Resumen Introducción Las cárceles constituyen un foco de violencia inherente y un ambiente propicio de lesiones traumáticas. Objetivo Describir el perfil de ingreso y evolución de personas privadas de libertad hospitalizadas en nivel terciario por trauma acontecido en 2 complejos penitenciarios, que ingresan a nuestro Servicio. Materiales y Método Estudio descriptivo, incluyó la revisión de fichas clínicas en nuestro hospital (HUAP), durante el periodo entre agosto de 2009 y diciembre de 2016. Resultados 88 consultas de personas privadas de libertad, donde se obtuvieron 46 consultas por lesiones traumáticas. Se observó una distribución simétrica para las variables edad, presión arterial media, frecuencia cardíaca, hematocrito, hemoglobina y recuento de leucocitos. El sitio del trauma más frecuente fue el tórax y el abdomen (incluyendo cara anterior y posterior completa), cada uno con 18 pacientes (39,13% cada uno). El diagnóstico de ingreso más frecuente fue neumotórax en doce sujetos. Los principales tratamientos efectuados fueron 16 laparotomías exploradoras (34,78; IC 95%: 22,68 a 49,23) y 12 pleurostomías (26,09; IC 95%: 15,60 a 40,26). La duración de la hospitalización distribuyó en forma asimétrica, con mediana de 3 días. Tuvimos 6 reingresos (13,04%) en los primeros 30 días posteriores al alta y una mortalidad. Conclusiones Los hechos de violencia en estos 2 centros penitenciarios en Santiago, son un diagnóstico que se presenta en la urgencia de nuestro hospital, con lesiones de distinta gravedad y tratamiento. Resulta necesario adelantarse a estos escenarios, donde ahora sabemos que gran parte de ellos necesitará algún procedimiento o intervención.


Background Prisons are a source of inherent violence and an environment conducive to traumatic injuries. Aim The objective of this paper is to describe the income and evolution profile of hospitalized people deprived of liberty at the tertiary level due to trauma that occurred in two prison detention centers in Santiago, that enters our service. Materials and Method Descriptive study, included the review of clinical records in our hospital, during the period between August 2009 and December 2016. Results 88 consultations of people deprived of liberty, where 46 consultations for traumatic injuries were obtained. A symmetric distribution was observed for the variables age, mean arterial pressure, heart rate, hematocrit, hemoglobin and leukocyte count. The most frequent trauma site was the thorax and abdomen (including front face and full back), each with 18 patients (39.13% each). The most frequent diagnosis of admission was pneumothorax in twelve subjects. The main treatments were 16 exploratory laparotomies (34.78, 95% CI: 22.68 to 49.23) and 12 pleurostomies (26.09, 95% CI: 15.60 to 40.26). The duration of hospitalization distributed asymmetrically, with a median of 3 days. We had 6 readmissions (13.04%) in the first 30 days after discharge and one mortality. Conclusions The violence in this two prison detention centers, in Santiago, is a diagnosis that appears in the urgency of our hospital, with lesions of different severity and treatment. It is necessary to anticipate these scenarios, where we now know that a large part of them will need some procedure or intervention.

ABSTRACT Objective: to assess the epidemiological profile of patients undergoing exploratory trauma laparotomy based on severity and prognosis criteria, and to determine the predictive accuracy of trauma scoring systems in terms of morbidity and mortality. Methods: retrospective cohort study and review of medical records of patients undergoing exploratory laparotomy for blunt or penetrating trauma at the Hospital de Pronto Socorro de Porto Alegre, from November 2015 to November 2019. Demographic data, mechanism of injury, associated injuries, physiological (RTS and Shock Index), anatomical (ISS, NISS and ATI) and combined (TRISS and NTRISS) trauma scores, intraoperative findings, postoperative complications, length of stay and outcomes. Results: 506 patients were included in the analysis. The mean age was 31 ± 13 years, with the majority being males (91.3%). Penetrating trauma was the most common mechanism of injury (86.2%), predominantly by firearms. The average RTS at hospital admission was 7.5 ± 0.7. The mean ISS and NISS was 16.5 ± 10.1 and 22.3 ± 13.6, respectively. The probability of survival estimated by TRISS was 95.5%, and by NTRISS 93%. The incidence of postoperative complications was 39.7% and the overall mortality was 12.8%. The most accurate score for predicting mortality was the NTRISS (88.5%), followed by TRISS, NISS and ISS. Conclusion: the study confirms the applicability of trauma scores in the studied population. The NTRISS seems to be the best predictor of morbidity and mortality.


RESUMO Objetivo: analisar o perfil epidemiológico dos pacientes submetidos a laparotomia exploradora por trauma com base em critérios de gravidade e prognóstico, e determinar a acurácia preditiva dos escores de trauma em termos de morbimortalidade. Métodos: estudo de coorte retrospectiva e revisão de prontuários dos pacientes submetidos a laparotomia exploradora por trauma contuso ou penetrante no Hospital de Pronto Socorro de Porto Alegre no período de novembro de 2015 a novembro de 2019. Foram avaliados dados demográficos, mecanismo do trauma, lesões associadas, índices fisiológicos (RTS e Shock Index), anatômicos (ISS, NISS e ATI) e mistos (TRISS e NTRISS), achados intraoperatórios, complicações pós-operatórias, tempo de internação e desfecho. Resultados: foram incluídos 506 pacientes na análise. A idade média foi de 31 ± 13 anos, com predomínio do sexo masculino (91,3%). O trauma penetrante foi o mecanismo de lesão mais comum (86,2%), sendo a maioria por arma de fogo. A média do RTS na admissão hospitalar foi 7,5 ± 0,7. A média do ISS e do NISS foi 16,5 ± 10,1 e 22,3 ± 13,6, respectivamente. A probabilidade de sobrevida estimada pelo TRISS foi de 95,5%, e pelo NTRISS de 93%. A incidência de complicações pós-operatórias foi de 39,7% e a mortalidade geral de 12,8%. O escore com melhor acurácia preditiva foi o NTRISS (88,5%), seguido pelo TRISS, NISS e ISS. Conclusões: o estudo confirma a aplicabilidade dos escores de trauma na população em questão. O NTRISS parece ser o sistema com melhor acurácia preditiva de morbimortalidade.

