ABSTRACT

ABSTRACT Introduction: With the preparation and development of the Winter Olympic Games, there has been an increase in the popularity of skiing. Driven by this demand, the professional development of coaches requires updates in the analysis of skiers' sports injuries, elaboration of corresponding prevention and treatment strategies, and resistance training to promote the development of athletes' physical quality, such as bone mass and mineral density. Objective: Study the sports injuries of skiers and explore needs and ways of applying resistance training in skiing. Methods: The questionnaire survey was used to explore the main types of injuries and their influencing factors. Then, 20 volunteers were selected for the resistance training experiment; each group of 10 people included five men and five women. The control group maintained a normal daily life, while resistance training was added to the experimental group three times a week, based on the control group. Results: The research results showed that the current proportion of skiing injuries was relatively high, mainly in mild injuries. After nine weeks in the experimental resistance training group, both men and women had positive bone mass development. Conclusion: Resistance training can improve physical quality and reduce the occurrence of sports injuries, demonstrating the benefits of its implementation in endurance training for skiers. RESUMO Introdução: Com a preparação e desenvolvimento dos Jogos Olímpicos de Inverno, houve um aumento da popularidade do esqui. Impulsionados por essa demanda, o desenvolvimento profissional dos treinadores requer atualizações nas análises de lesões esportivas dos esquiadores, elaboração de estratégias de prevenção e tratamento correspondentes, além de treinamentos de resistência de modo a promover o desenvolvimento da qualidade física dos atletas, como a massa óssea e sua densidade mineral. Objetivo: Estudar as lesões esportivas dos esquiadores e explorar necessidades e modos de aplicação do treino de resistência no esqui. Métodos: O método de pesquisa por questionário foi utilizado para explorar os principais tipos de lesões e seus fatores influenciadores. Em seguida, 20 voluntários foram selecionados para o experimento de treinamento de resistência, cada grupo de 10 pessoas incluiu 5 homens e 5 mulheres. O grupo controle manteve uma vida diária normal, enquanto ao grupo experimental foi adicionado um treinamento de resistência, três vezes por semana, com base no grupo controle. Resultados: Os resultados da pesquisa mostraram que a proporção atual de lesões por esqui foi relativamente alta, principalmente em lesões leves. Após 9 semanas no grupo experimental de treinamento de resistência, tanto homens quanto mulheres tiveram desenvolvimento positivo de massa óssea. Conclusão: O treinamento de resistência pode não só melhorar a qualidade física, mas também reduzir a ocorrência de lesões esportivas, demonstrando os benefícios de sua implementação no treinamento de resistência dos esquiadores. RESUMEN Introducción: Con la preparación y el desarrollo de los Juegos Olímpicos de Invierno, ha aumentado la popularidad del esquí. Impulsado por esta demanda, el desarrollo profesional de los entrenadores requiere actualizaciones en el análisis de las lesiones deportivas de los esquiadores, la elaboración de las correspondientes estrategias de prevención y tratamiento, así como el entrenamiento de la resistencia para promover el desarrollo de la calidad física de los atletas, como la masa ósea y la densidad mineral. Objetivo: Estudiar las lesiones deportivas de los esquiadores y explorar las necesidades y formas de aplicar el entrenamiento de resistencia en el esquí. Métodos: Se utilizó el método de encuesta por cuestionario para explorar los principales tipos de lesiones y sus factores de influencia. A continuación, se seleccionaron 20 voluntarios para el experimento de entrenamiento de resistencia, cada grupo de 10 personas incluía 5 hombres y 5 mujeres. El grupo de control mantuvo una vida diaria normal, mientras que al grupo experimental se le añadió un entrenamiento de resistencia, tres veces por semana, basado en el grupo de control. Resultados: Los resultados de la investigación mostraron que la proporción actual de lesiones de esquí era relativamente alta, principalmente en las lesiones leves. Después de 9 semanas en el grupo experimental de entrenamiento de resistencia, tanto los hombres como las mujeres tuvieron un desarrollo positivo de la masa ósea. Conclusión: El entrenamiento de resistencia no sólo puede mejorar la calidad física, sino también reducir la aparición de lesiones deportivas, lo que demuestra los beneficios de su aplicación en el entrenamiento de resistencia de los esquiadores. ABSTRACT Introduction The research on the promotion of physical fitness of the human body by exercise has also attracted widespread attention in sports and rehabilitation medicine. Object This article uses ultrasound to study the bone density health of the human body, thereby establishing the relationship between sports and bone density health. We hope to use the research results of this article to improve people's awareness of sports health. Method We used ultrasound to test the bone mineral density of two groups of students (exercise group and non-exercise group) to explore the relationship between different sports items and the students' bone mineral density. Results There is a significant difference in bone density between the two groups of students. There is no gender difference in bone density. Conclusion Students need to strengthen physical exercise in the growth and development stage to improve bone density. Level of evidence II; Therapeutic studies - investigation of treatment results.


RESUMO Introdução A pesquisa sobre a promoção da aptidão física do corpo humano por meio do exercício também tem atraído atenção generalizada na medicina esportiva e de reabilitação. Objeto este artigo usa o ultrassom para estudar a saúde da densidade óssea do corpo humano, estabelecendo assim a relação entre esportes e saúde da densidade óssea. Esperamos usar os resultados da pesquisa deste artigo para aumentar a conscientização das pessoas sobre a saúde no esporte. Método Usamos ultrassom para testar a densidade mineral óssea de dois grupos de alunos (grupo de exercícios e grupo de não exercícios) para explorar a relação entre diferentes itens esportivos e a densidade mineral óssea dos alunos. Resultados Existe uma diferença significativa na densidade óssea entre os dois grupos de alunos. Não há diferença de gênero na densidade óssea. Conclusão Os alunos precisam fortalecer os exercícios físicos na fase de crescimento e desenvolvimento para melhorar a densidade óssea. Nível de evidência II; Estudos terapêuticos - investigação dos resultados do tratamento.


