ABSTRACT

The COVID-19 pandemic has affected a large number of patients in all countries, overwhelming healthcare systems worldwide. In this scenario, surgical procedures became restricted, causing unacceptable delays in the treatment of certain pathologies, such as glioblastoma. Regarding this tumor with high morbidity and mortality, early surgical treatment is essential to increase the survival and quality of life of these patients. Association between COVID-19 and neurosurgical procedures is quite scarce in the literature, with a few reported cases. In the present study, we present a rare case of a patient undergoing surgical resection of glioblastoma with COVID-19.

ABSTRACT

RESUMEN A pesar de los avances en radioterapia, quimioterapia y los tratamientos de resección quirúrgica agresiva en el glioblastoma multiforme, el pronóstico sigue siendo sombrío. Con la presente revisión se describen, en un marco actual, las principales alternativas de tratamiento del glioblastoma multiforme. Se revisaron los principales artículos publicados en inglés, en revistas de alto impacto a nivel mundial, acerca de los principales avances en el tratamiento de este tumor. Se abordaron los importantes progresos neuroquirúrgicos en la resección del glioblastoma así como las implicaciones de las células madres tumorales en la génesis y control de la proliferación tumoral y el efecto de la hipoxia sobre la dinámica celular tumoral. Se explican las alteraciones del ADN que ocasionan tumorogénesis y las mutaciones del PTEN en el glioblastoma (AU).


SUMMARY Despite advances in radiotherapy, chemotherapy and aggressive surgical resection treatments in glioblastoma multiforme, the prognosis remains discouraging. With the current review, the main alternatives for the treatment of glioblastoma multiforme are described in a current context. The authors reviewed the main articles published in English, in high impact journals worldwide, on the main advances in the treatment of this tumor. The main neurosurgical advances in the resection of glioblastoma were addressed, as well as the implications of tumor stem cells in the genesis and control of tumor proliferation, as well as the effect of hypoxia on tumor cell dynamics. DNA alterations causing tumor genesis and PTEN mutations in glioblastoma are also explained (AU).

Introducción: El Glioblastoma (GB) o Astrocitoma grado IV (OMS), representan 15-20 por ciento de los tumores del SNC y aproximadamente 50 por ciento de los gliomas en adultos. Objetivo: Revelar el perfil epidemiológico del HSCMRP, correlacionar los hallazgos macroscópicos y microscópicos durante la cirugía de enero de 2011 a noviembre de 2015. Método: Estudio epimedeológico observacional, descriptivo, retrospectivo, 429 casos de tumores intracraneales a partir de los datos obtenidos de los archivos de La institución y los registros patológicos de los pacientes tratados quirúrgicamente. Resultados: Tumores encontrados 429 y 96 (22,37 por ciento) GB, edad media de 59 años, predominante séptima década 33 por ciento. Una relación entre mujeres y hombres fue de 1:1.12, respectivamente. Las quejas más frecuentes: dolor de cabeza (58 por ciento), confusión (41 por ciento), hemiparesia 37 por ciento. Comorbilidades frecuentes: hipertensión (64 por ciento), diabetes (22 por ciento) y fumadores (24 por ciento). La topografía más común fue la frente izquierdo. El tiempo medio de inicio de los síntomas a la cirugía fue de 39 días. Resección completa en 76 por ciento de los casos. La duración media de la recurrencia fue de 96 días, en 68% de los pacientes se observó una exuberancia de los vasos trombosados durante la cirugía Hallazgos patológicos: necrosis 98 por ciento, mitosis atípica 96 por ciento, proliferación microvascular 73 por ciento y polimorfismo nuclear 57 por ciento. Discusión: GB estado del arte. Conclusión: Nuestros resultados son similares con la literatura. Observación intraoperatoria de vasos trombosados y agresividad tumoral en pacientes con peor pronóstico y menor tiempo de recaída sugiere que es real, sin embargo. El pequeño número de casos, necesita más investigación, incluyendo otros hallazgos y resultados inmunohistoquímicos.


