ABSTRACT

Introdução: o cisto ósseo traumático (COT) é um pseudocisto que se apresenta assintomático e é descoberto frequentemente em exames de rotina. Outra lesão também presente nos maxilares é o odontoma, sendo dividido em dois subtipos, o composto e o complexo; os odontomas são geralmente descobertos como um achado acidental, visto que não apresentam sintomatologia. Objetivo: relatar um caso incomum de um COT, associado à odontoma composto (OC). Relato de caso: paciente do gênero masculino, 16 anos de idade, compareceu à clínica escola de odontologia da Universidade Federal de Campina Grande (UFCG), campus Patos, PB, referenciado pelo cirurgião-dentista após solicitar exame radiográfico para tratamento ortodôntico e observar lesão radiolúcida em região anterior da mandíbula. Durante a anamnese, o paciente não relatou nenhuma alteração sistêmica ou doença de base, mas relatou trauma de infância na região acometida. No exame clínico intraoral, não foi observado nenhum aumento de volume na região. Realizou-se palpação na região, não havendo relato de dor. Ao analisar a radiografia panorâmica, observou-se a presença de pequenas estruturas calcificadas com radiopacidade semelhante às estruturas dentárias, delimitada por uma linha radiolúcida, sugestiva de OC. Para melhor localização, delimitação, relação com estruturas anatômicas e planejamento cirúrgico da lesão, foi solicitado um exame de tomografia computadorizada de feixe cônico (TCFC). Considerações finais: com base nos achados clínicos e radiográficos, optou-se por abordagem cirúrgica da lesão cística e enucleação do OC, sob anestesia local. No pós-operatório de um ano, o paciente evoluiu satisfatoriamente sem queixas clínicas.(AU)


Introduction: traumatic bone cyst (TBC) is a pseudocyst that usually presents asymptomatically and is found frequently in routine exams. Another lesion also present in the jaws is odontoma. The odontoma is divided into two subtypes, the compound and the complex; odontomas are usually discovered as an accidental finding, since they do not present symptomatology. Objective: the present article aims to report an unusual case of a TBC associated with a composite odontoma. Case report: a 16-year-old male patient attended the Clinic School of Dentistry of the Universidade Federal de Campina Grande (UFCG), Patos-PB campus, referenced by the dentist after identify radiolucent lesion in the anterior region of the mandible on radiographic examination for orthodontic treatment. During the anamnesis, the patient did not report any systemic alteration or underlying disease, but reported trauma in the region affected in childhood. The intra oral clinical examination, was not observed any increase in volume in the region. Palpation was performed in the region, and there was no report of pain. When analyzing panoramic radiography the presence of small calcified structures with radiopacity similar to dental structures was observed, delimited by a radiolucent line, suggestive of compound odontoma. To better location, delimitation, compared with anatomy and surgical planning of the injury, it was requested an cone beam computed tomography (CBCT). Final considerations: based on the clinical and radiographic findings, we opted for a surgical approach to cystic lesion and enucleation of composite odontoma, under local anesthesia. In the one-year postoperative period, the patient progresses satisfactorily without clinical complaints.(AU)

ABSTRACT

Objetivo: apresentar um caso clínico de paciente com cisto ósseo solitário em mandíbula e sua evolução após o tratamento de exploração e curetagem cirúrgica por meio de achados radiográficos. Caso clínico: paciente do sexo masculino, 45 anos, compareceu à clínica da Faculdade de Estudos Administrativos ­ Fead. Ao exame clínico, os tecidos se apresentavam dentro dos padrões de normalidade e ao exame radiográfico observou-se área radiolúcida unilocular, limites regulares e bem definidos em região anterior de mandíbula entre ápices dos dentes 33, 32, 31, 41 e 42, sem deslocamento desses. Para confirmação do diagnóstico, além do exame clínico, foi necessário realizar a tomografia de feixe cônico e exploração cirúrgica. Após a exploração cirúrgica foi realizada a curetagem das paredes ósseas e a proservação do caso para acompanhar a sua evolução. Considerações finais: por ser uma lesão de comportamento não agressivo, responde bem a este tipo de tratamento, sendo notável sua melhora durante o período de acompanhamento.(AU)


Objective: To present a clinical case report of a patient with solitary bone cyst of the mandible and its evolution after exploration and surgical curettage treatment through radiographic findings. Case report: A 45-year-old male patient attended the clinic of the School of Administrative Studies - FEAD. The clinical examination showed normal tissues and the radiographic examination showed unilocular radiolucent area and regular and well-defined boundaries in the anterior mandible between the apexes of teeth 33, 32, 31, 41, and 42, without displacement. To confirm the diagnosis, in addition to the clinical examination, cone beam tomography and surgical exploration were required. After the surgical exploration, bone wall curettage and case proservation were performed to monitor the evolution. Final considerations: Considering it is a non-aggressive lesion, it responds well to this type of treatment, with considerable improvement during follow-up.(AU)

