ABSTRACT Purpose: This study investigated the relationship between blood pressure and intraocular pressure in treatmentnaive, non-glaucoma patients with different blood pressure statuses, focusing on the 24-h ocular volume and nocturnal blood pressure decline. Methods: Treatment-naive, non-glaucoma patients undergoing hypertension evaluation were enrolled as study participants. Simultaneous 24-h ambulatory blood pressure measurement and 24-h ocular volume recording with a contact lens sensor. We also compared ocular volume curve parameters between normotensive and hypertensive patients, as well as between those with and without nocturnal blood pressure decline. Results: A total of 21 patients, including 7 normotensive and 14 treatment-naive hypertensive individuals, were included in the study. of them, 11 were dippers and 10 were non-dippers. No significant difference in the 24-h ocular volume slope was observed between the hypertensive and normotensive patients (p=0.284). However, dippers had a significantly higher 24-h ocular volume slope (p=0.004) and nocturnal contact lens sensor output (p=0.041) than non-dippers. Conclusion: Nocturnal blood pressure decline, rather than the blood pressure level, is associated with the increased 24-h ocular volume slope and nocturnal ocular volume. Further studies are required to determine whether the acceleration of glaucoma progression in dippers is primarily due to low blood pressure, high intraocular pressure, or a combination of both.

Introduction: High blood pressure (HBP) is the leading cause of death from cardiovascular disease. Despite the advances, the percentage of undiagnosed and untreated hypertensive patients is 58.4%. The evaluation of cognitive damage in HBP focuses on preventing stroke, while functional damage is ignored. This inadequate management may be multifactorial. The objective was to analyze the opinions that doctors have about the relationship between high blood pressure and cognitive damage. Methodology: Observational, descriptive, cross-sectional study developed in the period between August 2020 and August 2023. Analysis of data obtained from a self-administered, anonymous and voluntary questionnaire. Revealing information on the professional profile, knowledge of HBP, its link with cognitive impairment (CD), diagnosis and treatment. Results: 222 professionals were included, 215 (96.8%) agree with the existence of a link between HBP and other cardiovascular risk factors in CD, and 218 (98.1%) acknowledge assisting patients at risk of suffering from CD. The CD evaluation is carried out in selected cases by 132 (59.4%) participants and 59 (26.7%) always do it. Of those who perform evaluation, 103 (54%) use the Mini Mental State Examination (MMSE), 10 (5.2%) use the Montreal Cognitive Assessment (MoCA) and 9 (4.7%) use the Clock Drawing Test. Regarding the decrease in blood pressure in elderly patients and the link with risk of CD: 54 (24.3%) do not recognize risk and 65 (29.2%) recognize a moderate-high risk. In reference to the implication of the treatment of cardiovascular disease and CD: 217 (97.7%) recognized a beneficial effect. Discussion: Given the recognition of the link between HBP and CD, it would be expected that CD would be investigated in the vast majority, however only 26.7% always evaluate it. There is no consensus on the method, the MMSE being the most used, with a low application of the MoCA test and/or Clock Drawing Test, the latter being the ones that evaluate executive function, mostly altered in CD linked to HBP. Although the treatment of cardiovascular disease is recognized as beneficial with respect to CD, the control of HBP in older adults is considered risky. A diagnosis is made of a situation where a disparity is evident between what one recognizes as knowing and what one claims to do. Conclusions: The role of vascular disease in functional brain damage is recognized, considering it necessary to know the cognitive status of patients, however there is a low application of screening tests that evaluate executive function. In this context, a gap between medical knowledge and practice is shown.
Introdução: A hipertensão arterial (HA) é a principal causa de morte por doenças cardiovasculares. Apesar dos avanços, o percentual de hipertensos não diagnosticados e não tratados é de 58,4%. A avaliação do dano cognitivo na hipertensão concentra-se na prevenção do acidente vascular cerebral, enquanto o dano funcional é ignorado. Esse manejo inadequado pode ser multifatorial. É objetivo fue analisar a opinião dos médicos sobre a relação entre hipertensão arterial e danos cognitivos. Metodologia: Estudo observacional, descritivo, transversal desenvolvido no período entre agosto de 2020 e agosto de 2023. Análise de dados obtidos a partir de questionário autoaplicável, anônimo e voluntário. Revelar informações sobre o perfil profissional, conhecimento sobre a HA, sua ligação com o comprometimento cognitivo (DC), diagnóstico e tratamento. Resultados: Foram incluídos 222 profissionais, 215 (96,8%) concordam com a existência de ligação entre hipertensão e outros fatores de risco cardiovascular na DC e 218 (98,1%) reconhecem ajudar pacientes com risco de sofrer de D.C. A avaliação da DC é realizada em casos selecionados por 132 (59,4%) participantes e 59 (26,7%) a fazem sempre. Dos que realizam avaliação, 103 (54%) utilizam o Mini Exame do Estado Mental (MEEM), 10 (5,2%) utilizam a Avaliação Cognitiva de Montreal (MoCA) e 9 (4,7%) utilizam o Clock Drawing Test. Em relação à diminuição da pressão arterial em pacientes idosos e a ligação com o risco de DC: 54 (24,3%) não reconhecem risco e 65 (29,2%) reconhecem risco moderado-alto. Em referência à implicação do tratamento de doenças cardiovasculares e DC: 217 (97,7%) reconheceram o efeito benéfico. Discussão: Dado o reconhecimento da ligação entre hipertensão e DC, seria de esperar que a DC fosse investigada na grande maioria, no entanto apenas 26,7% sempre a avaliam. Não há consenso sobre o método, sendo o MEEM o mais utilizado, com baixa aplicação do teste MoCA e/ou Clock Drawing Test, sendo estes últimos os que avaliam a função executiva, majoritariamente alterada nos DC vinculados à HA. Embora o tratamento das doenças cardiovasculares seja reconhecido como benéfico em relação à DC, o controle da HA em idosos é considerado arriscado. É feito um diagnóstico de uma situação em que é evidente uma disparidade entre o que se reconhece como saber e o que se afirma fazer. Conclusões: O papel da doença vascular no dano cerebral funcional é reconhecido, considerando-se necessário conhecer o estado cognitivo dos pacientes, porém há baixa aplicação de testes de triagem que avaliam a função executiva. Nesse contexto, evidencia-se uma lacuna entre o conhecimento e a prática médica.