Introduction: Glioblastoma (GB) or Astrocytoma grade IV (WHO), represent 15-20 percent of CNS tumors and approximately 50 percent of gliomas in adults. Objective: Reveal the epidemiological profile of HSCMRP, correlate macroscopic and microscopic findings during surgery treated from January 2011 to November 2015. Method: Observational epidemiological study, descriptive, retrospective, of medical records of 429 cases of intracranial tumors from data obtained from the files of the institution and pathological records of patients treated surgically. Results: Total tumors found 429 and 96 (22.37 percent) GB with a mean age of 59 years, predominant seventh decade of life 33 percent. A relationship between women and men was with little difference 1:1.12, respectively. The most common complaints were headache (58 percent), confusion (41 percent), hemiparesis 37 percent. Most prevalent comorbidities: hypertension (64 percent) and diabetes (22 percent) and smokers (24 percent). Most common topography were followed by left front lesions. The average time of onset of symptoms to surgery was 39 days. Complete resection in 76 percent of cases. The mean length of postoperative recurrence was 96 days, in 68 percent patients were noticed an exuberance of thrombosed vessels during surgery. Pathological findings: necrosis 98%, atypical mitosis 96%, microvascular proliferation 73 percent and nuclear polymorphism 57 percent. Discussion: GB state of art. Conclusion: Our results are very slightly with the literature. The association of intraoperative observation thrombosed vessels, and tumor aggressiveness in patients with worse prognosis and shorter time to relapse, suggests that it is real, however, the small number of cases, needs further investigation, including other findings and immunohistochemical results.

ABSTRACT Objective To analyze cases of recurrent glioblastoma subjected to reoperation at a Brazilian public healthcare service. Methods A total of 39 patients subjected to reoperation for recurrent glioblastoma at the Department of Neurosurgery, São Paulo Hospital, Federal University of São Paulo, from January 2000 to December 2013 were retrospectively analyzed. Results The median overall survival was 20 months (95% confidence interval – CI = 14.9–25.2), and the median survival after reoperation was 9.1 months (95%CI: 2.8–15.4). The performance of adjuvant treatment after the first operation was the single factor associated with overall survival on multivariate analysis (relative risk – RR = 0.3; 95%CI = 0.2–0.7); p = 0.005). Conclusion The length of survival of patients subjected to reoperation for glioblastoma at a Brazilian public healthcare service was similar to the length reported in the literature. Reoperation should be considered as a therapeutic option for selected patients.


RESUMO Objetivo Analisar o papel da reoperação em pacientes com glioblastoma recidivado em um serviço público no Brasil. Métodos Foram analisados retrospectivamente 39 pacientes submetidos à reoperação por recorrência de glioblastoma no Departamento de Neurocirurgia da Universidade Federal de São Paulo, no período de janeiro de 2000 até dezembro de 2013. Resultados A sobrevida global mediana foi de 20 meses (IC 95% = 14.9–25.2), e a sobrevida mediana após a reoperação foi de 9.1 meses (IC 95% = 2.8–15.4). A realização de tratamento adjuvante após a primeira cirurgia foi o único fator associado com a sobrevida global numa análise multivariada (RR = 0.3; IC 95% = 0.2–0.7; p = 0.005). Conclusão A sobrevida dos pacientes submetidos à reoperação em um serviço público no Brasil é semelhante à reportada pela literatura. A reoperação deve ser considerada como uma opção terapêutica em pacientes selecionados.