ABSTRACT

Podemos definir "quiste" como una bolsa conectivoepitelial, tapizada en su interior por epitelio y recubierta en su cara externa por tejido conectivo, que encierra un contenido líquido o semisólido. Según su patogénesis, los clasificamos como lesiones de desarrollo o de tipo inflamatorio (1, 17). Tanto los quistes de desarrollo como los inflamatorios se caracterizan por un crecimiento lento y una tendencia expansiva. A pesar de ser entidades con un comportamiento benigno, pueden alcanzar un tamaño considerable si no se diagnostican y se tratan a tiempo (2). El tipo de tratamiento de la lesión está condicionado por múltiples factores como el tamaño de la misma, su localización, su relación con estructuras anatómicas vecinas, la posible afectación de estructuras dentales, entre otras. (3) El objetivo es elegir la modalidad de tratamiento que conlleve el menor riesgo posible de recurrencia, el cuidado, en lo posible, de las estructuras nobles vecinas y, por supuesto, la erradicación de la lesión. (4) Dentro de los tratamientos de los quistes maxilares, podemos mencionar la enucleación total de los mismos o técnica de Parstch II, la marsupialización o técnica de Parstch I y la técnica descompresiva, también llamada técnica de Waldron- Axhausen. El objetivo de este trabajo es el de mostrar un caso clínico donde se realizó la técnica descompresiva para el tratamiento de un quiste inflamatorio en el maxilar inferior en íntimo contacto con el nervio dentario inferior y la basal mandibular (AU)


We can define "cyst" like a connective-epitelial bag, upholstered inside by epithelium and covered on the outside by connective tissue, enclosing a liquid or semisolid contents. According to its pathogenesis, classify them as development or inflammatory (1, 17). Injury both the inflammatory and developing cysts are characterized by slow growth and an expansionary trend. Despite being entities with a benign behavior, they can reach a considerable size if not diagnosed and treated in time. (2) The type of treatment of injury, is determined by multiple factors such as the size, its location, its relationship with neighboring anatomical structures, possible involvement of dental structures, among others. (3) The objective is to choose the modality of treatment involving the least possible risk of recurrence, care, as far as possible, of the neighboring noble structures and of course the elimination of the lesion. (4) Inside of the Maxillary cysts treatments we can mention the enucleation total thereof or technical Parstch II, the marsupialization or technique of Parstch I and the decompression technique, also called Waldron - Axhausen technique. The objective of this work is the show a clinical case where took place the decompression technique for the treatment of inflammatory cyst in the lower jaw in intimate contact with the inferior alveolar nerve and the mandibular basal (AU)

ABSTRACT

Objetivo: proteger y preservar las estructuras adyacentes en el tratamiento de quistes de gran tamaño. Caso clínico: en este artículo se compartirán dos casos clínicos de lesiones quísticas de pacientes que han concurrido al servicio de Cirugía Bucomaxilofacial del Hospital Interzonal de Agudos San Roque de Manuel B. Gonnet. Conclusión: se debe confeccionar un correcto diagnóstico y elaborar un plan de tratamiento oportuno que permita le eliminación de la lesión y la neoformación ósea.

ABSTRACT

Aneurysmal bone cysts are rare benign lesions of bone tissue, infrequent in the craniofacial skeleton about other structures like long bones or the spine. The rare jaw lesions encountered in the mandible and maxilla. On the other side, it is at the same time very exciting in terms of its differential diagnosis with other types of mandibular or maxillary bone lesions. We present the case of a 45-year-old female patient with an aneurysmatic cyst located at the left angle of the mandible describing the treatment for the same i.e. surgical excision and hemi mandibulectomy with reconstruction. We have focused on the differential diagnosis, mainly with the malignancies, which can be found at this location, along with therapeutic options classically described for these kinds of pathologies. In our patient, the surgical excision and hemi mandibulectomy allowed a complete removal of the lesion and recon plate was placed for rehabilitation.

