ABSTRACT

Abstract Arthroscopy is a surgical technique whose indication for wrist injuries has grown in recent years. Athletes are subject to traumatic injury to the wrist due to training overload or the intensity of the activity during competition. The need of a quick return to sports practice makes arthroscopy a very useful minimally invasive technique in these situations. The authors present indications of sports-related injuries to the wrist that can be treated by arthroscopy. A literature review is also presented.


Resumo A artroscopia é uma técnica cirúrgica que tem sido cada vez mais usada para a abordagem de lesões no punho. Atletas estão sujeitos a lesões traumáticas no punho devido à sobrecarga de treinamento ou à intensidade da atividade em competição. A necessidade de retornar o mais breve possível à pratica esportiva faz da artroscopia uma técnica minimamente invasiva muito útil nessas situações. Os autores apresentam as principais indicações de tratamento de lesões esportivas por artroscopia. Foi feita uma revisão da literatura.

ABSTRACT

INTRODUCCIÓN: la artrosis de la articulación basal del pulgar es la más común de las enfermedades degenerativas articulares de la mano. La trapezoidectomía con o sin interposición y con o sin ligamentoplastia, es una de las opciones de tratamiento quirúrgico. OBJETIVO: mostrar los resultados obtenidos con la técnica de trapezoidectomía parcial y artroplastia por interposición, así como caracterizar algunas variables que pueden estar en relación con esta enfermedad. MÉTODOS: estudio de intervención longitudinal prospectivo con pacientes diagnosticados e intervenidos por rizoartrosis de la mano, entre enero de 2009 y enero de 2013 y valorados un año después. La muestra quedó constituida por 45 pacientes, 29 mujeres y 16 hombres. RESULTADOS: existió predominio del sexo femenino y del grupo de edades entre 51 y 60 años, mayoritariamente en pacientes que desempeñaban labores de tipo manual, con estadios de degeneración avanzados y afectación de la mano derecha dominante. Ocurrieron pocas complicaciones menores y significativos cambios en la percepción del dolor, la oposición del pulgar y en el nivel de discapacidad presente antes de la intervención. CONCLUSIONES: la trapezoidectomía parcial con artroplastia interposicional tendinosa resulto altamente efectiva en el tratamiento de la artrosis carpometacarpiana del pulgar demostrado a través del índice QuickDASH.


INTRODUCTION: Osteoarthritis of the basal joint of the thumb is the most common degenerative joint disease of the hand. Trapezoidectomy, with or without interposition and with or without ligamentoplasty, is one of the surgical treatment options. OBJECTIVE: Show the results obtained with the technique of partial trapezoidectomy and interposition arthroplasty and characterize some variables that may be related to this disease. METHODS: A prospective longitudinal intervention study was carried out with patients diagnosed and treated for hand rizoarthrosis from January 2009 to January 2013, and they were assessed a year later. The sample was composed of 45 patients: 29 women and 16 men. RESULTS: There was predominance of females, and the age group between 51 and 60 years, mostly in patients who performed manual labor with advanced stages of degeneration and disruption of the dominant right hand. A few minor complications and significant changes occurred in the perception of pain, thumb opposition and the present level of disability before surgery. CONCLUSIONS: partial tendon interpositional trapezoidectomy with arthroplasty resulted highly effective in tretaing osteoarthritis of the thumb carpometacarpal shown through QuickDASH index.

Phosphoric acid has been suggested as an irrigant due to its effectiveness in removing the smear layer. Objectives : The purpose of this study was to compare the antimicrobial and cytotoxic effects of a 37% phosphoric acid solution to other irrigants commonly used in endodontics. Material and Methods : The substances 37% phosphoric acid, 17% EDTA, 10% citric acid, 2% chlorhexidine (solution and gel), and 5.25% NaOCl were evaluated. The antimicrobial activity was tested against Candida albicans, Staphylococcus aureus, Enterococcus faecalis, Escherichia coli, Actinomyces meyeri, Parvimonas micra, Porphyromonas gingivalis, and Prevotella nigrescens according to the agar diffusion method. The cytotoxicity of the irrigants was determined by using the MTT assay. Results : Phosphoric acid presented higher antimicrobial activity compared to the other tested irrigants. With regard to the cell viability, this solution showed results similar to those with 5.25% NaOCl and 2% chlorhexidine (gel and solution), whereas 17% EDTA and 10% citric acid showed higher cell viability compared to other irrigants. Conclusion : Phosphoric acid demonstrated higher antimicrobial activity and cytotoxicity similar to that of 5.25% NaOCl and 2% chlorhexidine (gel and solution). .

