ABSTRACT

The present study analyzes the psychometric properties of the Inventory of Family Integration (IFI) by means of a dyadic analysis based on the responses of fathers and mothers in families from the city of Arequipa, southern Peru. For this purpose, 264 married couples living with their families were non-probabilistically selected and administered the 52-item IFI. An analysis of dimensionality and reliability was carried out, and a factorial invariance analysis was applied under a dyadic approach and, finally, a comparative analysis using Cohen's d was performed. The results reported moderate correlations between the values of fathers and mothers in each family holon: personal, spousal, parental, sibling and familial. Likewise, the dimensionality fit of each holon in fathers and mothers was adequate with acceptable magnitudes, and the reliability indices calculated with Cronbach's alpha and McDonald's Omega tests were high. The five-factor internal structure and internal consistency in both fathers and mothers were corroborated.

ABSTRACT

Objective: draw parameters to structure concepts of the One Health approach through the thoughts of Alfred North Whitehead, Arthur George Tansley, Amartya Sen, and Norberto Bobbio. Methodology: this was original research, with a deductive approach, hermeneutic bias based on the selected thoughts and the One Health. Results: One Health is structured on the holistic and integrated assertion that human, animal, and environmental health are interconnected. From Whitehead's perspective, the One Health approach can be considered a dynamic and relational process, where humans, animals, and the environment constantly interact, interconnecting through relationships and processes, forming a whole. From Tansley's perspective, the idea of One Health can align with the ecosystem concept, where health cannot be analyzed in isolation in individuals, but necessarily through the complex interactions between humans, animals, and the environment. From Sen's development prism (rights and freedoms), the One Health approach can be seen to achieve it, through the interrelation of mechanisms, systems, and institutions focused on promoting health and well-being. In Bobbio's view, fundamental rights, democracy, and peace are ethical and primary ways to ensure rights, especially a right of nature (humans, animals, and the environment) in the joint pursuit of guarantees for peaceful coexistence. Conclusion: the One Health approach is not just a practical strategy, but also a renewed vision of the old perception that recognized the interconnection of all forms of life.

ABSTRACT

BACKGROUND: High-risk Human Papillomaviruses (HR-HPVs) are the etiological agents of cervical cancer and its preneoplastic lesions. HPV16 and 18 are the most frequent HR-HPV genotypes detected in cervical cancer. HR-HPV genome integration into the host cell is an important event in the carcinogenic process. However, it remains uncertain which stage of cervical carcinogenesis HPV16 and 18 integration occurs in the Chilean population. AIM: The goal of this study was to evaluate HPV16 and HPV18 integration in preneoplastic lesions of the cervix. METHODS: DNA was extracted from 108 cervical scrape samples with preneoplastic lesions. HPV was genotyped using PCR and non-radioactive hybridization. The integration status of HPV16 and HPV 18 was determined by evaluating the E2 gene presence through PCR. RESULTS: HPV16 and HPV18 tested positive in 36.1% and 12.0% of samples, respectively. HPV16 was found integrated in 23.1% of HPV 16 cases, while HPV 18 in 100% of samples positive for this viral genotype. CONCLUSIONS: HR-HPV integration is an early event in cervical carcinogenesis, occurring in nearly half of preneoplastic lesions and being more frequent in HPV18 than in HPV16. The evaluation of HR-HPV integration can be utilized as a complementary tool for detecting HPV in the Chilean population.

ABSTRACT

BACKGROUND:Bone wax is a filler that can be used for bone hemostasis.Although modification of bone wax formulations is attempted worldwide, its inertness is still the main challenge today. There is an urgent clinical need to develop novel orthopedic hemostatic materials with hemostasis, osteogenesis and antibacterial properties. OBJECTIVE:To review the development of orthopedic hemostatic materials including bone wax and its substitutes. METHODS:PubMed,Web of Science,WanFang,CNKI and VIP databases were searched for literature related to bone wax, hemostatic materials, and research progress of orthopedic hemostatic materials, and 136 articles were selected for inclusion in the review by reading the abstracts of the articles in the initial screening. RESULTS AND CONCLUSION:To replace traditional bone wax, researchers have developed various orthopedic hemostatic materials based on the needs of practical scenarios such as hemostasis and osteogenesis. However, relevant studies mostly focus on basic physical and chemical and performance tests, lack a systematic evaluation system, and lack sufficient reports of large animal experiments and clinical trials. Therefore, bone wax is still a recognized orthopedic hemostatic material at present. The fundamental reason is that the design of existing materials cannot timely meet the new needs of intraoperative hemostasis, postoperative osteogenesis and clinical practice. In the future, the structure, composition and function of existing hemostatic and osteogenic materials need to be integrated and redesigned to meet the increasing demand for hemostatic and osteogenic materials.