Evaluar la influencia del uso de fluoresceína sódica (FLS-Na) en la cirugía del glioblastoma (GB) sobre el grado de resección tumoral y la supervivencia en pacientes atendidos en el Instituto Nacional de Enfermedades Neoplásicas. Materiales y métodos. Se revisó un total de 238 casos de GB atendidos entre los años 2008 y 2013 y se seleccionó 150 casos de GB sometidos a resección quirúrgica, con información clínico-patológica y seguimiento adecuado. Resultados. La media de edad fue 51 años, el 58,7% de casos presento Karnofsky de al menos 90. Se administró FLS-Na en 80 casos (53,3%) y se obtuvo una resección subtotal y total en 69 (46%) y 81 (54%) de los casos, respectivamente. El grupo que recibió FLS-Na obtuvo mayores tasas de resección total que el grupo operado solo con luz blanca (77,5 vs 27,1%, p<0,001). La mediana de sobrevida global (SG) fue mayor en el grupo sometido a resección total que a subtotal (17 vs 7 meses, p<0,001). La mediana de SG en los que recibieron FLS-Na fue mayor que en los que no la recibieron (15,0 vs 8 meses, p=0,003). Otros factores que afectaron la SG fueron la edad (p=0,002), el Karnofsky (p=0,052) y la administración de radioterapia (p=0,016) y quimioterapia (p=0,011). Conclusiones. To evaluate the influence of the use of sodium fluorescein (FLS-Na) in surgery of glioblastoma (GB) on the degree of tumor resection and survival in patients treated at the National Institute of Neoplastic Diseases. Materials and methods. A total of 238 cases of GB treated between 2008 and 2013 were reviewed and 150 cases of GB who underwent surgical resection with clinicopathological information and adequate follow-up were selected. Results. The mean age was 51 years, 58.7% of the cases presented a Karnofsky score of at least 90. FLS-Na was administered in 80 cases (53.3%) and a subtotal and total resection was obtained in 69 (46%) and 81 (54%) cases, respectively. The group that received FLS-Na obtained higher rates of total resection than the group operated with white light alone (77.5 vs 27.1%, p<0.001). The median overall survival (OS) was higher in the group subject to total compared to subtotal resection (17 vs 7 months, p<0.001). The median OS in those who received FLS-Na was higher than in those who did not (15.0 vs 8 months, p=0.003). Other factors affecting OS were age (p=0.002), the Karnofsky score (p=0.052) and radiation therapy (p=0.016) and chemotherapy (p=0.011). Conclusions. ABSTRACT

O glioblastoma multiforme (GBM) é o tumor primário mais comum do sistema nervoso central (SNC). Em virtude do envelhecimento populacional, entre outros fatores, sua ocorrência em faixas etárias avançadas tem se mostrado crescente nas últimas décadas. Os objetivos da ressecção do GBM incluem: alívio do efeito de massa, citorredução tumoral, diagnóstico histopatológico, controle dos sintomas e melhoria nas taxas de sobrevivência. Por muito tempo acreditou-se que a idade seria fundamental na decisão pelo tratamento, embora alguns autores afirmem que a extensão da ressecção cirúrgica constitui o principal fator determinante do prognóstico, sobretudo para pacientes que foram eleitos para radioterapia e quimioterapia. A ressecção de mais de 98% do volume tumoral é capaz de interferir na sobrevivência, e ressecções menores proporcionaram sobrevivência semelhante à dos pacientes não abordados. The glioblastoma multiforme (GBM) is the most common primary tumor of the central nervous system (CNS). Due to the aging population, among other factors, its occurrence in older age groups has shown increasing in recent decades. The goals of resection of GBM include: relieve mass effect, tumor debulking, histopathological diagnosis, symptom control and improvement in survival rates. For a long time it was believed that age would be crucial in deciding the treatment, although some authors state that the extent of surgical resection is the main determinant of prognosis, especially for patients who were elected to radiotherapy and chemotherapy. The resection of more than 98% of the tumor volume can interfere with the survival and minor resections provided similar to survival of patients not operated. ABSTRACT

