ABSTRACT

The clinical experience in treatment of somatalgia with "point-zone-strip stimulation technique" of acupuncture of professor ZHANG Wei-hua was summarized. Professor ZHANG integrates the theories of the cutaneous region of meridian, biaoben, "taking tender point as acupoint" and local holography as a whole in treatment of somatalgia. The "point-stimulation technique" of acupuncture (subcutaneous needling technique) is adopted for the painful site less than 3 cm in diameter, the "zone-stimulation technique" (surrounding needling technique) is for the site larger than 3 cm in diameter and the "strip-stimulation technique" (cubit-tibia needling technique) is for various acute and chronic somatalgia. These three needling techniques are applicable singly or in combination in clinical practice.

ABSTRACT

Objective: To observe the efficacy of needling the Xi-Cleft points of yang meridians plus topical surrounding needling and pregabalin in treating postherpetic neuralgia (PHN) affecting the head and face and its influence on quality of life (QOL). Methods: Eighty patients with PHN affecting the head and face were randomized into an acupuncture-medication group and a Western medication group by their visiting sequence, with 40 cases in each group. The Western medication group took pregabalin capsules orally, 75 mg each time, twice a day. The dosage could increase to 150 mg each time and twice a day within 1 week based on the efficacy and tolerance. The treatment lasted for 5 weeks. The acupuncture-medication group was given additional surrounding needling at the herpes zoster on the head and face, and Xi-Cleft points of yang meridians were chosen according to the affected area for acupuncture, once every other day, for 5 weeks in total. Results: Prior to treatment, there were no significant differences in the visual analog scale (VAS) and QOL scores between the two groups (both P>0.05). After treatment, VAS and QOL scores dropped significantly in both groups (all P<0.01), and were significantly lower in the acupuncture-medication group than in the Western medication group (both P<0.01). The total effective rate was 67.5% in the Western medication group, versus 92.5% in the acupuncture-medication group, and the between-group difference was statistically significant (P<0.05). Conclusion: Based on oral administration of pregabalin, needling the Xi-Cleft points of yang meridians and topical surrounding needling can reduce PHN, notably improve patients' QOL, and produce more significant efficacy than oral administration of pregabalin alone.

ABSTRACT

OBJECTIVE@#To compare the efficacy difference between thumb-tack acupuncture with surrounding needling method plus medication and medication alone for herpes zoster (HZ) of stagnated heat in liver meridian type.@*METHODS@#According to random number table method, 60 patients with HZ of stagnated heat in liver meridian type were randomly divided into an observation group and a control group, 30 cases in each one. All the patients were treated with oral valaciclovir hydrochloride dispersible tablets (0.3 g per time, twice per day), mecobalamin tablets (0.5 mg per time, three times a day) and vitamin B (10 mg per time, three times a day) for 15 days. In addition, the patients in the observation group were treated with thumb-tack acupuncture at area 1 cm outside the herpes, with an interval of 3 cm between thumb-tack needles. The thumb-tack acupuncture was given once every 3 days, retained for 48 h, with an interval of 1 day between treatments, and totally 5 treatments were given. The index of herpetic evaluation (stopping time of herpes, scarring time, decrustation time), visual analogue scale (VAS), serum immune-related factors (IgG, IgM, IgA) and serum inflammatory factors (IL-4, IL-17, TNF-α, TGF-β1) were observed before and after treatment in the two groups.@*RESULTS@#After treatment, the stopping time of herpes, scarring time, decrustation time in the observation group were shorter than those in the control group (all 0.05), but the difference before and after treatment in the observation group was superior to that in the control group (<0.05). Compared before treatment, the levels of serum immune-related factors IgG, IgM, IgA were increased in the two groups after treatment (all <0.05), and the levels in the observation group after treatment were higher than those in the control group (all <0.05). Compared before treatment, the levels of serum inflammatory factors IL-4, IL-17, TNF-α, TGF-β1 were reduced in the two groups after treatment (all <0.05), and the levels in the observation group after treatment were lower than those in the control group (all <0.05).@*CONCLUSION@#The thumb-tack acupuncture with surrounding needling method plus medication have the advantages of rapid onset and analgesic effect for HZ of stagnated heat in liver meridian type, which could also improve serum immune-related factors and reduce inflammatory reaction.

ABSTRACT

OBJECTIVE@#To summarize the research progress of acupuncture-moxibustion and the related adjuvant therapy for promoting pressure ulcer healing in recent 10 years and explore the future research direction.@*METHODS@#By retrieving Chinese Journal Full-text Database (CNKI), VIP Database (VIP) and PubMed database, dated from January 2008 through to December 2017, the articles for the treatment of pressure ulcers with acupuncture-moxibustion and the related adjuvant therapy were collected. In terms of clinical research and experimental research, the research progress and the related effect mechanisms were analyzed and summarized.@*RESULTS@#Through screening, 78 articles for the treatment of pressure ulcer with acupuncture-moxibustion and the adjuvant therapy were collected finally, of which, 71 articles were Chinese and 7 articles were English. The therapies with the highest frequency for pressure ulcer were the simple moxibustion or moxibusiton combined with other adjuvant measures. The local surrounding needling therapy, electroacupuncture, transcutaneous electrical nerve stimulation (TENS), neuromuscular electrical stimulation (NMES) and laser therapy were followed by regarding the use frequency. All of these interventions promoted the healing of pressure ulcers to different degrees. The underlying mechanisms were related to the regulation of the local blood perfusion, differentiation and migration of vascular endothelial growth factor, angiogenesis, superoxide dismutase SOD and eNOS/NO signaling pathway so as to promote tissue migration and proliferation.@*CONCLUSION@#Acupuncture-moxibustion and the related adjuvant therapies achieve the definite therapeutic effects on pressure ulcers. The researches on the treatment of pressure ulcers at home and abroad are diversified, but it is still limited in the research on the effect mechanism.

ABSTRACT

This study was aimed to observe curative effect of periarthritis of shoulder with activating brain and re-gaining consciousness acupuncture method combined with surrounding needling acupuncture on shoulder joint. A to-tal of 120 patients were randomly divided into the treatment group (60 cases) and the control group (60 cases) on the basis of routine internal medicine treatment and rehabilitation exercises. Cases in the control group received conven-tional acupuncture treatment. And cases in the treatment group received activating brain and regaining conscious-ness acupuncture method combined with surrounding needling acupuncture on shoulder joint. The treatment course was 28 days. And then, the curative effect evaluation was conducted. The results showed that the effective rate was 73.33% in the control group and 93.34% in the treatment group. There was statistical significance on effective rate between both groups(P< 0.05). It was concluded that the activating brain and regaining consciousness acupuncture method combined with surrounding needling acupuncture on shoulder joint had significant curative effects for peri-arthritis of shoulder. This treatment method is worthy of further popularization and application.