RESUMEN Introducción: El trauma accidental o intencional es la causa número uno de muerte en personas jóvenes. Objetivo: Evaluar la capacidad pronóstica en la predicción de la muerte de índices pronóstico en pacientes con trauma abdominal cerrado. Métodos: Se desarrolló un estudio observacional y descriptivo con fases analíticas en un universo de 72 pacientes con trauma abdominal cerrado en el Hospital Universitario "Celia Sánchez Manduley", Manzanillo, Granma, entre enero de 2016 a diciembre 2017. Los datos fueron procesados en el sistema Excel, construyéndose la curva de características operativas del receptor para el análisis de la efectividad de los índices pronósticos. Resultados: Predominaron los pacientes masculinos (70,83 por ciento) con una edad media de 43,31 años. El bazo fue el órgano más lesionado (5,56 por ciento) y el shock séptico la principal causa directa de la muerte (4,17 por ciento). El índice de severidad de lesiones, la escala revisada de trauma, la escala CRAMS y la metodología TRISS resultaron en una sensibilidad, de predicción de la muerte, de 91,02 por ciento, 87,5 por ciento, 83,5 por ciento y 95,8 por ciento respectivamente a un punto de corte de 25 puntos, 6 puntos, 7 puntos y hasta 19 por ciento respectivamente. Conclusiones: La metodología TRISS resultó ser el índice pronóstico de mayor sensibilidad y en consecuencia el más recomendado para la práctica clínica habitual(AU)


ABSTRACT Introduction: Accidental or intentional trauma is the leading cause of death among young people. Objective: To evaluate the mortality prediction capacity of prognostic indexes in patients with closed abdominal trauma. Methods: An observational and descriptive study with analytical phases was developed in a population of 72 patients with closed abdominal trauma, at Celia Sánchez Manduley University Hospital in Manzanillo, Granma Province, between January 2016 and December 2017. The data were processed in the Excel system, and the receiver operating characteristics curve was constructed to analyze the effectiveness of the prognostic indexes. Results: There was a predominance of male patients (70.83 percent) with a mean age of 43.31 years old. The spleen was the most injured organ (5.56 percent). The septic shock was the main direct cause of death (4.17 percent). The injury severity index, the revised trauma scale, the CRAMS scale, and the TRISS methodology produced a mortality prediction sensitivity of 91.02 percent, 87.5 percent, 83.5 percent, and 95.8 percent, respectively, at a cut point of 25, 6, and 7 points, and up to 19 percent, respectively. Conclusions: The TRISS methodology turned out to be the prognostic index with the highest sensitivity and, consequently, the most recommended for routine clinical practice(AU)

RESUMEN Introducción: El trauma de abdomen tiene alta incidencia y con frecuencia afecta el colon y el recto, se aboga por el tratamiento mediante sutura primaria. Objetivo: Identificar diferencias en las características clínico-quirúrgicas y resultados terapéuticos de pacientes con lesiones traumáticas de colon y recto, tratados con sutura primaria y técnicas derivativas. Método: Se realizó un estudio multicéntrico descriptivo ambispectivo para determinar características clínico-quirúrgicas y resultados terapéuticos en pacientes con lesiones traumáticas de colon y recto en los hospitales Carlos J. Finlay, Joaquín Albarrán, Calixto García y Enrique Cabrera de La Habana, en el periodo 2010-2015. El universo estuvo conformado por todos los pacientes intervenidos quirúrgicamente por trauma de colon y/o recto. Resultados: La colostomía fue el proceder más frecuente (53,1 por ciento). Los pacientes en edades medias de la vida (promedio: 43,5 años), sexo masculino (71,4 por ciento), evolución hasta el tratamiento quirúrgico menor de 12 horas (73,4 por ciento); causadas por empalamientos y otros (49 por ciento) y heridas con arma blanca (38,8 por ciento). La magnitud del daño de colon y recto fue baja, con mayor afectación de colon izquierdo. La frecuencia de complicaciones graves y mortalidad fue alta en la colostomía. Conclusiones: La sutura primaria muestra mejores resultados terapéuticos y debe ser la técnica de elección en las lesiones traumáticas de colon y recto intraperitoneal(AU)


ABSTRACT Introduction: Abdominal trauma presents high incidence and often affects the colon and rectum; primary suture is the preferred treatment. Objective: To identify differences between the clinical-surgical characteristics and the therapeutic outcomes in patients with traumatic lesions of the colon and rectum, treated with primary suture and derivative techniques. Method: A multicentric, descriptive and ambispective study was carried out to determine the clinical-surgical characteristics and the therapeutic outcomes in patients with traumatic colon and rectal lesions in Carlos J. Finlay, Joaquín Albarrán, Calixto García and Enrique Cabrera Hospitals in Havana, in the period from 2010 to 2015. The study population was made up of all patients surgically intervened for colon and/or rectal trauma. Results: The colostomy was the most frequent procedure (53.1 percent). Patients in middle ages of life (average: 43.5 years), male sex (71.4 percent), evolution to surgical treatment under 12 hours (73.4 percent); caused by impalement and others (49 percent) and cutting wounds (38.8 percent). The magnitude of colon and rectum damage was low, with greater involvement of the left colon. The frequency of serious complications and mortality was high in the colostomy. Conclusions: Primary suture shows better therapeutic results and should be the technique of choice in traumatic lesions of the intraperitoneal colon and rectum(AU)