RESUMEN Introducción La investigación sobre la promoción de la aptitud física del cuerpo humano mediante el ejercicio también ha atraído una amplia atención en la medicina deportiva y de rehabilitación. Objeto Este artículo utiliza la ecografía para estudiar la salud de la densidad ósea del cuerpo humano, estableciendo así la relación entre el deporte y la salud de la densidad ósea. Esperamos utilizar los resultados de la investigación de este artículo para mejorar la conciencia de las personas sobre la salud deportiva. Método Utilizamos ultrasonido para probar la densidad mineral ósea de dos grupos de estudiantes (grupo de ejercicio y grupo sin ejercicio) para explorar la relación entre diferentes artículos deportivos y la densidad mineral ósea de los estudiantes. Resultados Existe una diferencia significativa en la densidad ósea entre los dos grupos de estudiantes. No hay diferencia de género en la densidad ósea. Conclusión Los estudiantes necesitan fortalecer el ejercicio físico en la etapa de crecimiento y desarrollo para mejorar la densidad ósea. Nivel de evidencia II; Estudios terapéuticos: investigación de los resultados del tratamiento.

ABSTRACT Introduction: Basketball sports will effect on the morphology and structure of the hand bones and joints. Objective: The article selected nine professional basketball players (basketball group) and 11 non-basketball players (control group) in the provincial youth team. A 64-row spiral computed tomography scan was used to scan the wrist and three-dimensional reconstruction. The volume of each carpal bone and the computed tomography value (bone density) were measured. Methods: To explore the influence of basketball sports on the hand bones and joints' morphological structure, the paper analyzes the structural characteristics of the computed tomography images of young male basketball players' wrist bones. Results: Compared with the carpal bones in the control group, the volume of the right navicular bone and the small polygonal bone, the left-hand navicular bone, the large triangular bone, and the small polygonal bone in the basketball group increased significantly (P<0.05). Conclusions: Basketball can increase the volume of the part of the wrist bones of adolescent male athletes and reduce the bone density; the morphological structure of the non-smashing wrist bones of basketball players has similar changes to that of the spikers. RESUMO Introdução: O basquetebol afetará a morfologia e a estrutura dos ossos e articulações da mão. Objetivo: o artigo selecionou nove jogadores profissionais de basquete (grupo de basquete) e 11 não jogadores de basquete (grupo de controle) da equipe juvenil da província. Uma tomografia computadorizada espiral de 64 linhas foi usada para digitalizar o punho e a reconstrução tridimensional. O volume de cada osso do carpo e o valor da tomografia computadorizada (densidade óssea) foram medidos. Métodos: Para explorar a influência dos esportes de basquete sobre os ossos da mão e a estrutura morfológica das articulações, o artigo analisa as características estruturais das imagens de tomografia computadorizada de ossos do punho de jovens jogadores de basquete do sexo masculino. Resultados: Em comparação com os ossos do carpo no grupo de controle, o volume do osso navicular direito e o osso poligonal pequeno, o osso navicular esquerdo, o osso triangular grande e o osso poligonal pequeno no grupo de basquete aumentaram significativamente (P <0,05). Conclusões: O basquete pode aumentar o volume dos ossos do punho de atletas adolescentes do sexo masculino e reduzir a densidade óssea; a estrutura morfológica dos ossos do pulso que não se quebram em jogadores de basquete tem mudanças semelhantes às dos espigões. Nível de evidência II; Estudos terapêuticos- investigação dos resultados do tratamento.


RESUMEN Introducción: el baloncesto afectará la morfología y la estructura de los huesos y articulaciones de la mano. Objetivo: El artículo seleccionó nueve jugadores de baloncesto profesionales (grupo de baloncesto) y 11 jugadores no baloncesto (grupo de control) en el equipo juvenil provincial. Se utilizó una tomografía computarizada en espiral de 64 filas para escanear la muñeca y la reconstrucción tridimensional. Se midieron el volumen de cada hueso del carpo y el valor de la tomografía computarizada (densidad ósea). Métodos: Para explorar la influencia de los deportes de baloncesto en la estructura morfológica de los huesos de la mano y las articulaciones, el artículo analiza las características estructurales de las imágenes de tomografía computarizada de los huesos de la muñeca de los jóvenes jugadores de baloncesto. Resultados: En comparación con los huesos del carpo en el grupo de control, el volumen del hueso navicular derecho y el hueso poligonal pequeño, el hueso navicular izquierdo, el hueso triangular grande y el hueso poligonal pequeño en el grupo de baloncesto aumentaron significativamente (P <0,05). Conclusiones: El baloncesto puede aumentar el volumen de los huesos de la muñeca de los deportistas varones adolescentes y reducir la densidad ósea; la estructura morfológica de los huesos de las muñecas que no se rompen de los jugadores de baloncesto tiene cambios similares a los de los atacantes. Nivel de evidencia II; Estudios terapéuticos- investigación de los resultados del tratamiento.