Abstract Introduction: Blood pressure (BP) assessment affects the management of arterial hypertension (AH) in chronic kidney disease (CKD). CKD patients have specific patterns of BP behavior during ambulatory blood pressure monitoring (ABPM). Objectives: The aim of the current study was to evaluate the associations between progressive stages of CKD and changes in ABPM. Methodology: This is a cross-sectional study with 851 patients treated in outpatient clinics of a university hospital who underwent ABPM examination from January 2004 to February 2012 in order to assess the presence and control of AH. The outcomes considered were the ABPM parameters. The variable of interest was CKD staging. Confounding factors included age, sex, body mass index, smoking, cause of CKD, and use of antihypertensive drugs. Results: Systolic BP (SBP) was associated with CKD stages 3b and 5, irrespective of confounding variables. Pulse pressure was only associated with stage 5. The SBP coefficient of variation was progressively associated with stages 3a, 4 and 5, while the diastolic blood pressure (DBP) coefficient of variation showed no association. SBP reduction was associated with stages 2, 4 and 5, and the decline in DBP with stages 4 and 5. Other ABPM parameters showed no association with CKD stages after adjustments. Conclusion: Advanced stages of CKD were associated with lower nocturnal dipping and greater variability in blood pressure.
Resumo Introdução: A avaliação da pressão arterial (PA) tem impacto no manejo da hipertensão arterial (HA) na doença renal crônica (DRC). O portador de DRC apresenta padrão específico de comportamento da PA ao longo da monitorização ambulatorial da pressão arterial (MAPA). Objetivos: O objetivo do corrente estudo é avaliar as associações entre os estágios progressivos da DRC e alterações da MAPA. Metodologia: Trata-se de um estudo transversal com 851 pacientes atendidos nos ambulatórios de um hospital universitário que foram submetidos ao exame de MAPA no período de janeiro de 2004 a fevereiro de 2012 para avaliar a presença e o controle da HA. Os desfechos considerados foram os parâmetros de MAPA. A variável de interesse foi o estadiamento da DRC. Foram considerados como fatores de confusão idade, sexo, índice de massa corporal, tabagismo, causa da DRC e uso de anti-hipertensivos. Resultados: A PA sistólica (PAS) se associou aos estágios 3b e 5 da DRC, independentemente das variáveis de confusão. Pressão de pulso se associou apenas ao estágio 5. O coeficiente de variação da PAS se associou progressivamente aos estágios 3a, 4 e 5, enquanto o coeficiente de variação da pressão arterial diastólica (PAD) não demonstrou associação. O descenso da PAS obteve associação com estágios 2, 4 e 5, e o descenso da PAD, com os 4 e 5. Demais parâmetros da MAPA não obtiveram associação com os estágios da DRC após os ajustes. Conclusão: Estágios mais avançados da DRC associaram-se a menor descenso noturno e a maior variabilidade da pressão arterial.

