ABSTRACT

Objective:To analyze the correlation between serum bile acid and glycolipid metabolism in patients with type 2 diabetic mellitus (T2DM) and metabolically associated fatty liver disease (MAFLD).Methods:A total of 387 T2DM patients hospitalized in Jinan Central Hospital in Shandong Province from July 2020 to November 2021 were selected, including 140 T2DM patients (T2DM group) and 247 T2DM patients with MAFLD (T2DM with MAFLD group). Clinical data were collected of all patients for retrospective analysis and compare the differences in clinical datas in the T2DM group and the T2DM with MAFLD group. Analyze the influencing factors of T2DM with MAFLD. The serum total bile acid (S-TBA) of the T2DM with MAFLD group was divided into T1, T2 and T3 groups by the method of trisection. Triglyceride glucose index (TyG) and hemoglobin glycation index (HGI) were divided into low TyG group, high TyG group and low HGI group, high HGI group by their median values. The incidence of hyperlipidemia, degree of insulin resistance and glycosylated hemoglobin variation was compared among the three groups. To analyze the influencing factors of TyG and HGI. The measurement data conforming to the normal distribution were compared between two groups by independent sample t-test, the comparison between multiple groups by ANOVA, the comparison between two groups of measurement data not conforming to the normal distribution by Mann Whitney U test, and the comparison between multiple groups by Kruskal Wallis H test, χ 2 test was used for comparison of count data between groups. The influencing factors of T2DM with MAFLD and high HGI were analyzed by multivariate binary Logistic regression. The influencing factors of TyG were analyzed using multiple stepwise linear regression. Results:There was no significant difference between the S-TBA of T2DM group and T2DM with MAFLD group ( P>0.05). TyG ( OR=1.788, 95% CI: 1.223-2.614), superoxide dismutase (SOD) ( OR=1.019, 95% CI: 1.009-1.030) and VFA (visceral fat area) ( OR=1.021, 95% CI: 1.013-1.028) were risk factors for the occurrence of T2DM with MAFLD ( P values are 0.003, 0.001 and 0.001, respectively). The incidence of high TyG in T3 group (61.0% (50/82)) as higher than that in T1 group (40.0% (32/80)), the incidence of high HGI in T3 group (61.0% (50/82)) was higher than that in T2 group (41.2% (35/85)), the morbidity of hypercholesterolemia in T3 group (15.9% (13/82)) was higher than that in T2 group (4.7% (4/85)), the differences were statistically significant (χ 2 values were 7.13, 6.55, 5.67; the P values were 0.008, 0.011, 0.017). S-TBA, hepatic steatosis index (HSI) and VFA were independent influencing factors of TyG (the P values were 0.003, <0.001, <0.001). S-TBA ( OR=0.940, 95% CI:0.887-0.997) and fasting C peptide ( OR=0.454, 95% CI:0.219-0.940) were protective factors for the occurrence of high HGI (the P values were 0.039, 0.034). Conclusion:S-TBA was positively correlated with the degree of insulin resistance and incidence of hypercholesterolemia in patients with T2DM and MAFLD, negatively correlated with the degree of glycated hemoglobin variation.

ABSTRACT

OBJECTIVE@#Scutellarin is a primary active composition come from Erigeron breviscapus. It is well known that scutellarin has anti-inflammatory and antioxidant physiological functions. In this study, we detected the effects of scutellarin on hepatocyte cell apoptosis in type 2 diabetes mellitus (T2DM) rats.@*METHODS@#Sprague Dawley (SD) (6-8 weeks, 160-180 g) rats were randomly divided into six groups: control, model, scutellarin low-dose, medium-dose, high-dose treatment, and rosiglitazone positive groups; with 10 SD rats in each group (n = 10). The changes of biochemical factors in serum were detected by automatic biochemical instrument, the pathological changes of liver tissue were detected by hematoxylin and eosin (HE) staining, the apoptosis of liver tissue and cells was detected by tissue staining and flow analyzer, and the expression of apoptosis-related factors were determined by qPCR, Western blot and immunohistochemistry in liver tissues or cells.@*RESULTS@#The results showed that scutellarin decreased the levels of fasting blood glucose, total cholesterol, triglyceride, and low-density lipoprotein and increased the levels of high-density lipoprotein. Meanwhile, scutellarin decreased the levels of alanine transaminase (ALT) and aspartate transaminase (AST) and improved liver function. In addition, scutellarin suppressed the secretion of interleukin-1 (IL-1), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) and reduced hepatocyte apoptosis. Furthermore, scutellarin inhibited the expression of cleaved Caspase-3, Bax, and cytochrome C (Cyt-C) and promoted the expression of Bcl-2.@*CONCLUSION@#Scutellarin can inhibit the apoptotic pathway, thereby relieving T2DM.