O objetivo do estudo foi analisar a composição corporal com utilização do DXA e correlacioná-la com a idade cronológica em adolescentes pós-púberes, de ambos os sexos. Participaram da pesquisa 46 adolescentes em fase pós-puberal, sendo 27 meninas (17,23±0,98) e 19 meninos (17,65±0,74) de ensino médio de uma escola estadual da Zona Sul da cidade do Rio de Janeiro. Foram realizadas uma anamnese, avaliação antropométrica e o DXA para avaliações. O teste U de Mann Whitney e o coeficiente de correlação de Spearman foram utilizados, adotando P<0,05 para significância. Verificou-se que as meninas apresentam composição corporal diferentedos meninos (P<0,05) e que com o aumento da idade elas tendem a diminuir o percentual de gordura, embora tenha sido observado um quantitativo alto de meninas com percentual de gordura elevado. Os meninos apresentaram uma tendência de aumento do percentual de gordura e da massa magra com o aumento da idade. As medidas de conteúdo e densidade mineral ósseo dentro da normalidade, com tendência de aumento com o avanço da idade cronológica. Conclui-se que os grupos masculino e feminino apresentaram comportamentos diferenciados quanto à composição corporal e os valores apresentados trazem mais um complemento à literatura a respeito de referências para a composição corporal, obtida com o DXA, em adolescentes pós-púberes.


The purpose of this study was to analyze body composition using DXA and correlate it with chronological age in post-pubertal adolescents of both genders. A total of 46 adolescents participated in the study, of which 27 were girls (17.23±0.98) and 19 boys (17.65±0.74) from a state school in the South District in the city of Rio de Janeiro. An anamnesis, anthropometric evaluation and DXA were performed for evaluations. The Mann-Whitney U test and the Spearman correlation coefficient were used, adopting P <0.05 for significance. Girls were found to have a different body composition than boys (P <0.05) and that, with increasing age, they tended to present a decrease in fat percentage, although in general a high number of girls presented a high fat level. The boys presented a tendency to increase the percentage of fat and lean mass with the increase of the age. Measurements of bone mineral content and density were considered as being within normality, with a tendency to increase with the advancing of the chronological age. Girls and boys present a different behavior regarding body composition and the values presented in this study bring an addition to the literature regarding body composition references through DXA in post-pubertal adolescents.

ABSTRACT

Phosphodiesterase-5 inhibitors (PDE-5Is) exert positive effects on bone healing and mineralization by activation the nitric oxide/cyclic guanosine monophosphate/protein kinase-G (NO/cGMP/PKG) signaling pathway. In this study, the effects of zaprinast and avanafil, two PDE-5Is, on the NO signaling pathway, estrogen levels, selected bone formation and destruction marker levels, whole-body bone mineral density (WB-BMD), right femur trabecular bone thickness (RF-TBT) and epiphyseal bone width, angiogenesis in the bone-marrow, and selected oxidative stress parameter levels were investigated in rats with ovariectomy-induced osteoporosis. Twenty four adult rats (8 months old) were equally divided into four groups. The first group was the sham operated group. Groups 2, 3 and 4 included ovariectomized rats. At six months after ovariectomy, the 3rd and 4th groups were administered 10 mg/kg zaprinast and avanafil daily as a single dose for 60 days, respectively. Increases in the activity of the NO/cGMP/PKG signalling-pathway, C-terminal collagen peptide levels, angiogenesis in the bone marrow, RF-TBT, epiphyseal bone width and WB-BMD were observed compared to the ovariectomized positive control group (OVX), while the pyridinoline and deoxypyridinoline levels were decreased in the OVX+zaprinast and OVX+avanafil groups (p<0.05). The malondialdehyde, ubiquinone10/ubiquinol10 and 8-hydroxy-2-deoxyguanosine/106deoxyguanosine levels were also increased in the ovariectomized groups compared to the sham group (p<0.05). Based on these results, the levels of bone atrophy and some markers of oxidative stress were increased due to acute estrogen deficiency induced by ovariectomy, but zaprinast and avanafil administration significantly prevented these changes

Abstract Introduction: Age-related hearing impairment is the most common sensory dysfunction in older adults. In osteoporosis, the mass of the ossicles will be decreased, affecting the bone density of the cochlea, and interfering with the sound transmission to the cochlea. Age related hearing loss might be closely related to osteoporosis. Objective: To determine the relationship between age-related hearing impairment and osteoporosis by investigating the relationship between hearing loss and cortical bone density evaluated from femur neck bone mineral density. Methods: We used data from the Korea National Health and Nutrition Examination Survey to examine the associations between osteoporosis and age-related hearing impairment from 2009 to 2011. Total number of participants was 4861 including 2273 men and 2588 women aged 50 years or older. Osteoporosis was defined as a bone mineral density 2.5 standard deviations below according to the World Health Organization diagnostic classification. Age-related hearing impairment was defined as the pure-tone averages of test frequencies 0.5, 1, 2, and 4 kHz at a threshold of 40 dB or higher on the more impaired hearing side. Results: Total femur T-score (p < 0.001), lumbar-spine T-score (p < 0.001) and, femur neck T-score (p < 0.001) were significantly lower in the osteoporosis group compared to the normal group. Thresholds of pure-tone averages were significantly different in normal compared to osteopenia, and osteoporosis groups. In addition, there were significantly higher pure-tone averages thresholds in the osteoporosis group compared to other groups (p < 0.001). After adjusting for all covariates, the odds ratio for hearing loss was significantly increased by 1.7 fold with reduced femur neck bone mineral density (p < 0.01). However, lumbar spine bone mineral density was not statistically associated with hearing loss (p = 0.22). Conclusion: Our results suggest that osteoporosis is significantly associated with a risk of hearing loss. In addition, femur neck bone mineral density was significantly correlated with hearing loss, but lumbar spine bone mineral density was not.