ABSTRACT

Introduction: Bone grafts are frequently used in the treatment of bone defects. Bone harvesting can cause postoperative complications and sometimes does not provide a sufficient quantity of bone. Therefore, synthetic biomaterials have been investigated as an alternative to autogenous bone grafts. Aim and Objectives: The aim of this study was to evaluate and compare bovine derived hydroxyapatite (BHA) and synthetic hydroxyapatite (SHA) graft material as bone graft substitute in maxillary cystic bony defects. Patients were analyzed by computerized densitometric study and digital radiography. Materials and Methods: In this study, 12 patients in each group were included randomly after clinical and radiological evaluation. The integration of hydroxyapatite was assessed with mean bone density, surgical site margin, and radiological bone formation characteristics, of the successful graft cases using computer densitometry and radio-visiograph. Statistical analysis was carried out using Mann-Whitney U-test, Wilcoxon matched pairs test and paired t-test. Results: By the end of 24 th week, the grafted defects radiologically and statistically showed similar volumes of bone formation. However, the significant changes observed in the formation of bone and merging of material and surgical site margin at 1 st week to 1 st month. The results were significant and correlating with all the parameters showing the necessity of the grafting for early bone formation. However, the bone formation pattern is different in both BHA and SHA group at 3 rd month interval with significant P value. Conclusion: Both BHA and SHA graft materials are biocompatible for filling bone defects, showing less resorption and enhanced bone formation with similar efficacy. Our study showed maximum bone healing within 12 weeks of grafting of defects. The BHA is economical; however, price difference between the two is very nominal.

Se realiza una revisión bibliográfica sobre quistes inflamatorios de los maxilares y se presenta una situación clínica de udna paciente niña de 9 años de edad que concurre a la guardia del Hospital de Odontología Infantil Don Benito Quinquela Martín, con quistes inflamatorios bilaterales (cuyas biopsias confirmaron el diagnóstico inicial) y desplazamiento de los gérmenes de segundos premolares inferiores derecho e izquierdo (3.5 y 4.5) hacia la basal. Considerando la edad de la paciente y el pronóstico favorable de las piezas dentarias en desarrollo, se decidió utilizar como tratamiento la técnica de Waldron-Axhausen o técnica descompresiva. La evolución de la paciente fue favorable, ya que se logró la remisión del proceso patológico y la normal erupción de dichas piezas dentarias...

ABSTRACT

O cisto nasolabial é um cisto de desenvolvimento não odontogênico raro, que acomete os tecidos moles entre a asa e base do nariz e o lábio superior podendo levar a assimetria facial. Sua patogênese é ainda muito discutida, no entanto, seu diagnóstico é clínico confirmado pelo exame anatomo-patológico. O tratamento clássico é a enucleação da lesão. O objetivo deste artigo é descrever as características, o diagnóstico e o tratamento do cisto nasolabial e relatar um caso clínico.


Nasolabial cyst is arare non-odontogenic development cyst, which affects soft-tissues between the ala and the base of the nose and the upper lip and may lead to facial asymmetry. Its pathogenesis is still controverse, although clinical diagnosis is confirmed by the anatomo-pathological examination. The treatment is enucleation of the lesion. The aim of this article is to describe the characteristics, diagnosis and the treatment of nasolabial cysts and report a clinical case.

ABSTRACT

O cisto ósseo traumático é uma entidade patológica caracterizada pela presença de uma cavidade óssea assintomática desprovida de revestimento epitelial, sendo raramente encontrado nos maxilares. OBJETIVO: Descrever as características clínico-cirúrgicas e radiográficas dos cistos ósseos traumáticos. MATERIAL E MÉTODO: Estudo de caráter retrospectivo dos pacientes diagnosticados com cisto ósseo traumático em um serviço de patologia oral no período de 1992 a 2007. Informações referentes às características clínicas, radiográficas e cirúrgicas foram coletadas. RESULTADOS: Vinte e seis casos de cisto ósseo traumático foram diagnosticados no período de 15 anos, 17 pertencentes ao sexo masculino e 09 ao sexo feminino. A maioria dos pacientes afetados pertencia às duas primeiras décadas de vida, não relatava sintomatologia dolorosa, bem como história de trauma na região da lesão. O padrão multilocular foi observado em apenas sete casos, dando às lesões uma aparência radiográfica tumoral. A presença de ar no interior da cavidade patológica foi relatada em aproximadamente 70% dos casos, sendo rara a presença de conteúdo serossanguíneo e seroso. CONCLUSÃO: A maior prevalência de casos em pacientes jovens, a infrequente história de trauma e o pequeno número de lesões com conteúdo serossanguíneo refletem a necessidade de se discutir a real patogênese do cisto ósseo traumático.


