Resumen Introducción: El tronco arterial persistente es una rara malformación cardíaca congénita que provoca diversas complicaciones en el sistema cardiovascular. Se caracteriza por la presencia de un tabique ventricular defectuoso, una única válvula troncal y un tronco arterial común entre la arteria pulmonar y aorta, conllevando a una mezcla entre la sangre arterial y venosa, debido a un cortocircuito cardíaco bidireccional predominante de izquierda a derecha que compromete el suministro de flujo sanguíneo, nutrientes y oxigenación sistémica. Las manifestaciones clínicas incluyen desaturación con cianosis, hipoxemia, taquicardia, taquipnea, alteraciones en la contractilidad cardíaca, pulsos distales anómalos, pérdida de peso, fatiga y hepatomegalia. Objetivo: El propósito de esta investigación es establecer hipótesis sobre los diversos mecanismos compensatorios que se activan a nivel sistémico para contrarrestar los efectos de esta malformación. Reflexión: Se sugiere que se producen respuestas biomoleculares similares en los sistemas cardiovascular, pulmonar y renal, reduciendo la producción de óxido nítrico y provocando respuestas vasoconstrictoras. A nivel hepático, se generan factores de crecimiento y se inician procesos de angiogénesis para aumentar la perfusión sanguínea. En el cerebro, se activan enzimas para incrementar el flujo sanguíneo y proporcionar oxígeno y nutrientes esenciales. Conclusión: A pesar de estos mecanismos compensatorios, no logran contrarrestar por completo las manifestaciones clínicas, conduciendo a una serie de problemas de salud, como hipertensión pulmonar, insuficiencia cardíaca, hepatomegalia, hipoperfusión de órganos y déficits neurológicos. Estos factores convergen para generar una compleja condición cardíaca que desencadena respuestas adaptativas en el cuerpo que terminan siendo una afección médica desafiante y potencialmente grave.
Abstract Introduction: Persistent truncus arteriosus is a rare congenital cardiac malformation that causes various complications in the cardiovascular system. It is characterized by the presence of a defective ventricular septum, a single truncal valve and a common truncus arteriosus between the pulmonary artery and aorta, leading to a mixture between arterial and venous blood, due to a predominantly left-to-right bidirectional cardiac shunt that compromises the supply of blood flow, nutrients, and systemic oxygenation. Clinical manifestations include desaturation with cyanosis, hypoxemia, tachycardia, tachypnea, alterations in cardiac contractility, abnormal distal pulses, weight loss, fatigue, and hepatomegaly. Aim: The purpose of this research is to establish hypotheses about the various compensatory mechanisms that are activated at a systemic level to counteract the effects of this malformation. Reflection: It is suggested that similar biomolecular responses occur in the cardiovascular, pulmonary, and renal systems, reducing nitric oxide production and causing vasoconstrictive responses. At the liver level, growth factors are generated and angiogenesis processes are initiated to increase blood perfusion. In the brain, enzymes are activated to increase blood flow and provide oxygen and essential nutrients. Conclusion: Despite these compensatory mechanisms, they fail to completely counteract the clinical manifestations, leading to a series of health problems such as pulmonary hypertension, heart failure, hepatomegaly, organ hypoperfusion, and neurological deficits. These factors converge to generate a complex cardiac condition that triggers adaptive responses in the body that end up being a challenging and potentially serious medical condition.

Abstract During the preanesthetic assessment of the obstetric patient, it is critical to assess the patient's cardiovascular baseline condition, identify any potential risks, and facilitate behavioral modification to develop an individualized management strategy aimed at minimizing complications. Cardiac point-of-care ultrasound (POCUS) is a valuable instrument for assessing the morphology and function of the heart prior to surgery. Cardiac POCUS is not designed to replace comprehensive transthoracic echocardiography, which is the realm of cardiovascular anesthesiology and cardiology. However, when used in conjunction with anamnesis, physical examination, electrocardiogram, and previous laboratory results, cardiac POCUS is a valuable adjunct in the diagnostic toolbox of anesthesiologists. It allows for direct visualization of the heart and great vessels, with added benefits of speed, availability, and low risk for the patient. The purpose of this manuscript is to explore and describe the advantages of cardiac POCUS in the preanesthetic period of obstetric patients and its potential value for anesthesiologists through the identification of potentially hazardous conditions that may require individualized preoperative management.
Resumen Durante la valoración preanestésica de la paciente obstétrica, es fundamental evaluar la condición cardiovascular basal de la paciente, identificar riesgos potenciales y facilitar modificaciones conductuales para desarrollar una estrategia de manejo individualizada, dirigida a minimizar las complicaciones. La ecografía cardíaca a la cabecera del paciente (POCUS) es un instrumento valioso para evaluar la morfología y la función del corazón antes de un procedimiento quirúrgico. El POCUS cardiaco no está diseñado para sustituir a la ecocardiografía transtorácica formal, que corresponde al área de anestesiología cardiovascular y a la cardiología. Sin embargo, cuando se usa en conjunto con la anamnesis, el examen físico, el electrocardiograma y los resultados de exámenes de laboratorio previos, el POCUS cardiaco es un complemento valioso dentro del arsenal diagnóstico de los anestesiólogos. Permite la visualización directa del corazón y de los grandes vasos, con beneficios adicionales de velocidad, disponibilidad y bajo riesgo para la paciente. El objetivo del presente manuscrito es explorar y describir las ventajas del POCUS cardiaco durante el periodo preanestésico en pacientes obstétricas y su valor potencial para los anestesiólogos, a través de la identificación de condiciones eventualmente peligrosas que pudieran requerir un manejo preoperatorio individualizado.

