ABSTRACT

The mandibular advancements performed in orthognathic surgeries can be stabilized with several techniques when using stable internal fixation. This study aims to comparatively evaluate, in vitro, the mechanical strength in a polyurethane mandibular model for four fixation techniques for sagittal split ramus osteotomy mandibular. 60 samples were divided into 4 groups, with 15 units for each group: group A, group B, group C and group D. Advances of 5 mm were made for each subgroup and fixed with 2.0 mm system plates and monocortical screws in the replicas of human hemimandibles in polyurethane resin. The samples were submitted to mechanical tests of linear loading, being evaluated the peak load and peak deformation. Technique B presented higher peak load (Kgf) and techniques A and B presented higher peak strain (p<0.05). Technique D presented lower peak load and lower peak strain (p<0.05). It is concluded that the study based on the development of new techniques for fixation for sagittal osteotomy of the mandibular ramus is of great importance for the advancement of orthognathic surgery, provided by the technical innovation of more favorable plate models.


Los avances mandibulares realizados en cirugías ortognáticas pueden estabilizarse con varias técnicas cuando se utiliza fijación interna estable. Este estudio tuvo como objetivo evaluar comparativamente, in vitro, la resistencia mecánica en un modelo mandibular de poliuretano para cuatro técnicas de fijación para la osteotomía sagital de la rama mandibular. Se dividieron 60 muestras en 4 grupos, con 15 unidades para cada grupo: grupo A, grupo B, grupo C y grupo D. Se realizaron avances de 5 mm para cada subgrupo y se fijaron con placas de sistema de 2,0 mm y tornillos monocorticales en las réplicas de hemimandíbulas humanas en resina de poliuretano. Las muestras fueron sometidas a pruebas mecánicas de carga lineal, siendo evaluadas la carga máxima y la deformación máxima. La técnica B presentó mayor pico de carga (Kgf) y las técnicas A y B presentaron mayor pico de deformación (p<0,05). La técnica D presentó menor carga máxima y menor tensión máxima (p<0,05). Se concluye que el estudio basado en el desarrollo de nuevas técnicas de fijación para la osteotomía sagital de la rama mandibular es de gran importancia para el avance de la cirugía ortognática, proporcionada por la innovación técnica de modelos de placas más favorables.

ABSTRACT

In addition to functionally correcting skeletal facial deformities, orthognathic surgery also has a great impact on aesthetics and facial proportions. However, patients who have soft tissue changes, such as tissue flaccidity, may not achieve an ideal result, and require complementary surgical techniques that promote aesthetic refinement. The literature on less invasive techniques is limited, and therefore, this work aims to report a surgical technique performed concomitantly with genioplasty to refine the harmony of the submandibular region, in a simple and effective way. Patient with skeletal class II deformity, with lack of mandibular definition and submandibular flaccidity, underwent bimaxillary advancement orthognathic surgery with advancement genioplasty to correct chin retrusion. Due to limited results with bone repositioning alone, plication of the suprahyoid musculature was performed in association with the same approach used in the genioplasty. As a result, there was traction in the cervical region, with a consequent decrease in the submental-cervical angle and improvement in submandibular aesthetics. Furthermore, there was no need for extensive surgical intervention and no impairment of the functional results of the orthognathic surgery itself. Therefore, it can be concluded that plication of the suprahyoid musculat ure concomitantly with genioplasty is a technique that promotes aesthetic gains in the cervical region.