RESUMEN Introducción: En el diagnóstico y seguimiento de la hipertensión arterial (HTA) la mayoría de las guías de práctica recomiendan el monitoreo ambulatorio de la presión arterial (MAPA). En este sentido, existe cada vez más evidencia que respalda la superioridad de la hipertensión arterial nocturna (HTAN) como predictor de eventos cardiovasculares. Se sabe poco sobre la relación con los eventos cardiovasculares según la gravedad de la HTAN. Además, no está claro a partir de qué valor de presión arterial nocturna comienza a aumentar el riesgo. Objetivos: Conocer si la presencia de HTAN y sus niveles de gravedad se asocian con resultados cardiovasculares adversos durante el seguimiento. Material y métodos: Estudio observacional. Realizamos un análisis de los datos obtenidos en un centro médico de alta complejidad de Buenos Aires, recopilados a partir de estudios de MAPA de 24 horas. Examinamos las características clínicas de los pacientes, los resultados de laboratorio, los estudios de imagen y sus resultados durante el período de seguimiento. Nuestro estudio incluyó personas de 18 años o más a las que se les había diagnosticado hipertensión. Definimos HTAN como aquellos casos con valores de presión arterial ≥ 120/70 mmHg durante el periodo nocturno. Resultados: Fueron incluidos 981 pacientes en el análisis final. De ellos, el 53 % eran hombres; la edad media era de 59,4 años. Presentaban HTAN 63,6 % (n=624). Clasificamos la HTAN en cuatro estratos de gravedad para comparar, según el valor de presión arterial sistólica nocturna: 83-119 mmHg, 120-139 mmHg, 140-159 mmHg y 160-220 mmHg. Se registraron eventos adversos cardiovasculares mayores en 8 (2,2 %), 17 (4,1 %), 8 (5,6 %) y 7 (11,3 %) sujetos, respectivamente, y esta diferencia entre grupos fue estadísticamente significativa (p=0,007). El análisis multivariado de regresión de Cox demostró que la presencia de HTAN fue un predictor independiente de eventos cardiovasculares adversos (HR 3,60; IC 95% 1,12-11,5; p=0,033), incluso al considerar la presencia de hipertensión arterial diurna. Conclusión: En esta cohorte contemporánea, la HTAN y su gravedad se asociaron independientemente con la incidencia de eventos cardiovasculares adversos.
ABSTRACT Background: In the diagnosis and follow-up of arterial hypertension (HTN) most practice guidelines recommend ambulatory blood pressure monitoring (ABPM). In this regard, there is increasing evidence supporting the superiority of nocturnal hy pertension (NHTN) as a predictor of cardiovascular events. Little is known about the relationship with cardiovascular events according to the severity of NHTN. Furthermore, it is unclear from what nighttime pressure value the risk begins to increase. Objectives: The aim of this study was to determine whether the presence of NHTN and its severity levels are associated with adverse cardiovascular outcomes during follow-up. Methods: An observational study was performed analyzing data collected from 24-hour ABPM studies obtained in a high complexity medical center in Buenos Aires. We examined patients' clinical characteristics, laboratory findings, imaging studies and their results during the follow-up period. Our study included ≥18 year-old persons who had been diagnosed with hypertension. We defined NHTN as those cases with blood pressure values ≥120/70 mmHg during the nighttime period. Results: The final analysis included 981 patients. Among these, 53% were men, mean age was 59.4 years and NHTN was present in 63.6% of cases (n=624). Nocturnal HTN was classified into four severity strata for comparison, according to the nighttime systolic blood pressure value: 83-119 mmHg, 120-139 mmHg, 140-159 mmHg and 160-220 mmHg. Major adverse cardiovascular events were recorded in 8 (2.2%), 17 (4.1 %), 8 (5.6%) and 7 (11.3%) subjects, respectively, and this differ ence between groups was statistically significant (p=0.007). Multivariate Cox regression analysis showed that the presence of NHTN was an independent predictor of adverse cardiovascular events (HR 3.60; 95% CI 1.12-11.5; p=0.033), even when considering the presence of daytime hypertension. Conclusion: In this contemporary cohort, NHTN and its severity were independently associated with the incidence of adverse cardiovascular events.

Introducción. La trombosis de la vena porta es la oclusión parcial o completa de la luz de la vena porta o sus afluentes por la formación de trombos. Se asocia a desenlaces adversos y a un peor pronóstico. La frecuencia de aparición viene en aumento, a menudo de manera incidental, debido al uso cada vez mayor de imágenes diagnósticas. Metodología. Estudio observacional de una serie de casos de pacientes mayores de 18 años a quienes se les documentó trombosis venosa portal en hígado no cirrótico en el periodo comprendido entre enero de 2012 y diciembre de 2019 en un hospital de cuarto nivel de la ciudad de Medellín, Colombia. Resultados. Se documentaron 94 trombosis portales, la media de edad fue 44 ± 15 años, el 56 % eran mujeres. El promedio de aparición de síntomas fue de 14 días. La presentación de la trombosis fue aguda en un 41 %, crónica en 44 % y de instauración aguda sobre una trombosis crónica en un 15 %. La presentación clínica fue asintomática en el 33 %, el dolor abdominal fue la presentación más común con el 62 %. La trombofilia adquirida de mayor ocurrencia fueron las neoplasias mieloproliferativas crónicas en un 18 %, seguida del síndrome antifosfolípido en un 6 %. El método diagnóstico más usado fue la TAC trifásica en un 58 % seguido de la ultrasonografía en un 35 %. El 66 % de los pacientes fueron anticoagulados, siendo la warfarina el principal anticoagulante usado en un 56 %. El 16 % presento algún tipo de sangrado, aunque ninguno fue sangrado mayor. Conclusiones. La trombosis portal sigue siendo en muchos casos un hallazgo incidental. Se encontró un número inusual de neoplasias mieloproliferativas crónicas. La anticoagulación es segura y eficaz, aunque los anticoagulantes orales directos aún tienen un uso restringido.
Introduction. Portal vein thrombosis is the partial or complete occlusion of the lumen of the portal vein or its tributaries by thrombus formation. It is associated with adverse outcomes and a poorer prognosis. Its frequency is increasing, often incidentally, due to the growing use of diagnostic imaging. Methodology. This is an observational study of a case series of patients over 18 years old who were documented with portal vein thrombosis in a non-cirrhotic liver between January 2012 and December 2019 in a fourth-level hospital in the city of Medellín, Colombia. Results. Ninety-four cases of portal vein thrombosis were documented. The mean age was 44 ± 15 years, and 56 % were women. The average onset of symptoms was 14 days. Thrombosis presentation was acute in 41 %, chronic in 44 %, and acute on chronic in 15 %. Clinically, 33 % were asymptomatic, and abdominal pain was the most common presentation at 62 %. The most common acquired thrombophilia was chronic myeloproliferative neoplasms at 18 %, followed by antiphospholipid syndrome at 6 %. The most used diagnostic method was triphasic CT at 58 %, followed by ultrasonography at 35 %. Sixty-six percent of the patients received anticoagulation, with warfarin being the main anticoagulant used at 56 %. Sixteen percent experienced some type of bleeding, although none were major. Conclusions. Portal vein thrombosis remains, in many cases, an incidental finding. An unusual number of chronic myeloproliferative neoplasms were found. Anticoagulation is safe and effective, although the use of direct oral anticoagulants remains restricted.

