ABSTRACT

ObjectiveTo explore the anti-inflammatory mechanism of Huangqintang based on the inflammation model in RAW264.7 cells. MethodHuangqintang was prepared and the safe dose to RAW264.7 cells was screened out. The RAW264.7 cells were seeded in 24-well plates and incubated with Huangqintang and lipopolysaccharide (LPS), successively. The concentrations of nitric oxide (NO), interleukin (IL)-6, tumor necrosis factor (TNF)-α, and prostaglandin E2 (PGE2) were measured by Griess assay and enzyme-linked immunosorbent assay (ELISA), respectively. Meanwhile, RAW264.7 cells were inoculated in 6-well plates, and normal group, LPS group, LPS+Huangqintang group, nuclear factor-κB (NF-κB) p65 inhibitor PDTC group, p38 mitogen-activated protein kinase (MAPK) inhibitor SB203580 group, extracellular signal-regulated kinase (ERK) inhibitor PD98059 group, c-Jun N-terminal kinase (JNK) inhibitor SP600125 group, and Janus kinase (JAK) inhibitor AG490 group were set up. After the cells were incubated with corresponding inhibitors and Huangqintang and stimulated by LPS, RNA and protein were extracted. The mRNA and protein expression levels of NF-κB p65, p38 MAPK, ERK, JNK, and JAK were detected by Real-time fluorescence-based quantitative polymerase chain reaction (Real-time PCR) and Western blot, respectively, to explore the anti-inflammatory mechanism of Huangqintang by regulating the NF-κB, MAPK, and JAK/signal transducer and activator of transcription protein (STAT) signaling pathways. ResultAfter stimulation with LPS, the concentrations of NO, IL-6, TNF-α, and PGE2 in the cells of the model group increased significantly(P<0.05,P<0.01). Compare with the model group, after incubation with Huangqintang, the secretion of NO, IL-6, TNF-α, and PGE2 showed a downward trend (P<0.05,P<0.01). Compared with the normal group, the model group showed increased mRNA expression of p38 MAPK, ERK, JNK, JAK, and NF-κB p65 and total protein expression in cells after stimulation with LPS (P<0.05,P<0.01). Compare with the model group,after incubation with Huangqintang, the total protein and mRNA expression of p38 MAPK, ERK, JNK, JAK, and NF-κB p65 in inflammatory cells decreased (P<0.05,P<0.01). Meanwhile, the expression of NF-κB p65 total protein and mRNA in each inhibitor group showed a downward trend (P<0.05,P<0.01). ConclusionHuangqintang can inhibit the inflammatory response through the NF-κB, MAPK, and JAK-STAT signaling pathways.

ABSTRACT

ObjectiveTo investigate the protective effect and underlying mechanism of Gandou Fumu decoction (GDFMT) on renal fibrosis in a mouse model of Wilson's disease. MethodSixty adult male toxic milk (TX) mice were randomly divided into a model group, high-, medium-, and low-dose GDFMT groups, and a positive control (penicillamine) group, and another 12 wild-type mice were assigned to the normal group. The high-, medium-, and low-dose GDFMT groups were administered GDFMT at 13.92, 6.96, 3.48 g·kg-1, respectively, and the positive control group received penicillamine at 0.1 g·kg-1, while the model and normal groups were given an equal volume of 0.9% saline solution by gavage once a day for 4 consecutive weeks. Enzyme-linked immunosorbent assay (ELISA) was used to measure the levels of blood urea nitrogen (BUN), creatinine (CRE), type Ⅲ procollagen (PC-Ⅲ), and type Ⅳ collagen (C-Ⅳ) in the serum. Histological changes in the mouse kidneys were examined by hematoxylin-eosin (HE) and Masson's trichrome staining. Immunofluorescence was used to assess the protein expression of leptin, Janus kinase 2 (JAK2), and signal transducer and activator of transcription (STAT) in renal cells. Real-time polymerase chain reaction (Real-time PCR) was performed to analyze the mRNA expression levels of leptin, leptin receptor(OB-R), JAK2, and STAT. Western blot was used to detect the expression of transforming growth factor-β1 (TGF-β1) and monocyte chemoattractant protein-1 (MCP-1). ResultCompared with the normal group, the model mice exhibited a significant increase in BUN, CRE, PC-Ⅲ, and C-Ⅳ levels (P<0.01). Compared with the model group, the high- and medium-dose GDFMT groups and the penicillamine groups showed significant decreases in these parameters (P<0.05, P<0.01), with the high-dose GDFMT group demonstrating the most significant reduction (P<0.01). The histological examination of renal tissue revealed fibrosis in the model group, while the fibrotic damage was mitigated to varying degrees after drug intervention, with improvement in fibrosis. Immunofluorescence results showed that leptin, JAK2, and STAT3 protein expression levels were significantly upregulated in the renal fibrosis of the model group. After GDFMT intervention, the fluorescence intensity decreased, with the high-dose GDFMT group showing the lowest intensity. Real-time PCR results demonstrated that leptin, OB-R, JAK2, and STAT3 mRNA expression levels were significantly elevated in the model group compared with those in the normal group, while the high- and medium-dose GDFMT groups and the penicillamine group showed significant reductions in their expression levels (P<0.05, P<0.01). Western blot analysis revealed that TGF-β1 and MCP-1 expression levels were significantly increased in the model group (P<0.01), and the high- and medium-dose GDFMT groups exhibited significant reductions in their expression levels (P<0.01). ConclusionGDFMT can alleviate renal fibrosis damage in TX mice, and its mechanism of action may be related to the regulation of leptin and the JAK/STAT signaling pathway.