RESUMO Objetivo: avaliar a evolução de pacientes vítimas de trauma abdominal grave, nos quais o protocolo de transfusão maciça foi acionado, e que foram submetidos à Tomografia Axial Computadorizada (TAC) no Pronto Socorro (PS), com o intuito de verificar o prognóstico do paciente e a eficiência diagnóstica da TAC nesse cenário. Métodos: estudo retrospectivo, longitudinal e observacional, feito em centro de referência para trauma. Foram selecionados 60 pacientes vítimas de trauma abdominal grave que ativaram o protocolo de transfusão maciça, divididos em dois grupos: os submetidos à TAC no PS e os que não foram. Verificou-se a acurácia da TAC, comparou-se o número de óbitos nos dois grupos, o tempo de internamento e os hemocomponentes transfundidos. Resultados: dos 60 pacientes, 66,67% receberam concentrados de hemácias ainda no PS; 33,3% foram submetidos à TAC na admissão, pela melhora hemodinâmica, e 66,7% não realizaram o exame na entrada. O percentual de óbitos foi de 35% em ambos os grupos. A diferença entre as médias do tempo de internamento entre os grupos não foi estatisticamente significativa, assim como a média da quantidade de concentrado de hemácias transfundido. No grupo que fez TAC, 45% não necessitaram de laparotomia exploratória. Conclusão: a TAC pôde ser realizada de maneira rápida em pacientes com instabilidade hemodinâmica na chegada ao PS, não influenciou significativamente a mortalidade e poupou alguns doentes de uma laparotomia exploratória desnecessária.


ABSTRACT Objective: to evaluate the evolution of severe abdominal trauma patients, for whom the massive transfusion protocol was triggered, and who were submitted to Computed Axial Tomography (CAT) in the emergency room (ER), in order to verify the patient's prognosis and the diagnostic efficiency of CAT in this scenario. Methods: retrospective, longitudinal and observational study performed at a referral center for trauma care in Curitiba, Parana, Brazil. We selected 60 severe abdominal trauma patients who had massive transfusion protocol activation and divided them into two groups: patients who underwent CAT at ER and patients who did not. We verified the diagnostic accuracy of CAT-scan examination and compared the number of deaths, hospitalization time, and transfused blood components in both groups. Results: considering the 60 patients, 66.67% received red blood cells at ER; 33.3% underwent CAT on admission due to hemodynamic improvement, and 66.7% did not perform the examination at the entrance. The percentage of deaths was 35% in both groups. Considering the two groups, the difference between the mean lengths of hospital stay was not statistically significant, as well as the difference between the mean numbers of transfused red blood cells. In the group that underwent CAT, 45% did not require exploratory laparotomy. Conclusion: CAT could be rapidly performed in patients with hemodynamic instability on arrival at ER, sparing some patients from an unnecessary exploratory laparotomy and not significantly influencing mortality.

RESUMO Objetivo: avaliar a influência do local de ocorrência do trauma nos escores de trauma de pacientes submetidos à laparotomia de emergência. Métodos: estudo retrospectivo observacional analítico. Foram incluídos 212 pacientes submetidos à laparotomias exploratórias no período de janeiro de 2015 e dezembro de 2017. Informações sobre o local do acidente e dados vitais dos pacientes foram obtidas com base na coleta de dados por meio de prontuários eletrônicos e físicos. Foram analisados os índices de trauma de pacientes provenientes de Curitiba e Região Metropolitana e o local em que o paciente foi socorrido (estabelecimento físico ou via pública). Resultados: entre os 212 pacientes estudados, 184 (86,7%) foram trazidos pelo Serviço de Atendimento Pré-Hospitalar provenientes da cidade Curitiba e 28 (13,3%) provenientes de Região Metropolitana de Curitiba. Foram socorridos em estabelecimentos físicos 25 pacientes (17,6%), enquanto 117 (82,4%) foram socorridos em via pública. Observou-se maiores valores de ISS (Injurity Severity Score) dos pacientes procedentes da Região Metropolitana em relação aos procedentes de Curitiba (29,78 vs 22,46, P=0,009), enquanto valores maiores do TRISS (Trauma Trauma and Injury Severity Score) foram observados em pacientes procedentes de Curitiba em relação aos da Região Metropolitana (90,62 vs 81,30; P=0,015). Pacientes socorridos em via pública apresentaram menor valor de RTS (Revised Trauma Score) (6,96 vs 7,65; P=0,024) e TRISS (86,42 vs 97,21; P=0,012). Conclusão: pacientes vítimas de trauma procedentes de locais mais distantes do atendimento no centro de referência apresentaram pior prognóstico à admissão e foi observado pior prognóstico em pacientes socorridos em via pública.


ABSTRACT Objective: to evaluate the influence of the site of trauma occurrence on the trauma scores of patients submitted to emergency laparotomy. Methods: this is a retrospective, observational, analytical study. We included 212 patients who underwent exploratory laparotomies in the period of January 2015 and December 2017. We obtained information about the accident site and vital data of the patients based on data collection through electronic and physical records. We analyzed the trauma indices of patients from Curitiba and its Metropolitan Region and the place where the patient was rescued (physical establishment or public road). Results: among the 212 patients studied, 184 (86.7%) were brought by the Prehospital Care Service from the city of Curitiba, and 28 (13.3%), from the Metropolitan Region of Curitiba. Twenty-five patients (17.6%) were rescued in physical establishments, while 117 (82.4%) were rescued on public roads. We observed higher values ​​of Injurity Severity Scores (ISS) in patients coming from the Metropolitan Region than in those coming from Curitiba (29.78 vs 22.46, P=0.009), but higher values ​​of Trauma and Injury Severity Scores (TRISS) in patients from Curitiba than the ones from the Metropolitan Region (90.62 vs 81.30, P=0.015). Patients rescued in public roads presented lower Revised Trauma Scores (RTS) (6.96 vs 7.65, P=0.024) and TRISS (86.42 vs 97.21; P=0.012). Conclusion: trauma victims from sites more distant from the referral center and rescued on public roads presented worse prognosis.