Objetivo: identificar características clínicas das paradas cardiopulmonares e reanimações cardiopulmonares ocorridas em ambiente intra-hospitalar. Método: estudo quantitativo, prospectivo e observacional, a partir de informações de prontuários de pacientes submetidos a manobras de reanimação devido à parada cardiopulmonar entre janeiro e dezembro de 2021. Utilizou-se um instrumento baseado nas variáveis do modelo de registro Utstein. Resultados: em 12 meses foram registradas 37 paradas cardiopulmonares. A maioria ocorreu na unidade de terapia intensiva respiratória, com causa clínica mais prevalente hipóxia. 65% dos pacientes foram intubados no atendimento e 57% apresentaram ritmo atividade elétrica sem pulso. A duração da reanimação variou entre menos de cinco a mais de 20 minutos. Como desfecho imediato, 57% sobreviveram. Conclusão: dentre os registros analisados, a maior ocorrência de paradas cardiopulmonares foi na unidade de terapia intensiva respiratória, relacionada à Covid-19. Foram encontrados registros incompletos e ausência de padronização nas condutas.
Objective: identify the clinical characteristics of cardiopulmonary arrests and cardiopulmonary resuscitations in the in-hospital environment. Method: this is a quantitative, prospective and observational study based on information from the medical records of patients who underwent resuscitation maneuvers due to cardiopulmonary arrest between January and December 2021. An instrument based on the variables of the Utstein registration protocol was used. Results: thirty-seven cardiopulmonary arrests were recorded in 12 months. The majority occurred in a respiratory intensive care unit, with hypoxia being the most prevalent clinical cause. Sixty-five percent of the patients were intubated and 57% had pulseless electrical activity. The duration of resuscitation ranged from less than five to more than 20 min. As for the immediate outcome, 57% survived. Conclusion: among the records analyzed, the highest occurrence of cardiopulmonary arrests was in respiratory intensive care units, and they were related to Covid-19. Moreover, incomplete records and a lack of standardization in cardiopulmonary resuscitation procedures were found.

Método: estudio cuantitativo, prospectivo y observacional, realizado a partir de información presente en historias clínicas de pacientes sometidos a maniobras de reanimación por paro cardiorrespiratorio entre enero y diciembre de 2021. Se utilizó un instrumento basado en las variables del modelo de registro Utstein. Resultados: en 12 meses se registraron 37 paros cardiopulmonares. La mayoría ocurrió en la unidad de cuidados intensivos respiratorios, la causa clínica más prevalente fue la hipoxia. El 65% de los pacientes fue intubado durante la atención y el 57% presentaba un ritmo de actividad eléctrica sin pulso. La duración de la reanimación varió entre menos de cinco y más de 20 minutos. Como resultado inmediato, el 57% sobrevivió. Conclusión: entre los registros analizados, la mayor cantidad de paros cardiopulmonares se dio en la unidad de cuidados intensivos respiratorios, relacionada con Covid-19. Se encontraron registros incompletos y falta de estandarización en el procedimiento.

Cardiac disease is an important cause of maternal mortality and morbidity in ante partum and postpartum period. Incidence of heart disease in pregnancy is <1%. Pre pregnancy diagnosis, counselling, antenatal supervision, delivery at equipped centre and management with multidisciplinary approach favours the maternal and fetal outcome. This consists of case series of 16 patients with cardiac disease admitted in the department of obstetrics and gynecology at Gulbarga institute of medical sciences, Kalaburgi from January 2022 to June 2023.