ABSTRACT

BACKGROUND: Osteoporotic fracture combined with type 2 diabetic mellitus in female patients is often accompanied by dyslipidemia and hyperglycemia. In addition to insulin treatment, statins are often prescribed for combination therapy, but the combined effect of these two drugs on fracture healing has not been reported in such patients. OBJECTIVE; To investigate the effect of lovastatin combined with insulin on the fracture healing of bilateral ovariectomized rats suffering from type 2 diabetic mellitus. METHODS: The study was approved by the Ethical Committee of North China University of Science and Technology. Thirty-two female Sprague-Dawley rats were divided into control, diabetic ovariectomized, insulin and combined groups. A model of type 2 diabetes and osteoporosis fracture was established in all rats except for the control group. At 7 days after the type 2 diabetic model was successfully established by injection of streptozotocin, the rats in the insulin and combined groups received the subcutaneous injection of insulin (2-4 U in the morning, and 4-6 U in the evening) until the end of the experiment. The rats in the combined group were given 20 mg/kg lovastatin via gavage daily, and those in the other two groups were not treated. All rats were sacrificed at 3 weeks after fracture. Radiographic, clinical and histomorphometric detections of the callus were performed. The expression levels of bone morphogenetic protein 2, vascular endothelial growth factor and collagen type II were detected. All above results were used to analyze the fracture healing in each group. RESULTS AND CONCLUSION: (1)The radiographic score, micro-CT index, histologic score and the expression levels of vascular endothelial growth factor and collagen type II in the diabetic ovariectomized group were significantly poorer than those in the control group (P < 0.05). (2) All above indexes in the insulin group were significantly improved compared with the diabetic ovariectomized group (P < 0.05), which promoted the fracture healing of model rats. (3) After combined with lovastatin, although the expression levels of vascular endothelial growth factor and bone morphogenetic protein 2 in the callus were significantly increased (P < 0.05), there was no significant improvement in the radiographic appearance and microstructure of the callus.

@#AIM: To analyzed the species of bacteria and antibiotic susceptibility in the conjunctival sac of non- diabetic patients and diabetic patients, to decrease the rate of postoperative endophthalmitis through the proper use of antibiotic drop and preoperative disinfection. <p>METHODS: Totally 124 patients(124 eyes)underwent cataract operation in Yangsi Hospital during November 2013 to January 2017 were divided into two groups: diabetic group(37 patients 37 eyes)and control group(87 patients 87 eyes). Bacterial culture in the conjunctival sac and antibiotic susceptibility test was operated in three time-points: before preoperative disinfection(T0), after preoperative disinfection(T1)and after operation(T2). <p>RESULTS: The bacterial culture rate in the conjunctival sac of diabetic patients was significantly higher than that in non-diabetic patients. Most of strains of bacteria were sensitive to vancomycin, levofloxacin and tobramycin. For penicillin, azithromycin and tetracycline, the rate of antibiotic resistance was much higher than other antibiotic. <p>CONCLUSION: Due to the change of the microenvironment of ocular surface, the bacterial culture rate of the conjunctival sac was significantly increased in diabetic patients. The main cultured bacteria in conjunctival sac were staphylococcus epidermidis and acnes propionate. Most of cultured bacteria in conjunctival sac were sensitive to levofloxacin and tobramycin. Levofloxacin and tobramycin can be used as preoperative antibiotic eye drop.