Objectives: To assess the epidemiological profile of children and adolescents with intestinal stoma, assisted at the Presidente Dutra University Hospital (HUPD), São Luís (MA). Methods: Observational, descriptive, retrospective, and individuated study. Data were collected from 110 children and adolescents with elimination intestinal stoma, from January 2006 to February 2013. The following variables were analyzed: age, gender, patient origin, and stomata indication, type, and temporal character. After data collection, descriptive analysis was made by Bioestat 5.0 program. Results: Of 110 patients, 78.2% were male and 21.8% female. The average age was 9.4 years old. 55.5% came from the Maranhão state countryside, and 44.5% from the capital, São Luís. Regarding stoma type, colostomies made up 88.2%, and were 11.8% ileostomies, all temporary stoma. The main cause was trauma (42.7%), with firearm abdominal puncture being the most frequent cause (71.5% of the category); followed by congenital anomalies (38.2%), and obstructive (5.4%) and inflammatory (4.5%) causes. Conclusion: Studied children and adolescents were mostly male, from Maranhão state countryside. The main cause was trauma, and colostomy was the most common stoma type.


Objetivos: Avaliar o perfil epidemiológico de crianças e adolescentes portadores de estomas intestinais atendidos no Hospital Universitário Presidente Dutra (HUPD) em São Luís (MA). Métodos: Estudo observacional, descritivo, retrospectivo e individuado em que foram coletados dados de 110 crianças e adolescentes com estomas intestinais de eliminação de janeiro de 2006 a fevereiro de 2013. Analisaram-se as variáveis: idade, sexo, procedência dos pacientes, indicação, tipo e caráter temporal dos estomas. Após coleta dos dados, foi feita análise descritiva pelo programa Bioestat 5.0. Resultados: Dos 110 pacientes, 78,2% eram do sexo masculino e 21,8% do sexo feminino. A idade média foi de 9,4 anos. 55,5% era procedente do interior do Estado do Maranhão e 44,5% da capital, São Luís. Com relação ao tipo: 88,2% eram colostomias e 11,8% ileostomias, todos os estomas temporários. A causa principal foi o trauma (42,7%), sendo a perfuração abdominal por arma de fogo a mais frequente (71,5% da categoria); seguido pelas anomalias congênitas (38,2%), causas obstrutivas (5,4%) e inflamatórias (4,5%). Conclusão: As crianças e adolescentes estudados eram, em sua maioria, do sexo masculino, proveniente do interior do Estado do Maranhão, tendo como principal causa a traumática e a colostomia como o tipo de estoma mais comum.

ABSTRACT

Objetivo: caracterizar los traumas abdominales atendidos en la Clínica Multiperfil. Luanda, Angola en el periodo de enero del 2010 a junio del 2012. Métodos: se realizó un estudio descriptivo prospectivo y transversal en 147 pacientes que presentaron traumatismo abdominal, atendidos en la Clínica Multiperfil, Luanda, Angola. Resultados: se clasificó el trauma abdominal en abierto (38 porciento) y cerrado (61,9 porciento) con predominio de este último. Los órganos más afectados, fueron el hígado para el 37,6 porciento seguido por el intestino delgado, con el 25,8 porciento. Se empleó la conducta quirúrgica a un total de 57,1 porciento de los casos, de ellos un 36,7 porciento forman parte del trauma abierto. Se realizó laparotomía terapéutica en el 39,4 porciento de los pacientes, seguido de la cirugía de control de daños (14,2 porciento). Predominó el shock hipovolémico en un 17 porciento. Fallecieron un total de 12,2 porciento pacientes y en las primeras 24 horas un 5,4 porciento de este. Con un predominio en los casos mayores de 50 años (66,6 porciento) con signos de shock al ingreso (100 porciento). Un GCS inferior a 8 (88,8 porciento), y la ISS mas de 25 (100 porciento). Conclusiones: el trauma abdominal es frecuente en la población de Luanda, su diagnóstico y tratamiento resulta difícil por la variedad y número de lesiones, presentando ocasionalmente complicaciones fatales que repercuten en la mortalidad(AU)


Objective: To characterize abdominal trauma treated at Multiperfil Clinic in Luanda, Angola, from January 2010 to June 2012. Methods: A descriptive, prospective and cross-sectional study was performed in 147 patients with abdominal trauma, who were admitted to Multiperfil Clinic in Luanda, Angola from January 2010 to June 2012. Results: The abdominal trauma was classified into open (38 percent) and closed (61.9 percent), being the last one predominant. The most affected organs were liver (37.6 percent) followed by the small intestine( 25.8 percent). The surgical procedure was applied to 57.1 percent of patients of whom 36.7 percent suffered open traumas. Therapeutic laparotomy was performed in 39.4 percent of cases, followed by damage control surgery in 14.2 percent. Hypovolemic shock prevailed (17 percent); 12.2 percent of patients died and 5.4 percent of this amount within the first 24 hours. Especially in older than 50 years (66.6 percent) with signs of shock at admission (100 percent) a GCS less than 8 (88.8 percent), and the ISS over 25 (100 percent). Conclusions: Abdominal trauma is common in the population of Luanda; diagnosis and treatment are difficult because of the variety and number of lesions, presenting occasionally fatal complications that have an impact on the mortality rate(AU)

La realización del presente estudio es identificar la epidemiología y describir las características de presentación del trauma abdominal, para tener el conocimiento de base y poder proponer en un futuro estrategias de manejo y mejorar la calidad de atención hacia nuestros pacientes; ya que esta patología esta en ascenso y afecta principalmente a la población económicamente activa y el Hospital de Vitarte por su ubicación geográfica es un centro de gran importancia en la atención de estos casos.