Resumo Introdução: A perda auditiva associada ao envelhecimento é a disfunção sensorial mais comum em idosos. Na osteoporose, a massa dos ossículos diminui e afeta a densidade óssea da cóclea, o que irá interferir na transmissão do som para a mesma. A perda auditiva associada à idade pode estar intimamente relacionada à osteoporose. Objetivo: Determinar a relação entre deficiência auditiva relacionada à idade e osteoporose, investigar a relação entre perda auditiva e densidade óssea cortical avaliada a partir da densidade mineral óssea do colo do fêmur. Método: Utilizamos dados da Korea National Health and Nutrition Examination Survey para examinar as associações entre osteoporose e perda auditiva associada ao envelhecimento de 2009 a 2011. O número total de participantes foi de 4.861, incluiu 2.273 homens e 2.588 mulheres com 50 anos ou mais. A osteoporose foi definida como densidade mineral óssea com 2,5 desvios-padrão abaixo da média, de acordo com a classificação diagnóstica da Organização Mundial da Saúde. A perda auditiva associada ao envelhecimento foi definida como as médias de tom puro das frequências de teste de 0,5, 1, 2 e 4 kHz a um limiar de 40 dB ou superior no lado da audição mais afetado. Resultados: O T-score total do fêmur (p < 0,001), o T-score da coluna lombar (p < 0,001) e o T-score do colo do fêmur (p < 0,001) foram significantemente menores no grupo com osteoporose em comparação ao grupo normal. Os limiares de médias de tom puro foram significantemente diferentes nos grupos normais em comparação com aqueles com osteopenia e osteoporose. Além disso, houve limiares significantemente maiores de médias de tom puro no grupo com osteoporose em comparação com os outros grupos (p < 0,001). Após o ajuste para todas as covariáveis, a odds ratio da perda auditiva mostrou estar significantemente aumentada em 1,7 vez com densidade mineral óssea reduzida no colo do fêmur (p < 0,01). No entanto, a densidade mineral óssea da coluna L não se associou estatisticamente à perda auditiva (p = 0,22). Conclusão: Nossos resultados sugerem que a osteoporose está significantemente associada ao risco de perda auditiva. Além disso, a densidade mineral óssea da coluna lombar não se correlacionou com a perda auditiva, apenas a densidade mineral óssea do colo do fêmur foi significantemente correlacionada.

ABSTRACT Objective: To systematically review the literature to verify the relationship between neuromuscular fitness indicators in childhood/adolescence and bone strength variables in adulthood. Data sources: A systematic review was conducted in PUBMED, SCOPUS, SPORTDiscus, Web of Science, PsycINFO, LILACS, and SciELO, covering the entire period until March 2019. Data synthesis: The search identified 1149 studies. After duplicity analysis and eligibility criteria, four studies were reported. In one study, baseline was childhood and, in the others, adolescence. In childhood, when adjusting the model for age and body mass index, a statistically significant relation was found for girls: standing long jump with quantitative ultrasound index (β=0.11; p<0.05) and with speed of sound (β=0.14; p<0.01). However, when controlling muscular performance in adulthood, the relationship was no longer significant. In adolescence, coefficients ranged from 0.16 for neuromotor battery and bone mineral density (BMD) in the lumbar region to 0.38 for hanging leg lift test and BMD of arms. The explained variance varied between 2% (bent arm hang for BMD total) and 12% (hanging leg-lift for BMD arms), therefore, a higher performance in neuromuscular fitness in adolescence was associated with better bone strength in adulthood. Conclusions: In adults, bone strength variables showed significant correlation from low to moderate magnitude with neuromuscular fitness indicators in adolescence, but not in childhood, after controlling for adult performance in neuromuscular fitness. However, there is limited evidence to support the neuromuscular fitness in early life as a determinant of bone strength in adulthood.


RESUMO Objetivo: Revisar sistematicamente a literatura para verificar a relação entre indicadores da aptidão neuromuscular na infância/adolescência e variáveis de resistência óssea na idade adulta. Fonte de dados: Revisão sistemática com pesquisa de artigos conduzida no PUBMED, SCOPUS, SPORTDiscus, Web of Science, PsycINFO, LILACS and SciELO, abrangendo todo o período da base até março de 2019. Síntese dos dados: A busca identificou 1.149 estudos, após análise de duplicidade e dos critérios de elegibilidade, quatro estudos foram relatados. Em um dos estudos, o baseline foi a infância e, nos demais, a adolescência. Na infância, ao ajustar o modelo por idade e índice de massa corporal, foi encontrada relação estatisticamente significativa para as meninas: salto em distância com índice quantitativo de ultrassom (β=0,11; p<0,05) e com velocidade do som (β=0,14; p<0,01). Entretanto, ao se fazer o controle do desempenho muscular na idade adulta, a relação deixou de ser significativa. Na adolescência, coeficientes variaram de 0,16 para bateria neuromotora e densidade mineral óssea (DMO) lombar a 0,38 para o teste de elevação de pernas em suspensão e a DMO dos braços. Variação explicada entre 2% (suspensão na barra e DMO do corpo total) e 12% (elevação de pernas em suspensão e DMO dos braços), portanto, melhor desempenho na aptidão neuromuscular na adolescência, mais resistência óssea na idade adulta. Conclusões: Em adultos, variáveis de resistência óssea apresentaram correlação significativa em magnitude baixa a moderada com indicadores da aptidão neuromuscular na adolescência, mas não na infância, após controlar pelo desempenho adulto na aptidão neuromuscular. ABSTRACT

