ABSTRACT

Objective: To explore the influence of tanshinone 11 A on the expressions of molecular chape rone Cosmc. advanced oxidation protein products (AOPP) in the kidney tissue of the allergic purpura nephritis mice and the levels of extracellular signal-regulated kinase/mitogen activated protein kinase (ERK/MAPK) signaling pathway proteins, and to clarify mechanism of tanshinone 11 A in the treatment of allergic purpura nephritis. Methods: A total of 38 mice were divided into control group∗ model group (allergic purpura nephritis model) and tanshinone 11 A group (allergic purpura nephritis model • tanshinone 11 A treatment), with 12 in each group; the other 2 mice were used to verify the success of modeling. The urine red blood cell counts and urine protein levels of the mice in various groups were measured, and the pathomorphology of kidney tissue of the mice in various groups were observed by periodate schiff-reaction (PAS) staining. The AOPP protein levels in the kidney tissue of the mice were determined by enzyme linked immunosorbent assay (ELISA), and the ERK and phosphorylated ERK (p-ERk) protein expression levels were determined by Western blotting method. The expression levels of ERK. p-ERK and Cosmc mRNA in kidney tissue of the mice were determined by RT-PCR. Results: In control group, there was no hyperplasia of glomerular basement membrane and no glomerular sclerosis; in model group, the proliferation of glomerular basement membrane was obvious∗ the inflammatory cells were infiltrated, and glomerular sclerosis appeared; in tanshinone 11 A group, glomerular basement membrane hyperplasia and inflammatory infiltration were not obvious in the mice, there was only small amount of infiltrated inflammatory cells. Compared with control group, the urine red blood cell count, the 24 h urine protein level, the p ERK mRNA and AOPP protein levels of the mice in model group and tanshinone 11 A group were increased ( P<0. 05 or P<0. 01); the Cosmc mRNA expression levels were decreased (P<0. 01). ABSTRACT

Objective To investigate the clinical significance of interleukin-6 (IL-6) and leukotriene B4 (LTB4) in the early diagnosis of allergic purpura(HSP) in children and its effect on prognosis.Methods A total of 68 children with Henoch-Schonlein purpura enrolled in our hospital from May 2014 to May 2016 were selected.The levels of IL-6 and LTB4 in the early stage and early recovery stage of children were detected.The significance of early diagnosis and the impact on prognosis were analyzed.Results Compared with the control group,the levels of IL-6 and LTB4 in acute and early recovery stage of children with Henoch-Schonlein purpura were significantly higher(P<0.05),and those in the acute stage of children were higher than those in the early recovery stage(P<0.05).The higher the level of IL-6 and LTB4 in the serum,the greater the possibility of renal damage in patients,and the difference was statistically significant (P<0.05).The higher the level of serum IL-6 and LTB4,the higher the cure rate(P<0.05) and the lower the recurrence rate(P<0.05).The AUC of the combined detection of HSP was 0.894 and the sensitivity was 0.811,which were better than those of IL-6 and LTB4(P<0.05).Conclusion Combined detection of IL-6 and LTB4 is of great clinical significance in the early diagnosis and prognosis of HSP.

ABSTRACT

Objective To study the effect of low molecular weight heparin calcium combined with Bailing capsule in treatment of children with Henoch-Schonlein purpura nephritis and the efficacy of serum hs-CRP, MMP-9, APN and CysC.MethodsFrom January 2013 to December 2015, 84 children with Henoch-Schonlein purpura nephritis were enrolled in our hospital.According to the order of admission, the observation group and control group were divided into observation group and control group.The control group were given low molecular weight heparin calcium, the observation group were treated with low molecular weight heparin calcium combined with Bailing capsule for treatment.The levels of serum hs-CRP, MMP-9, APN, CysC, inflammatory factors and immune levels were observed in the two groups.ResultsAfter treatment, the clinical effective rate of the observation group was significantly higher than that of the control group[92.86% (39/42) vs 69.05% (29/42)] (P<0.05).There were no significant differences in hs-CRP, MMP-9, APN and CysC levels between the two groups.After treatment, the levels of hs-CRP, MMP-9 and CysC in the two groups were significantly higher than those before treatment (P<0.05).APN was significantly higher than that before treatment (P<0.05).Compared with the control group, the levels of hs-CRP, MMP-9 and CysC in the observation group were lower than those in the control group (P<0.05), and the APN level was lower than that in the control group (P<0.05).There were no significant differences in TNF-α, IL-6, IL-10 and IL-18 levels between the two groups before treatment.After treatment, The levels of TNF-α, IL-6 and IL-18 in the two groups were significantly lower than those before treatment (P<0.05).There were no significant differences in IgA, IgG and IgM levels between the two groups.After treatment, the levels of IgA, IgG and IgM in the two groups were significantly lower than those before treatment (P<0.05).The levels of IgA, IgG and IgM in the observation group were lower than those in the control group (P<0.05).There was no significant difference in adverse reaction rate between the two groups.ConclusionLow molecular weight heparin combined with Bailing capsule can effectively reduce hs-CRP, MMP-9, CysC level and APN level in children with Henoch-Schonlein purpura nephritis, improve clinical symptoms, good clinical efficacy and high safety.