OBJETIVO: Identificar fatores preditivos de lesões abdominais em vítimas de trauma fechado. MÉTODOS: Análise retrospectiva dos dados das vítimas de trauma fechado com idade superior a 13 anos, em um período de 15 meses. Comparamos as variáveis entre os doentes com lesões abdominais diagnosticadas por tomografia computadorizada e/ou laparotomia - grupo I (Abbreviated Injury Scale abdome>0, grupo I) e os demais - grupo II (Abbreviated Injury Scale abdome=0,). RESULTADOS: Foram incluídos 3783 casos, com média etária de 39,1 +17,7 anos (14 a 99 anos), sendo 76,1% do sexo masculino. Foram identificadas lesões abdominais em 130 doentes (3,4%). Os traumatizados com lesões abdominais apresentaram, significativamente, menor média etária (35,4 + 15,4 anos vs. 39,2 + 17,7 anos), menor média da pressão arterial sistólica à admissão (114,7 + 32,4mmHg vs. 129,1 + 21,7mmHg), menor média na escala de coma de Glasgow à admissão (12,9 + 3,9 vs. 14,3 + 2,0), maior média de AIS em segmento cefálico (0,95 + 1,5 vs. 0,67 + 1,1), maior média de AIS em segmento torácico (1,10 + 1,5 vs. 0,11 + 0,6) e maior média de AIS em extremidades (1,70 ± 1,8 vs. 1,03 ± 1,2). Os maiores Odds ratio foram presença de tórax flácido (21,8) e fraturas de pelve (21,0). CONCLUSÃO: As lesões abdominais foram mais frequentemente observadas nos doentes com instabilidade hemodinâmica, alteração na escala de coma de Glasgow, lesões graves em crânio, tórax ou extremidades.


OBJECTIVE: To identify predictors of abdominal injuries in victims of blunt trauma. METHOD: retrospective analysis of trauma protocols (collected prospectively) of adult victims of blunt trauma in a period of 15 months. Variables were compared between patients with abdominal injuries (AIS>0) detected by computed tomography or/and laparotomy (group I) and others (AIS=0, group II). Student's t, Fisher and qui-square tests were used for statistical analysis, considering p<0.05 as significant. RESULTS: A total of 3783 cases were included, with a mean age of 39.1 ± 17.7 years (14-99), 76.1% being male. Abdominal injuries were detected in 130 patients (3.4%). Patients sustaining abdominal injuries had significantly lower mean age (35.4 + 15.4 vs. 39.2 + 17.7), lower mean systolic blood pressure on admission (114.7 + 32.4 mmHg vs. 129.1 + 21.7 mmHg), lower mean Glasgow coma scale (12.9 + 3.9 vs. 14.3 + 2.0), as well as higher head AIS (0.95 + 1.5 vs. 0.67 + 1.1), higher thorax AIS (1.10 + 1.5 vs. 0.11 + 0.6) and higher extremities AIS (1.70 ± 1.8 vs. 1.03 ± 1.2). Patients sustaining abdominal injuries also presented higher frequency of severe injuries (AIS>3) in head (18.5% vs. 7.9%), thorax (29.2% vs. 2.4%) and extremities (40.0% vs. 13.7%). The highest odds ratios for the diagnosis of abdominal injuries were associated flail chest (21.8) and pelvic fractures (21.0). CONCLUSION: Abdominal injuries were more frequently observed in patients with hemodynamic instability, changes in Glasgow coma scale and severe lesions to the head, chest and extremities.

OBJETIVO: avaliar o perfil epidemiológico e o desfecho das vítimas de trauma abdominal submetidas à laparotomia em hospital de urgência. MÉTODOS: estudo observacional, descritivo, longitudinal, com abordagem prospectiva, mediante entrevista de 100 pacientes com trauma abdominal submetidos ao tratamento cirúrgico e à avaliação dos seus prontuários. Período da coleta dos dados: setembro a novembro de 2011. RESULTADOS: Os pacientes mais acometidos pelo trauma abdominal foram do sexo masculino, de cor parda, na faixa etária de 25-49 anos, com baixa escolaridade, solteiros, católicos, com rendimento de um a dois salários mínimos. Houve uma predominância do trauma no ambiente urbano, no período noturno e no final de semana. O motivo mais frequente do trauma foi a tentativa de homicídio, associado ao uso de álcool e drogas ilícitas e o mecanismo a arma branca. A dor mostrou-se o sinal de alerta mais presente. A região mais afetada foi abdome superior e o fígado o órgão mais acometido. O tempo de internação hospitalar durou em torno de quatro a dez dias. A maioria teve alta sem sequela. Ocorreram dois óbitos. CONCLUSÃO: Foi marcante a associação do trauma abdominal com homens sob efeito de álcool e/ou drogas ilícitas, refletindo o contexto da violência interpessoal na sociedade atual. A despeito da magnitude do trauma, o desfecho foi satisfatório, apesar da ocorrência de óbitos, o que denota a importância dos hospitais de urgência de manter no seu corpo clínico uma equipe cirúrgica treinada.


OBJECTIVE: To evaluate the profile and outcome of victims of abdominal injuries who underwent laparotomy in the Emergency Hospital of the state of Sergipe (HUSE). METHODS: This was an observational, descriptive longitudinal study with prospective approach, through interviews of 100 patients with abdominal trauma who underwent surgery and evaluation of their medical records. The study period was from September to November of 2011 in the area of trauma care of the HUSE. RESULTS: the most affected individuals were male, mulattos, aged 25-49 years, with low education, single, Catholic, with an income of 1-2 minimum wages. There was a predominance of trauma in the urban areas, at night and on weekends. The most frequent cause of trauma was the attempted of murder associated with the use of alcohol and illicit drugs, and the most frequent mechanism, stabbing. The pain was the most frequent symptom. The most affected region was the upper abdomen and liver was the most affected organ. The hospital stay averaged 4-10 days. Most patients were discharged without sequelae. There were 2 deaths. CONCLUSION: In the HUSE, the association of abdominal trauma with men under the influence of alcohol and illegal drugs was striking, reflecting the context of interpersonal violence in current society. Despite the magnitude of the traumas, the outcome was satisfactory, although deaths, occurred, demonstrating the importance of keeping a trained surgical team in emergency hospitals.