Paciente do sexo masculino, 56 anos, apresentou-se para excisão de glioblastoma intracraniano multiforme. Após ser rotineiramente monitorado, o paciente foi pré-oxigenado. Anestesia e paralisia foram induzidas com propofol (200 mg), fentanil (50 µg) e vecurônio (9 mg). Laringoscopia direta com lâmina Macintosh número 3 revelou um cisto pedunculado de 2x2 cm, que surgia do lado direito da valécula e impedia a intubação endotraqueal. Enquanto o paciente permanecia anestesiado, consultamos rapidamente um otorrinolaringologista e o cisto foi aspirado por uma seringa com agulha de calibre 22G sob laringoscopia direta. Aspiramos 10 cc de líquido. Intubação traqueal foi feita em seguida sem intercorrências com sonda de 9,0 aramada e com balão. Uma opção para a intubação com fibra óptica pode ser a aspiração cuidadosa do cisto para facilitar a intubação.


A 56-year-old man presented himself for an intracranial glioblastoma multiforme excision. After being routinely monitored, he was preoxygenated. We induced anesthesia and paralysis with 200 mg propofol, 50 µg fentanyl and 9 mg vecuronium. Direct laryngoscopy with a Macintosh 3 blade revealed a 2x2 cm cyst, pedunculated, arising from the right side of the vallecula preventing the endotracheal intubation. While the patient remained anesthetized, we urgently consulted an otolaryngologist and aspirated the cyst with a 22-gauge needle and syringe under direct laryngoscopy. We aspirated 10 cc of liquid content. This was followed by an uneventful tracheal intubation with a 9.0 enforced spiral cuffed tube. An alternative to fiberoptic intubation may be careful cyst aspiration to facilitate the intubation.


Un paciente del sexo masculino, de 56 años, llegó para una resección de glioblastoma intracraneal multiforme. Posteriormente a la rutina de monitorización, el paciente fue pre-oxigenado. La anestesia y la parálisis se indujeron con propofol (200 mg), fentanilo (50 µg) y vecuronio (9 mg). La laringoscopia directa con lámina 3 Macintosh arrojó un quiste pedunculado de 2x2 cm que surgía al lado derecho de la valécula e impedía la intubación endotraqueal. Mientras el paciente permanecía anestesiado, consultamos rápidamente un otorrinolaringólogo y el quiste fue aspirado por una jeringa con una aguja calibre 22G bajo laringoscopia directa. Aspiramos 10 cc de líquido. La intubación traqueal se hizo enseguida sin intercurrencias con una sonda de 9,0 y un alambre en espiral y con balón. Una opción para la intubación con fibra óptica puede ser la aspiración cuidadosa del quiste para facilitar la intubación.

Introducción: el glioblastoma multiforme (GBM) representa la forma más agresiva de los gliomas. La sobrevida a cinco años con el tratamiento quirúrgico exclusivo es nula. El agregado radioterapia (RT) y quimioterapia (QT) prolongan la so-brevida. Objetivo: evaluar el impacto del tratamiento en la sobrevida de pacientes portadores de GBM.Pacientes y métodos: se incluyeron los pacientes con diagnóstico histológico de GBM asistidos en la Unidad de Neuro-Oncología del Hospital de Cínicas, de 1980 a 2000. Se recabaron datos patronímicos, clínicos, paraclínicos y evolutivos de las historias clínicas. Se calculó la sobrevida mediante el método de Kaplan Meier y Mantel Cox.Resultados: se estudiaron 65 pacientes con una edad mediana de 58 años (rango: 6-79). Recibieron cirugía 100%(completa en 22 pacientes, subtotal en 9, parcial en 32 y biopsia en 2). El 92% recibió RT posoperatoria y 25% QT. La mediana de sobrevida fue de 46,6 semanas y la sobrevida global al año fue de 28%. La probabilidad de sobrevida al año fue 0 en los pacientes que recibieron cirugía exclusiva; 0,23 en los que se agregó RT, y 0,62 en los tratados con QT (p=0,0001). El análisis multivariable identificó como variables independientes: el tratamiento multimodal incluyendo quimioterapia(p=0,003) y buen estado neurológico posoperatorio (p=0,007).Conclusiones: la mediana de sobrevida de esta serie fue similar a lo descrito en la literatura, existiendo una sobrevida mayor en los pacientes con buen estado neurológico tratadoscon cirugía seguida de RT y QT.


