ABSTRACT

Objective:To detect serum level of complement factor B (CFB), and to explore its correlations with clinical parameters and prognosis in children with primary IgA nephropathy (IgAN).Methods:A total of 204 children with primary IgAN confirmed by kidney biopsy in the Department of Nephrology of the First Affiliated Hospital of Zhengzhou University from December 2014 to April 2017 were included in IgAN group.During the same period, 84 healthy children were included in healthy control group.Their mean age was (11.0±3.5) years and (10.9±3.2) years, respectively.Patients in IgAN group were divided into low CFB group (102 cases) and high CFB group (102 cases) according to the medium serum level of CFB measured by enzyme-linked immunosorbent assay. Spearman′ s coefficient was employed to analyze correlation amongst various parameters.Multivariable-adjusted Cox proportional ha-zards models were used to evaluate the relationship between serum CFB level and prognosis in children with IgAN. Results:Serum CFB levels were significantly higher in IgAN group than that in healthy control group [290.9 (186.2-453.9) mg/L vs.218.9 (155.0-321.3) mg/L, Z=-3.372, P=0.001]. Serum levels of CFB were negatively correlated with serum albumin ( r=-0.388, P<0.001) and estimated glomerular filtration rate ( r=-0.416, P<0.001), but positively correlated with serum creatinine ( r=0.305, P<0.001) and 24 h urinary protein ( r=0.456, P<0.001) in IgAN group.The incidences of crescents (C1-2) (70.6% vs.29.4%, χ2=34.588, P<0.001) and C 3 deposition (+ + -+ + + ) (63.7% vs.44.1%, χ2=7.892, P=0.005) were significantly higher in high CFB group than those in low CFB group. Kaplan- Meier analysis showed that high CFB levels predicted worse renal outcome in pediatric IgAN patients ( χ2=17.509, P<0.001). Multivariate Cox regression analysis showed that the high CFB level was the independent risk factor for the poor renal outcome ( HR=2.517, 95% CI: 1.284-4.932, P=0.007). Conclusions:High serum levels of CFB are associated with decreased renal function, increased urinary protein excretion, crescentic formation and poor renal outcome in pediatric IgAN patients.

ABSTRACT

Objective To reveal the early elastic dysfunction of common carotid artery may have occurred in systemic lupus erythematosus( SLE) patients ,the circumferential strain parameters of common carotid artery intima in SLE patients at different serum complement levels were quantitatively evaluated by layer-specific strain imaging . Methods Seventy-two patients suffered from SLE without evidence of atherosclerosis were enrolled ,and the results of laboratory examination of serum complement C 3 in patients with SLE were recorded in detail ,49 cases of SLE patients whose serum C3 was lower than the normal reference range(SLE C3+ group) ,23 cases of SLE patients whose serum C3 was in the range of normal reference value(SLE C3 - group) . And a matched healthy control group ( n =38) were selected . Carotid ultrasound were performed in all three groups ,and the common carotid artery conventional parameters ,such as common carotid artery intima-media thickness ( CCA-IM T ) ,peak systolic velocity ( PSV ) ,end-diastolic velocity(EDV) ,resistance index(RI) and circumferential strain related parameters ,such as circumferential strain of common carotid artery intima(CSendo) and circumferential strain rate(CSr) were measured and derived.Therelatedparametersamongthosegroupswerecomparedfordifference.Results ①Therewas no significant difference in RI among the three groups(all P >0 .05) . The values of CCA-IMT in the SLE C3+ group and SLE C3- group were larger than that in the control group( all P <0 .05) . ②The values of CSendo and CSr in the SLE C3+ group and SLE C3 - group were lower than those in the control group ,the values of SLE C3+ group were significantly lower than those in the control group ( P <0 .001) ,and there was no significant difference between the SLE C3- group and control group( P >0 .05) . ③The CSendo and CSr in the SLE C3+ group were lower than those in the SLE C3- group(all P < 0 .05) . Conclusions CSendo which is provided by new ultrasonic technology of layer-specific strain combined with recent changes in serum complement C3 may provide visual evidence for assessing early elastic disfunction of carotid intima in SLE patients .