ABSTRACT

El objetivo del estudio fue determinar los factores epidemiológicos que intervienen en la morbilidad y mortalidad de los pacientes con traumatismo hepático. Se trató de un estudio retrospectivo, descriptivo, transversal. La población fueron todos los pacientes con diagnóstico de traumatismo hepático del Hospital Nacional Arzobispo Loayza que se atendieron durante el periodo 2005 a 2010, siendo una población total de 48 casos; con 2 casos excluidos, en un periodo de 5 años. No Hubo muestra de estudio. Toda la información fue analizada usando el programa estadístico SPSS 18 y procesada en una computadora Pentium IV, expresando los datos cuantitativos en media y desviación estándar y los datos cualitativos en porcentajes. El género que predominó fue el masculino con 93,5 por ciento y la edad media 36 años. El mecanismo de trauma de mayor frecuencia fue el abierto con 56,6 por ciento. El grado de lesión hepática III y II fueron los más frecuentes con 30,4 por ciento y 28,3 por ciento cada una, respectivamente. Un segmento hepático se afectó en el 56,5 por ciento de los casos, dos y tres segmentos en el 15,2 por ciento de los casos, siendo el segmento V el de mayor frecuencia lesionado con 41,3 por ciento. Además del hígado, el órgano más afectado fue el diafragma con 13 por ciento; seguido del intestino delgado y estómago con 8,7 por ciento cada uno. En el 95,7 por ciento de los pacientes, el tratamiento de elección fue el quirúrgico y sólo dos pacientes tuvieron tratamiento conservador. De los 44 casos que tuvieron tratamiento quirúrgico, al 34,1 por ciento se les realizó empaquetamiento hepático, al 20,4 por ciento, se les realizó hepatorrafía, al 27,3 por ciento se les realizó laparotomía exploratoria no terapéutica. Como complicación producto del traumatismo hepático en el intraoperatorio tenemos el shock hipovolémico en 8,7 por ciento y en el postoperatorio la colección intrabdominal con 8,7 por ciento, así como un caso de fistula biliar correspondiente al...

Introducción: El colon es el segundo órgano más frecuentemente lesionado en las heridas por trauma penetrante de abdomen. En México, las lesiones por arma blanca o de fuego van en aumento. Nuestro objetivo fue evaluar el principal manejo para las lesiones traumáticas de colon. Material y métodos: Estudio retrospectivo y aleatorizado de 178 pacientes con trauma abdominal y lesiones de colon, en un lapso de cinco años (enero de 2003 a junio de 2008) en el Hospital General de Balbuena. Se comparó el uso del cierre primario y cirugía derivativa con colostomía. Se analizó sexo, grupo de edad, tipo de herida, grado de lesión y mortalidad. Resultados: De 178 pacientes, 156 fueron hombres (87.6 %) y 22 mujeres (12.4 %). El grupo de edad con mayor afección fue el de 21 a 30 años; 74 pacientes (41.6 %) presentaron heridas por instrumento punzocortante y 104 pacientes (58.4 %) heridas por arma de fuego. El principal manejo fue mediante cierre primario: 92 casos (51.7 %) versus 86 (48.3 %) para cirugía derivativa; sin embargo, en las heridas por arma de fuego el principal manejo fue la colostomía (67 casos). La mortalidad fue de 17 casos (9.55 %) debido a causas diversas como lesiones a múltiples órganos de manera asociada. Conclusiones: En las lesiones colónicas debe individualizarse el tratamiento, según la etiopatogenia, grado de lesión y lesiones asociadas.


BACKGROUND: Colon trauma is frequent and its prevalence is difficult to establish because of the different factors that intervene in its origin. In Mexico, traumatic colon injuries, albeit stab wounds or gunshot wounds, are on the rise. Our objective was to evaluate the most appropriate management for traumatic colon injuries. METHODS: We conducted a retrospective study of 178 case files of patients with abdominal trauma and colon lesions during a 5-year period from January 2003 to June 2008 from the General Hospital of Balbuena, Mexico City. The study compared the use of primary closure vs. colostomy, analyzing variables such as sex, age, type of wound, severity of lesion and mortality. RESULTS: There were a total of 178 patients; 156 were male (87.6%) and 22 were female (12.4%). The most affected age group was between 21 and 30 years; 74 patients (41.6%) had stab wounds and 104 patients (58.4%) had gunshot wounds. Management consisted mainly of primary closure in 92 cases (51.7%) vs. colostomy in 86 patients (48.3%). However, 64% of gunshot wounds were treated with colostomy. Reported mortality was 9.55% and this was due to different factors such as multiple organ injury. CONCLUSIONS: Treatment of traumatic colon injury should be case specific, taking into account the mechanism of the lesion, its severity and associated injuries.