ABSTRACT

Objective:To research the correlation of serum levels of IL-21,TGF-β1,TNF-α and IgA1 in children with allergic purpura and the purpura nephritis.Methods:57 cases with allergic purpura who were treated in our hospital from November 2015 to May 2016 were selected and 29 cases were diagnosed with general allergic purpura,and another 28 cases were diagnosed with purpuric nephritis.30 healthy children were selected as the control group.Then the serum levels of IL-21,TGF-β1,TNF-α,IgAl,immunoglobulin A (IgA),C3 and C4 between the three groups were observed and compared.Results:The serum levels of IL-21,TGF-β1,TNF-α,IgA1 and IgA in the purpuric nephritis group were higher than those of the control group and the general allergic purpura group,and the differences were statistically significant (P<0.05);The serum levels of IL-21,TGF-β1,TNF-α,IgA1 and IgA in the general allergic purpura group were higher than those of the control group,and the differences were statistically significant (P<0.05);There was no statistically significant difference about the C3 and C4 in the three groups (P>0.05).Conclusion:The serum levels of IL-21,TGF-β1,TNF-α and IgA1 may be involved in the development ofhenoch-schonlein purpura and purpura nephritis.

ABSTRACT

Objective To study the effect of multi drug combination therapy of children allergic purpura.Methods 80 cases of children with Henoch Schonlein purpura in our hospital from May 2014 to May 2016 were selected,these children were randomly divided into methylprednisolone,cimetidine and loratadine treatment group(treatment group)and methylprednisolone treatment group(control group),40 cases in each groups,the clinical curative effects,rash regression time,joint pain relief time,abdominal pain relief time,hematochezia disappeared time,urine recovery time,hospitalization time,incidences of adverse reaction and recurrences of the two groups were statistically analyzed.Results The total treatment efficiency of the treatment group was 95%(38/40),which was significantly higher than the control group 70%(28/40),the difference was statistically significant(P<0.05),the rash regression,joint pain relief time,abdominal pain relief time,hospitalization time of the treatment group were significantly shorter than the control group(P<0.05),but the differences the stool disappear time,urine recovery time,incidences of adverse reactions,recurrence rate between the two groups were not significant.Conclusion The effect of multi drug combination therapy of children with allergic purpura is better than single drug alone,can shorten the time of disappearance of symptoms of patients,reduce hospital stay.

ABSTRACT

To observe the effect of tripterygium glycosides combined with Danshen injection on blood coagulation mechanism in children with allergic purpura nephritis, and investigate its treatment efficacy through a randomized controlled study. The results showed that before treatment, there were no significant differences in levels of D-D, APTT, PT, FIB and PLT between treatment group and control group; while after treatment, there were significant differences in levels of D-D, PT, FIB and PLT between two groups, but no difference in level of APTT. The effective rate was 90.38% in treatment group, significantly higher than 79.25% in the control group. There was no significant difference in incidence of adverse reactions between two groups. The results suggested that tripterygium glycosides combined with Danshen injection can enhance treatment efficacy and improve blood coagulation mechanism of children in the treatment of purpura nephritis, with high safety and no adverse effects.