ABSTRACT

OBJECTIVE:To observe clinical efficacy of urinary kallidinogenase in the treatment of acute cerebral watershed in-farct (WSI). METHODS:128 patients with WSI were randomly divided into control group and treatment group,each of the 64 cases. Control group was given Shuxuening 15 ml added into 0.9% Sodium chloride 250 ml,ivgtt,qd;treatment group received urinary kallidinogenase 0.15 PNA added into 0.9% Sodium chloride 100 ml,ivgtt,qd. Both groups were treated for consecutive 14 days. Neurologic impairment score(NIHSS)and clinical efficacy were observed in 2 groups before treatment and 3,7 and 14 days after treatment. The blood specimens were collected after 7 and 14 days treatment,to determine serum levels of TCC. RESULTS:After treatment,NIHSS and total effective rate of treatment group were significantly higher than those of control group,with statis-tical significance(P0.05);7 days af-ter treatment,TCC level of 2 groups increased significantly,to 14 days,and a declive;the treatment group was higher than the control group,with statistical significance (P<0.05). CONCLUSIONS:Urinary kallidinogenase can improve clinical efficacy of WSI significantly,and promote neurologic impairment symptom and TCC levels.

ABSTRACT

Objective To investigate immunological changes in the serum of elderly patients with primary nephrotic syndrome (PNS).Methods We made a retrospective analysis of clinically relevant immunological parameters and pathological data of PNS patients 60 years old and above (elderly group)in our hospital for kidney biopsy between September 2010 and November 2013.The data of similar patients but younger than 60 years old (control group)were collected for comparison.Results ① The average levels of IgG in serum of the two groups were both lower than the normal level.And the patients with a decreased level accounted for 59.3% in the elderly group and 78.2% in the control group.The results exhibited a significant difference (P 0.05).③ IgG had a negative correlation with urinary protein and cholesterol in serum (the values of r were-0.107,-0.238,respectively,P <0.05).However,there was a positive correlation between IgG and albumin (r=0.252,P <0.05).The results in both groups were consistent.④ Membranous nephropathy was the most common pathological type in aged patients while in control group IgA nephropathy was the most common.Conclusion ① The level of IgG in the serum decreases in most aged patients with PNC.The serum IgA and IgG/IgM ratio are elevated while concentrations of C3 and C4 change little.②The change of IgG is correlated with urinary protein, serum albumin and cholesterol in aged patients with PNS.

ABSTRACT

To characterize the serum complement innate immune system in three species of crocodilians native to southeast Mexico. Methodology: Plasma collected from three wild crocodilian species native to southeast Mexico were exposed to sheep red blood cells (SRBCs) to measure hemolysis, which is used as an indication of serum complement immune activity. Results: Incubation of different volumes of plasma from Crocodylus acutus, Crocodylus moreletii, and Caiman crocodilus resulted in a volume-dependent increase in SRBC hemolysis. However, while maximum hemolysis for C. acutus and C. moreletii were both approximately five-fold higher than that of Ca. crocodilus. A kinetic study revealed that the hemolysis was rapid, with near-maximum activity recorded at 30 min for C. acutus and C. moreletii. However, Ca. crocodilus activity exhibited a significant increase (P<.5) only between one and two hours. A thermal analysis showed that the SRBC hemolysis was maximal at temperatures to which these species thermoregulate. The thermal profiles were similar for all three species, although the activity was lower for Ca. crocodilus (P<.01). The SRBC hemolysis was strongly inhibited by mild heat treatment (56°C,30 min) and also by EDTA, indicating that the hemolytic activity was probably due to the presence of crocodilian serum complement activity. The EDTA-inhibited activity was restored by the addition, of a 20mM excess of Ca2+or Mg2+, but not Fe2+orCu2+, thus exhibiting the specific need for Ca2+orMg2+. Conclusions: The serum complement activities of C. acutus and C. moreletii are much higher (P<.01) than Ca. crocodilus, which may be related to the maximum sizes and increased aggressive natures of C. acutus and C. moreletii.

