ABSTRACT

BACKGROUND: There is an inflammatory response in the lesion tissue of ischemic cerebral infarction, and the expression of miR-150-5p is significantly decreased. Whether miR-150-5p inhibits the release of inflammatory factors and alleviates the injury of ischemic cerebral infarction tissue through the Toll-like receptor-5/nuclear factor-KB pathway remains unclear. OBJECTIVE: To investigate the role and preliminary mechanism of miR-150-5p in ischemic cerebral infarction in rats. METHODS: (1) The rat models of middle cerebral artery occlusion were constructed and the rat models were divided into five groups: Control, miR-150-5p agomir, agomir control, miR-150-5p antagomir and antagomir control groups. (2) The rats in the control group was given the intracerebroventricular injection of normal saline, and the rats in the latter four groups were given the intracerebroventricular injection of miR-150-5p agomir (miR-150-5p agonist), agomir negative control, miR150-5p antagomir (miR150-5p inhibitor) and antagomir negative control, respectively. (3) After 7 days, the brain was graded by modified neurological severity score, the cerebral infarct volume was measured by MRI, and the histopathological changes were observed by hematoxylin-eosin staining. The expression levels of miR-150-5p, interleukin-6, tumor necrosis factor-a, Toll-like receptor-5 and nuclear factor-KB p65 in brain tissues were detected by qRT-PCR, ELISA and western blot assay, respectively. The target relationship between miR150-5p and Toll-like receptor-5 was verified by luciferase assay by retrieving the bioinformatics website Targetscan to predict the binding sites of miR-150-5p and Toll-like receptor-5. RESULTS AND CONCLUSION: (1) Compared with the control group, the modified neurological severity score, and levels of interleukin-6, tumor necrosis factor-a, Toll-like receptor-5 and nuclear factor-KB p65 proteins were significantly decreased in the miR-150-5p agomir group (P 0.05). (3) TargerScan website prediction results and luciferase reporter gene analysis results showed that miR-150-5p and Toll-like receptor-5 had a targeted binding site. (4) These results imply that miR-150-5p can inhibit the inflammatory signaling pathway of Toll-like receptor-5/nuclear factor-KB p65 in brain injury caused by ischemia and reduce the inflammatory response, thereby alleviating the damage of nerve function and playing a protective role.

ABSTRACT

Objective:To investigate the effect of bone marrow mesenchymal stem cell transplantation on immunoinflammatory response in experimental cerebral infarction rats,and to provide a method for the treatment of cerebral infarction.Methods:①Experimental rat model of ischemic cerebral infarction was established.Sixty SD rats in the lab from March 2015 to May 2017 were selected to complete the ischemic cerebral infarction model established by modified Zea-longa suture method.②Acquisition and culture of stem cells.In addition,5 rats were used to wash the bone marrow cavity with DMEM medium to collect bone marrow cells and the rat bone marrow mesenchymal stem cells could be isolated and prepared by adherent method.③Treatment of model rats.After successful modeling,60 rats were randomly divided into model group,control group and experimental group.After modeling,the model group only performed vascular ligation and did not insert the line plug;the control group injected equal volume of PBS at the lesion site;the experi-mental group injected 10 μl of third-generation bone marrow mesenchymal stem cells at the lesion site and measured the immune levels of the three groups of rats.To evaluate the effect of bone marrow mesenchymal stem cells in experimental rats with ischemic cerebral in-farction.Results:①In the inverted microscope,the volume of the first generation of MSCs became polygonal or fusiform on the third day,and the cells started to aggregate.The third generation of cells proliferated rapidly and arranged in a certain direction with swirling growth,and had multiple layers,the boundaries are unclear;②Fluorescence microscopy showed that:99% of cells obtained by fluorescence labeling,the obtained cells were bone marrow mesenchymal stem cells;③Control group and model group 1 month after treatment,there was no significant difference in neurological score between 2 months and 3 months(P>0.05).Neurological scores at 1, 2 and 3 months after treatment in the observation group were lower than those in the control group and the model group(P<0.05);④The levels of IL-6 and IFN-γ in the observation group were higher than those in the control group at 3 months after treatment(P<0.05).The levels of IL-10 and TGF-β in the observation group at 3 months after treatment were lower than those in the control group(P<0.05).⑤The percentage of CD4+CD25+Tregs in CD4+T cells at 1 month,2 months and 3 months after treatment in observation group was higher than that in control group and model group(P<0.05).Conclusion:Transplantation of bone marrow mesenchymal stem cells can help regulate rat immunity,reduce the level of inflammatory cytokines and promote the early recovery of neurological function.