ABSTRACT

O trauma da mão é uma das lesões mais frequentes nas emergências. O trauma complexo envolve lesões de múltiplas estruturas e tecidos da mão. O objetivo principal deste trabalho é esclarecer os conceitos envolvidos no estudo do paciente com trauma complexo da mão, mais especificamente, na avaliação inicial e manejo primário da mão traumatizada. Procuramos orientar a conduta dos médicos nas diversas situações clínico-cirúrgicas, que envolvem esta patologia, objetivando um julgamento rápido e adequado, interferindo positivamente no desenvolvimento natural da doença, como também nas sequelas decorrentes ao traumatismo. Nesta primeira parte, são abordados os fundamentos e conceitos considerados essenciais na abordagem do traumatismo da mão, lesão vascular, lesão nervosa e lesão tendínea, enfatizando os benefícios de uma conduta multifatorial e assertiva, abarcando as diversas disciplinas envolvidas no atendimento primário do paciente vítima de trauma (AU)


Trauma of the hand is one of the most frequent injuries in emergencies. Complex trauma involves injuries to multiple tissues and structures of the hand. The main purpose of this work is to clarify the concepts involved in the study of patients with complex trauma of the hand, more specifically, the initial assessment and primary management of the traumatized hand. We seek to guide the conduct of physicians in different clinical and surgical situations involving this pathology. Aiming for a quick and proper judgment, interfering positively with the natural development of the disease and also with sequelae resulting from trauma. In this first part the article discusses the fundamentals and concepts considered essential in the management of hand trauma, vascular injury, nerve injury and tendon injury, emphasizing the benefits of a multifactorial and assertive approach, covering the various disciplines involved in primary care of the trauma patient (AU)

ABSTRACT

Las lesiones ocacionadas por artefactos pirotécnicos y/o explosivos son altamente incapacitantes, constituyendo un problema de salud pública. Se estudiaron 25 paciente, 96 por ciento del sexo masculino, el grupo etario mas afectado estuvo entre 16 y 25 años con 40 por ciento. 92 por ciento eran dextromanos: la mano izquierda se lesionó en un 72 por ciento. Las lesiones más frecuentes fueron las amputaciones parciales en el 19,23 por ciento y las heridas del espacio interdigital en 17,95 por ciento; el pulgar fue afectado en el 96 por ciento. La influencia del alcohol estuvo presente en el 80 por ciento, siendo estos procedencia ilegal en el 84 por ciento.

ABSTRACT

Date palm thorn injuries are common in Middle Eastern countries where there are many date palm plantations. Most of the injuries happen to workers in the plantation or to children. Injuries, if detected, can be treated without subsequent complications, but in children the diagnosis can very easily be missed resulting in late complications. The hand, being the most exposed part of the body, is the most affected. Embedded thorns can produce lesions mimicking those of osteomyelitis. The foot is exposed to injuries in people who walk barefoot in the plantations. The author presents five cases of date thorn injury which presented with late complications. All three patients with hand injuries had periostitis, and one of them had an intraosseous thorn producing osteomyelitis. Two patients presented with osteolytic lesions of the metatarsals with intraosseous thorn in one patient. All cases recovered completely leaving behind no sequlae of the bony infection

Se presenta un paciente que acude al cuerpo de guardia de Ortopedia, por haber sufrió un trauma en la mano derecha cuando realizaba labores agrícolas. Se diagnóstico sinostosis total del carpo fusionada con el segundo y tercer metacarpianos, unilateral. El diagnóstico fue casual. Se realizó una revisión de la literatura científica sobre esta entidad y no se encontró caso similar


This paper presented a patient who went to the Emergency Orthopedic Service for a trauma in his right hand that occurred while he was working in farming. The diagnosis was total unilateral synostosis of the carpus fused with the second and third metacarpals. The diagnosis was made by chance. A scientific literature review was made but no other similar case was found


Un patient allant au Service des urgences en orthopédie dû à un traumatisme de la main droite est présenté. Une synostose totale du carpe par soudure unilatérale du deuxième et le troisième métacarpien est diagnostiqué. Le diagnostic est fortuit. Une revue bibliographique sur cette entité a été faite, mais on n'a pas trouvé un cas pareil

ABSTRACT

Injuries caused by venomous and poisonous aquatic animals may provoke important morbidity in humans. The phylum Echinoderma include more than 6000 species of starfish, sea urchins, sand dollars, and sea cucumbers some of which have been found responsible for injuries to humans. Initial injuries by sea urchins are associated with trauma and envenomation, but later effects can be observed. Sea urchin granuloma is a chronic granulomatous skin disease caused by frequent and successive penetration of sea urchin spines which have not been removed from wounds. The authors report a typical case of sea urchin granuloma in a fisherman and its therapeutic implications.