Introducción: La capacitación en reanimación cardiopulmonar (RCP) y uso correcto del desfibrilador externo automático (DEA) permite la intervención de los ciudadanos legos en situaciones de paro cardíaco extrahospitalario (PCEH). En Paraguay, la Ley n° 5.578/2016 establece el uso obligatorio del DEA en centros de concurrencia masiva (CCM). Objetivo: Evaluar la efectividad de un programa de capacitación intensiva en reanimación cardiopulmonar y uso del desfibrilador externo automático entre representantes de centros de concurrencia masiva en Asunción julio-setiembre 2023. Metodología: Estudio cuasiexperimental antes/después en representantes de CCM (Universidades, Shoppings, Clubes sociales/deportivos Gimnasios, Consecionario Automotríz, Complejos residenciales y Supermercados). Se obtuvieron datos de universidades del Ministerio de Educación y Ciencias, de fuentes municipales y la Cámara de Comercio. Se capacitó entre 1-3 representantes/CCM mediante talleres teórico-práctico sobre RCP básico y uso correcto del DEA en PCEH con un cuestionario validado (alfa de Cronbach: 0,75). Se compararon datos con χ² y prueba t o U de Mann-Whitney (p-valor significativo =<0,05). Resultados: De 194 participantes, la edad meda fue; 29±5años; el 60,31% (117) del sexo masculino, 38,65%(70) representantes de shoppings, 60,82% (118) tenian formación universitaria, y el 45,36% (88) se desempeñaban en atención al cliente. El 59, 228% (115) no habia recibido capacitación en RCP anteriormente y el 86,08% (167) tampoco tenia entrenamiento en uso del DEA. Se pudo observar una diferencia estadisticamente significativa entre la puntaje antes y después, observandose una diferencia de 4,791ï± 1,012 puntos (p=0,0001). Discusión: El programa de capacitación intensiva es efectivo para elevar el nivel de conocimiento y práctica en RCP básica y uso del DEA entre representantes legos de CCM.
Introduction: Training in cardiopulmonary resuscitation (CPR) and correct use of the automated external defibrillator (AED) allows the intervention of lay citizens in situations of out-of-hospital cardiac arrest (EHPA). In Paraguay, Law No. 5,578/2016 establishes the mandatory use of the AED in mass attendance centers (CCM). Methodology: quasi-experimental before/after study in CCM representatives (Universities, shopping malls, Social/Sports Clubs, Gyms, Automotive Dealership, Residential Complexes and Supermarkets). Data were obtained from universities of the Ministry of Education and Sciences, municipal sources, and the Chamber of Commerce. Between 1-3 representatives/MCCs were trained through theoretical-practical workshops on basic CPR and correct use of the AED in HCWP with a validated questionnaire (Cronbach's alpha: 0.75). Data were compared with χ² and Mann-Whitney t-test or U test (p-significant value =<0.05). Results: Of 194 participants, the mean age was 29±5; 60.31% (117) were male, 38.65% (70) were shopping mall representatives, 60.82% (118) had a university education, and 45.36% (88) worked in customer service. 59.228% (115) had not previously received CPR training and 86.08% (167) had no AED training. A statistically significant difference was observed between the before and after score, with a difference of 4.791(1.012 points (p=0.0001). Discussion: The intensive training program is effective in raising the level of knowledge and practice in basic CPR and AED use among lay CCM representatives.

Poland syndrome is characterized by the absence or hypoplasia of other chest muscles: small pectoral muscle, anterior tightened muscle, grandorsal muscle, and deltoid muscle. Anomalies of the thoracic cage: agenesis or hypoplasia of one or more costal segments, pectus carinatum, excavatum, clavicular hypoplasia, pulmonary herniation. Abnormalities of the mammary region: agenesis or hypoplasia of the breast, areola and nipple. The reported incidence of Poland’s syndrome ranges from 1 in 7,000 to 1 in 100,000. Males are affected more frequently by a 2:1 to 3:1 ratio Poland’s syndrome has also been diagnosed in 1 of 19,000 mammograms. The right side of the body was found to be involved in 60% to 75% of patients - associated malformations: genitourinary malformations, cardiac malformation, hepatic and biliary tract malformations. Case report Our case involves a 7-month-old boy with no medical history of cardiac or respiratory complaints. This is a male child with around 65cm height, and 7kg weight. There is no family history of consanguineous marriage and no family history of congenital anomalies. All siblings are alive and healthy. The child’s milestones are within normal limits. There is aplasia of the right sternocostal head and clavicular heads and abdominal head of the pectoralis major muscle, there is a hypoplasia of the right nipple and breast and absence of subcutaneous fat on the same side. Ribs are visible on the right chest wall. Physical examination shows no history of hand and digit anomaly and no rib anomalies. Chest wall asymmetry with right chest wall showing depression on axillary floor. The anterior axillary fold is absent. On attempting abduction of the shoulder, it shows the absence of the sternocostal head of the pectoralis major.