ABSTRACT

Objetivo: Informar si la evolución > 6 horas, grado de contaminación y lesión, sitio anatómico lesionado, PATI (penetrating abdominal trauma index) > 25 y presencia de otras lesiones en trauma de colon, se asocian a mayor morbimortalidad en pacientes con lesión colónica a quienes se les realizó cierre primario. Material y métodos: Estudio prospectivo, observacional, longitudinal, descriptivo, en el Hospital Central “Dr. Ignacio Morones Prieto”, San Luis Potosí. Se incluyeron pacientes con trauma abdominal sometidos a cirugía que presentaron lesión colónica. Análisis estadístico básico con χ2. Resultados: 481 pacientes fueron intervenidos por trauma abdominal; 77 (16.1 %) tuvieron lesión colónica, de los cuales 90 % (n = 69) se intervino en las primeras seis horas; 91 % fue lesión penetrante. El colon transverso fue el más lesionado (38 %, n = 29); las lesiones grado I y II representaron 75.3 % (n = 58). Se efectuó cierre primario en 76.66 % (n = 46), resección con anastomosis en 8.3 % (n = 5) y colostomía en 15 % (n = 9). Hubo lesiones asociadas en 76.6 % (n = 59) y contaminación en 85.7 % (n = 66); 82.8 % (58) tuvo PATI < 25; complicaciones asociadas al procedimiento operatorio, 28.57 % (n = 22); reintervenciones, 10 % (n = 8); estancia hospitalaria promedio, 11.4 días; mortalidad no relacionada a lesión de colon, 3.8 % (n = 3). Conclusiones: El cierre primario es un procedimiento seguro para el tratamiento de lesiones colónicas. Los pacientes con cierre primario presentaron menor morbilidad (p < 0.009). Los pacientes con cirugía en las primeras seis horas (p < 0.006) y estabilidad hemodinámica (p < 0.014) tuvieron menor riesgo de complicación.


BACKGROUND: Primary repair of colon injuries is an accepted therapeutic option; however, controversy persists regarding its safety. Our objective was to report the evolution and presence of complications in patients with colon injury who underwent primary closure and to determine if the time interval (>6 h), degree of injury, contamination, anatomic site injured, PATI (Penetrating Abdominal Trauma Index) >25, and the presence of other injuries in colon trauma are associated with increased morbidity and mortality. METHODS: This was a prospective, observational, longitudinal and descriptive study conducted at the Central Hospital "Dr. Ignacio Morones Prieto," San Luis Potosí, Mexico, from January 1, 2003 to December 31, 2007. We included patients with abdominal trauma with colon injury subjected to surgical treatment. chi(2) was used for basic statistical analysis. RESULTS: There were 481 patients with abdominal trauma who underwent surgery; 77(16.1%) had colon injury. Ninety percent (n = 69) were treated in the first 6 h; 91% (n = 70) were due to penetrating injuries, and gunshot wound accounted for 48% (n = 37). Transverse colon was the most frequently injured (38%) (n = 29). Grade I and II injuries accounted for 75.3% (n = 58). Procedures included primary repair (76.66 %) (n = 46); resection with anastomosis (8.3%) (n = 5); and colostomy (15%) (n = 9). Associated injuries were present in 76.6% (n = 59). There was some degree of contamination in 85.7% (n = 66); 82.8% (58) had PATI <25. Complications associated with the surgical procedure were observed in 28.57% (n = 22). Reoperation was necessary in 10% (n = 8). Average hospital stay was 11.4 days. Mortality was 3.8% (n = 3); none of these were related to colon injury. CONCLUSIONS: Primary repair is a safe procedure for treatment of colon injuries. Patients with primary repair had lower morbidity (p <0.009). Surgery during the first 6 h (p <0.006) and in hemodynamically stable patients (p <0.014) had a lower risk of complications.

El traumatismo abdominal representa del 15-20 por ciento del total de las lesiones traumáticas. Nuestro objetivo fue conocer el comportamiento del traumatismo abdominal predominante en el lesionado complejo. Se revisaron las historias clínicas de 24 pacientes ingresados en la Unidad de Cuidados Intermedios Quirúrgicos del Hospital Militar Central Dr Luis Díaz Soto durante el año 2002, con el diagnóstico de trauma abdominal predominante. Hubo una mayor incidencia del traumatismo cerrado (70.8 por ciento), predominando en el sexo masculino (75 por ciento). El lavado peritoneal diagnóstico y la punción abdominal fueron los procedimientos más utilizados. Las lesiones más frecuentes fueron del hígado, el intestino delgado y el hematoma retroperitoneal. La hepatorrafia (30 por ciento) fue la técnica quirúrgica más empleada. Las complicaciones (29.2 por ciento) más frecuentes fueron la sepsis intraabdominal, de la herida quirúrgica y los trastornos hidroelectrolíticos. La mortalidad fue del 8.3 por ciento. La tendencia del comportamiento del traumatismo abdominal no ha logrado variarse en los últimos años, su frecuencia se mantiene estable y el tratamiento quirúrgico continúa siendo el más empleado.


The abdominal traumatism represents the 15-20 percent of the total of the traumatic lesions. Our objective was to know the behavior of the abdominal traumatism, predominant in the patients with complex lesions. We went through the clinical records of 24 patients who entered the Surgical Intermediate Care Unit of the Central Military Hospital Luis Díaz Soto during 2002, with diagnosis of predominant abdominal trauma. There was a higher incidence of the closed traumatism (70.8 percent), predominating the male sex (75 percent). The peritoneal diagnostic lavage and the abdominal punction were the most used procedures. The most frequent lesions were those in the liver, the small intestine and the retroperitoneal hematoma. Hepatorraphy (30 percent) was the most used surgical technique. The most frequent complications (29.2 percent) intra-abdominal sepsis, complications of the surgical injury and electrolytic disturbs. There was 8.3 percent of mortality. There was not any variation of the abdominal traumatism behavior tendency achieved in the last years, its frequency is steady and the surgical treatment is still the most used.