Background: Hypertension, often referred to as the "silent killer," usually presents no symptoms, making many people unaware of their condition. Fishing, as a profession, poses unique health risks due to harsh environmental conditions, irregular work patterns, and strenuous physical demands. Fishermen are particularly vulnerable due to unhealthy diets, sleep disturbances, physical inactivity, tobacco and alcohol use, and obesity. Estimating the prevalence of hypertension among fishermen is crucial for understanding the extent of this hidden health issue within this high-risk group and for implementing targeted interventions to improve their health outcomes. Methods: A cross-sectional study was conducted among Chennai's fishermen using systematic random sampling. Data were collected using a semi-structured questionnaire, and blood pressure was recorded following JNC criteria. Results: The study included 180 fishermen with a mean (SD) age of 49 (12). The prevalence of hypertension was 38.9%. Significant associations were found between hypertension, age, and years of fishing experience. Notably, 77.2% of fishermen lacked knowledge about hypertension, and 86.1% were unaware of its risk factors. Isolated systolic hypertension was found in 16% of participants, while 36% had isolated diastolic hypertension. Conclusions: In Chennai, hypertension is highly prevalent among fishermen and is significantly associated with age, years of fishing experience and smoking. There was no significant association with family history of hypertension, stress, or fried fish consumption. Awareness and knowledge about hypertension were very low among the fishermen. Although isolated systolic and diastolic hypertension were relatively minimal, regular hypertension screening for this community is essential.

Background: Chandra Nadi Shuddhi Pranayama is a type of alternate nostril breathing which is the control-positive inhalation and exhalation process. Alternate nostril breathing technique shows the best result in health improvement especially in controlling the blood pressure. It activates the parasympathetic nervous system which dilates blood vessels, decreases the heart rate which leads to reduce blood pressure. Many studies have been done to see effects of breathing exercises on cardiovascular system however, no such studies were done to see specific and immediate effect of Chandra Nadi Shuddhi on cardiovascular system. In this article, we will look how Chandra Nadi Shuddhi improves blood pressure, heart rate, and rate pressure product. Methods: Ethical clearance and participant consent was taken. Study design was experimental study. The 80 subjects were taken by convenient sampling. Participants were asked to perform 27 rounds of the Chandra Nadi Shuddhi Pranayama and outcome measures were assessed before and immediately after performing Chandra Nadi Shuddhi. The normality of the data was assessed using parametric paired t test. Results: Immediate effect of Chandra Nadi Shuddhi Pranayama showed that there was statistically significant decrease in blood pressure, heart rate and rate pressure product (p=0.0001, 0.0045, 0.0001 respectively) Conclusions: There is immediate effects of Chandra Nadi Shuddhi Pranayama on individuals with normal blood pressure.These results suggest a possible role for this technique in helping to control hypertension, lower anxiety and depression, improve breathing and immunity.

Background: We studied the gynaecological, obstetrical and family history, complications arisen, timing and mode of delivery, and post-delivery maternal and neonatal health conditions for preeclamptic patients in Rajshahi, Bangladesh. Methods: Using a cross-sectional longitudinal design, 90 women hospitalized with preeclampsia in 7 hospitals of Rajshahi, Bangladesh were considered, of which two-thirds were from tertiary referral Rajshahi medical college hospital. The data were collected by interviewing the patients, physical examinations and analyzing patients� pathological profile. For statistical analyses, SPSS software was employed. Results: The 58% of the preeclamptic patients became pregnant earlier, of which 56% had NVD, 24 % C/S and 20% abortion. The 85% patients had history of chronic constipation, ashma, blood transfusion, UTI, hypertension, liver disease, diabetes and preeclampsia. Regarding past surgical history, 40% had Appendisectomy, DE and C, MR, left Salphingo-oophorectomy and CS. Their gestational age ranged 40-32 weeks, averaging 37 weeks. About three-fourths of the patients� deliveries were made by CS. Generally, patients� BP fell down after delivery. One patient out of 88 had died after giving birth, but her female infant (weighing 2.0 kg) was in good condition. Only one case of twin-pregnancy was recorded. After giving births, 28% of mother had no complications, whereas 56% had mild complications and 16% had severe complications. Male children dominated (60%) over female children (40%). A total of 9 (10%) neonatal deaths were recorded. Among alive infants, 42% were premature. About 28% of the newborn infants had no complications, while 16% had mild complications and 56% had severe complications including asphyxia, IUGR, etc. Conclusions: After delivery (CS was 75%), mothers� health was good, but the infants� health deteriorated. Consequently, 1% maternal death and 10% neonatal deaths were recorded.

