ABSTRACT

Objective To explore the value of dynamic nomogram constructed by multi spiral computed tomography(MSCT)features combined with inflammatory indicators in predicting the status of microvascular invasion(MVI)of hepatocellular carcinoma(HCC)before surgery.Methods The clinical and imaging data of 137 patients with postoperative pathologically confirmed HCC were analyzed retrospectively.According to the status of the MVI,they were divided into positive group(44 cases)and negative group(93 cases).Multivariate logistic regression analysis was used to screen independent risk factors for predicting the MVI status of HCC patients,and a joint prediction model was constructed,which was displayed in the form of a dynamic nomogram.The receiver operating characteristic(ROC)curve,calibration curve and Hosmer-Lemeshow test were used to evaluate the diagnostic efficiency,calibration and goodness of fit of the model,Akaike information criterion(AIC)and Bayesian information criterion(BIC)were used for comparison between the models,and a 5-fold cross-validation and decision curve analysis(DCA)were also used to evaluate the stability and clinical applicability of the model.Results Multivariate logistic regression analysis showed that necrosis and delayed-phase enhancement(DEd),and alkaline phosphatase to lymphocyte ratio(ALR)were independent risk factors for predicting MVI status in HCC patients.The area under the curve(AUC)of the dynamic nomogram was 0.721,with the sensitivity of 0.705 and the specificity of 0.656.The AIC and BIC values were 152.372 and 158.212,respectively.The calibration curve and the Hosmer-Lemeshow test showed that the model had a high degree of calibration and goodness of fit(χ2=2.372,P=0.967),the average AUC of the 5-fold cross-validation was 0.787,and the DCA showed that the nomogram model had a good clinical applicability.Conclusion The dynamic nomogram model constructed by MSCT features combined with inflammatory indicators is feasible to predict the MVI status of HCC patients before surgery,and the dynamic nomogram can directly generate the prediction results of different individuals.

ABSTRACT

Con el objetivo de identificar la relación entre el consumo de frutas y vegetales, los indicadores del estrés oxidativo e inflamación se realizó un estudio transversal con 39 adolescentes. De los indicadores del estrés oxidativo se determinaron en el suero sanguíneo el malondialdehído, los productos avanzados de la oxidación de proteínas y el poder reductor férrico y en eritrocitos el glutatión reducido y como indicador de inflamación el recuento leucocitario y el conteo diferencial. Se encontraron asociaciones significativas a través de los cuartiles del consumo de vegetales y frutas con el potencial reductor férrico y el glutatión reducido con aumento de ambos, así como, con el malondialdehído y los productos de la oxidación avanzada de proteínas, con disminución de estos. Se concluye que el mayor consumo de vegetales y frutas se asoció con variaciones en los biomarcadores de estrés oxidativo, con aumento de los indicadores de las defensas antioxidantes y disminución de los de daño oxidativo inflamatorio.


SUMMARY With the objective of identifying the relationship between the consumption of fruits and vegetables, the indicators of oxidative stress and inflammation, a cross-sectional study was carried out with 39 adolescents. Of the indicators of oxidative stress, malondialdehyde, advanced products of protein oxidation and ferric reducing power were determined in the blood serum, and reduced glutathione in erythrocytes and, as an indicator of inflammation, the leukocyte count and the differential count. Significant associations were found across the quartiles of vegetable and fruit consumption with ferric reducing potential and reduced glutathione with an increase in both, as well as with malondialdehyde and products of advanced protein oxidation, with a decrease in these. It is concluded that the greater consumption of vegetables and fruits was associated with variations in the biomarkers of oxidative stress, with an increase in the indicators of antioxidant defenses and a decrease in those of inflammatory oxidative damage.


Com o objetivo de identificar a relação entre o consumo de frutas e hortaliças, os indicadores de estresse oxidativo e inflamação, foi realizado um estudo transversal com 39 adolescentes. Dos indicadores de estresse oxidativo, malondialdeído, produtos avançados de oxidação de proteínas e poder redutor férrico foram determinados no soro sanguíneo, e glutationa reduzida em eritrócitos, e contagem de leucócitos e contagem diferencial foram indicadores de inflamação. Foram encontradas associações significativas entre os quartis de consumo de hortaliças e frutas com potencial redutor de ferro e glutationa reduzida com aumento de ambos, bem como com malondialdeído e produtos avançados de oxidação proteica, com diminuição destes. Conclui-se que o maior consumo de hortaliças e frutas esteve associado a variações nos biomarcadores de estresse oxidativo, com aumento dos indicadores de defesas antioxidantes e diminuição dos de dano oxidativo inflamatório.