ABSTRACT

Abstract Objective To compare the efficacy and safety between baricitinib (BARI) and tofacitinib (TOFA) for the treatment of the rheumatoid arthritis (RA) patients receiving methotrexate (MTX) in clinical practice. Methods This retrospective study recruited 179 RA patients treated with BARI (2-4 mg/d) or TOFA (10 mg/d) at The First Affiliated Hospital of Guangxi Medical University from September 2019 to January 2022. The rate of low disease activity (LDA) was used as the primary end point. Secondary end points included the Disease Activity Scale-28 (DAS-28)-C-reactive protein (CRP); the rate of DAS28-CRP remission; visual analogue scale (VAS) for pain, swollen joint, and tender joint counts; and adverse events at the 6-month follow-up. Several factors affecting LDA achievement were also analyzed. Results Seventy-four patients were treated with BARI and 105 were treated with TOFA, including 83.24% females, with a median (IQR) age of 56.0 (53.0-56.0) years old and disease duration of 12.0 (6.0-12.0) months. There was no difference of the rate of LDA between the BARI and TOFA treatment groups. All disease indices in the two groups were significantly improved, including a significantly lower VAS in the BARI group (P < 0.05), reflecting the drug efficacy after 1 and 6 months of treatment. The incidence of adverse reactions was similar in these two groups. Conclusion The treatment efficacy and safety of BARI and TOFA in the RA patients were similar, but BARI was more effective in pain relief than TOFA. An older baseline age was more likely to achieve LDA in the BARI group, while a low baseline erythrocyte sedimentation rate (ESR) was more likely to achieve LDA in the TOFA group.

ABSTRACT

ObjectiveTo investigate the effects of gallic acid (GA) on human colon cancer HCT-116 and Caco-2 cell activities, intracellular Janus kinase (JAK)/signal transducer and activator of transcription factor (STAT) signaling pathway, and the expression of anti-apoptotic protein B-cell lymphoma-2 (Bcl-2) and pro-apoptotic protein B-cell lymphoma-2-associated X protein (Bax), so as to explore its underlying molecular mechanism. MethodFollowing the classification of cells into GA group, blank group, and 5-fluorouracil (5-FU, 0.05 g·L-1) group, the HCT-116 and Caco-2 cells were treated with GA (0.02, 0.05, 0.1, 0.15, 0.2 g·L-1) for 12, 24, 48, and 72 h, respectively, and the cell proliferation inhibition rats were determined by cell counting kit-8 (CCK-8) assay to select the GA concentration that effectively inhibited proliferation. The colony formation ability was detected by crystal violet staining and the migration of cells by scratch test. The level of reactive oxygen species (ROS) was measured using a fluorescent probe (DCFH-DA). The expression of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) in cell supernatant were determined using the enzyme-linked immunosorbent assay (ELISA) kits. The expression levels of JAK2, phosphorylated (p)-JAK2, STAT3, p-STAT3, Bcl-2, and Bax were assayed by Western blot. ResultCCK-8 assay showed that after 12, 24, 48, and 72 h of treatment, GA (0.02, 0.05, 0.1, 0.15, 0.2 g·L-1) inhibited the proliferation of HCT-116 and Caco-2 cells in a dose- and time-dependent manner, and the inhibition rates were higher than those in the blank control group. Compared with the 5-FU group, GA (0.2 g·L-1) enhanced the inhibition of cell proliferation in a time-dependent manner. Compared with the blank control group, GA (0.1, 0.15, and 0.2 g·L-1) significantly decreased the number of cell colonies (P<0.01), increased the inhibition rate of cell colony formation (P<0.01), diminished the scratch healing rate (P<0.05, P<0.01), elevated the fluorescence intensity of intracellular ROS (P<0.01), and down-regulated the expression of IL-6 and TNF-α in the supernatant (P<0.01) in a dose-dependent manner. Compared with the 5-FU group, GA (0.2 g·L-1) decreased the scratch healing rate (P<0.01), enhanced the fluorescence intensity of intracellular ROS (P<0.01), and down-regulated the levels of IL-6 and TNF-α in cell supernatant (P<0.01). According to Western blot analysis, compared with the blank control group, GA (0.1, 0.15, 0.2 g·L-1) obviously lowered the expression of p-JAK2, p-STAT3, Bcl-2, p-JAK2/JAK2, p-STAT3/STAT3, and Bcl-2/Bax (P<0.01) and raised Bax protein expression (P<0.05, P<0.01) in a dose-dependent manner. Compared with the 5-FU group, GA (0.2 g·L-1) down-regulated the expression of p-JAK2, p-STAT3, Bcl-2, p-JAK2/JAK2, p-STAT3/STAT3, and Bcl-2/Bax (P<0.05, P<0.01) and up-regulated the expression of Bax protein (P<0.05, P<0.01). ConclusionGA significantly inhibits the proliferation of HCT-116 and Caco-2 cells, which may be related to the increased accumulation of intracellular ROS, down-regulation of inflammatory factors IL-6 and TNF-α, p-JAK2 and p-STAT3 protein expression in JAK/STAT signaling pathway, and Bcl-2, and up-regulation of Bax.