ABSTRACT

Objective To investigate the expression of IL-10 in serum of patients with type 2 diabetic mellitus (T2DM) and its relationship with carotid atherosclerosis.Methods Totally 168 cases of T2DM patients treated in our hospital from May 2013 to December 2016 were retrospectively analyzed.According to the IMT value,the patients were divided into the normal group,the thickening of the group and carotid atherosclerosis plaque formation group.In addition,65 cases of healthy volunteers in our hospital physical examination center at the same period were treated as control group.The levels of serum inflammation index,blood glucose,blood lipid and other indicators of all subjects were compared,at the same time,the correlation between IL-10 and other detection index was analyzed.Additionally,the risk factors of T2DM patients with carotid atherosclerosis were analyzed.Results Compared with the control group,the levels of IL-6,blood glucose,blood lipid and other indicators in T2DM patients increased significantly(P<0.05),while the level of IL-10 decreased significantly(P<0.05);with the increase of IMT,the levels of IL-6,blood glucose,blood lipid and other indicators in T2DM patients increased significantly,but the level of IL-10 decreased significantly,meanwhile the differences between the groups were statistically significant (P<0.05).Compared with normal group,the levels of IL-6,blood glucose,blood lipid and other indicators of patients in thickening group and plaque group significantly elevated,but the level of IL-10 decreased significantly;the levels of IL-6,blood glucose,blood lipid and other indicators of patients in plaque group were obviously higher than those of thickening group,and IL-10 level was significantly lower than thickening group,and the differences were statistically significant (P<0.05);the levels of IL-10 in the serum were negatively correlated with blood glucose,blood lipid and other indicators (P<0.05);multifactor non-conditional Logistic analysis showed that IL-6,Hcy and UA were the risk factors for atherosclerosis in T2DM patients,and IL-10 was the protective factor.Conclusion The level of IL-10 of patients with T2DM is closely related to the occurrence and development of carotid artery atherosclerosis,which provides a new target for clinical treatment.

ABSTRACT

The present study aimed at exploring the therapeutic potential of standard extract of Bombax ceiba L. leaves (BCE) in type 2 diabetic mellitus (T2DM). Oral administration of BCE at doses of 70, 140, and 280 mg·kg, to the normal rats and the high-fat-diet- and streptozotocin-induced T2DM rats were carried out. Effects of BCE on blood glucose, body weight, and a range of serum biochemical parameters were tested, and histopathological observation of pancreatic tissues was also performed. HPLC-ESI-Q/TOF-MS/MS analysis indicated that the chemical composition of BCE mainly contained mangiferin, isoorientin, vitexin, isomangiferin, isovitexin, quercetin hexoside, 2'-trans-O-cumaroyl mangiferin, and nigricanside. BCE caused a significant decrease in the concentrations of fasting blood glucose, glycosylated hemoglobin, total cholesterol, triglyceride, low density lipoprotein-cholesterol, serum insulin, and malondialdehyde, and increases in oral glucose tolerance, high density lipoprotein-cholesterol, and superoxide dismutase in the T2DM model rats. Moreover, considerable pancreatic β-cells protection effect and stimulation of insulin secretion from the remaining pancreatic β-cells could be observed after BCE treatment. The results indicated that BCE exhibited an excellent hypoglycemic activity, and alleviated dyslipidemia which is associated with T2DM. Antioxidant activity and protecting pancreatic β-cells are the possible mechanisms involved in anti-diabetic activity of BCE.

Objective To investigate the relationship between renal vascular resistance index( RI) and serum Apelin level in type 2 diabetic mellitus ( T2DM )?Methods Seventeen cases newly diagnosed T2DM patients with RI increased but without microalbuminuraia in Minhang Hospital Affiliated to Fudan University from December 2011 to December 2014 were selected as observation group,17 newly diagnosed T2DM patients with RI normal during the same period were selected as control group?Fasting plasma glucose(FPG),glycosylated hemoglobin A1C(HbAlC),blood lipids and fasting insulin(FINS),hepatic functional and renal function were tested in all the subjects?Serum Apelin level was detected by enzyme?linked immunosorbent assay?Results Compared with control group,serum Apelin level was significantly higher in observation group((179?2±122?4)μg/L vs?(56?7±50?6) μg/L,t=3?814,P<0?05)?Partial correlation analysis showed that the RI was positive correlated with Apelin ( r= 0?364, P= 0?040 )?Conclusion The serum Apelin levels elevated in newly diagnosed T2DM patients with RI increased, and RI is positively correlated with Apelin, which indicate that Apelin play an important role in the pathophysiology of early renal damage in patients with TsDM.