Cardiomyopathy is defined as a heterogeneous group of myocardial disorders wherein the cardiac muscle is functionally and structurally abnormal, without the presence of any congenital heart disease (CHD), coronary artery disease (CAD), valvular disease, and hypertension sufficient enough to lead to abnormality of the myocardium. Based on etiology, cardiomyopathies are classified into primary (acquired, mixed, or genetic) and secondary, which results in different phenotypes like hypertrophic, restrictive, dilated, etc. patterns. Hypertrophic cardiomyopathy is the most common type of primary cardiomyopathy among all cardiomyopathies usually presenting as exertional dyspnea, heart failure, atypical chest pain, syncope, and sudden cardiac death (SCD). Dilated cardiomyopathy is genetic or acquired, causing classic symptoms of heart failure with reduced ejection fraction. Restrictive cardiomyopathy is mostly associated with systemic disease and is rare. Diagnosis of cardiomyopathy includes a detailed evaluation of history, and physical examination followed by a workup including blood test, genetic testing, electrocardiography, and echocardiography testing. Treatment includes initially staging the therapy for heart failure, restriction of physical activity, evaluation of the need for implantable cardioverter-defibrillators, optimization of drugs, and consideration of heart transplantation in refractory cases. Genetic testing of families is now available as an emerging modality for early diagnosis and prevention in relatives of diagnosed cases. This review evaluates the causes, early diagnosis, and early treatment and prevention modalities for cardiomyopathies to reduce morbidity and mortality caused by it.

A systemic inflammatory disease known as rheumatoid arthritis (RA) is distinguished by excessive cardiovascular disease (CVD) morbidity and death. Traditional CV risk factors may partially contribute to CV disease in RA. Shared inflammatory mediators, post-translational modifications of peptides/proteins and subsequent immune responses, changes in the composition and function of lipoproteins, increased oxidative stress, and endothelial dysfunction are some of the mechanisms that link RA and CVD. The detailed pathogenetic pathway by which this association between RA and CVD might be explained is still not entirely known. It is crucial for controlling cardiovascular risk in people with RA. Optimizing care of traditional risk factors in addition to those inherent to RA is necessary to lessen the burden caused by CVD. The potential effect of planned Cardiac risk management in these individuals is highlighted by findings for under diagnosis and inadequate treatment of conventional CVD risk factors in RA. Present cardiovascular standards suggest RA patients to be examined for and treated for CVD risk factors without appropriate treatment goals. Utilizing potent anti-rheumatic medications that can reduce disease activity and treating the conventional CV risk factors should both be part of the therapy of CV risk in RA. There is currently insufficient scientific data to develop therapy targets for RA-related CVD risk factors. Thus, more study is required on the traditional CVD risk factor screening and management in RA patients.

Cardiovascular disease acts as a predominant cause for death in India. Digital technology has shown great potential in delivering cardiac rehabilitation remotely to patients with minimal cost implications. The main aim of this study was to analyze the comprehensiveness and effectiveness of digital cardiac rehabilitation intervention for cardiovascular patients on the Indian Cardiology Continuum. A comprehensive literature search was performed in Pubmed, Springer-link, Elsevier, Embase and Google Scholar. The selected tenure for this review was 10 years (16 January 2014 to 16 January 2024). Authors separately conducted the assessment of the selected studies by using the quality assessment of diagnostic accuracy studies. Microsoft excel 2020 was used for statistical analysis and graphical designs. A total of 3 articles were included in the review. The median sample size of the included studies is 153 (30-304), mean study timeline is 7.67 months (3-17), and median follow-up period is 6 months (3-12). Digital therapeutics showed significant reduction in Systolic Blood Pressure (34.67 mmHg) and Diastolic Blood Pressure (21.97 mmHg) in stage III hypertensive patients. Medication adherence of patients using digital therapeutics was 90%, while 10% patients remained non-adherent/dropped out of the trial. Digital therapeutics builds a lucrative bridge to help cardiovascular patients cross over into improved QoL, in place of traditional face-to-face CR regimes. Further research is required to improve comprehensiveness and clinical effectiveness of the digital cardiac rehab programs on a long-term basis in India.

Abstract Sudden cardiac death is a common occurrence. Out-of-hospital cardiac arrest is a global public health problem suffered by ≈3.8 million people annually. Progress has been made in the knowledge of this disease, its prevention, and treatment; however, most events occur in people without a previous diagnosis of heart disease. Due to its multifactorial and complex nature, it represents a challenge in public health, so it led us to work in a consensus to achieve the implementation of cardioprotected areas in Mexico as a priority mechanism to treat these events. Public access cardiopulmonary resuscitation (CPR) and early defibrillation require training of non-medical personnel, who are usually the first responders in the chain of survival. They should be able to establish a basic and efficient CPR and use of the automatic external defibrillator (AED) until the emergency services arrive at the scene of the incident. Some of the current problems in Mexico and alternative solutions for them are addressed in the present work.
Resumen La muerte súbita cardíaca (SCD) es un acontecimiento común. El paro cardiaco extrahospitalario (OHCA) es un problema de salud pública mundial que sufren ≈3.8 millones de personas al año. Se ha avanzado en el conocimiento de esta enfermedad, su prevención y tratamiento, sin embargo, la mayoría de los eventos se producen en personas sin diagnóstico previo de cardiopatía. Debido a su carácter multifactorial y complejo, representa un reto en salud pública, lo que obliga a trabajar en un consenso para lograr la implementación de "Espacios Cardio protegidos" en México, como mecanismo prioritario de atención a estos eventos. La reanimación cardiopulmonar básica (RCPB) y la desfibrilación temprana de acceso público requieren de entrenamiento al personal no médico, que suelen ser los primeros respondientes para iniciar la cadena de la supervivencia. Ellos deberían instaurar una RCPB eficiente y el uso del desfibrilador automático externo (AED) hasta que lleguen al lugar del incidente los servicios de emergencias. El presente trabajo menciona algunos de los problemas actuales en México y algunas opciones de solución para los mismos.