ABSTRACT

Trauma is the second most common cause of death regardless of age in Iran and the health system annually defrays a vast cost for trauma patients. The majority of cases comprise abdominal trauma. This study was performed to determine individual and pathologic characteristics of patients with blunt abdominal trauma in Gorgan - Iran. This descriptive and retrospective study was done on 239 patients with blunt abdominal trauma in 5 Azar hospital, Gorgan - Iran from March 2001 to March 2005. 197 [82.4%] of cases were male and 42 [17.6%] were female. The mean age of patients was 23.4 years. The mean of hospital stay was 8.8 days. The most common cause of blunt abdominal trauma was traffic injury. The most common damaged organs were spleen [49.4%] and liver [21.3%] respectively. Mortality rate was 16.3%. The hypovolumic shock was the major cause of mortality. This study showed that epidemiology of trauma is similar to those in other regions of our country. Therefore, programming and attempts of governmental organizations are needed to prevent and decrease traffic accidents

ABSTRACT

O objetivo do estudo foi mensurar os gastos diretos do Sistema Único de Saúde (SUS) com internações por causas externas em São José dos Campos, São Paulo, Brasil. Foram estudadas as internações por lesões decorrentes de causas externas, respectivamente capítulos XIX e XX da CID-10, no primeiro semestre de 2003, no Hospital Municipal Dr. José de Carvalho Florence. Foram analisados os valores pagos através do SUS, após a verificação da qualidade dos dados nos prontuários de 976 internações. Os maiores gastos totais foram por internações decorrentes de acidentes de transporte e quedas. O maior gasto médio de internação foi por acidentes de transporte (R$ 614,63), seguido das agressões (R$ 594,90). As lesões que representaram maior gasto médio foram as fraturas de pescoço (R$ 1.191,42) e traumatismo intracraniano (R$ 1.000,44). As internações com maior custo-dia foram fraturas do crânio e dos ossos da face (R$ 166,72) e traumatismo intra-abdominal (R$ 148,26). Os resultados encontrados demonstraram que os acidentes de transporte, as quedas e as agressões são importantes fontes de gastos com internações por causas externas no município.


The objective of this study was to identify direct expenditures for hospitalizations due to external causes in the Unified National Health System (SUS) in the city of São José dos Campos, São Paulo State, Brazil. Admissions to the Dr. José de Carvalho Florence Municipal Hospital resulting from external causes or injuries - ICD-10, chapters XIX and XX respectively - were analyzed for the first semester of 2003. 976 patient admission forms were analyzed, after data evaluation. Admissions with the highest total cost were those resulting from motor vehicle accidents and falls. The highest mean cost for hospitalization for injury was due to motor vehicle accidents (BRL 614.63), followed by assault (BRL 594.90). The highest mean cost for hospitalization due to injury was for cervical fractures (BRL 1,191.42) and head injuries (BRL 1,000.44). Hospitalizations with the highest daily cost were skull and facial fractures (BRL 166.72) and abdominal trauma (BRL 148.26). The study confirmed that motor vehicle accidents, falls, and assault are an important source of costs due to hospitalization for injuries.

ABSTRACT

OBJECTIVE: The present study was undertaken to categorize frequency of abnormalities detected by upper abdominal ultrasound. MATERIAL AND METHOD: The present study was conducted on 3,398 people between the ages of 16 and 93 years old, who had the annual health check-up between October 2003 and May 2004 at the Health Promotion Center Bangkok Hospital Medical Center (all of those participating were defined as "patients"). Of these, 1,930 are men (56.8%) and 1,468 are women (43.2%). In the present study, the data of age, gender, and upper abdominal ultrasound findings were assessed. RESULTS: Of the 3,398 subjects, 1,782 patients (52.4%) had positive findings; 1,147 of which were men (64.4%) and 635 were women (35.6%). Fatty liver, being the most common positive finding, was observed in 1,219 patients (35.9%); 831 of which were men (68.2%) and 388 were women (31.8%). The finding of a liver mass, as the second positive finding, was observed in 209 patients (6.2%); of which were 124 patients (59.3%) with hepatic cyst, 51 patients (24.4%) with hemangioma, 16 patients (7.7%) with indetermined nodule, 11 patients (5.3%) with calcification, and 7 patients (3.4%) with malignant tumor while 664 patients (19.5%) had other positive findings. The top four included gallbladder polyp observed in 181 patients (5.3%), renal cyst observed in 155 patients (4.6%), gallstone observed in 141 patients (4.2%), and renal stone observed in 101 patients (3.0%). CONCLUSION: The prevalence of abnormalities detected by upper abdominal ultrasound was 52.44%. The most common finding was fatty liver (35.87%). Although the majority of positive findings were benign conditions, some of them might have an adverse effect to the health condition of patients in the long run. The ultrasound findings also yielded the patient management to prevent subsequent disease and to perform medical treatment or follow up.

ABSTRACT

Penetrating trauma is the commonest cause of surgical admissions in Baghdad following invasion to Iraq 2003. In this study, the factors that affect the morbidity and mortality in patients with penetrating colonic injuries were evaluated. Eighty six [86] patients [74 males, 12 females; age range 7 to 60 years] with penetrating colonic trauma, most of them of high velocity injuries were evaluated according to age, gender, type of penetration, trauma location and severity of colonic injury, associated injury, interval between the trauma and definitive operation, hemodynamic status, blood transfusion requirement, fecal contamination, surgical procedure, postoperative complication and mortality. The mechanisms of injuries were bullets injury in 66 [76.74%] patients, blast injury [shell] in 18 [ 20.93%] and stab injury in 2 [2.3%] patients. Injuries were evaluated according to TRISS and PATI [penetrating abdominal trauma index]. Multiple organs injuries found in 68 [79%] patients, wherease only the large bowels injured in 18 [21%] patients. Primary repair was performed in 36 [42%] of the 86 patients, and colostomy was performed in 50[58%] patients. From those with primary closure [36 patients], PATI was over 25 in 10 patients and below 25 in 26 patients. 15 patients had resection segment [destructive injuries, primary anastomosis], 7 patients had more than 25% of colonic wall injuries, and 10 had less than 25%. Symptoms of shock were present in 66 patients at admission, mortality rate was 6.97%. These results suggest that the use of primary closure should be expanded in penetrating colon trauma and that, even with moderate and severe degrees of colon injury, primary closure provides an outcome equivalent to that provided by colostomy. The primary repair of the penetrating colon trauma can be performed confidently in patients with PATI suggests that is should be included along with other injury severity indices in trauma data bases