RESUMEN Introducción . El consumo excesivo de sal (cloruro de sodio, NaCl) en la dieta conduce al desarrollo de hipertensión arterial (HTA) y daño de órgano blanco. Se sabe que los canales ClC-K1 y ClC-5 son reguladores esenciales del anión cloruro (Cl-), pero la contribución de este anión a los efectos deletéreos de la sal es aún desconocida. Objetivo . El objetivo de este trabajo fue evaluar la participación del Cl- en la respuesta inflamatoria y oxidativa renal y en el desarrollo de HTA. Material y métodos . Ratas Wistar macho se dividieron en cuatro grupos (n=8/grupo) y se alimentaron con diferentes dietas durante 3 semanas. control (grupo C); NaCl 8 % (grupo NaCl); dieta alta en Na+. citrato de sodio (Na3C6H5O7) 11,8 % (grupo Na); dieta alta en Cl-. cloruro de calcio (CaCl2) 3,80 %, cloruro de potasio (KCl) 3,06 % y cloruro de magnesio (MgCl2) 1,30 % (grupo Cl). Se determinó la presión arterial sistólica (PAS), función renal, marcadores de estrés oxidativo y de inflamación en corteza renal, y la expresion renal de los canales de cloruro ClC-K1 y ClC-5. Resultados . Se observó un aumento de la PAS, actividad de glutatión peroxidasa (GPx) y expresión renal de factor nuclear kappa B (NFkB) y receptor de angiotensina II tipo 1 (AT1R) en los grupos NaCl y Cl- (p<0,05). La producción de sustancias reactivas del ácido tiobarbitúrico (TBARS) aumentó en los grupos experimentales con respecto a C. La expresión de la proteína de Parkinson 7 (PARK7) disminuyó en el grupo Cl en comparación con C (p< 0,05). Los grupos NaCl y Cl- mostraron una mayor expresión de ClC-K1, mientras que ClC-5 se redujo en el grupo NaCl en comparación con C (p<0,05). Conclusión . El Cl- sería corresponsable, junto con el Na+, de desencadenar daño oxidativo e inflamatorio renal y aumentar la presión arterial; por ello se deduce la importancia de reducir la ingesta de ambos iones como medida preventiva no farmacológica para la prevención y control de la HTA. El rol de los canales ClC-K1 y ClC-5 como mediadores de este proceso queda aún por confirmarse.
ABSTRACT Background . Excessive consumption of salt (sodium chloride, NaCl) in the diet leads to the development of hypertension (HTN) and target organ damage. It is known that the ClC-K1 and ClC-5 channels are essential regulators of the chloride (Cl-) anion, but the contribution of this anion to salt-harmful effects remains unknown. Objective . The aim of this study was to evaluate the participation of Cl- in the renal inflammatory and oxidative response and in the development of HTN. Methods . Male Wistar rats were divided into four groups (n=8/group) and fed with different diets for 3 weeks. control (C group); NaCl 8% (NaCl group); high Na+ diet. sodium citrate (Na3C6H5O7) 11.8% (Na group); high Cl- diet. calcium chloride (CaCl2) 3.80%, potassium chloride (KCl) 3.06% and magnesium chloride (MgCl2) 1.30% (Cl group). Systolic blood pressure (SBP), renal function, oxidative stress and inflammation markers in the renal cortex, and renal expression of the chloride ClC-K1 and ClC-5 channels were assessed. Results . An increase in SBP, glutathione peroxidase (GPx) activity, and renal expression of nuclear factor kappa B (NFkB) and angiotensin II type 1 receptor (AT1R) were observed in the NaCl and Cl groups (p<0.05). The production of thiobarbituric acid reactive substances (TBARS) increased in the experimental groups compared with C. The expression of Parkinson disease protein 7 (PARK7) decreased in the Cl group compared with C (p< 0.05). The NaCl and Cl groups showed increased expression of ClC-K1, while ClC-5 was reduced in the NaCl group compared with C (p<0.05) Conclusion . Cl- would be co-responsible together with Na+ in triggering oxidative and inflammatory kidney damage and increasing blood pressure. This indicates the importance of reducing the intake of both ions as a non-pharmacological preventive measure for the prevention and control of HTN. The role of ClC-K1 and ClC-5 channels as mediators of this process remains to be confirmed.
ABSTRACT
RESUMEN Introducción . La hipertensión pulmonar (HP) se asocia con elevada morbimortalidad y su pronóstico está determinado por la función del ventrículo derecho y el acople ventrículo-arterial pulmonar. Diversos parámetros hemodinámicos han sido propuestos para caracterizar el riesgo de evolución adversa. Objetivos . Determinar la capacidad de predicción de eventos graves, de diversos parámetros hemodinámicos medidos por cateterismo derecho. Material y métodos . Estudio multicéntrico, descriptivo, de cohorte retrospectiva de pacientes con HP. Se dividió a los datos hemodinámicos en alto o bajo riesgo según lo definido por las recomendaciones europeas de 2022, o según media o mediana de nuestra población. Resultados . Fueron incluidos 324 pacientes con HP, con edad media 61,5 años y 69% de sexo femenino; 62,1 % de los pacientes del Grupo 1 y 19,2 % del Grupo 3. La tasa de eventos (muerte o internaciones) en un seguimiento mediano de 23 (RIC 14-44) meses fue 60 % y la mortalidad global 24,5 %. Los valores de corte para definir riesgo de las diferentes variables fueron. índice cardíaco (IC) 2,72 L/min/m2, índice de volumen sistólico (IVS) 33,1 mL/lat./m2, resistencia vascular pulmonar (RVP) 6 unidades Wood (UW), índice de pulsatilidad de la arteria pulmonar (IPAP) 3,76, índice de trabajo sistólico del ventrículo derecho (ITSVD) 11,6 g.m/m2, compliance vascular pulmonar 1,84 mL/mmHg. La RVP elevada se asoció a mayor frecuencia de eventos combinados, mientras que valores de alto riesgo de IC, IVS, RVP y compliance presentaron asociación con mayor mortalidad en el seguimiento. Conclusiones . Si bien el IC y el IVS presentaron una adecuada predicción de riesgo, variables como la RVP y la compliance mostraron similar, e incluso mejor predicción de eventos graves en HP. Se necesitan nuevos estudios que validen estos parámetros en el seguimiento.
ABSTRACT Background. Pulmonary hypertension (PH) is associated with high morbidity and mortality and its prognosis is determined by the right ventricular function and right ventricular-pulmonary artery coupling. Several hemodynamic parameters have been proposed to better characterize the risk for adverse disease progression. Objective. The aim of this study was to determine the predictive ability of various hemodynamic parameters that can be calculated during RHC to predict serious events. Methods. In this retrospective multicenter and descriptive cohort study of patients with PH, hemodynamic variables values were divided as high or low risk based on the 2022 European guidelines, or mean or median values in our population. Results. A total of 324 patients with PH were included; mean age was 61.5 years and 69% were women; 62.1 % of the patients were in Group 1 and 19.2 % in Group 3. In a median (IQR) follow-up of 23(14-44) months, the event rate (death or hospitalization for heart failure) was 60.6% and all-cause mortality was 24.5%. The cut-off values associated with risk for the different hemodynamic variables were: cardiac index (CI) 2.72 L/min/m2, systolic volume index (SVI) 33.1 ml/m2, pulmonary vascular resistance (PVR) 6 Wood units (WU), pulmonary artery pulsatility index (PAPI) 3.76, right ventricle systolic work index (RVSWI) 11.6 g.m/m2, and pulmonary arterial compliance 1.84 mL/mm Hg. Elevated PVR was associated with a higher rate of composite events, while high-risk CI, SVI, PVR, and compliance were associated with higher mortality at follow-up. Conclusions. While CI and SVI are adequate predictors of risk, PVR and compliance demonstrate similar or even better risk prediction in patients with PH. Additional research is necessary to validate these parameters during follow-up.