Introduction: glioblastoma multiforme (GBM) is the most agressive kind of gliomas. The 5-year survival rate for exclusive surgical treatment is null. Additional chemotherapy and radiotherapy increases survival.Objective: to evaluate the impact of treatment in the survival of patients suffering from GBM.Method: patients with a histological diagnosis of GBM who were seen at the Neuro-Oncology Unit of the Clínicas Hospital, from 1980 through 2000. Clinical, paraclinical and patronymic data was collected, the same as information on the evolution of medical records. Survival rates were calculated through the Kaplan Meier and Mantel Cox method. Results: 65 patients with an average age of 58 years old were studied, (range: 6-79). All of them underwent surgery (complete in 22 patients, subtotal in 9, partial in 32 and biopsy in 2 patients). Ninety two percent of patients were treated with radiotherapy and 25% with chemotherapy. Median survival was 46.6 weeks and global survival one year after was 28%. The probability for survival one year after was 0 in patients who were treated only with surgery; 0.23 in those who also received radiotherapy and 0.62 with chemotherapy QT (p=0,0001).Multivariable analysis identified the following independent variables: multimodal treatment including chemotherapy (p=0,003) and a good postoperative general condition (p=0,007). Conclusions: median survival in this series was similar to that described in literature, there existing a longer survival rate for patients with a good neurologic condition who are treated with radiotherapy and chemotherapy after surgery.


Introdução: o glioblastoma multiforme (GBM) é a forma mais agressiva dos gliomas. A sobrevida a os cinco anos é nula quando o paciente recebe exclusivamente tratamento cirúrgico. Observa-se um aumento da sobrevida quando ademais da cirurgia se agrega radioterapia (RT) e quimioterapia (QT).Objetivo: avaliar o impacto do tratamento na sobrevida de pacientes portadores de GBM. Pacientes e métodos: foram incluídos todos os pacientes com diagnóstico histológico de GBM atendidos na Unidade de Neuro-Oncologia do Hospital de Clínicas, de 1980 a 2000. Dos prontuários dos pacientes foram coletados os dados de identificação, clínicos, resultados de exames e estudos e os relacionados com a evo-lução do caso. A sobrevida foi calculada utilizando o método de Kaplan Meier e Mantel Cox.Resultados: foram estudados 65 pacientes com idade média de 58 anos (rango: 6-79 anos). Todos os pacientes (100%) foram submetidos à cirurgia (completa em 22 pacientes, subtotal em 9, parcial em 32 e biopsia em 2). Noventa e dois por centos (92%) receberam RT pós-operatória e 25% QT. A mediana de sobrevida foi de 46,6 semanas e a sobrevida global aos 12 meses foi de 28%. A probabilidade de sobrevida aos 12 meses foi zero nos pacientes que foram submetidos exclusivamente à cirurgia; 0,23 nos que se agregaram RT, e 0,62 nos tratados com QT (p=0,0001). A análise multivariada identificou como variáveis independentes: o tratamento multimodal incluindo quimioterapia (p=0,003) e bom estado neurológico pós-operatório (p=0,007).Conclusões: a mediana de sobrevida desta serie foi similar às descritas na literatura, existindo uma sobrevida maior nos pacientes com bom estado neurológico tratados com cirurgia seguida de RT e QT.

