ABSTRACT
Objective To investigate the expression of activating transcription factor 6(ATF6)and interferon α(IFN-α)and their significance in laryngeal squamous cell carcinoma(LSCC)tissue.Methods A total of 100 LSCC patients admitted to Clinical Medical College of Henan University of Science and Technology/the First Affiliated Hospital of Henan University of Science and Technology from March 2015 to March 2020 were selected,and their clinicopathological features such as tumor location,degree of differentiation,and lymph node metastasis were collected and organized.Immunohistochemical method was applied to detect the expression of ATF6 and IFN-α in tissues.Spearman method was used to analyze the correlation between ATF6 and IFN-α expression in LSCC tissue.Kaplan-Meier method was applied to analyze the relationship between ATF6 and IFN-α expression in LSCC tissue and 3-year survival rate of patients.Cox regression was used to analyze the influencing factors of 3-year mortality in LSCC patients.Results The positive rate of ATF6 in LSCC tissue(76.00%)was higher than that in normal tissues adjacent to cancer(13.00%),the positive rate of IFN-α in LSCC tissue(29.00%)was lower than that in normal tissues adjacent to cancer(74.00%),and the difference was statistically significant(χ2=80.352,40.536,all P<0.05).The proportions of ATF6 positive expression in LSCC patients with TNM stage Ⅲ+Ⅳ,deep infiltration depth,and lymph node metastasis were significantly higher than those in LSCC patients with TNM stage Ⅰ+Ⅱ,shallow infiltration depth,and no lymph node metastasis(χ2=7.310,9.223,5.123,all P<0.05).The proportions of IFN-α negative expression in LSCC patients with TNM stage Ⅲ+Ⅳ,deep infiltration depth,and lymph node metastasis were significantly higher than those in LSCC patients with TNM stage Ⅰ+Ⅱ,shallow infiltration depth,and no lymph node metastasis(χ2=8.564,5.021,5.203,all P<0.05).There was a negative correlation between ATF6 and IFN-α expression in LSCC tissues(r=-0.415,P<0.05).The 3-year survival rate of LSCC patients in the ATF6 positive expression group(50.00%)was significantly lower than that in the ATF6 negative expression group(83.33%),while the 3-year survival rate of LSCC patients in the IFN-α positive expression group(82.76%)was significantly higher than that in the IFN-α negative expression group(47.89%)(Log rank χ2=8.002,10.854,all P<0.05).ATF6(HR=1.735,95%CI:1.159~2.598)and IFN-α(HR=0.624,95%CI:0.439~0.886)were influencing factors for the mortality of LSCC patients.Conclusion The positive expression rate of ATF6 increased and the positive expression rate of IFN-α decreased in LSCC tissues.They were closely related to the clinical pathological characteristics and prognosis of patients.

Objective To analyze the accuracy and influencing factors of miniprobe ultrasonic endoscope in evaluating submucosal infiltration of colorectal laterally spreading tumor(LST).Method A retrospective analysis was conducted on the clinical data of 213 patients(268 lesions in total)with colorectal LST who underwent endoscopic submucosal dissection(ESD)treatment from June 2018 to August 2021.We summarized the clinical pathological characteristics and miniprobe ultrasonic endoscope examination results of LST,then analyzed the accuracy of miniprobe ultrasonic endoscope examination and the risk factors affecting the accuracy of miniprobe ultrasonic endoscope examination.Results The accuracy rate of miniprobe ultrasonic endoscope examination was 93.28%,and there was a statistically significant difference in the accuracy rate of miniprobe ultrasonic endoscope examination between different lesion surface morphologies(P = 0.000).Multivariate Logistic regression analysis showed that mix-ed nodule and false depression lesions were risk factors for inaccurate EUS assessment.Conclusion Colorectal LST is a special type of tumor,and miniprobe ultrasonic endoscope examination has a high accuracy in evaluating its infiltration depth.The surface morphology of the lesion is a risk factor that affects the accuracy of miniprobe ultrasonic endoscope examination.When the lesion is a nodule mixed type or pseudo depressed type,it can easily lead to inaccurate miniprobe ultrasonic endoscope examination.
ABSTRACT
Objective: To investigate the risk factors for the development of deep infiltration in early colorectal tumors (ECT) and to construct a prediction model to predict the development of deep infiltration in patients with ECT. Methods: The clinicopathological data of ECT patients who underwent endoscopic treatment or surgical treatment at the Cancer Hospital, Chinese Academy of Medical Sciences from August 2010 to December 2020 were retrospectively analyzed. The independent risk factors were analyzed by multifactorial regression analysis, and the prediction models were constructed and validated by nomogram. Results: Among the 717 ECT patients, 590 patients were divided in the within superficial infiltration 1 (SM1) group (infiltration depth within SM1) and 127 patients in the exceeding SM1 group (infiltration depth more than SM1). There were no statistically significant differences in gender, age, and lesion location between the two groups (P>0.05). The statistically significant differences were observed in tumor morphological staging, preoperative endoscopic assessment performance, vascular tumor emboli and nerve infiltration, and degree of tumor differentiation (P<0.05). Multivariate regression analysis showed that only erosion or rupture (OR=4.028, 95% CI: 1.468, 11.050, P=0.007), localized depression (OR=3.105, 95% CI: 1.584, 6.088, P=0.001), infiltrative JNET staging (OR=5.622, 95% CI: 3.029, 10.434, P<0.001), and infiltrative Pit pattern (OR=2.722, 95% CI: 1.347, 5.702, P=0.006) were independent risk factors for the development of deep submucosal infiltration in ECT. Nomogram was constructed with the included independent risk factors, and the nomogram was well distinguished and calibrated in predicting the occurrence of deep submucosal infiltration in ECT, with a C-index and area under the curve of 0.920 (95% CI: 0.811, 0.929). Conclusion: The nomogram prediction model constructed based on only erosion or rupture, local depression, infiltrative JNET typing, and infiltrative Pit pattern has a good predictive efficacy in the occurrence of deep submucosal infiltration in ECT.