ABSTRACT

Objetivo: evaluar la asociación entre los niveles séricos de Proteína C Reactiva e índices Apoproteína B/Apoproteína A1, Apoproteína B/LDL colesterol, LDL/HDL colesterol, índice aterogénico, lipoproteína (a) y componentes C3 y C4 del complemento sérico; así como la capacidad predictiva de la proteína C Reactiva, C3 y C4 complemento sobre los parámetros del lipidograma mencionados, se realizó un estudio transversal en pacientes portadores de Artritis Reumatoide y controles sanos de la provincia Matanzas. Métodos: las determinaciones de los parámetros individuales fueron realizadas por método inmunoturbidimétrico y enzimocolorimétrico. El Software estadístico SPSS, versión 18,0 fue empleado para el procesamiento de los resultados. Resultados: la correlación de Spearman detectó asociación de la proteína C Reactiva con los índices ApoB/LDL colesterol y LDL/HDL colesterol exclusivamente en los pacientes, Rho de Spearman= 0,439 (p=0,002); -0,300 (p=0,043), respectivamente; mientras manifestó asociación de esta con el C4 complemento en ambos grupos, Rho de Spearman= 0,355 (p=0,015); 0,376 (p=0,000), pacientes y controles, respectivamente. La proteína C reactiva predijo el índice ApoB/LDL colesterol mediante el análisis de regresión lineal en los pacientes: R²=0,192 F=10,488 (p=0,002), en tanto las proteínas del complemento C3 y C4 estimaron significativamente el nivel de lipoproteína(a); R²=0,170 F=4,396 (p=0,018). Los resultados apoyan la hipótesis del vínculo entre respuesta inflamatoria y predominio de Lipoproteínas de baja densidad más proaterogénicas; así como la posible estimación de marcadores del riesgo coronario relacionados con el metabolismo lipoproteico a partir de los niveles séricos de Proteína C Reactiva, C3 y C4 complemento en pacientes con Artritis Reumatoide...


Objective: evaluate the association between serum levels of C-reactive protein and the indices apoprotein B/apoprotein A1, apoprotein B/LDL cholesterol, LDL/HDL cholesterol, atherogenic index, lipoprotein (a) and serum complement components C3 and C4, as well as the prediction capacity of C-reactive protein, C3 and C4 complement with respect to the above mentioned lipidogram parameters. A cross-sectional study was conducted of patients with rheumatoid arthritis and healthy controls from the province of Matanzas. Methods: individual parameters were determined by immunoturbidimetry and enzymatic colorimetry. Results were processed with the statistical software SPSS version 18.0. Results: spearman rank correlation spotted an association of C-reactive protein with indices ApoB/LDL cholesterol and LDL/HDL cholesterol exclusively in patients, Spearman's Rho = 0.439 (p=0.002); -0.300 (p=0.043); -0.300 (p=0.043), respectively; and an association of C-reactive protein with C4 complement in both groups, Spearman's Rho = 0.355 (p=0.015); 0.376 (p=0.000), patients and controls, respectively. C-reactive protein predicted the ApoB/LDL cholesterol index by linear regression analysis in patients: R²=0.192 F=10.488 (p=0.002), whereas C3 and C4 complement proteins significantly estimated the level of lipoprotein (a): R²=0.170 F=4.396 (p=0.018). Results support the hypothesis about the link between inflammatory response and the predominance of more proatherogenic low density lipoproteins, as well as the potential estimation of coronary risk markers related to lipoprotein metabolism based on serum levels of C-reactive protein, C3 and C4 complement in patients with rheumatoid arthritis...

ABSTRACT

1. Complement levels were studied in 41 normal adults and 40 cancer cases2. Complement levels measured by the two mehtods, CIA50 and CH50 units, showed statistically higher levels among cancer cases as compared with normal adults. Mean titers were 31o7 CIA50 units per 0.5 ml. and 35.7 CH50 units per ml. for normal adults and 4385 CIA50 units per 0.5 ml. and 51.8 CH50 units per ml. for cancer cases3. There was a tendency towards correlation in the two measurements so that a high level of CIA50 units was accompanied by a similar increase in CH50 units4. The significnce of the high complement levels among cancer cases and its relationship to resistance is discussed. (Conclusions)