Objetivo: É apresentada neste trabalho a casuística cirúrgica de ressecção de gliomas em um hospitalescola quaternário da cidade de São Paulo, no período de 19 meses. Método: Foram coletados dados, prospectivamente, de todos os pacientes adultos com diagnóstico de glioma, submetidos à ressecção cirúrgica no hospital referido, no período de novembro de 2007 a junho de 2009, por meio de preenchimento de um protocolo pelo médico-assistente de cada caso. Resultados: No período estudado, foram realizadas 288 cirurgias para ressecção de tumores encefálicos. Dessas, 88 cirurgias foram destinadas à ressecção de gliomas do sistema nervoso central. Corresponde a 30,56% das cirurgias para ressecção de tumores encefálicos, sendo esse o maior grupo, com média anual de 55,5 gliomas operados. Conclusão: O predomínio dos astrocitomas de alto grau nos casos operados é semelhante aos dados da literatura, demonstrando a importância dessa doença. A gravidade e a considerável incidência desses tumores em nossa população mostram a necessidade de constante investimento e aprimoramento tecnológico e humano para o adequado tratamento desses pacientes.


Objective: We present in this work the gliomas surgery casuistic of a quaternary medical school hospital of São Paulo city during a 19 months period. Method: Data were collected, prospectively, of all patients with glioma operated at Hospital das Clínicas of São Paulo Medical School, in the period between November 2007 to June 2009. Results: In the studied period 288 surgeries were held for encephalic tumors resection. Of these, 88 surgeries were aimed at resection of central nervous system gliomas. That represents 30.56% of encephalic tumors surgery, the largest group with an annual average of 55.5 gliomas operated. Conclusion: The prevalence of high grade astrocytomas operated is similar to literature data, demonstrating the importance of this disease on the health of a community. The seriousness and considerable incidence of these tumors in our population show the necessity of constant investment in technological and human enhancement for the proper treatment of these patients.

OBJETIVO: Identificar fatores prognósticos e avaliar a evolução clínica de pacientes com diagnósticode glioblastoma submetidos a cirurgia e radioterapia, com ou sem quimioterapia adjuvante. MATERIAL E MÉTODO: trabalho retrospectivo com 48 pacientes portadores de glioblastoma tratadosno período de 1997 a 2007. Todos os pacientes foram classificados segundo critérios do recursive partitioning analysis (RPA). RESULTADOS: Observaram-se predominância do sexo feminino, idade maior ou igual a 50 anos, performance status maior ou igual a 70, e as classes mais prevalentes,de acordo com a classificação RPA, foram a V e VI. Em 72,9% dos pacientes foi realizada ressecção parcial da lesão e em 27,1%, ressecção subtotal ou total. Quimioterapia foi administrada em47,9% dos pacientes e a dose de radioterapia foi de 50û60 Gy em 72,9% dos casos. A sobrevida global mediana observada foi de 52 semanas. CONCLUSÃO: Os dados obtidos mostram que a sobrevida global de pacientes portadores de glioblastoma foi semelhante aos resultados encontrados na literatura e dependente de fatores como a adição de quimioterapia, dose de radioterapia eíndice de Karnofsky.


OBJECTIVE: To identify prognostic factors and evaluate the clinicaloutcome of patients with glioblastoma treated with surgery and radiotherapy combined or not with chemotherapy. MATERIAL AND METHOD: In this retrospective study, 48 patients with glioblastoma were treated between 1997 and 2007. All patients wereclassified according the recursive partitioning analysis (RPA) criteria.RESULTS: The majority of patients were female, with 50 years of age or above. Performance status of 70 or greater were found in 70.8% of cases, and RPA classes V and VI prevailed. Seventy-two percent of patients were submitted to partial resection and 27.1% to total or subtotal resection. Chemotherapy wasadministered in 47.9% of patients and doses between 50 and 60 Gy were delivered in 72.9%. The median overall survival was 52 weeks. CONCLUSION: Our data show an overall survival that approaches the related in others reports and were dependent of factors such as chemotherapy, dose of radiation and Karnofsky performance status.