ABSTRACT

BACKGROUND:The appearance of the crescent sign in femoral head necrosis is a"turning point"in the progression of the disease, and repairing and stabilizing the bone-cartilage interface is particularly important in preventing further progression and collapse of the femoral head.Tissue engineering offers potential advantages in the simultaneous repair and integration of the bone-cartilage interface. OBJECTIVE:To review potentially suitable techniques addressing the subchondral separation in femoral head necrosis. METHODS:Relevant articles from January 1970 to April 2023 were searched in PubMed,Web of Science, and China National Knowledge Infrastructure(CNKI)using English search terms"femoral head necrosis, avascular necrosis of femoral head, osteonecrosis of femoral head"and Chinese search terms"femoral head necrosis, subchondral bone, cartilage, integration of cartilage and subchondral bone".A total of 114 articles were included for review and analysis. RESULTS AND CONCLUSION:(1)Structural defects, ischemic and hypoxic environment, inflammatory factors, and stress concentration may cause subchondral separation in osteonecrosis of the femoral head.Subchondral bone collapse and failure of hip-preserving surgery may be associated.Integration of tissue engineering scaffolds with the bone-cartilage interface is one potential approach for treating subchondral separation in osteonecrosis of the femoral head.(2)Current literature suggests that multiphase scaffolds, gradient scaffolds, and composite materials have shown improvements in promoting cell adhesion, proliferation, and deposition of bone and cartilage matrix. These advancements aid in the integration of scaffolds with the bone-cartilage interface and have implications for the treatment of subchondral separation in osteonecrosis of the femoral head.(3)Surface modifications of scaffolds can enhance interface integration efficiency, but they have their advantages and disadvantages.Scaffolds providing different environments can induce differentiation of mesenchymal stem cells and facilitate integration between different interfaces.(4)Future scaffolds for subchondral separation in osteonecrosis of the femoral head are expected to be composite materials with gradient and differentiated biomimetic structures.Surface modifications and stem cell loading can promote integration between the bone-cartilage interface and scaffolds for therapeutic purposes, but further experimental verification is still needed.Challenges include synchronizing scaffold degradation rate with repair progress and ensuring stability between different interfaces.

ABSTRACT

Objective:To analyze the correlation between the grouping and weighting of two sets of disease combination systems, namely diagnosis-related groups(DRG) and diagnosis-intervention packet(DIP), and to establish a multidimensional analysis and evaluation mode by applying DRG, DIP, and clinical pathway to guide the standardized diagnosis and treatment and management of disease types. Methods:DRG grouping and DIP simulation full enrollment were applied to patients discharged from a tertiary Grade A general hospital in 2019. The correlation analysis between DRG, DIP, and clinical pathway inclusion(entry), correlation analysis between relative weight of DRG group and DIP standard score, and correlation analysis between clinical pathway entry and cost structure of the two disease groups were conducted by using chi-square test, Pearson correlation analysis, t-test, structural change value, degree of structural change, and incremental contribution rate. Results:Among the 130 395 patients, 41 460 cases entered the clinical pathway, 127 535 cases were enrolled in DRG, and 104 227 cases were enrolled in DIP. There was a correlation between the enrollment of DRG, DIP, and clinical pathway( P<0.05), and there was also a correlation between the relative weight of DRG groups and the enrollment of clinical pathway. The relative weight of the DRG disease group was positively correlated with the DIP standard score( r2=0.761 7, P<0.001). There was a significant difference in hospitalization costs between patients with and without clinical pathway access for some diseases( P<0.05), and different cost categories had different impacts on the total costs. Conclusions:The weight assignment and value orientation of DRG and DIP disease types are consistent, and the multi-dimensional fusion evaluation mode for DRG-DIP-clinical pathway is feasible. The correlation analysis of DRG, DIP, and clinical pathways can serve as the basis for disease classification and cost structure evaluation, which could help to carry out hospital′s refined management and optimize disease structure.