ABSTRACT

Objective To explore the pharmacodynamic action of the extract from raw materials of Rhizoma Anemarrhenae and Cortex Phellode ndri, a classic herb pair in traditional Chinese medicine, on mice with diabetic peripheral neuropathy(DNP). Methods The mice were administrated with intraperitoneal injection of streptozotocin (STZ, 30 mg/kg) and fed with high-fat diet to establish the DNP mouse model. One hundred and sixty male C57BL/6j mice were divided into normal group, model group, metfomin group(130 mg/kg), Xuezhikang group(200 mg/kg), high- and low-dose Rhizoma Anemarrhenae extract group(360, 90 mg/kg) , high-and low-dose Cortex Phellodendri extract group(135, 45 mg/kg), 20 mice in each group. The medication lasted for 24 weeks. On medication week 8, 16 and 24, the plasma levels of glucose (GLU), total cholesterol (TC), triglyceride(TG), low density lipoprotein-cholesterol(LDL-C) and insulin(INS) were determined respectively. On medication week 10, oral glucose tolerance test(OGTT)was carried out. On medication week 24, the plasma nuclear factor kappa B(NF-κB) activity and the plasma levels of transforming growth factor-beta 1(TGF-β1), superoxide dismutase(SOD) and reduced glutathione(GSH) were detected. Results Five weeks after injection of STZ, the body mass of the model group was firstly increased and then decreased, and FBG was increased(P<0.05 compared with the normal group). Compared with the normal group, FBG, TC, TG, LDL-C and INS levels were increased(P<0.01 or P<0.001), plasma NF-κB activity, TGF-β1 and SOD levels were enhanced, and GSH level was decreased in the model group, the difference being statistically significant (P<0.01 or P<0.001). Contrasted with the model group, FBG, TC, TG, LDL-C and INS levels in high-and low-dose Rhizoma Anemarrhenae extract group and in high-and low-dose Cortex Phellodendri extract group were decreased to various degrees. After continuous medication, plasma NF-κB activity and TGF-β1 and SOD levels in high-dose Rhizoma Anemarrhenae extract group and in high-dose Cortex Phellodendri extract group were reduced(P<0.05 or P<0.001), and plasma GSH level was increased(P<0.01). Conclusion Extract from raw materials of Rhizoma Anemarrhenae and Cortex Phellodendri has obvious hypoglycemic effect and protective effect on experimental mice with DPN.

Objective To analyze the variation tendency of correlative test indexes in the patients with type 2 diabetic mellitus (T2DM) and diabetic nephropathy (DN) .Methods The urinary microalbumin (M‐Alb) ,blood glucose and renal function were de‐termined in 167 cases of T2DM .Then the cases were divided into the simple DM group ,early DN group and clinical DN group ac‐cording to the excretion rates of urinary M‐Alb .The determined results were compared among 3 group and analyzed .Results The age in the simple DM group was smaller than that in the early DN group and clinical DN group (P<0 .05) .The positive rates of u‐rinary M‐Alb and urinary protein were highest in the clinical DN group ,the differences among 3 groups had statistical significance (P<0 .05) .The difference of blood glucose indexes had no statistical difference between the simple DM group and early DN group , the clinical DN group was apparently higher than that in the simple DM group and early DN group (P<0 .05) .The levels of urea (Urea) and creatinine(Cr) in the simple DM group were significantly lower than those in the early DN group and clinical DN group ,the differences among 3 groups had statistical significance (P<0 .01) .Conclusion The severe the DN ,the higher the urina‐ry M‐Alb ,blood glucose and renal function indicators ,the higher the positive rates of urinary protein and urinary glucose ,the more significant the tendency of renal function damage .

ABSTRACT

Objective To investigate the relationship between serum uric acid level and body fat depots as well as the possible influencing factors of the relationship in type 2 diabetic patients.Methods Five hundred and thirty two patients hospitalized for type 2 diabetes were recruited in this study.According to serum uric acid,the subjects were classified into normal uric acid group (group NUA,n =426) and high uric acid group ( group HUA,n =106 ).Anthropometry ( BMI,waist circumference,hip circumference and WHR),serum uric acid level,blood glucose,renal function and lipid levels were compared between the two groups.Results Serum uric acid level was closely related to body fat distribution:(1) in group HUA,WHR,BMI,BUN,SCr and TG were significantly higher than that of group NUA ( [ 0.94 ± 0.07 ] vs.[ 0.91 ± 0.07 ],[ 26.91 ± 3.90 ]kg/m2 vs.[23.84 ±3.80] kg/m2,[8.66 ±5.94] mmol/L vs.[6.29 ±3.64] mmol/L,[93.9 ± 67.6 ] μmol/Lvs.[63.9 ± 20.6 ] μmol/L,[ 2.65 ± 1.92 ] mmol/L vs.[ 1.86 ± 1.46 ] mmol/L,respectively; all P < 0.05 ] ;while HDL-C was lower in Group HUA than in group NUA ( [ 1.09 ± 0.32 ] mmol/L vs.[ 1.24 ± 0.45 ] mmol/L,P <0.05).(2) Multiple regression analysis revealed that the serum uric acid levels were positively correlated with BMI,SCr,and TG ( b =0.201,0.500,0.153,respectively,P < 0.05 ),but negatively correlated with HbA(1)c and HDL-C ( b =- 0.168 and - 0.121,P < 0.05 ).Conclusion In patients with type 2 diabetes,serum uric acid levels are closely related with the BMI and Glucose/lipid metabolism,which suggests that body weight control is as important as the uric acid lowering treatment in patients with type 2 diabetes to cut the risks of cardiac and vascular diseases.