RESUMEN Introducción: La terapia de resincronización cardíaca (TRC) es un tratamiento eficaz en pacientes con insuficiencia cardíaca (IC), baja fracción de eyección del ventrículo izquierdo (FEVI) y QRS ancho. Sin embargo, hay un porcentaje de estos que son no respondedores, lo que implicaría peores resultados clínicos. Objetivos: Valorar las diferencias en parámetros ecocardiográficos de remodelado reverso y tasa de eventos [hospitalización por insuficiencia cardíaca (HIC), muerte de todas las causas (MT), trasplante cardíaco (TXC) y terapias apropiadas (TA)] entre pacientes respondedores vs. no respondedores a la TRC. Material y métodos: Se incluyeron 343 pacientes con TRC. Se los clasifico en respondedores y no respondedores según parámetros clínicos y ecocardiográficos. Se realizó seguimiento a 2 años, en que se evaluó el remodelado reverso y la incidencia de HIC, MT, TXC y TA. Resultados: De los 343 pacientes, 17% fueron no respondedores y 83% respondedores. A los 6 meses y 12 meses no hubo diferencias significativas en cuando a diámetros ventriculares, pero si en la FEVI (p<0,001), que aumentó más en los respondedores. A los 24 meses los respondedores presentaron menor diámetro diastólico (p=0,004), menor diámetro sistólico (p=0,003) y mayor FEVI (p<0,001) Los no respondedores tuvieron significativamente mayor incidencia de HIC (p< 0,001), TXC (p=0,001) y TA (p=0,002) y un exceso de MT en el límite de la significación estadística (p= 0,056). Conclusiones: Los pacientes respondedores a la TRC presentaron mayor remodelado reverso y mejor evolución clínica, en forma acorde a los resultados de estudios observacionales internacionales.
ABSTRACT Background: Cardiac resynchronization therapy (CRT) is an effective treatment in patients with heart failure (HF), low left ventricular ejection fraction (LVEF) and wide QRS. However, there are a percentage of these patients who are non-responders, implying worse clinical outcomes. Objectives: The aim of this study was to assess the differences in echocardiographic parameters of reverse remodeling and event rates [hospitalization for heart failure (HHF), all-cause mortality (ACM), heart transplantation (HTX) and appropriate therapies (AT)] between responder vs. non-responder patients to CRT. Methods: A total of 343 patients with CRT, classified into responders and non-responders according to clinical and echocardiographic parameters, were included in the study. A 2-year follow-up was performed, in which reverse remodeling and the incidence of HHF, ACM, HTX and AT were evaluated. Results: Among the 343 patients, 17% were non-responders and 83% responders. At 6 and 12 months there were no significant differences in ventricular diameters, but significant differences in LVEF (p<0.001), with greater increase in responders. At 24 months, responders had smaller diastolic diameter (p=0.004), smaller systolic diameter (p=0.003) and higher LVEF (p<0.001). Non-responders had significantly higher incidence of HHF (p<0.001), HTX (p=0.001) and AT (p=0.002), and an excess of ACM at the limit of statistical significance (p= 0.056). Conclusions: Patients responding to CRT presented greater reverse remodeling and better clinical evolution, in accordance with the results of international observational studies.