ABSTRACT

Objective To investigate the clinical significance of the urine micro albumin(MA)and urine enzyme detections in early renal injury in children with allergic purpura.Methods 120 patients with allergic purpura were chosen as the research sub-jects and divided into the early renal injury group and non-renal injury group according to whether having the renal injury.Contem-poraneous 40 healthy children were chosen as the healthy control group.The urine MA and urine enzyme levels were detected and compared among the various groups.Results MA was (17.5 ±3.8)mg/L in the study group,transferring(TRF)was (0.56 ± 0.34)mg/L,β2-MG was (0.35 ±0.07 )mg/L,NAG was (18.6 ±5.1 )U/g,which were significantly higher than those in the healthy control group,the differences were statistically significant (P <0.05 );in the early renal injury group,MA was (42.1 ± 21.3)mg/L,TRF was (1.65±0.54)mg/L,β2-MG was (0.43±0.06)mg/L and NAG was (23.1±5.12)U/L,which were signif-icantly higher than those in the non-renal injury group,the differences had statistically significance (P <0.05).Conclusion The de-tection of urine MA and urine enzyme can be used as the indexes for monitoring the early renal injury in children with allergic pur-pura,and is conducive to early discovery,in time intervention and reduce the occurrence of complications.

ABSTRACT

Objective To investigate the detection value of the positive rate of MP-IgM in children with Henoch-Schonlein purpura (HSP).Methods 80 HSP patients were chosen as the research subjects (the observation group),the other 40 healthy children were chosen as research subjects(the control group).The positive rate of MP-IgM,and positive changes in immune function of children with HSP combined with MP-IgM were analyzed in two groups.Results 32 cases were MP-IgM positive in the observation group,the positive rate was 40%.The MP-IgM positive rate in the observation group was significantly higher than that in the control group(P <0.05).The percentage of CD4+ in the observation group was significantly lower than that in the control group,the percentage of CD8+ was significantly higher than that in the control group,CD4+/CD8+ was significantly lower than that in the control group,NK was significantly lower than that in the control group(P <0.05).The percentage of CD8+ in MP-IgM positive group was significantly higher than that in the negative group,the percentage of CD4+/CD8+ was significantly lower than that in the negative group,the percentage of NK was significantly lower than that in the negative group (P < 0.05).Conclusion MP infection is an important cause of HSP,it can aggravate the immune dysfunction and play an important role in the occurrence and development of HSP.

ABSTRACT

OBJECTIVE: To observe the efficacy of SNMC combined with calcium gluconate in the treatment of allergic purpura. METHODS: 86 cases of allergic purpura were randomly divided into treatment group (n=46)and control group(n=40). Control group were injected with 10% calcium gluconate solution 10 mL added into 5% glucose injection 100 mL (i.v. gtt) once a day for 14 days. Treatment group were given SNMC injection 20~80 mL added into 5% glucose 250 mL (i.v. gtt) at the same time administered with 10% calcium gluconate solution 10 mL added into 5% glucose injection 100 mL once a day (i.v. gtt) for 14 days. ABSTRACT

[Abstract] Objective To evaluate the clinical and endoscopic manifestations of abdominal type allergic purpura in adult patients as the evidences in reaching early diagnosis. Methods The clinical and endoscopic manifestations of 32 patients with abdominal type allergic purpura were analyzed retrospectively. Re-SllltS All the patients complained of paroxysmal abdominal colic, 59. 4% patients complained of nausea and vomiting, 50% patients had hematochezia and haematemesis, the occult blood test was positive in all patients, and 25% patients had elevated level of amylase in the blood and urine. Purpura was found in 2-10 days after the presentation of abdominal pain. Endoscopy found hyperaemia, edema, bleeding spots, erosion and ulcer in gastroenterologic mucosa. Duodenal, ileum and caecum had more severe mucous lesions, but the esophageal mucosas in all patients were normal. The clinic and endoscopic misdiagnosis rates were 87.5% and 85.7% respectively. Glucocorticoids were the especially effective medicine in its treatment, while those agents for relieving spasm or antacids were ineffective. Conclusion Many scattered bleeding spots and snow flower-like ulcers on the gastrointestinal mucosa are the most important findings in adult abdominal type allergic purpura patients. Clinic manifestations of these patients are very severe abdominal colic in indefinite sites and merely with mild abdominal objective signs is the important characteristic feature.

The platelet count was below 10,000/mm2 in 97% cases of the thrombocytopenic purpura, while within normal range in non-thrombocytopenic purpura. 5) The bleeding time and the clotting time were within normal ranges in most of all cases. The Rumpel-Leede test revealed positive result in 85% cases of I.T.P., while in only 20% of allergic purpura.