ABSTRACT

Objective:To observe the therapeutic effect of modified Fuyuanwan combined with auricular acupressure bean on stage Ⅱ, Ⅲ diabetic nephropathy and its effect on serum janus kinase (JAK)/ signal transducer and activator of tranions (STAT) signaling pathway. Method:A total of 180 cases were randomly divided into control group and observation group, 90 cases in each group. Losartan potassium, modified Fuyuanwan combined with auricular acupressure bean were given respectively for 12 weeks. Renal function indexes [blood urea nitrogen (BUN), serum creatinine (SCr), urinary albumin excretion rate (UAER), 24 h urinary protein quantitative (24 h Upor)], relative abundance of intestinal flora (verruca microflora, scleriobacteriae, deferribacter, proteobacteria), oxidative stress indicators [advanced oxidation protein products (AOPPs), reactive oxygen species (ROS), glutathione peroxidase (GSH-Px), total superoxide dismutase (TSOD)], renal blood flow index [end-diastolic blood flow velocity (EDV), peak systolic value (PSV), pulse index (PI), blood flow resistance index (RI)], JAK/STAT signaling pathway [JAK, phosphorylated JAK (p-JAK), STAT, phosphorylated STAT (p-STAT) were observed before and after treatment. The safety indexes of two groups were evaluated after treatment. The efficacy was observed after treatment and followed up for 1 years and 2 years. Result:After treatment and follow-up for 1, 2 years, the total effective rates of patients in observation group were 97.8% (87/89), 81.6% (71/87), 59.8% (49/82), respectively, observation group which were significantly higher than those in control group of 79.3%(69/87),57.8%(48/83),37.2%(29/78) (<italic>χ</italic><sup>2</sup>=4.016, <italic>χ</italic><sup>2</sup>=4.503, <italic>χ</italic><sup>2</sup>=4.769, <italic>P</italic><0.05). Compared with control group after treatment, UAER, BUN, SCr, 24 h Upor, firmicutes, actinobacillus, proteobacteria, AOPPs, ROS, PI, RI, p-JAK, p-STAT3 in observation group were significantly decreased (<italic>P</italic><0.05,<italic>P</italic><0.01), microflora verruca, GSH-PX, TSOD, JAK, STAT3 were significantly increased (<italic>P</italic><0.05,<italic>P</italic><0.01), EDV and PSV were significantly accelerated (<italic>P</italic><0.05,<italic>P</italic><0.01). The incidence of adverse reactions was 1.1% (1/89) in observation group, lower than 13.8% (12/87) in control group (<italic>χ</italic><sup>2</sup>=5.127, <italic>P</italic><0.05). Conclusion:Modified Fuyuanwan combined with auricular acupressure bean can significantly improve the curative effect of stage Ⅱ, Ⅲ diabetic nephropathy, and its mechanism of action may be related to the serum JAK/STAT signaling pathway.