Se denomina transgénero mujer (TM) a un varón biológico con identidad de género femenina. El tratamiento hormonal cruzado (THC) es una de las opciones para lograr caracteres sexuales del género autopercibido. Realizamos un estudio de diseño transversal, observacional y analítico para evaluar la densidad mineral ósea, composición corporal y fuerza muscular antes de iniciar la hormonización. Un total de 26 TM en condiciones de ingresar en el estudio fueron comparadas con hombres cisgénero de similar edad (mediana 23,5 vs. 25,5 años). Basalmente, las TM presentaron menor densidad ósea en columna lumbar (1,040 vs. 1,280 g/cm2; p=0,01), cadera total (0,970 vs. 1,070 g/cm2; p=0,01) y cuerpo entero (1,080 vs. 1,220 g/cm2; p<0,01). Observamos, además, menor masa muscular en brazos (5,033 vs. 6,212 kg; p<0,01) y piernas (16,343 vs. 18,404 kg; p=0,02), acompañada de menor fuerza muscular de puño (p<0,01). Concluimos que las TM presentaron características diferentes de la biología masculina aun sin haber iniciado el THC. Sugerimos incluir la evaluación de la densidad mineral ósea en la evaluación inicial de esta población, dados los hallazgos identificados. (AU)


A trans-woman (TW) is a biologically male person with female gender identity. Cisgender denotes a person whose sense of personal identity and gender corresponds with its birth sex. Cross-sex hormone therapy (CSHT) is one of the options to achieve secondary characteristics of the self-perceived gender. We performed a cross-sectional study. Bone mineral density (BMD), body composition, and muscle strength before starting CSHT were assessed. Twenty-six TW (median age 23.5 years) and cisgender males (median age 25.5 years) were matched for age. TW had less BMD at the lumbar spine (1.040 vs 1.280 g/cm2; p=0.01), total hip (0.970 vs 1.070 g/cm2; p=0.01), and total body (1.080 vs 1.220 g/cm2; p<0.01). They also had less skeletal muscle mass in the arms (5.033 vs 6.212 kg; p<0.01) and legs (16.343 vs 18.404 kg; p=0.02), associated with lower grip strength (p<0.01). It appears that bone and muscle characteristics of TW before starting CSHT differ from cisgender men. Taking these findings into account, we suggest the inclusion of BMD in the initial evaluation of TW. (AU)

Abstract Objective: To analyze the effect of height catch-up at school age on the bone mass of adolescents of a Brazilian birth cohort. Methods: A cohort study using data obtained from the three moments (birth, 7/9 years and 18/19 years of age) of the Cohorts-RPS study. Height catch-up was defined based on the difference between the schoolchild's z-score and birth z-score. The adolescents' bone mineral mass was analyzed using the z-score index for the lumbar spine measured by dual emission X-ray absorptiometry. A theoretical model was developed for the proposed analysis using directed acyclic graphs paired through the nearest-neighbor matching propensity score using the STATA software, version 14.0. The level of significance was set at 5%. Results: Of the 297 studied children, 24.5% achieved height catch-up. The bone mass below the expected for age was observed in 5.39% of the subjects. The mean lumbar spine z-score was −0.34 (±1.01). After the adjustment, no effect was observed between height catch-up at school age and bone mass in adolescents (Coeff = 0.598; 95% CI −0.117 to 1.313; p = 0.101). Conclusion: Even using the directed acyclic graphs and the causal inference method by adjusting the propensity score, the height catch-up did not seem to affect bone mass in adolescents, a result perhaps related to the sample size.


Resumo: Objetivo: Analisar o efeito do catch-up de estatura na idade escolar na massa óssea em adolescentes em uma coorte brasileira de nascimento. Métodos: Estudo de coorte, utilizando dados dos três momentos (nascimento, aos 7/9 anos e 18/19 anos) da pesquisa Coortes-RPS. Catch-up de estatura foi definida a partir da diferença entre o Z-score do escolar e Z-score do nascimento. Para a análise da massa óssea em adolescentes foi utilizado o índice Z-score da coluna lombar medido pela densitometria por dupla emissão de raio X. Para análise proposta, foi construído modelo teórico usando os gráficos acíclicos direcionados e pareado por escore de propensão do tipo vizinho mais próximo utilizando o software STATA versão 14.0. O nível de significância adotado foi de 5%. Resultados: Das 297 crianças, 24,58% realizaram o catch-up estatural. Massa óssea abaixo do esperado para a idade foi de 5,39%. O Z-score médio da coluna lombar foi −0,34 (± 1,01). Após ajuste, não foi observado efeito entre catch-up de estatura na idade escolar e massa óssea no adolescente (Coef = 0,598; IC 95% −0,117-1,313; p = 0,101). Conclusão: Mesmo com os gráficos acíclicos direcionados e o método de inferência causal por ajuste do escore de propensão, o catch-up de estatura parece não afetar a massa óssea em adolescentes, resultado talvez relacionado ao tamanho da amostra.