The traumatic bone cyst is characterized by the presence of an asymptomatic sinus devoid of epithelial lining, which is rarely found in the jaws. OBJECTIVE: To describe the clinical, surgical and radiographic findings of traumatic bone cysts. MATERIAL AND METHOD: A retrospective study was made of patients diagnosed with traumatic bone cysts at an oral pathology department from 1992 to 2007. Data on the clinical, radiographic and surgical complications were gathered. RESULTS: Twenty-six cases of traumatic bone cyst were diagnosed in 15 years; 17 were male and 09 were female. Most patients were within first two decades of life and had no pain or history of trauma in the affected area. The multilocular pattern was observed in only seven cases, its radiographic appearance suggests a tumor. Air was found inside the lesion in about 70% of cases; serous fluid with blood and blood only were uncommon within the lesions. CONCLUSION: A higher prevalence in young patients, absence of a history of trauma, and a small number of lesions containing serous fluid with blood reflects the need to discuss the true pathogenesis of traumatic bone cysts.

Los quistes de los maxilares son cavidades patológicas con una pared conjuntivo-epitelial con contenido líquido, semilíquido o pastoso. A veces se manifiestan clínicamente o se descubren por hallazgos radiográficos. En la presente publicación se presenta un caso de un quiste maxilar asintomático, descubierto radiográficamente, cuyo tamaño abarca desde el canino derecho hasta la rama ascendente opuesta, con conservación de la vitalidad pulpar de las piezas involucradas, a las que no se les realizó tratamiento endodóntico previo o posterior.

ABSTRACT

The solitary or traumatic bone cyst (TBC) is an uncommon non-epithelial lined cavity of the jaws. The lesion is mainly diagnosed in young patients most frequently during the second decade of life. The majority of them are located in the mandibular body between the canine and the third molar. Clinically, the lesion is asymptomatic and is often accidentally discovered on routine radiological examination. The definite diagnosis of traumatic cyst can only be determined at surgery. Often, the material for histological examination may be scanty or non-existent. We present a documented case of a solitary bone cyst involving the body of the mandible. A brief review of literature regarding the main characteristics of the lesion is provided.

ABSTRACT

O cisto do ducto nasopalatino (CDNP) ou cisto do canal incisivo é considerado um cisto não-odontogênico, de patogênese incerta, que acomete indivíduos entre a 2ª e 5ª décadas de vida e exibe crescimento lento e assintomático. Radiograficamente nota-se párea radiolúcida situada na região anterior da maxila, próximo ao ápice dos incisivos centrais superiores. O diagnóstico diferencial pode ser feito com o cisto radicular, queratocisto, entretanto dados clínicos, radiográficos e histopatológicos auxiliam na conclusão do caso. Este estudo descreve um relato de caso de cisto do ducto nasopalatino observado após exame radiográfico de rotina. Foi realizada biópsia excisional para diagnóstico da lesão. O controle clínico e radiográfico pós-operatório mostrou ausência de recorrência e excelente reparo

El quiste paradental se desarrolla a partir del epitelio odontogénico de la zona superficial del ligamento periodontal de un diente vital. Se localiza por vestibular y distal de un molar inferior semierupcionado, más frecuentemente el tercer molar, con una historia asociada de pericoronaritis. Un quiste similar ocurre en la superficie vestibular de primeros molares en niños denominándose quiste mandibular bucal infectado. ABSTRACT

Pain following any surgical procedure is difficult to combat by man. Analgesics can be given for some amount of symptomatic relief. The need for a long lasting local anaesthetic is with the intention to reduce the most severe nature of pain, and decrease the analgesic consumption. This study was conducted to compare the efficacy of, Bupivacaine over Lidocaine for minor oral surgical procedures. The popularly used lidocaine works for 3 to 3.9 hours. This is not sufficient since the most severe nature of pain is felt six to eight hours post surgery, where as Bupivacaine has duration of action of seven to eight hours. Hence the post operative pain experienced following administration of bupivacaine was found to be considerably lesser in degree than compared to the lidocaine group. To avoid severe pain and discomfort to the patient following any minor oral surgical, the use of bupivacaine is recommended.

ABSTRACT

Los quistes dentígeros son unos de los más prevalentes entre los quistes odontogénicos. Su verdadera histogénesis permanece sin aclarar, la mayoría de los autores apoya un origen de desarrollo, pero tambiéns e ha creído que existe un origen inflamatorio. El propósito de este artículo es presentar un caso de quiste dentígero inflamatorio asociado con un segundo premolar mandibular en una pciente de 10 años de edad, relacionado con un diente primario con tratamiento pulpar y explicar la patogénesis de este nuevo tipo de quiste dentígero