RESUMEN La comprensión de los mecanismos para el desarrollo de la ascitis ha evolucionado a lo largo de los años, involucrando al hígado, peritoneo, corazón y riñones, pero no al intestino, como responsable clave en su formación. Poco se ha descripto tanto de su papel fisiopatológico como de las posibles alternativas terapéuticas clínicas y quirúrgicas. A fin de validar empíricamente este concepto se relata la situación de un hombre de 39 años, con trombosis aislada de la vena mesentérica superior y fallo intestinal. La necesidad de realizar una derivación quirúrgica nos llevó a construir un shunt desde una vena colateral de la mesentérica superior a la vena esplénica. La reducción de la presión del sistema mesentérico superior de 35 a 6 mm Hg sirvió para la resolución de los signos y síntomas clínicos en un mes. Este caso obliga a revisar conceptos fisiopatológicos básicos sobre la producción de ascitis, contribuye a la descripción de un nuevo tipo de shunt y amplía las opciones terapéuticas quirúrgicas en el tratamiento de la hipertensión venosa esplácnica.
ABSTRACT The understanding of the mechanisms for the development of ascites has evolved over the years to include the liver, peritoneum, heart and kidneys, but not the intestine, as key players in its formation. The pathophysiological role of the intestine as well as the possible clinical and surgical therapeutic options have been poorly described. In order to empirically validate this concept, we report the situation of a 39-year-old man with isolated thrombosis of the superior mesenteric vein and intestinal failure. The need to perform a surgical shunt led us to construct a shunt from a collateral vein of the superior mesenteric vein to the splenic vein. The reduction in pressure in the superior mesenteric venous system from 35 to 6 mm Hg resulted in resolution of clinical signs and symptoms within one month. This case study forces us to review the basic pathophysiological concepts of ascites formation, contributes to the description of a new type of shunt, and expands the surgical therapeutic options in the treatment of splanchnic venous hypertension.

Resumen Introducción : Los dispositivos electrónicos y tecno logías de la comunicación se emplean cada vez más frecuentemente para brindar atención médica a los pacientes con hipertensión arterial. El objetivo prin cipal del trabajo fue evaluar la percepción de los mé dicos sobre la utilidad de las tecnologías digitales en hipertensión arterial. Los objetivos secundarios fueron evaluar la prevalencia de uso de tecnologías digitales en el seguimiento de pacientes con hipertensión arterial e identificar las barreras percibidas por los médicos para la implementación de las mismas en la República Argentina. Métodos : Estudio observacional, de corte transversal, multicéntrico, basado en un cuestionario autoadminis trado, en el ámbito de la República Argentina y coordi nado por el grupo de Epidemiología de la Sociedad Ar gentina de Hipertensión Arterial. Población: Médicos que realizan atención de pacientes con hipertensión arterial. Resultados : Se obtuvieron 247 respuestas, 35.6% fue ron mujeres y 64.4% hombres; el 92.3 % consideró de utilidad el uso de tecnologías digitales para mejorar la adherencia terapéutica. Reciben resultados de presión arterial en domicilio por algún medio digital 54.7%. Realizan habitualmente teleconsultas 41.3%, solamente el 6.1% por plataformas digitales específicas de salud, el 29.1% por plataformas no específicas, telefónicamente 21.9% y correo electrónico (asincrónicas) 10.9%. Las principales barreras percibidas fueron: disponibilidad, remuneración económica, reglamentación, problemas de conectividad y falta de tiempo. Conclusión : La percepción de los profesionales es favorable, considerándolas útiles para mejorar la ad herencia terapéutica, por lo que podría tener un efecto facilitador en la implementación de las mismas, aunque las barreras percibidas forman parte de cuestiones or ganizacionales a resolver.
Abstract Introduction : Electronic devices and communication technologies are increasingly used to provide medical care to patients with high blood pressure. The main objective was to evaluate doctors' perception on the usefulness of digital technologies in hypertension. The secondary objectives were to evaluate the prevalence of use of digital technologies in the follow-up of patients with hypertension and to identify the barriers perceived by doctors for their implementation in the Argentine Republic. Methods : Observational, cross-sectional, multicenter study, based on a self-administered questionnaire, in the Argentine Republic, and coordinated by the Epidemiol ogy group of the Argentine Society of Arterial Hyper tension. Population: Doctors who care for patients with high blood pressure. Results : 247 responses were obtained, 35.6% were women and 64.4% men; 92.3% indicated that the use of digital technologies was useful to improve therapeutic adherence; 54.7% received blood pressure results at home through some digital means; 41.3% regularly carry out teleconsultations, only 6.1% through specific digital health platforms, 29.1% through non-specific platforms, by telephone 21.9% and by email (asynchronous) 10.9%. The main perceived barriers were: availability, financial remuneration, regulations, connectivity problems and lack of time. Conclusion : The perception of professionals is fa vorable, as they considered these technologies useful to improve therapeutic adherence, which could have a facilitating effect on their implementation, although the perceived barriers are part of organizational issues to be resolved.