Abstract Objective: The objective of this study was to evaluate the effects of two low-dose combined oral contraceptives on bone metabolism in adolescents for one year. Methods: This was a quasi-experimental study. The adolescents were divided into three groups: oral contraceptives 1 (n = 42) (20 µg EE/150 µg desogestrel), oral contraceptives 2 (n = 66) (30 µg EE/3 mg drospirenone), and a control group (n = 70). Adolescents underwent anthropometric assessment and densitometry (dual-energy X-ray). Bone age and bone formation markers (osteocalcin and bone alkaline phosphatase) were evaluated. The oral contraceptives users were evaluated again after 12 months. Linear regression analysis was used to indirectly study the effect of each additional year of chronological age on anthropometric and densitometric variables as well as on bone markers in the control group. Results: At study entry, no significant differences were observed between the oral contraceptives 1, oral contraceptives 2, and controls in the analyzed variables. Linear regression analysis showed an increase in bone mineral density and bone mineral content for each additional year. There was a significant reduction in bone alkaline phosphatase levels; no significant difference was observed for osteocalcin in control individuals. Comparison of dual-energy X-ray variables at baseline and after one year showed no significant differences in the oral contraceptives 1 or oral contraceptives 2 groups. A significant reduction in bone alkaline phosphatase and osteocalcin levels was observed in both the oral contraceptives 1 and oral contraceptives 2 groups. Conclusion: Adolescent women gain peak bone mass during this phase of life. Two low-dose combined oral hormonal contraceptives were associated with lower bone gain and lower bone formation markers than in untreated controls.


Resumo: Objetivo: O objetivo deste estudo foi avaliar os efeitos de dois contraceptivos orais combinados de baixa dosagem por um ano sobre o metabolismo ósseo em adolescentes. Métodos: Este foi um estudo quase experimental. As adolescentes foram divididas em três grupos: contraceptivos orais 1 (n = 42) (20 µg de EE/150 µg de desogestrel), contraceptivos orais 2 (n = 66) (30 µg EE/3 mg de drospirenona) e grupo controle (n = 70). As adolescentes foram submetidas à avaliação antropométrica e densitometria (raio-X de dupla energia). Foram avaliados a idade óssea e os marcadores de formação óssea (osteocalcina e fosfatase alcalina óssea). As usuárias de contraceptivos orais foram novamente avaliadas após 12 meses. A análise de regressão linear foi utilizada para estudar, indiretamente, o efeito de cada ano adicional da idade cronológica sobre as variáveis antropométricas e densitométricas e sobre os marcadores ósseos no grupo de controle. Resultados: No início do estudo, não foram observadas diferenças significativas nas variáveis analisadas entre as usuárias de contraceptivos orais 1, contraceptivos orais 2 e o grupo controle. A análise de regressão linear mostrou um aumento na densidade mineral óssea e no conteúdo mineral ósseo para cada ano adicional. Houve uma redução significativa nos níveis de fosfatase alcalina óssea e não foi observada diferença significativa para osteocalcina nos indivíduos controles. A comparação das variáveis do raio-X de dupla energia no início e após um ano não mostrou diferença significativa no grupo de contraceptivos orais 1 ou contraceptivos orais 2. Foi observada uma redução significativa nos níveis de fosfatase alcalina óssea e osteocalcina nos dois grupos contraceptivos orais 1 e contraceptivos orais 2. Conclusão: As adolescentes atingiram o pico de massa óssea durante essa fase da vida. Duas formulações de contraceptivos hormonais orais de baixa dosagem, após um ano de uso, se associaram a menor incremento na densidade mineral óssea e menor concentração de marcadores de formação óssea quando confrontados com resultados de adolescentes não usuárias de contraceptivos.

Abstract Background Osteoporosis (OP) is common in patients with chronic obstructive pulmonary disease (COPD). The relationship between OP and COPD has been primarily studied in male patients, and few reports are available in postmenopausal women. Objective The purpose of this study was to investigate the association between bone mineral density (BMD) and COPD in postmenopausal women. Methods This cross-sectional study included 133 clinically stable female ex-smokers with confirmed COPD, and 31 age-matched “ex-smoker” female controls. We analyzed groups according to their airway obstruction category. BMD was measured on dual-energy X-ray absorptiometry images of the left femoral neck. Results Patients with COPD had lower T-scores and higher prevalence of osteopenia/OP than the control group. In the COPD group, the airway obstruction category was significantly associated with the T-score after adjustment for confounders. Multivariate logistic regression analysis showed COPD was an independent marker for increased risk of osteopenia/OP in postmenopausal women. Conclusions COPD and airway obstruction category were strongly related to BMD. Postmenopausal women with COPD, especially those with severe airway obstruction, had a higher prevalence rate and a higher risk of osteopenia and OP than female controls without COPD.

Estudios recientes han sugerido que los estímulos mecánicos (vibraciones) de alta frecuencia y baja magnitud pueden ejercer un efecto positivo sobre la morfología ósea y beneficiar su cantidad y calidad. La plataforma vibratoria es una máquina popular que se introdujo en la última década como una nueva promesa contra el tratamiento de la osteoporosis. Actualmente, en el mundo hay más de 200 millones de mujeres posmenopáusicas que sufren osteoporosis. Esta enfermedad es una de las más comunes y costosas de la salud pública. El ejercicio físico complementado con el tratamiento vibratorio puede que sea considerado como una estrategia efectiva para la prevención y tratamiento de la osteoporosis posmenopáusica. Esta revisión ofrece una visión general de cuestiones significativas relacionadas con la terapia con la plataforma vibratoria para la prevención y tratamiento de la osteoporosis en mujeres postmenopáusicas. El objetivo de esta revisión ha sido conocer los últimos avances de entrenamiento con plataformas vibratorias para la mejoría de la masa ósea en mujeres posmenopáusicas. Existe una gran discrepancia respecto al uso de estas como tratamiento osteoporósico, uso de diferentes tipos de plataformas, distintas frecuencias, amplitud, aceleración o duración del tratamiento. La escasa literatura estableció que la plataforma vibratoria Galileo es la que más se utiliza en dicha población, pero se necesitan más intervenciones para concretar los beneficios y daños de este tratamiento en mujeres postmenopáusicas(AU)


