiovascular, orientando sobre a melhor estratégia terapêutica. Esta revisão procura fornecer uma descrição geral e integrada das diferentes técnicas de monitorização, bem como aspectos fisiológicos relevantes para a sua compreensão e utilização terapêutica. A monitorização hemodinâmica acompanhada de um conhecimento adequado da fisiologia cardiovascular permite determinar o estado do sistema cardiovascular, a condição hemodinâmica do doente e a estratégia terapêutica neces sária, a sua interpretação deve partir da integração e correlação de vários parâmetros hemodinâmicos.

Introducción. La variación de la velocidad máxima aórtica con la ventilación mecánica (ΔVpeakAo) ha demostrado ser el mejor predictor de respuesta a volumen en pediatría. Existe evidencia en adultos de que la variación de velocidad máxima de flujo carotídeo (ΔVpeakCar) es predictor de respuesta a fluidos. Al momento es escasa la información sobre este índice en pediatría. Su beneficio se basa en la no-invasividad, y que para su medición no es necesaria la ecocardiografía ni el acceso al tórax del paciente. Objetivo. El objetivo general de este trabajo fue estudiar la correlación y la concordancia de ΔVpeakCar con ΔVpeakAo en una población pediátrica bajo ventilación mecánica. Metodología. Se incluyeron pacientes de 0 a 12 años. Se registraron flujos aórtico y carotídeos máximos y mínimos y se calculó ΔVpeakCar y ΔVpeakAo. Para analizar correlación y concordancia entre las variables se utilizó el test de Pearson, análisis de Bland-Altman y análisis de los 4-cuadrantes. Resultados. Se estudiaron 58 pacientes, 13 lactantes (menores 12 meses), 21 preescolares (12-60 meses) y 24 escolares (mayores a 60 meses). Se observó una correlación significativa entre ΔVpeakAo y ΔVpeakCar (r=0,85; p<0,05) con un coeficiente de determinación de r2=0,72. El análisis de Bland-Altman mostró un sesgo del 0,15% (IC95%, -0.7-1.0) con un límite de concordancia del -6,1 a 6,2%. La concordancia fue 85%, con un sesgo angular de 4,5°±31°. El análisis por subgrupos mostró un r2 de 0.89 en escolares, 0.56 en preescolares y 0.45 en lactantes. La concordancia fue de 100% en escolares, 95% en prescolares y 93% en lactantes. Discusión y conclusiones. El registro de ΔVpeakCar fue viable. Al analizar la capacidad de ΔVpeakCar de sustituir a ΔVpeakAo en el total de la muestra, no es buena La correlación y concordancia son mejores en escolares. Es necesario continuar estudiando este nuevo índice.


Introduction. The variation in maximum aortic velocity with mechanical ventilation (ΔVpeakAo) has proven to be the most effective predictor of fluid response in pediatrics. While there is evidence in adults that the variation in maximum carotid flow velocity (ΔVpeakCar) predicts fluid response, information on this index in pediatrics remains limited. Its advantage lies in its non-invasive nature, eliminating the need for echocardiography or thoracic access for recording. Objective. This study aims to examine the correlation and concordance between ΔVpeakCar and ΔVpeakAo in a pediatric population. Methodology. The study included patients aged 0 to 12 years. Maximum and minimum aortic and carotid flows were recorded, and ΔVpeakCar and ΔVpeakAo were calculated. Correlation and agreement between variables were analyzed using the Pearson test, Bland Altman analysis, and 4-quadrant analysis. Results. A total of 58 patients were studied, comprising 13 infants (under 12 months), 21 preschoolers (12-60 months), and 24 school-aged children (over 60 months). A significant correlation was observed between ΔVpeakAo and ΔVpeakCar (r=0.85; p<0.05) with a coefficient of determination, r²=0.72. The Bland-Altman analysis revealed a bias of 0.15% (95% CI, -0.7-1.0) with an agreement limit of -6.1% to 6.2%. The concordance rate was 85%, with an angular bias of 4.5°±31°. Subgroup analysis showed r² values of 0.89 in school-aged children, 0.56 in preschoolers, and 0.45 in infants. Concordance rates were 100% in school-aged children, 95% in preschoolers, and 93% in infants. Discussion and Conclusions. The measurement of ΔVpeakCar proved feasible. However, when considering its ability to replace ΔVpeakAo, the results are suboptimal. Correlation and concordance are stronger in school-aged children. Further investigation into this new index is warranted.

However, only a few studies are done wherever hemodyanamics effects when bupivacaine versus levobupivacaine have compared. Aim of this study to examine the consequences of bupivacaine vs levobupivacaine in patients undergoing lower abdominal surgeries and their effects on hemodynamics ( the blood pressure, HR, and SpO2). Method: A randomised double-blind management study conducted when approval from IEC Ref. code-92nd IIB Thesis / P4 . The study has fifty patients, with ASA I – II physical status, as well as each sex and 2-10 years cluster age and who set up for abdominal surgery notably lower abdominal . 25 patients set in every group and every which way allotted: cluster Bupiva and Levobupiva . All patients were well well-read regarding the procedure, drugs, and effects of drugs, and untoward complications. well-read written consent was taken from each patient before involving them within the study. primary objective is to examine the impact of those medicine on Blood pressure, HR, and SPO2 & Secondary objective was to check rescue physiological condition reduction within the surgical period. After the comparison between these 2 clusters, The mean SBP of group Bupiva was found below that of group Levobupiva . and located to be important at fifteen min, one hour, a pair of hr, and four hr and insignificant at baseline zero min, thirty min, half dozen hr (p=0.092), twelve hr, eighteen hr, and twenty four h.differences is insignificant in comparison the DBP at any purpose of your time from baseline to 24 hr. On comparing the center rate, variations were found insignificant from baseline (p = 0.897), 0 min (p = 0.651), 30 min (p = 0.096), 1hr (p = 0.192), 2 hr (p=0.390), four hour (p=0.525), half dozen hr (p=0.469), twelve hr (p=0.443), eighteen hr (p=0.288) to twenty four hr (p=0. 390) except at fifteen min (p=0.010) that was the mean pulse rate of cluster Bupiva is over that of group Levobupiva at one,5 min. On comparison the SPO2differences were insignificant at baseline (p = 0.620), 15 min (p = 0.108), thirty min (p=0.353), 1 hr (p = 0.789), 2 hr (p = 0.364), 4 hr (p = 0.292) and 6 hr (p = 0. 198) however important at 0 min (p = 0.002), 12hr (p = 0.012), 18hr (p = 0.002) and24 hr (p = 0.028) wherever the mean SPO2 of cluster Bupiva was abundant below the group Levobupiva. Stable hemodynamics for a extended period were provided by Levobupivacaine as compared to Bupivacaine.