Os acidentes por animais aquáticos traumatizantes e venenosos podem provocar morbidez importante em humanos. Equinodermos marinhos incluem mais de 6000 espécies de estrelas-do-mar, ouriços-do-mar, "bolachas-de-praia" e pepinos-do-mar. Vários equinodermos têm sido responsabilizados por acidentes em humanos. Granulomas por ouriço-do-mar são lesões de caráter granulomatoso, crônicas, causada por acidentes com espículas de ouriço-do-mar. Os autores relatam um caso típico de granulomas por ouriço-do-mar ocorrido em um pescador e enfatizam as implicações terapêuticas aplicadas.

ABSTRACT

This paper aims to study the author's experience with the open palm technique originally described by McCash for the treatment of serve Dupuytren's contractures. All patients presented hand ulnar border (ring and little fingers) impairment and belonged to stages III to IV of the classification proposed by Tubiana. In a period of 6 years, 35 patients were submitted to regional fasciectomy. On average, 18 days were necessary (extremes of 15 and 25) to complete scar healing. We obtained an average articular residual deficit of 25 to the ring finger and 30 to the little finger at the proximal interphalangeal articulation level.

ABSTRACT

La artritis reumatoide es una enfermedad que puede afectar a todas las articulaciones y tendones de mano y muñeca, lo que repercute en las actividades cotidianas del paciente. Los tendones frecuentemente afectados son el extensor propio y común del meñique, el extensor del anular, el extensor común del medio. El extensor propio del índice es el que se lesiona con menos frecuencia. Fueron estudiados 70 pacientes con artritis reumatoide en el Hospital ABC de 1971 a 1999; 13 de éstos presentaban ruptura de tendones extensores de los dedos a nivel de la muñeca. A todos se les realizó tenosinovectomía, sinovectomía de muñeca y exéresis del extremo distal del cúbito, así como sutura laterolateral del extremo proximal y distal de los tendones del meñique al extensor común del anular en ocho casos, sutura de los extremos proximal y distal del común del meñique y común del anular al tendón común del medio y reforzado con el tendón del extensor propio del índice en cuatro casos e injerto del palmar menor a los tendones extensor común al medio, anular y meñique en un caso. En todos se obtuvieron buenos resultados.

ABSTRACT

Se hizo un estudio prospectivo de los casos tratados con un colgajo vascular de la cara lateral del brazo para reconstruir la mano después de lesiones severas de la misma. Los pacientes fueron todos los registrados en el año 1991. Fueron 8 casos, 5 varones y 3 mujeres, con edades entre 13 y 55 y promedio de 28. Se hicieron 3 en la etapa temprana (primera sena) y 5 en la etapa tardía. Las lesiones fueron por machacamiento en 2, por proyectil de arma de fuego en 2 y respectivamente un caso de quemadura, accidente vial, explosión de cohete y secuelas de otra reconstrucción fallida de la mano. El colgajo puede contener todos los planos deseados, es decir piel, panículo adiposo, fascia, músculo y hueso. La superficie cubierta varió de 38.5 a 88 cm² con promedio de 50. Los resultados fueron excelentes en 2 casos, buenos en 5 y regular en uno

ABSTRACT

Entre 1961 y 1992 se realizó un estudio retrospectivo revisándose 367 historias clínicas que reportaron 450 fracturas de los dedos de la mano atendidas en la emergencia del Hospital Militar que ameritaron tratamiento hospitalario. Se encontró que las edades estaban comprendidas entre 3 a 45 años con un promedio de 28,62 por ciento, siendo el hombre más afectado que la mujer en una relación de 50 a 1. Las fracturas abiertas representaron un 70,78 por ciento. El trauma directo y las heridas por armas de fuego fueron las causas más importantes. Las lesiones en piel representaron un 13,35 por ciento de las asociadas a la fractura. El tratamiento realizado fue conservador 82,44 por ciento y quirúrgico 17,56 por ciento de acuerdo a las características propias de cada fractura. Las complicaciones fueron observadas en su mayoría en los pacientes tratados conservadoramente

A fratura de Bennett é uma lesäo comum e de tratamento variado. Os autores relatam um caso de fratura de Bennett em que houve associaçäo à fratura do trapézio e chamam a atençäo para esse tipo de lesäo, que é raro e aumenta a incidência de maus resultados.