Recent studies have suggested that mechanical stimuli (vibrations) of high frequency and low magnitude can exert a positive effect on bone morphology and benefit quantity and quality. The vibrating platform is a popular machine introduced in the last decade as a new promise against the treatment of osteoporosis. Currently, there are more than 200 million postmenopausal women in the world suffering from osteoporosis. This disease is one of the most common and expensive in public health. Physical exercise supplemented with vibrational treatment may be considered an effective strategy for the prevention and treatment of postmenopausal osteoporosis. This review offers an overview of significant issues related to therapy with the vibration platform for the prevention and treatment of osteoporosis in postmenopausal women. The objective of this review is to know the latest advances in vibratory platforms training for the improvement of bone mass in postmenopausal women. There is a great discrepancy regarding the use of vibratory platforms as osteoporosis treatment, the use of different types of platforms, different frequencies, amplitude, acceleration or duration of treatment. The limited literature established that Galileo vibration platform is the most used in this population, but more interventions are needed to grasp the benefits and harms of this treatment in postmenopausal women(AU)

ABSTRACT BACKGROUND: Sports participation plays an important role in bone gain during childhood and adolescence. The aim here was to identify sex-related determinants of bone mineral density (BMD) differences between male and female adolescents, with emphasis on the role of sports participation. DESIGN AND SETTING: Longitudinal study conducted in a public university in Presidente Prudente, Brazil. METHODS: The sample comprised 48 adolescents aged 11-17 years, of both sexes, who were matched according to sex, age and sports participation. BMD was the main outcome, while muscle mass, sports participation, calendar age and biological maturation were treated as covariates. Participants were followed up after nine months. RESULTS: At baseline, BMD values were similar between the sexes. However, adjustment for covariates showed that BMD was higher among girls at all sites, with a contribution from lean soft tissue (LST) in the model (partial eta-squared, ES-r = 0.619 in upper limbs; 0.643 in lower limbs; 0.699 in spine; and 0.599 in whole body). Sports participation only explained the upper-limb variance (ES-r = 0.99). At the follow-up, the results resembled the baseline except in the lower limbs (P = 0.109), in which BMD was similar between the groups. BMD gain over time was similar between girls and boys in all segments, and baseline LST affected upper-limb and whole-body BMD accrual (ES-r = 0.396 and 0.107, respectively). CONCLUSION: Whole-body and specific-site BMD differed between baseline and follow-up. However,BMD accrual was similar between the sexes, given that muscle mass constituted the most relevant determinant of the difference between them.

ABSTRACT

En esta investigación se planteó como objetivo la verificación del comportamiento dosimétrico del Sistema de Planificación de Tratamiento (TPS) de Radioterapia mediante las curvas de calibración de Densidades Electrónicas Relativas (DER). Este estudio se realizó en el Hospital de la Sociedad de Lucha Contra el Cáncer (SOLCA) Núcleo Loja, usando un fantoma antropomorfo CIRS 062M y un tomógrafo Toshiba Activion 16. Para determinar la nueva curva de calibración DER se tomaron los valores de densidades electrónicas especificadas en el manual del fantoma y las Unidades Hounsfield de la imagen tomográfica. Se realizó controles de calidad dosimétricos y verificación dosimétrica en tres casos clínicos: tórax, pelvis y cráneo; para realizar las pruebas dosimétricas se utilizó un acelerador CLINAC CX, cámara de ionización PTW tipo Farmer con volumen sensible de 0,6 cm3 y un electrómetro PTW UNIDOS E. Los resultados mostraron que las medidas para cada inserto del fantoma en ningún caso excedieron los límites establecidos de ± 20 UH, para el tomógrafo y el TPS; las pruebas de control de calidad no superaron el límite máximo de desviaciones en el cálculo de dosis absorbida por el TPS y la obtenida por medición de ± 4 % establecida por la IAEA y las verificaciones dosimétricas en tórax, pelvis y cráneo, determinaron que las desviaciones en el cálculo de la dosis absorbida por el TPS y la obtenida por medición no superaban la tolerancia del ± 5 % establecida por la ICRU.


In this research, the aim was to verify the dosimetric behavior of the Radiotherapy Treatment Planning System (TPS) using the Relative Electron Density (DER) calibration curves. This study was carried out at the SOLCA (Society of Fight Against Cancer) hospital in Loja, using an CIRS model 062M anthropomorphic phantom and a Toshiba Activion 16 tomograph. To determine the new DER calibration curve, the values of the electron densities specified in the manual of the phantom and the Hounsfield Units of the tomographic image were taken. Dosimetric quality controls were made in the location of three clinical cases: thorax pelvis and skull; used a CLINAC CX accelerator was used to perform the dosimetric tests, PTW ionization chamber type Farmer with sensitive volume of 0.6 cm3 and a PTW UNIDOS E electrometer. The results showed that the measurements for each insert of the phantom in no case exceeded the established limits of ± 20 UH, for the tomograph and the TPS; the quality control tests did not exceed the maximum limit of deviations in the calculation of dose absorbed by the TPS and the one obtained by measurement of ± 4% established by the IAEA and the clinical planning in the thorax, pelvis and skull, determine that the deviations in the calculation of the dose absorbed by the TPS and that obtained by measurement, they do not exceed the tolerance of ± 5% established by the ICR.