Abstract Introduction: Every year, five million people around the world experience an out-of-hospital cardiac arrest (OHCA) and less than 40 % receive any assistance before the arrival of the Emergency Medical Services (EMS). Ambulance operators (AO) take care of people experiencing an OHCA, stabilize and then transfer them. In Medellín, Colombia, there is a public AO and several private providers, but the information about an OHCA and the operational characteristics during the response to the event are limited. Objective: To estimate the incidence of OHCA and to explore the factors associated with survival after the event in Medellín city. Methods: Retrospective, population-based cohort study. All the medical records of patients experiencing an OHCA who were assessed and treated by ambulance operators, (AO) of the Emergency Medical Services (EMS) and private agencies in Medellin city were reviewed. Descriptive statistics were used for data analysis and the annual incidence of the event standardized for the general population was estimated. Potential survival-associated factors reported as OR with their corresponding 95% CI were explored. Results: A total of 1,447 patient records with OHCA between 2018 and 2019 were analyzed. The event incidence rate for the number of cases assessed was 28.1 (95 % CI 26.0-30.3) and 26.9 (95 % CI 24.929.1) cases per 100,000 inhabitants/year for 2018 and 2019, respectively; the incidence rate of treated OHCA was 2.6 (95 % CI 2.0-3.3) and 3.2 (95 % CI 2.5-4.0) per 100,000 inhabitants/year, for 2018 and 2019. Survival on arrival at hospitals of treated cases was 14.2 % (95 % CI 5.5-22.8) and 15.5 % (95 % CI 7.4-23.5) for 2018 and 2019, respectively. Conclusions: This study portrays the operating and care characteristics of the population experiencing OHCA in Medellín city. The incidence rate of the event and the survival were lower than those reported in the literature.
Resumen Introducción: Cada año, cinco millones de personas en el mundo presentan paro cardiaco prehospitalario (PCEH), de los cuales menos del 40 % reciben ayuda antes de la llegada de los sistemas de emergencia médica (SEM). Los operadores de ambulancias (OA) atienden a los que sufren un PCEH, su estabilización y posterior traslado. En Medellín, Colombia, existe un OA público y agencias privadas, pero la información acerca del PCEH y las características operacionales durante la respuesta a este evento es escasa. Objetivo: Estimar la incidencia del PCEH y explorar factores asociados a la supervivencia del evento en la ciudad de Medellín. Métodos: Estudio de cohorte retrospectivo de base poblacional. Se revisaron todos los registros médicos de pacientes que presentaron un PCEH que fueron evaluados y tratados por OA del SEM y de agencias privadas de Medellín. Se usaron estadísticos descriptivos para los datos y se estimó la incidencia anual del evento estandarizada para la población general. Se exploraron posibles factores asociados a la supervivencia, reportados como OR con su respectivos IC 95 %. Resultados: Se analizaron 1.447 registros de pacientes con PCEH presentados entre 2018 y 2019. La tasa de incidencia del evento para los casos evaluados fue de 28,1 (IC 95 % 26,0-30,3) y 26,9 (IC 95 % 24,9-29,1) casos por 100.000 habitantes/año, para 2018 y 2019, respectivamente; la tasa de incidencia del PCEH tratado fue de 2,6 (IC 95 % 2,0-3,3) y 3,2 (IC 95 % 2,5-4,0) por 100.000 habitantes/año, para 2018 y 2019. La supervivencia a la llegada a los hospitales de los casos tratados fue 14,2 % (IC 95 % 5,5-22,8) y del 15,5 % (IC 95 % 7,4-23,5) para 2018 y 2019, respectivamente. Conclusiones: Se muestra las características operativas y de atención de la población que presenta un PCEH en Medellín. La tasa de incidencia del evento y la supervivencia fueron menores a las reportadas en la literatura.

Abstract Postoperative acute kidney injury is an underdiagnosed condition. Its incidence is variable and depends on demographic, clinical, and surgical stress-associated factors; hence the pathophysiology is multifactorial. It is extremely important to acknowledge those risk factors early and use tools to estimate the risk of developing the condition, in order to adopt perioperative measures to mitigate its occurrence and impact. Some of the complications resulting from this condition include prolonged ICU stay, higher susceptibility for infections, hospitalization-related complications, progression to acute and chronic kidney failure, and even the need for transient or permanent renal replacement therapies (RRT) in addition to diseases that increase the cardiovascular risk, such as systemic high blood pressure and/or coronary heart disease that result in increased comorbidities and mortality, with subsequent increases in healthcare costs, lower quality of life and increased burden of the disease in the short and long term. A systematic search of the literature was conducted in PubMed, Google Schoolar, and Lilacs, under the terms MeSh and DeCs using Boolean operators; a review was conducted of the summary of the articles identified and 57 of them were selected for their comprehensive reading. This narrative review summarizes the relevant information on this pathology for prevention and identification purposes, for the adequate management of patients undergoing major non-cardiac surgery.
Resumen La lesión renal aguda posoperatoria es una entidad subdiagnosticada. Su incidencia es variable y depende de factores demográficos, clínicos y los inherentes al estrés quirúrgico; por ende, la fisiopatología es multifactorial. Es de vital importancia reconocer precozmente dichos factores de riesgo durante la valoración preanestésica y hacer uso de herramientas para calcular el riesgo de desarrollarla para, de esta forma, adoptar medidas perioperatorias para mitigar su aparición e impacto. Entre las complicaciones derivadas de esta entidad se encuentran la estancia en unidades de cuidado intensivo prolongadas, mayor susceptibilidad de infecciones, complicaciones derivadas de la hospitalización, progresión a enfermedad renal aguda y crónica e incluso necesidad de terapias de reemplazo renal (TRR) transitorias o permanentes, además de enfermedades que aumentan el riesgo cardiovascular como hipertensión arterial sistémica y/o enfermedad coronaria, lo que genera aumento en morbimortalidad, con aumento en costos asociado a la atención en salud, menor calidad de vida y mayor carga de la enfermedad a corto y largo plazo. Se realizó una búsqueda sistemática de la literatura en bases de datos PubMed, Google Schoolar, Lilacs, bajo los términos MeSh y DeCs, usando los operadores booleanos, se revisó el resumen de los artículos encontrados y se seleccionaron 57 artículos para su lectura completa. Esta revisión narrativa sintetiza la información relevante sobre esta patología que permita prevenirla y reconocerla para el manejo adecuado de los pacientes que van a cirugía mayor no cardiaca.