Resumo O objetivo do artigo é avaliar a prevalência de adesão ao tratamento anti-hipertensivo na população brasileira, com base nos estudos revisados por pares, que utilizaram instrumentos elaborados e/ou adaptados exclusivamente para este fim. Revisão sistemática com meta-análise, baseada nas recomendações do Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). A busca foi realizada nas bases BDENF, SciELO, Cuiden, PsycINFOe, CINAHL, Embase, LILACS, MEDLINE, e nos buscadores acadêmicos AgeLine, Google Scholar e ScienceDirect. O protocolo foi registrado no PROSPERO (CRD42021292689). Modelos de efeitos aleatórios foram usados para meta-análise das prevalências obtidas dos estudos individuais. Incluíram-se 104 estudos na meta-análise sobre tratamento anti-hipertensivo na população brasileira, totalizando 38.299 pacientes. O instrumento mais utilizado foi o teste de Morisky-Green de quatro itens (49,5%). A prevalência de adesão estimada pela foi de 44,4% (IC95%: 39,12%-49,94%, I2 = 91,17, p < 0,001), apresentando alta heterogeneidade. A prevalência de adesão ao tratamento anti-hipertensivo encontrada nos estudos nacionais foi insatisfatória, demonstrando que essa problemática continua sendo um grande desafio.
Abstract This article aims to evaluate the adherence to antihypertensive treatment prevalence in the Brazilian population based on peer-reviewed studies which used instruments exclusively designed and/or adapted for this purpose. A systematic review with meta-analysis based on the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). The search was carried out in the BDENF, SciELO, Cuiden, PsycINFOe, CINAHL, Embase, LILACS, and MEDLINE databases, as well as the AgeLine, Google Scholar and ScienceDirect academic search engines. The protocol was registered with PROSPERO (CRD42021292689). Random effects models were used for a meta-analysis of the prevalence obtained from individual studies. A total of 104 studies were included in the meta-analysis on antihypertensive treatment in the Brazilian population, totaling 38,299 patients. The most used instrument was the four-item Morisky-Green Test (49.5%). The adherence prevalence estimated by the meta-analysis was 44.4% (95%CI: 39.12%-49.94%, I2 = 91.17, p < 0.001), showing high heterogeneity. The adherence to antihypertensive treatment prevalence found in national studies was unsatisfactory, demonstrating that this problem continues to be a major challenge.

Hypertension is often regarded as a silent killer because the disease often causes no symptoms in the sufferers. This disease does not only need treatment, but family support is required to prevent and treat family members who are suffering. The objective of the study is to analyze the application of warm water for foot bath therapy to lower blood pressure in people suffering from hypertension. The design used is descriptive with case studies, through family nursing process. Monitoring of warm water foot bath was carried out for 3 days, before and after measuring the blood pressure Results: there was a decrease in blood pressure after 3 days of action. In family 1, the initial blood pressure was 160/90 mmHg, after 3 days of action it decreased to 145/85 mmHg. Family 2 before therapy 150/100 mmHg, decreased to 140/86 mmHg after soaking feet in warm water. The decrease in blood pressure observed in both families was more than 10 mmHg. The application of foot bath therapy using warm water which is carried out for 3 days, is effective in reducing blood pressure in hypertension cases. Suggestions are recommended for families and subjects to be able to apply independently warm water foot bath therapy and carry out routine blood pressure checks at the nearest healthcare facility.

Background: Hypertension is a major public health problem. Globally 1.28 billion adults aged 30-79 years have hypertension and among this 46% were undiagnosed. Kerala is one state undergoing epidemiological transition and having high prevalence of hypertension. So, we did this study to estimate the proportion of hypertension among non-medical employees of Medical College Thiruvananthapuram. Methods: A cross-sectional study was conducted among 202 non-medical employees of medical college Thiruvananthapuram. Data was collected using an investigator administered questionnaire. Anthropometric indices were measured and recorded. Blood pressure (BP) was recorded twice in a gap of 15 min for each participant in sitting position using mercury sphygmomanometer. Mean systolic BP (SBP) and diastolic BP (DBP) was calculated to confirm hypertension. Participants those who were already detected as hypertensive and on treatment were also noted. Data was analysed using SPSS software version 27. Results: Prevalence of hypertension in the study population was 61/202 (30.19%). Among the hypertensive 39 (63.9%) were already detected and on treatment and 22 (36.1%) were undiagnosed. Among already detected hypertensive 13 (33.3%) were found to be inadequately treated. Hypertension was found to be associated with waist circumference and BMI. Conclusions: The study population was found to have high prevalence of hypertension and having association with waist circumference and BMI. The rule of halves for depicting the trend of hypertension in study population can be used as a tool to deduce various interventions in the community.