Abstract Purpose: To evaluate the effects of food restriction on fracture healing in growing rats. Methods: Sixty-eight male Wistar rats were assigned to two groups: (1) Control and (2) Dietary restriction. After weaning the dietary restricted animals were fed ad libitum for 42 days with 50% of the standard chow ingested by the control group. Subsequently, the animals underwent bone fracture at the diaphysis of the right femur, followed by surgical stabilization of bone fragments. On days 14 and 28 post-fracture, the rats were euthanized, and the fractured femurs were dissected, the callus was analyzed by dual-energy X-ray absorptiometry, micro-computed tomography, histomorphometry, mechanical tests, and gene expression. Results: Dietary restriction decreased body mass gain and resulted in several phenotypic changes at the bone callus (a delay in cell proliferation and differentiation, lower rate of newly formed bone and collagen deposition, reductions in bone callus density and size, decrease in tridimensional callus volume, deterioration in microstructure, and reduction in bone callus strength), together with the downregulated expression of osteoblast-related genes. Conclusion: Dietary restriction had detrimental effects on osseous healing, with a healing delay and a lower quality of bone callus formation.

Abstract Purpose: To investigate the effects of dietary restriction on the growth plate and long bone tissue in growing rats. Methods: Sixty male Wistar rats were randomly assigned to two groups: Control (Con) and Diet-restricted (Res). After weaning, the Res rats were offered 50% of the chow ingested by the control (ad libitum food intake). The animals were subdivided into two subgroups with follow-ups up to 56 or 70 days. After euthanasia, the growth plate of tibias was analyzed by histomorphometry, micro-computed tomography, and mechanical test. The trabecular and compact bones were evaluated by histomorphometry, dual-energy X-ray absorptiometry, and micro-computed tomography (μCT). Real-time PCR was used to analyze gene expression. Results: Although dietary restriction did not alter gene expression, several phenotypic changes were seen in the growth plate; i.e., decrease in volume, reduction in total area and height, decrease in the area ossified zones, mechanical weakening, reduction in mass of trabecular and cortical bone, lower bone density, deterioration of the trabecular and cortical microarchitecture, and trabeculae with lower collagen deposition. Conclusion: Dietary restriction had severe detrimental effects on the growth plate and trabecular and cortical bone.

ABSTRACT Objective: To compare tissue composition, total and regional bone mineral content and bone mineral density, static hand grip and knee joint isokinetic strength between amateur soccer players and Control Group. Methods: Cross-sectional study. Air displacement plethysmography was used to estimate body volume and, in turn, density. Body composition, bone mineral content and bone mineral density were assessed for the whole body and at standardized regions using dual energy X-ray absorptiometry. Static grip strength was assessed with an adjustable dynamometer, and peak torque derived from isokinetic strength dynamometer (concentric muscular knee actions at 60°/s). Magnitude of the differences between groups was examined using d-Cohen. Results: Compared to healthy active adults, soccer players showed larger values of whole body bone mineral content (+651g; d=1.60; p<0.01). In addition, differences between groups were large for whole body bone mineral density (d=1.20 to 1.90; p<0.01): lumbar spine, i.e. L1-L4 (+19.4%), upper limbs (+8.6%) and lower limbs (+16.8%). Soccer players attained larger mean values in strength test given by static hand grip protocol (+5.6kg, d=0.99; p<0.01). Conclusion: Soccer adequately regulates body composition and is associated better bone health parameters (bone mineral content and density at whole-body and at particular sites exposed to mechanical loadings).


RESUMO Objetivo: Comparar a composição de tecidos, o conteúdo mineral ósseo e a densidade mineral óssea totais e por regiões do corpo, a força de preensão manual estática, e força isocinética da articulação do joelho, entre um grupo de jogadores de futebol amadores e um Grupo Controle. Métodos: Estudo transversal utilizando pletismografia de ar deslocado para estimar o volume corporal, para subsequente cálculo da densidade corporal. A composição de tecidos, o conteúdo mineral ósseo e a densidade mineral óssea foram avaliados para o corpo todo e regiões padronizadas através da absorciometria de raios-X de dupla energia. A força de preensão manual estática foi avaliada por um dinamômetro ajustável. Os momentos máximos de força das ações musculares concêntricas para os extensores e flexores do joelho foram avaliados pela dinamometria isocinética (60°/s). Foi calculado o valor d-Cohen para apreciar a magnitude do efeito das diferenças entre grupos. Resultados: Os futebolistas apresentaram níveis superiores de conteúdo mineral ósseo em comparação com os adultos ativos do Grupo Controle (+651g; d=1,60; p<0,01) e obtiveram valores superiores de densidade mineral óssea (d: 1,20 a 1,90; p<0,01) para a coluna lombar, L1-L4 (+19,4%), membros superiores (+8,6%) e membros inferiores (+16,8%). Para a força de preensão (estática) a diferença foi moderada (d=0,99; p<0,01) com valores mais elevados apresentados pelo futebolistas (+5,6kg; d=0,99; p<0,01). Conclusão: A prática de futebol promove uma regulação adequada da composição corporal (tecidos magro e gordo) e ganhos na densidade mineral óssea, mais acentuada em partes do corpo com maior exposição aos impactos mecânicos da atividade motora.