Resumen Introducción: Aproximadamente el 33% de los pacientes con enfermedades cardiovasculares requerirán cirugía cardíaca al menos una vez en la vida; sin embargo, menos de una cuarta parte de la población mundial tiene acceso a la atención quirúrgica cardíaca cuando la necesita. A pesar del progreso que ha tenido Colombia en cuanto a la prestación de atención quirúrgica cardíaca en las últimas décadas, se sabe poco sobre el acceso a la atención cardíaca en todo el país. Por lo tanto, la cirugía global busca estudiar y construir sobre la situación actual en áreas de acceso limitado a la atención médica quirúrgica y fortalecer los sistemas de salud. Objetivo: Describir la situación actual en Colombia en términos de personal quirúrgico y de infraestructura disponible, para entender mejor las brechas existentes en el acceso a la atención quirúrgica cardíaca para las poblaciones necesitadas. Materiales y método: Los datos sobre la fuerza laboral de la cirugía cardiaca se obtuvieron a partir de una encuesta a cirujanos registrados en el directorio de cirugía cardíaca y la Red de Cirugía Cardiotorácica de Colombia, en tanto que los de procedimientos del 2018 al 2019, se obtuvieron de los datos del gobierno nacional. Resultados: En Colombia había 110 cirujanos cardíacos o 1.8 cirujanos cardíacos por millón de habitantes, de los cuales el 85% eran hombres. Las densidades en cada uno de los 32 departamentos de Colombia variaron desde 4.6 cirujanos por millón de habitantes (Bogotá), a ningún cirujano en 14 departamentos. Se registraron 52 instituciones, con una mediana de 250 camas (rango intercuartílico 130-350). Uno de cada cinco departamentos de cirugía cardíaca ofreció un programa de subespecialidad en cirugía cardíaca. La revascularización miocárdica fue el procedimiento realizado con mayor frecuencia. Conclusiones: Este estudio identificó datos sobre la situación actual de la cirugía cardíaca en Colombia. A pesar de la disponibilidad relativamente favorable de mano de obra quirúrgica cardíaca en Colombia, la variación geográfica y los factores sociales y económicos apuntan a una necesidad urgente de evaluar las políticas de calidad de atención relacionadas con la atención quirúrgica cardíaca en poblaciones desatendidas.
Abstract Introduction: Approximately 33% of patients with cardiovascular diseases will require heart surgery at least once in their lifetime; yet, less than a quarter of the worlds population has access to cardiac surgical care when needed. Despite Colombias progress in cardiac surgical care delivery in recent decades, little is known regarding access to cardiac care across the country. Therefore, global surgery seeks to study and build upon the current situation in areas of limited access to surgical healthcare and to strengthen health systems. Objetive: Describe the current situation in Colombia in terms of surgical personnel and available infrastructure, to better understand the existing gaps in access to cardiac surgical care for populations in need. Materials and method: Data on the cardiac surgical workforce were obtained from a survey of surgeons registered in the cardiac surgery directory and the Cardiothoracic Surgery Network in Colombia. Procedural data from 2018-2019 were obtained from national government data. Results: There were 110 cardiac surgeons or 1.8 cardiac surgeons per million inhabitants in Colombia, of which 85.0% were male. Densities in each of the 32 departments of Colombia varied from 4.6 surgeons per million inhabitants (Bogotá) to no surgeons in 14 departments. There were 52 institutions registered, with a median of 250 beds (interquartile range 130-350). One in five cardiac surgery departments offered a certified cardiac surgery fellowship program. Coronary artery bypass grafting was the most frequently performed procedure. Conclusions: This study identified data regarding the current situation of cardiac surgery in Colombia. Despite relatively favorable cardiac surgical workforce availability in Colombia, geographical variation and social and economic factors point to an urgent need to evaluate the quality-of-care policies related to cardiac surgical care in underserved populations.

