ABSTRACT

Abstract In this study, the manufacturing process of lamivudine (3TC) and zidovudine (AZT) tablets (150+300 mg respectively) was evaluated using statistical process control (SPC) tools. These medicines are manufactured by the Fundação para o Remédio Popular "Chopin Tavares de Lima" (FURP) laboratory, and are distributed free of charge to patients infected with HIV by the Ministry of Health DST/AIDS national program. Data of 529 batches manufactured from 2012 to 2015 were collected. The critical quality attributes of weight variation, uniformity of dosage units, and dissolution were evaluated. Process stability was assessed using control charts, and the capability indices Cp, Cpk, Pp, and Ppk (process capability; process capability adjusted for non-centered distribution; potential or global capability of the process; and potential process capability adjusted for non-centered distribution, respectively) were evaluated. 3TC dissolution data from 2013 revealed a non-centered process and lack of consistency compared to the other years, showing Cpk and Ppk lower than 1.0 and the chance of failure of 2,483 in 1,000,000 tablets. Dissolution data from 2015 showed process improvement, revealed by Cpk and Ppk equal to 2.19 and 1.99, respectively. Overall, the control charts and capability indices showed the variability of the process and special causes. Additionally, it was possible to point out the opportunities for process changes, which are fundamental for understanding and supporting a continuous improvement environment.

Resumen Introducción. El cumplimiento de la meta de eliminación de la malaria en Ecuador en el 2020 exige contar con la capacidad requerida para el diagnóstico microscópico ajustado a los estándares de calidad de la Organización Mundial de la Salud (OMS) y de la Organización Panamericana de la Salud (OPS) y proveer el tratamiento adecuado a los pacientes. Objetivo. Conocer la idoneidad o competencia de los microscopistas de la red pública local para el diagnóstico parasitológico de la malaria y el desempeño de los laboratorios intermedios de referencia. Materiales y métodos. Se hizo un estudio descriptivo de corte transversal a partir de la información obtenida en los talleres de evaluación de idoneidad en el diagnóstico microscópico de la red de laboratorios en las coordinaciones zonales de salud utilizando un panel de láminas para evaluar la concordancia del diagnóstico. Además, se calificó el desempeño de los laboratorios intermedios en el diagnóstico en el marco del programa de evaluación externa del desempeño. Los resultados se compararon con los obtenidos por el laboratorio supranacional de Perú. Resultados. En los 11 talleres realizados, se evaluó la idoneidad de 191 microscopistas, de los cuales 153 (80,1 %) aprobaron las pruebas. Las medianas de los indicadores fueron las siguientes: concordancia entre la detección y el resultado, 100 % (Q1- Q3: 96-100); concordancia en la especie, 100 % (Q1- Q3: 93-100); concordancia en el estadio, 93,0 % (Q1- Q3: 86-95) y concordancia en el recuento, 77 % (Q1- Q3: 71-82). En el programa de evaluación externa de desempeño, los tres laboratorios intermedios obtuvieron una concordancia del 100 % en el resultado y una del 96 % en la especie. Conclusiones. Los indicadores de competencia de la red local y de desempeño de los laboratorios intermedios alcanzaron altos estándares de calidad acordes con el proceso de entrenamiento implementado en el país.


Abstract Introduction: To reach the goal of malaria elimination in Ecuador for the year 2020, it is necessary to have a laboratory network with the capacity to perform microscopic diagnosis according to the WHO/PAHO quality standards and to provide the adequate treatment of cases. Objective: To determine the level of competence for parasitological diagnosis of the microscopists from the local public network and the performance of intermediate reference laboratories. Materials and methods: We conducted a cross-sectional study based on the information collected in workshops carried out to appraise the competence for microscopic diagnosis of the local laboratory network (zonal health coordinating offices 1 to 8) using a slide panel to evaluate diagnosis agreement, as well as the diagnostic performance of the intermediate laboratories using an external quality assessment program. The results were compared against the reference standards of the supranational laboratory in Perú. Results: We evaluated the competencies of 191 microscopists in 11 workshops and 153 (80.1%) of them were approved. The medians of the indicators were the following: concordance for parasite detection, 100% (Q1- Q3: 96-100), concordance for species identification, 100% (Q1- Q3: 93-100), and concordances for stage identification, 93.0% (Q1- Q3: 86-95) and parasite counting, 77.0% (Q1- Q3: 71-82). In the external quality assessment, the three intermediate laboratories obtained 100% in parasite detection concordance and 96% for species detection concordance. Conclusions: The results for the primary network and the performance indicators for the intermediate laboratories showed the high-quality standards of the training program implemented in the country.