ABSTRACT

Background:Nutritional status and inflammatory indicators in patients with gastrointestinal tumors have a significant impact on patient prognosis.Aims:To investigate the association between preoperative nutritional/inflammation-related indicators and the prognosis of colorectal cancer patients underwent radical surgery,and to identify valuable prognostic assessment indicators.Methods:Colorectal cancer patients underwent radical surgery from 2018 to 2020 were enrolled,and a patient cohort was established.Clinical data were collected,correlation of preoperative nutritional/inflammation-related indicators with prognosis were analyzed,and predictive nutritional/inflammation-related indicators were screened.Results:A total of 146 patients were collected in this study.The results of Cox proportional risk model univariate analysis showed that family history of tumor,TNM staging,CEA,AGR,PLR,and SII were significantly associated with survival(P<0.05).Multivariate analysis revealed that preoperative AGR(HR=0.227,95%CI:0.087-0.595;P=0.003),NLR(HR=2.472,95%CI:1.058-5.778;P=0.037),PLR(HR=2.692,95%CI:1.162-6.235;P=0.021)and SII(HR=5.237,95%CI:1.857-14.771;P=0.002)were associated with prognosis of colorectal cancer patients,and AGR had the best prognostic ability.Conclusions:Four preoperative nutritional/inflammatory indicators,AGR,NLR,PLR,and SII,are associated with prognosis of colorectal cancer patients underwent radical surgery.

ABSTRACT

ABSTRACT BACKGROUND: Acute ischemic stroke (AIS) is the most common type of stroke. Inflammation is the primary factor in the pathogenesis of atherosclerosis. Use of immature granulocytes (IGs) has been recommended as a new indicator of systemic inflammation. However, data on the association between echocardiographic epicardial fat tissue thickness (EFT) and IGs in patients with AIS are limited. OBJECTIVE: To evaluate the association between the presences of IGs, epicardial fat tissue and AIS. DESIGN AND SETTING: Prospective study in a tertiary-care university hospital in Antalya, Turkey. METHODS: Our study included 53 AIS patients and 41 healthy controls with age and gender compatibility. Blood samples and transthoracic echocardiography of all participants were compared. RESULTS: IG levels were significantly higher in patients with AIS than in controls (0.62 ± 0.36 versus 0.28 ± 0.02, P < 0.001). The mean EFT was 3.74 ± 0.61 mm in the control group and 6.33 ± 1.47 mm in the AIS patient group. EFT was significantly greater in AIS patients than in controls (P < 0.001). For the optimum cut-off value for IG (0.95), the area under the curve (AUC) was determined to be 0.840; sensitivity was determined to be 81.1% and specificity, 92.5%. For the optimum cut-off value for EFT (4.95 mm), the AUC was determined to be 0.953; sensitivity was determined to be 90.6% and specificity, 90%. CONCLUSIONS: IG and echocardiographic EFT are clinical markers that can be used to predict AIS risk.

ABSTRACT

Objective:To analyze any changes in the functional connectivity between the seed points of the dorsolateral prefrontal cortex (DLPFC) and the whole brain, as well as any fluctuations in the low-frequency amplitude among persons with post-stroke depression (PSD). The aim was to develop correlations among functional imaging results, clinical scales, and inflammation indicators including high-sensitivity C-reactive protein (hs-CRP), interleukin 6 (IL-6), interleukin 2 (IL-2), interleukin 10 (IL-10), interleukin 17a (IL-17a) and interferon-γ (IFN-γ).Methods:Between 2016 and 2020, 55 ischemic stroke survivors were tested. The 28 scoring 7 or more on the Hamilton Depression Scale (HAMD-17) formed the PSD group, while the 27 others formed the control group. Functional magnetic resonance images were collected, and serum inflammation indicators were determined.Results:When seed points in the left DLPFC were used, in the PSD group the frontal cortex (FC) decreased in one cluster, with a voxel of 129mm3 and the MNI coordinates (x=9, y=30, z=33) indicating that the anatomical automatic labeling (AAL) brain regions were the Cingulum_Ant_L, Cingulum_Mid_R and the frontal_Sup_Medial_L. When the right DLPFC was used as the seed point the FC again decreased in one cluster, with voxels of 44mm 3 and the MNI coordinates (x=-27, y=12, z=47) referring to the AAL brain region of the frontal_Mid_L. In the PSD group, the FC value of abnormal brain areas with the R-DLPFC as the seed point was positively correlated with time since stroke. In the control group, the FC value of abnormal brain areas with L-DLPFC as the seed point was negatively correlated with MoCA, while with R-DLPFC as the seed point it was positively correlated with IFN-γ. The FC values of abnormal areas of the brain showed no significant correlation with other clinical scales, inflammation indicators or lesion volume. Conclusion:Abnormal functional connections within the executive control network and between the salience networks may participate in the mechanism of PSD, and may be related to the time since stroke, cognitive functioning, and IFN-γ levels.