Resumen El uso de dispositivos cardiacos implantables ha aumentado en los últimos años, lo que ha generado un aumento en el número de procedimientos de extracción de electrodos a medida que se intervienen pacientes con un mayor número de comorbilidades. En la actualidad, la técnica percutánea de extracción es de elección en la mayoría de los pacientes debido a los excelentes resultados alcanzados y a una morbi-mortalidad asociada inferior a la de la extracción quirúrgica. No obstante, algunos pacientes pueden presentar retos que aumentan el grado de dificultad técnica del procedimiento, entre los que se encuentran variantes anatómicas, calcificaciones extensas o la necesidad de extraer electrodos de fijación pasiva. Se expone el caso de la extracción de un electrodo disfuncionante en una paciente portadora de marcapasos por bloqueo auriculoventricular completo, con estimulación diafragmática por electrodo ventricular de fijación pasiva implantado a través de una vena cava superior izquierda persistente en una vena posterolateral del seno coronario.
Abstract The use of cardiac electronic devices has increased over the last decades, which has generated an increase in the number of electrode extraction procedures, as a greater number of patients with multiple comorbidities undergo cardiac electronic device implantation. Currently, the percutaneous technique is preferable in most patients, given its excellent clinical results and lower morbidity and mortality compared to surgical extraction. Nonetheless, some clinical scenarios increase procedural difficulty, including anatomical variants, or the need to extract passive fixation leads. We present a case of a patient with pacemaker due to complete atrioventricular block, with diaphragmatic stimulation caused by a passive fixation ventricular electrode that was implanted in a posterolateral vein of the coronary sinus through a persistent left superior vena cava, who underwent successful transvenous lead extraction.

SUMMARY: The average volumes of normal heart chambers in computed tomography (CT) are used not only as clinical criterions for heart disease diagnosis, but also as references in cardiology. With the development of artificial intelligence (AI), numerous CT data can be analyzed and segmented automatically. This study aimed to determine the average volumes of the four chambers in healthy adult hearts and present surface models with the average volume. Coronary CT angiographs of 508 Korean individuals (330 men and 178 women, 20 - 39 years old) were obtained. An automatic segmentation module for 3D Slicer was developed using machine learning in Anatomage KoreaTM. Using the module, the four chambers and heart valves in the CT were segmented and reconstructed into surface models. Surface models of the four chambers of identical hearts in the CT were produced using SimplewareTM. The volumes of structures were measured using Sim4life Light and statistically analyzed. After determining the average volumes of the four chambers, surface models of the average volumes were constructed. In both software measurements, the atrial volumes of females increased with age, and the ventricular volumes of males decreased significantly with age. The atrial and ventricular volumes of Simpleware were larger and smaller than those of Anatomage, respectively, because of errors in the Simpleware. Regarding the volume measurement, our module developed in this study was more accurate than the Simpleware. The average volume and three-dimensional models used in this study can be used not only for clinical purposes, but also for educational or industrial purposes.
Los volúmenes medios de las cámaras cardíacas normales en la tomografía computarizada (TC) se utilizan no sólo como criterios clínicos para el diagnóstico de enfermedades cardíacas, sino también como referencia en cardiología. Con el desarrollo de la inteligencia artificial (IA), numerosos datos de TC se pueden analizar y segmentar automáticamente. Este estudio tuvo como objetivo determinar los volúmenes promedio de las cuatro cámaras en corazones adultos sanos y presentar modelos de superficie con el volumen promedio. Se obtuvieron angiografías coronarias por TC de 508 individuos coreanos (330 hombres y 178 mujeres, de 20 a 39 años). Se desarrolló un módulo de segmentación automática para 3D Slicer utilizando aprendizaje automático en Anatomage KoreaTM. Utilizando el módulo, las cuatro cámaras y valvas cardíacas de la TC se segmentaron y reconstruyeron en modelos de superficie. Se produjeron modelos de superficie de las cuatro cámaras de corazones idénticos en la TC utilizando SimplewareTM. Los volúmenes de las estructuras se midieron utilizando Sim4life Light y se analizaron estadísticamente. Después de determinar los volúmenes promedio de las cuatro cámaras, se construyeron modelos de superficie de los volúmenes promedio. En ambas mediciones de software, los volúmenes atriales de las mujeres aumentaron con la edad y los volúmenes ventriculares de los hombres disminuyeron significativamente con la edad. Los volúmenes atrial y ventricular de Simpleware eran mayores y menores que los de Anatomage, respectivamente, debido a errores en Simpleware. En cuanto a la medición de volumen, nuestro módulo desarrollado en este estudio fue más preciso que el Simpleware. Los modelos tridimensionales y de volumen medio utilizados en este estudio se pueden utilizar no solo con fines clínicos, sino también con fines educativos o industriales.