ABSTRACT

A abordagem sobre a elaboração de índices representativos da qualidade constitui recurso cada vez mais utilizado para o monitoramento ou comparação de uma ou mais situações, permitindo expressar de forma simplificada as condições avaliadas. Na área de meio ambiente, os resultados fornecidos pelos laboratórios de controle ambiental assumem um importante papel, proporcionando situações de julgamento e decisão por parte de quem utiliza esses resultados, pois o mérito dessas decisões pode ser influenciado pela qualidade desses resultados. Para tanto, objetivou-se propor um índice de qualidade aplicável a laboratórios de controle ambiental, avaliando a sua representatividade, a realidade do laboratório com base no seu histórico de qualidade e aplicabilidade do índice no acompanhamento de condições gerais de desempenho. A pesquisa, de caráter exploratório, foi constituída a partir da seleção de dez especialistas em laboratórios de controle ambiental, representantes dos setores público, privado e academia, entrevistados nas sucessivas etapas de proposição do índice. A primeira etapa constituiu entrevista aos especialistas com uso da técnica de Brainstorming que culminou com a seleção de vinte parâmetros de consulta. Nas etapas seguintes, seguiram-se os preceitos da técnica DELPHI para extrair e refinar os julgamentos do grupo, bem como, selecionar os parâmetros finais de avaliação do IQL, critérios de pontuação e as três faixas de enquadramento dos laboratórios (inadequada, em fase de adequação ou adequada). Foram realizados experimentos de aplicação do IQL em seis laboratórios de controle ambiental, também representantes dos setores público, privado e da academia, a razão de duas unidades por setor, dentre os quais se encontravam laboratórios acreditados pelo INMETRO, conforme a norma ISO IEC 17.025. ABSTRACT

O estudo foi realizado com a participação de 120 trabalhadores da Divisão de Laboratório Central do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, com o objetivo de determinar associações de fatores demográficos, história clínica e ocupacional, estresse relacionado ao trabalho e fatores ergonômicos, com relatos de sintomas osteomusculares e faltas ao trabalho relacionadas aos sintomas. Os sintomas apresentaram associação significante com a história pregressa de doenças reumáticas ou ortopédicas e com posturas e movimentos inadequados na realização do trabalho. As faltas ao trabalho em função dos sintomas foram associadas ao baixo grau de escolaridade e ao estado civil dos trabalhadores.This study was developed with 120 workers of the Central Laboratory Division of "Hospital das Clínicas", University of São Paulo - School of Medicine, with the objective of determining the association of demographic factors, clinical and occupational case history, work related stress and ergonomic workplace analysis with the outcomes: musculoskeletal symptoms and related absences from work. ABSTRACT

In the present work, a device designed for concentrating cells from biological fluids is described. The instrument consists of a tube in which the inner cavity has a conical shape at one of its ends and a small orifice is found at the botton, while the tubes's exterior maintains its cylindrical shape. The tube is placed inside a second tube that ends on a flat surface on which a glass cover slide is placed. The sample to be studied is placed in the inner tube of the assembled device and spun in a regular clinical centrifuge. Cells are collected on the glass slide, fixed and stained for microscopical studies. The device was tested using 23 samples of cerebrospinal fluid (CSF) from patients with lymphoproliferative diseases. An adequate number of intact cells was recovered for observation, and a precise diagnosis was possible. Cells from three aliquots of each CSF sample were concentrated by this method, and by the more expensive standard commercial cytocentrifuge, with similar results. El crecimiento y la evolución del Laboratorio de Investigación Pediátrica representan un tema de considerable interés, ya que no sólo permite hacer consideraciones acerca del papel que juega el liderazgo científico en la solución de los problemas del desarrollo, sino también discutir acerca de la maduración de las instituciones universitarias y de los frutos que pueden resultar de su accionar. En este sentido, el Laboratorio de Investigación Pediátrica constituye un ejemplo casi único en nuestro continente, que vale la pena tener en cuenta al crear otras instituciones

Foi efetuada uma classificaçäo, quanto ao atendimento e exames realizados, dos laboratórios de análises e patologia clínica operando na regiäo metropolitana de Belo Horizonte, MG (Brasil), em 1978. Foram comparadas as 3 categorias de laboratórios (particulares, institucionais e hospitalares), segundo tipos de exames realizados, clientela atendida e pessoal técnico responsável pelos laboratórios. Com base nos resultados obtidos na análise, foi composto um "perfil" típico para cada categoria de laboratório estudado