ABSTRACT

Objective To investigate the clinical safety and efficacy of mechanical thrombectomy(MT) with Solitaire stent compared with the selective intra-arterial thrombolysis (IAT) in the treatment of acute cerebral infarction.Methods Totally 82 cases patients with severe acute ischemic stroke caused by middle cerebral artery stenosis from January 2014 to May 2016 in the stroke treatment center of the first people's Hospital in ZunYi city who were applied with the mechanical thrombectomy or the selective intra-arterial thrombolysis was included,and a comparative analysis was conducted on the mTICI rating to assess the interventional recanalization,the NIHSS score after the treatment,bleeding rate,and conditions of neurological functional recovery 90 days after operation.Results There were 42 cases applied with the mechanical thrombectomy (MT group),40 cases applied with the intra-arterial thrombolysis (IAT group).The total effective rate reached 85.70% in the MT group and 62.50% in the IAT group,the difference was significant (P < 0.05).Compared with NIHSS score before operation,the score after the treatment showed a decreased trend.And the NIHSS score of MT group was better than that of the IAT group in a week after the treatment (P < 0.05).The bleeding rate was lower in the MT group with statistically significant difference (P < 0.05).Conclusion Compared to IAT,MT can provide broader time window,higher recanalization rate and better outcome in patients with severe acute ischemic stroke.

ABSTRACT

Objective To observe the clinical effect of edaravone combined with citicoline for treatment of acute ischemic cerebral infarctio.Methods Eighty-two cases patients of acute cerebral infarction were selected from March 2012 to August 2013 in the General Hospital of XingTai Mining group.According to admission order,they were randomly divided into observation group and control group,and each group of 41 cases.Single blind studies were carried out and on the basis of conventional treatment methods,the control group was given ozagrel sodium 80 mg,2 times/d,continuous 14 days.Observation group applied 30 mg edaravone,2 times/d,continuous 14 days.Besides this,observation group were given citicoline 1 000 mg,ontinuous 14 days.Comparison of therapeutic effects were observed for 14 days.Results Before and after treatment,the national institutes of health stroke scale (NIHSS) score,Barthel index,superoxide dismutase (SOD) of observation group were (14.1±3.2) and (5.1±1.6),(51.6±13.5) and (73.1±15.9),(145.3±32.6) U/L and (226.9± 39.7) U/L,which of control group were (13.7 ± 3.3) and (10.2± 3.0),(53.4± 13.8) and (59.9±14.1),(143.6±33.8) U/L and (179.4±35.7) U/L,and there were significant difference between before and after treatment of two groups(observation group:t=16.108,6.600,10.171,P<0.01;control group:t =5.025,2.110,4.663,P<0.05),and between observation group and control group after treatment(t =0.605,3.977,5.697,P<0.01).Furthermore observation group's total effective rate was 97.6% (40/41),significantly better than that of control group,and the difference was significant (80.5% (33/41),x2 =4.49,P <0.05).Conclusion It is better that edaravone combined with citicoline for the treatment of acute ischemic cerebral infarction which can effectively protect the brain tissue,improve the patients' neurologic deficits and their ability of daily life.

ABSTRACT

Objective To investigate the levels and clinical significance of the ratio of apolipoprotein B(apoB)and apolipoprotein A1(apoA1)in the patients with cerebral infarction,and to determine the critical value of apoB/apoA1 inthe patients with ischemic cerebral infarction.Methods 126 inpatients with cerebal infraction diagnosed by MRI enhancement scanning of the head in our hos-pital.From November 1,2012 to May 15,2013 were selected.Among them,51 cases were common carotid artery intima-media thickening(carotid artery intima-media >0.9 mm)and 75 cases were carotid artery intima-media non-thickening by the color ultra-sonic examination;81 cases had atherosclerosis plaque and 45 cases had no atherosclerosis plaque.57 individuals with healthy physi-cal examination were selected as the control group.The fasting serum samples were collected from all the research subjects.The in-dexes of TG,CHOL,HDL-C,LDL-C,apoB,apoA,etc.were detected.Then the independent t-test was adopted to analyze and com-pare the ratio of apoB/apoA1 and others related indictors(including TG,AIP,apoB,apoA1,etc.)in the various groups.The ROC curves were made,the area under the curve was read and the specificity and sensitivity of apoB/apoA1 for diagnosing ischemic cere-bral infarction were calculated.Results (1)AIP and the ratio of apoB/apoA1 in the ischemic cerebral infarction patients were high-er than those in the control group,the rise rate of apoB/apoA1 ratio in the ischemic cerebral infarction patients was 5.43 times of that in the control group,especially the patients with carotid atherosclerosis or carotid artery intima-media thickenning were more significant,while TG and apoB had no obvious changes.(2)the ratio of apoB/apoA1 had no obvious difference between the carotid artery intima-media thickening group and the carotid artery intima-media non-thickening group,while which in the atherosclerotic plaques group was significantly increased compared with no carotid atherosclerosis group and 1.6 times of that in the normal control group.So it could be considered that the apoB/apoA1 ratio was a specific indicator for atherosclerosis.(3)In the diagnosis of ische-mic cerebral infarction,the area under the apoB/apoA1ROC curve was 0.86.its specificity and sensitivity were higher than other in-dexes.Conclusion The apoB/apoA1 ratio is the most specific and sensitive index in the patients with ischemic cerebral infarction and has good correlation with ischemic cerebral infarction,especially in the presence of carotid atherosclerosis plaque it is more sen-sitive,and is a good index of laboratory diagnosis of ischemic cerebral infarction.