ABSTRACT

Diabetes, which has shown an explosive increase in terms of its incidence, is regarded as a serious disease that must be overcome for the sake of human life. Among animal models used for testing of drug efficacy, the mini-pig model has shown a rapid upload due to its many similarities with human, particularly concerning the pharmacokinetics of compounds after subcutaneous administration, the structure and function of the gastrointestinal tract, the morphology of the pancreas, and overall metabolic status. Based on these various advantages, we sought to develop an animal model of type II diabetic mellitus using the Micro-pig, which differs from other miniature pigs. We used six male Micro-pigs for induction of a moderate insulin deficient model with nicotinamide (NIA)/streptozotocin (STZ) treatment and three animals for control. For evaluation of incidence of type II diabetes, we measured blood glucose level, and performed oral glucose tolerance test and immunohistochemistry on pancreatic tissue using insulin antibody. Compared to control animals, all animals treated with NIA/STZ showed high levels of glucose and low levels of insulin. In addition, we observed the partially destroyed beta cell population from tissue of the pancreas in treated animals. Based on these results, we report that the Micro-pig model developed in this study can be used for testing of the efficacy of therapeutic agents for treatment of Type 2 diabetic mellitus.

ABSTRACT

Glycemic indices (GIs) and insulin responses are useful for measuring biological effects and consequences of carbohydrates when designing healthy diets, particularly for people with or at risk of developing diabetes and others disorders such as metabolic syndrome. In this study, we investigated GIs and insulin (as measured by C-peptide) responses of patients with type 2 diabetes mellitus (T2DM) to Bangladeshi potatoes and sweet potatoes. Using a cross-over design, ten T2DM subjects consumed equivalent carbohydrate amounts (50 g of total carbohydrate) of either the vegetables or white bread (WB) (as reference food). Serum glucose levels were determined after 0, 15, 30, 45, 60, 90, 120, 150 and 180 minutes, and C-peptide levels were determined at 0 and 180 minutes. Glycemic Indices and Glycemic Loads (GLs) were calculated. Both plain and sweet potatoes showed a significantly higher serum glucose response compared to the reference food. The similar glycemic response between plain and sweet potatoes was reflected in their GI values: 162  50 and 191  66, respectively. The GL values were 8 and 11, respectively. The substantially higher glycemic response and GI values of the two potatoes were not the consequence of a suppressed insulin response. Compared to the mean values of the international table, Bangladeshi potatoes and sweet potatoes are very high GI foods. However, based on the dietary practices in our society, potatoes and sweet potatoes may be used as low and medium GL foods, respectively. This work may help create a better food exchange table for diabetic patients.

ABSTRACT

Objective To study the clinical effect of herbs of replenishing qi, nourishing yin and promoting blood combined with western medicine on symptoms and hemorrheology indexes of aged diabetic mellitus (DM). Methods Sixty cases with DM-2 were randomly divided into two groups. The treatment group was given herbs combined with western medicine and the control group was given western medicine treatment only. The treatment course was 1 month. The clinical symptoms, blood glucose and hemorrheology indexes were observed in both groups. Results There were 11 marked effective cases, 15 effective cases and 4 ineffective cases in treatment group, while 2, 12 and 16 cases in control group. The total effective rate was 86.7% in treatment group and 46.7% in control group. There was a significant difference between the two groups. The whole blood viscosity, platelet aggregation, D-dimer and fingernail microcirculation were significantly improved in the treatment group. Conclusion The herbs of replenishing qi, nourishing yin and promoting blood combined with western medicine can improve the clinical symptoms and hemorrheology of aged DM.

ABSTRACT

Objective To investigate the possible mechanism of Compounded Sour Medicinal Herbs (CSMH) in preventing and amelioration type 2 diabetic mellitus (T2DM) rats chronic complication, by analyzing the effects of CSMH on the deposition of advanced glycation end products (AGEs) and gene expression of their receptor myocardial tissue in T2DM rats. Methods The rats were injected with streptozotocin (STZ) through peritoneum, and fed with high-fat and high-caloric diets to induce T2DM animal models. Then the rats were randomly divided into three groups:untreated group (model group), AG treated group and CSMH treated group. The deposition of AGEs and gene expression of RAGE in the myocardial tissue of T2DM rats were detected separately by fluorescence and real time PCR method.