ABSTRACT

Emphasizing the correspondence between western medicine pathology and traditional Chinese medicine pathogenesis, ZHANG Cigong proposes “dual diagnosis and single treatment”. “Dual diagnosis” refers to the determination of pathology, etiology and disease names in western medicine as well as the syndromes of traditional Chinese medicine (TCM), while “single treament” is the use of Chinese herbal medicine (CHM). By analyzing the records of gastric disease cases by ZHANG Cigong and the related discussions, this paper summarized the “dual diagnosis and single treatment” of gastric disease, including the three-step diagnosis logic of gastric disease, the correspondence between pathology and pathogenesis, etiological diagnosis and TCM syndrome differentiation, as well as CHM for the treatment of acute gastritis, gastric ulcer and hyperacidity, duodenal ulcer, chronic gastritis, dyspepsia and gastrointestinal neurosis, and upper gastrointestinal bleeding. Under the thinking mode of organic integration of logical thinking, clinical practice, and scientific research, he corresponded the basic concepts of “Qi and blood, cold and hot, warm and cool” to “substance and function” in traditional Chinese and western medicine, exploring the essence of interoperability between Chinese and Western medicine, thereby expanding the scope of TCM syndrome differentiation and its efficacy to the microscopic physiological and pathological level. It is believed that ZHANG Cigong's academic ideas provide reference to the “win-win” approach of modern integration between Chinese and western medicine, and should follow the premise of learning from each other's strengths and complementing each other to fully highlight their advantages.

ObjectiveTo explore the potential mechanism of Zuogui Jiangtang Jieyu Formula (左归降糖解郁方, ZJJF) for diabetic rats with depression. MethodsSixty rats were randomly divided into normal group, model group, wingless MMTV integration site family member 5a (Wnt5a) agonist group, ZJJF group, and ZJJF plus Wnt5a inhibitor group, with 12 rats in each group. Except for the normal group, the rats were fed with high-fat chow, streptozotocin injection, and chronic mild unpredictable stress combination, to establish model of diabetes mellitus complicated with depression. After successful modelling, rats in the Wnt5a agonist group were given bilateral hippocampal stereotactic injections of Wnt5a agonist Foxy-5 with 5 μl each for 7 consecutive days; rats in ZJJF group were given 20.52 g/(kg·d) of ZJJF by gavage; rats in ZJJF plus Wnt5a inhibitor group were given the drug by gavage, and bilateral hippocampal stereotactic injections of Wnt5a inhibitors Box5, with the same dosage and injection method as above. The normal group and model group were given 10 ml/(kg·d) of normal saline by gavage. All groups were gavaged for 4 consecutive weeks. At the end of the intervention, the depression-like behaviour of rats was evaluated using the forced swimming experiment (immobility time) and the absent field experiment (number of activities); the blood glucose and insulin levels of rats were measured and the insulin resistance index was calculated; the dendritic morphology of dentate gyrus neurons in the hippocampus was observed using Golgi staining; the level of dentate gyrus neuron proliferation was measured using 5-bromodeoxyuracil nucleoside (Brdu) injection and immunofluorescence; RT-qPCR and Western blot were used to detect the mRNA and protein expression of Wnt5a, Ras homologue genomic member A (RhoA) and Rho homologue-associated coiled-coil protein kinase 1 (ROCK1) in the dentate gyrus. ResultsCompared with the normal group, rats in the model group had significantly higher blood glucose, insulin and insulin resistance indices, longer immobility time, fewer activities, lower Brdu integral optical density values and Wnt5a, RhoA, ROCK1 protein and mRNA expression in the dentate gyrus of the hippocampus (P<0.05 or P< 0.01); the dendritic branches of rat hippocampal dentate gyrus neurons could be seen to be significantly reduced or broken, and their length shortened. Compared with the model group, the blood glucose, insulin and insulin resistance indices of rats in ZJJF group and the ZJJF plus Wnt5a inhibitor group significantly reduced (P<0.05 or P<0.01); the immobility time of rats in the Wnt5a agonist group and ZJJF group was significantly shortened, the number of activities increased, the Brdu integral optical density values elevated, and the Wnt5a, RhoA, ROCK1 protein and mRNA expression elevated (P<0.05 or P<0.01), and the number of dendritic branches of hippocampal dentate gyrus neurons significantly increased, the length lengthened, and the complexity of dendrites increased. Compared with the Wnt5a agonist group, rats in the ZJJF group showed significant decrease in blood glucose, insulin and insulin resistance indices, prolongation of immobilisation time, reduction in the number of activities, and reduction in the Brdu integral optical density value; except for the Wnt5a mRNA in ZJJF group, Wnt5a, RhoA, ROCK1 protein and mRNA expression reduced in both ZJJF group and ZJJF plus Wnt5a inhibitor group (P<0.05 or P<0.01). Compared with ZJJF group, Wnt5a, RhoA, ROCK1 protein and mRNA expression were reduced in ZJJF plus Wnt5a inhibitor group (P<0.05 or P<0.01). ConclusionZJJF can improve hyperglycemia and depressive-like behaviours in rat models of diabetes with depression, and its antidepressant effects may be related to the activation of hippocampal Wnt5a/RhoA signaling and promotion of dentate gyrus neuron dendritic growth.
