ABSTRACT

Severe traumatic patients should be transported to level 1 trauma center within one hour. We investigated simulated transport time and distance from eastern Gyeongnam province to nearest level 1 trauma center in South Korea. This scenario was simulated on web mapping service. Transport time and distance was measured by motor vehicle. We decided that one hour is optimal for appropriate trauma care. Estimated transport time were 40 min, 48 min, 55 min and 1 hour 8 min, respectively (Gimhae, Yangsan, Changwon and Miryang). Nearest level 1 trauma center was Pusan National University Hospital outside Gyeongnam province. Transport of traumatic patient was based on real transfer time above administrative district.

ABSTRACT

The response and transport intervals of an ambulance dispatch are affected by various factors. The present ambulance system relies on the driver’s experience, knowledge of local road map and estimations of directions. These may contribute to delays, misdirection and inefficient utilization of resources. The objectives of this study were to assess the effects of GPS navigation device on the response and transport time intervals of an ambulance service operating in urban setting. This was a three-month experimental study involving simulated ambulance dispatch based in a hospital. A total of 50 simulated cycles were constructed. Travel time, arrival time and distance were documented from each cycle. The median actual response time using map and GPS navigation device was 11.82 min and 10.47 min respectively. The median actual transport time using map and GPS navigation device was 11.00 min and 10.74 min respectively. The differences in response time and transport time between map and GPS navigation were not statistically significant (p= 0.215 and p=0.710). The application of GPS reduced the error value between estimated and actual response time by 8.73%. It also reduced the mean error value between estimated and actual transport time by 1.42%. The result shows that the GPS navigation device allowed for more accurate travel time estimation and its application in an urban setting such as Kuala Lumpur has the potential to reduce ambulance response time.

ABSTRACT

The aim of this work was to evaluate different transport times for pacama juvenile (Lophiosilurus alexandri) previously trained to feed formulated diets. Four hundred and eighty-six pacama juveniles (2.52±0.71g) were submitted to 24 hours of fasting before transport. The animals were divided into nine plastic bags, with density of 54 juveniles in 5L of clean water for each bag after that inflated with pure oxygen and then sealed. Fish were transported for three hours and bags were open at 5, 6 and 7.5 hours after closing. For each time three replications were used. Survival was monitored for 96 hours post-transport. The longest time tested showed the lowest values of dissolved oxygen and pH, while the total ammonia was similar in the different treatments. Survival was 100% for different transport times after 96 hours. Some juveniles returned to feed 48 hours and the majority 72 hours after opening the bags, suggesting the efficiency of transport made at different times.

ABSTRACT

The aim of this study was to assess changes of Hsp70 and HSF-1 protein and mRNA expression in stress-sensitive organs of pigs during transportation for various periods of time. Twenty pigs were randomly divided into four groups (0 h, 1 h, 2 h, and 4 h of transportation). A significant increased activity of AST and CK was observed after 1 h and 2 h of transportation. Histopathological changes in the heart, liver, and stomach indicated that these organs sustained different degrees of injury. Hsp70 protein expression in the heart and liver of transported pigs did not change significantly while it increased significantly (p < 0.05) in the stomach. Hsp70 mRNA levels decreased significantly (p < 0.05) in the heart after 4 h of transportation. However, mRNA expression increased significantly in the liver after 1 (p < 0.05) and 4 h (p < 0.01) of transportation, and increased significantly in the stomach of the transported pigs after 1, 4 (p < 0.01), and 2 h (p < 0.05). HSF-1 levels were reduced at 1 and 4 h (p < 0.05) only in the hearts of transported pigs. These results indicate that Hsp70 mediates distinct stress-related functions in different tissues during transportation.

ABSTRACT

BACKGROUND: Despite continous efforts to improve the prehospital trauma care system in Korea, the preventable death rate has been reported to be high. The purpose of this study was to evaluate the prehospital trauma care system in Seoul by analyzing 119 rescue databases. METHODS: The 119 rescue data bases of 22,275 trauma patients, who were transported to the secondary and tertiary hospital in Seoul by Seoul 119 rescue services from January 1, 2000 to December 31, 2000, were analyzed. RESULTS: The response time(mean 3.7 min.) showed no time, weekly, and regional variations. However, the transport time from field to hospital showed was high in the morning rush hour(7:00 to 10:00 am), and was gradually decreased and the lowest between 10:00 pm and 7:00 am. There was also a regional variation in the transport time, which was short in the central area and long in the peripheral area of Seoul. Prehospital cares were given to the 10,999 trauma patients(49.4%). Of the 464 unresponsive patients(2%), only 236 patients were identified in transported hospitals. The outcomes of these unresponsive patients were DOA(54%), survival(19%), death in ER(14%), transfer to other hospitals(8%), and death after admission(5%) in order. CONCLUSION: We suggest that this study may be helpful to the establishment and improvement of the prehospital trauma care system as well as the determination of the adequate numbers and locations of trauma center in Seoul.