Background: Chronic kidney disease (CKD) is public health burden and rising rapidly in prevalence in developing countries especially in Sub-Saharan Africa. CKD increases with advancing age and comorbidities like hypertension, diabetes and obesity. However, there is no data on CKD the prevalence and risk factors associated among the hypertensive adult patients in Wajir county. This study aimed at estimating the prevalence and establishing risk factors associated with CKD among hypertensive patients in Wajir county and sought to recommend viable preventive measures. Methods: Analytical cross-sectional study design utilized, non-probability consecutive sampling was adopted to obtain sample size of 293 respondent attending medical outpatient clinic at Wajir county referral hospital between September to December 2023. The study utilised primary data that were collected using structured questionnaires and secondary data from the laboratory result. Results: Key findings revealed majority of hypertensive patients have CKD (45.40%), highlighting a critical health concern in the region. The study found most CKD cases were in early stages (stages 1 and 2), some had progressed to severe stages (stages 4 and 5). Regression analysis identified several demographic and clinical factors associated with CKD development, including age, gender, and education, and employment status, family history of CKD and difficulty in medication compliance. Conclusions: there is high prevalence of chronic kidney disease among hypertensive adults in Wajir county. With nearly half of hypertensive patients also suffering from CKD, there is an undeniable public health crisis at hand in Wajir county. Study recommended initiation of targeted screening programs.

Background: The aim of this research was to study the chronotherapy in patients with differentiated dipper and nondipper hypertension, researching the differences between sodium in 24 hours urine and serum urotensin II. Methods: About 32 nondipper and 29 dipper hypertension patients who have study admission criteria, sodium levels in 24 hours urine, serum urotensin II levels and routine biochemical laboratory tests of were studied. The statistical relationship between the results was analyzed. Results: In the patient group we examined, nondipper patients were found older and predominantly seen in the female patient population. We determined that the dipper patients differed significantly in family history from nondipper patients. Although it is not statistical significance, we observed accordance of treatment who nondipper patients was worse, we have found that serum urotensin II levels can be used at this distinction. Conclusions: It is important to distinguish nondipper and dipper hypertension in terms of the prognosis of hypertension and the efficacy of the treatment. For this reason, we want to underline that nondipper patients are older and predominantly seen in the female patient population. Although left ventricular mass index is important in the course and outcome of hypertension, we determined that is was not useful in distinguishing dipper-nondipper hypertension. We think that serum urotensin II levels will be useful in determining the efficacy of the treatment used, the prognosis of hypertension and the differentiation of nondipper-dipper hypertension.

Background: Predicting esophageal varices in cirrhosis using the measurement of splenic stiffness is an exciting area of study. Vein enlargement in the lower esophagus, known as varices, is a common symptom of cirrhosis caused by portal hypertension. These varicose veins are potentially fatal since they bleed easily. The present study aims to predict the OV in cirrhosis patients by splenic stiffness in and around this region. Methods: After receiving permission from the Institutional Ethics Committee, the study was performed. The present study authors measured splenic stiffness and liver stiffness by using FibroScan in 112 consecutive cirrhotic patients who met the inclusion criteria. Patients were also assessed by hepatic venous pressure gradient (HVPG), upper gastrointestinal endoscopy, liver stiffness, liver spleen diameter to platelet ratio score, and platelet count to spleen diameter ratio. Results: Among 112 patients enrolled, 64 patients had valid liver stiffness and splenic stiffness measurement, and 64 patients had EV (small, n=23 and large n=42). There was a significant difference in median liver stiffness (54.2 vs. 21.3 kPa, p<0.05), splenic stiffness (56.1 vs. 30 kPa, p<0.05), liver spleen diameter to platelet ratio score (6.3 vs. 2.7, p<0.05), and platelet count to spleen diameter ratio (798 vs 1,241, p<0.05) between patients with OV and those without OV. Conclusions: Non-invasive markers, such as splenic stiffness, may help identify individuals with cirrhosis who are at risk of having esophageal varices, especially large ones, and who are at risk of bleeding, the study's authors conclude.

Background: Pregnancy induced hypertension in women is a major cause of pregnancy related maternal, fetal, and neonatal morbidities and mortalities. Aim was to compare the clinical and hematological profile of newborns of mothers with and without pregnancy induced hypertension.Methods: This hospital-based case control study was conducted in Lala Lajpat Rai Hospital, Kanpur. Clinical and hematological profile of newborns of mothers with or without PIH were compared.Results: The low gestational age (35.44�89 weeks), low birth weight, thrombocytopenia (1.31�65 lakhs/mm3), absolute neutropenia (7.71�3�24�3cells/mm3) and deranged coagulation profile in newborns was found to have significant association with PIH in their mothers (p<0.05).Conclusions: This study concluded that newborns of hypertensive mothers carry a risk for prematurity, small for gestational age, infection and bleeding in early neonatal life.

Background: Right ventricular (RV) dysfunction is prevalent in children with congenital heart disease (CHD), particularly in association with pulmonary hypertension (PH). Transthoracic echocardiography (TTE) is crucial for detecting, assessing severity, and monitoring CHD patients with PH. The current study aimed to compare RV function assessed by TTE between CHD patients with PH and healthy children.Methods: The case group comprised CHD patients diagnosed with PH via TTE and confirmed by cardiac catheterization. The control group included children with normal echocardiograms. A total of 40 patients were enrolled based on predefined criteria.Results: The study included 20 cases and 20 controls. Echocardiographic parameters revealed significant differences between the case and control groups, with increased RV dimensions in the case group, except for RV subcostal wall thickness. RV systolic function parameters, including tricuspid annular plane systolic excursion (TAPSE) and tricuspid annular systolic velocity (S'), were significantly lower in the case group. RV diastolic function parameters, including E/A ratio, E/e', and deceleration time (DT), were also significantly different between the groups. The RV global function parameter, tissue Doppler imaging-derived myocardial performance index (TD MPI), was significantly higher in the case group.Conclusions: Echocardiographic evaluation of RV function, including TAPSE, S', TD MPI, E/A ratio, E/e', and DT, in CHD children with PH correlates with PH severity. This assessment can aid in early detection of PH severity before irreversible changes occur and serve as an alternative to right heart catheterization.