ABSTRACT

Glioblastoma multiforme [GBM] is a highly malignant glial tumor seen commonly in the cerebral hemispheres, but rarely encountered in the cerebellum. It may occur at any age, but is seen more often in adult age groups. Despite its rarity, GBM should be considered in patients with a ring-enhancing lesion in the cerebellum. No consensus regarding the best management has yet been established. However, multimodal treatment is currently available to deal with these lesions: wide excision with radiochemotherapy may improve and prolong the patient's life. Although the outcome remains dismal, we emphasize that timely multi modal treatment may provide the patient a better outcome and longer life. Herein, we report 2 new cases of cerebellar GBM and discuss their outcome and present a review of the relevant literature

ABSTRACT

Brain stem gliomas are a heterogeneous group of neoplasms arising mostly in paediatric patients. Tectal plate gliomas represent a particular type of brain stem tumours usually with a benign, indolent clinical course, presenting with signs of raised intracranial hipertension due to supra-tentorialhydrocephalous caused by aqueductal stenosis. Seldom high-grade lesions arise in this location with tremendous therapeutic implications. When a malignant tumour is clinically and radiographically suspected a biopsy should be performed to obtain histhological confirmation. Treatment is then planned in a case-by-case basis. We present the case of a glioblastoma of the tectal plate in a 22 years-old woman operated upon by a supracerebellar-infratentorial approach.


Os gliomas do tronco cerebral são um grupo heterogêneo de neoplasias que acometem habitualmente crianças. Os gliomas da placa quadrigeminal representam um tipo particular de tumores do tronco cerebral, habitualmente com um curso benigno e indolente, surgindo com sinais de hipertensão intracraniana devido a hidrocefalia supra-tentorial provocada por compressão do aqueduto cerebral. Raramente surgem lesões de alto grau nesta região, mas as implicações terapêuticas são tremendas. Quando existe suspeita clínica e imagiológica de que se trata de lesão maligna, esta deve ser biopsada para se obter confirmação histológica. O tratamento deve então ser planejado caso a caso. Apresentamos o caso de glioblastoma da placa quadrigeminal em uma paciente de 22 anos intervencionado por via supracerebelar-infratentorial.

ABSTRACT

Glioblastoma multiforme (GBM) cerebelar é um tumor raro. Apresentamos o terceiro caso revisado em literatura neurológica brasileira, sendo o último descrito há mais de 15 anos. Devido a sua agressividade, o tratamento deve ser instituído rapidamente, porém certa indefinição quanto à conduta pode ocorrer, pois o diagnóstico de GBM pode não ser lembrado. Apresentamos um caso de GBM cerebelar em um homem de 46 anos. Também fazemos uma revisão a respeito de seu comportamento, quadro clínico e avanços quanto à investigação por imagem, aspectos histopatológicos, formas de tratamento e suas características peculiares.

ABSTRACT

O glioblastoma, um tumor anaplásico de linhagem astrocitária, é o mais freqüente tumor cerebral. Localiza-se preferencialmente nos hemisférios cerebrais; seu crescimento primário no cone medular é muito raro, e o manejo e prognóstico deste tipo de lesão são distintos dos outros tumores observados nesta localização. Apresentamos o caso de um homem de 39 anos com tumor intramedular com diagnóstico histo-patológico de glioblastoma.

ABSTRACT

Los glioblastomas multiformes (OMS grado IV) son las neoplasias más frecuentes y malignas del sistema nervioso central, una variante histológica de éstos, denominada, de células gigantes representa aproximadamente 1% de todos los tumores cerebrales y 5% de todos los glioblastomas. Presentamos el caso de un paciente de sexo femenino a quien se le diagnosticó y trató un glioblastoma de células gigantes de localización intra y paraventricular derecho. Se enfatiza la importancia de las características histológicas de este tumor en relación a su pronóstico.


Glioblastomas (WorldHealth Organization, (WHO), grade IV) are the most frequent and malignant neoplasms of the human nervous system, Giant cells glioblastomas, a subtype of these, account for less than 1% of all brain toumors and up to 5% of glioblastomas. We present the case of a female who was diagnosed and treated for a right intra and paraventricular giant cell glioblastoma. We enfatize the importance of histological features of this toumor related to its prognosis.