Además de corregir funcionalmente las deformidades faciales esqueléticas, la cirugía ortognática también tiene un gran impacto en la estética y las proporciones faciales. Sin embargo, los pacientes que tienen cambios en los tejidos blandos, como la flacidez, pueden no lograr un resultado ideal y requieren técnicas quirúrgicas complementarias que promuevan el refinamiento estético. La literatura sobre técnicas menos invasivas es limitada, por lo que este trabajo tiene como objetivo reportar una técnica quirúrgica realizada concomitantemente con la genioplastia para refinar la armonía de la región submandibular, de manera simple y efectiva. Se trató a un paciente con deformidad esquelética clase II, con falta de definición mandibular y flacidez submandibular, intervenida de cirugía ortognática de avance bimaxilar con genioplastia de avance para corregir la retrusión del mentón. Debido a los resultados limitados solo con el reposicionamiento óseo, se realizó la plicatura de la musculatura suprahioidea en asociación con el mismo abordaje utilizado en la genioplastia. Como resultado, hubo tracción en la región cervical, con la consiguiente disminución del ángulo submentoniano-cervical y mejoría en la estética submandibular. Además, no hubo necesidad de una intervención quirúrgica extensa ni deterioro de los resultados funcionales de la cirugía ortognática en sí. Por lo tanto, se puede concluir que la plicatura de la musculatura suprahioidea concomitantemente con la genioplastia es una técnica que promueve ganancias estéticas en la región cervical.

ABSTRACT

El bad split es un término clínico que refiere a una fractura no planificada que ocurre al momento de realizar una osteotomía sagital de rama mandibular (OSRM). Afecta aproximadamente al 2,3% de los pacientes y se han descrito factores de riesgo tales como la presencia de terceros molares mandibulares, edad avanzada al momento de la cirugía, técnica de osteotomía inadecuada, entre otros. Se recomienda efectuar manio-bras preventivas para evitar la aparición de patrones de fractura no deseados al realizar la OSRM. Sin embargo, al momento de pesquisar un bad split, éste debe ser tratado por un equipo capacitado y de manera oportuna para evitar retardo en la consolidación, infecciones y secuestros óseos que puedan comprometer los resultados de la cirugía. En este artículo se presenta el manejo de un caso clínico de bad split bilateral intraoperatorio por el Servicio de Cirugía Maxilofacial del Hospital San José, enfatizando su tratamiento quirúrgico.


Bad Split is a clinical term referring to an unplanned fracture that occurs during the bilateral sagittal split osteotomy (BSSO). It affects approximately 2,3% of the patients undergoing orthognathic surgery and several risk factors have been described such as the presence of mandibular third molars, advanced age at the moment of orthognathic surgery, inadequate osteotomy technique, etc. Preventive maneuvers are recommended in order to avoid the appearance of undesired fracture patterns during BSSO. However, if a bad split is detected it must be managed and treated by a qualified team to avoid further complications such as delayed bone consolidation, bone infection and necrosis. In this article we present the management of a case of bilateral bad split by the Maxillofacial Surgery Service of Hospital San José, emphasizing on its surgical treatment.

Introducción: La cirugía ortognática está especialmente indicada en casos complejos, donde los procedimientos conservadores fracasan o son insuficientes. El diagnóstico, planificación y tratamiento deben estar a cargo de un equipo multidisciplinario que trabaje en estrecha relación. Objetivo: Reportar el trabajo multidisciplinario en el tratamiento ortodóncico quirúrgico de una desarmonía dentomaxilofacial compleja. Presentación de caso: Paciente masculino, 19 años de edad, mestizo, clase III esquelética grave, hiperdivergente con mordida abierta y asimetría del tercio inferior, que recibió tratamiento prequirúrgico descompensador de ortodoncia. Se realizó cirugía ortognática bimaxilar con genioplastia de ascenso. Conclusiones: El tratamiento de las desarmonías dentomaxilofaciales es complejo y necesita la concurrencia de varias especialidades. Para la resolución del presente caso fue necesario el trabajo coordinado y simultáneo de especialistas de ortodoncia, prótesis dental, cirugía maxilofacial, periodoncia, psicología y otorrinolaringología. Se alcanzaron resultados estéticos y funcionales acorde a los objetivos del tratamiento propuestos y hubo estabilidad un año después de la cirugía(AU)


Introduction: Orthognathic surgery is especially indicated in complex cases, where conservative procedures fail or are insufficient. Diagnosis, planning and treatment should be carried out by a multidisciplinary team that works closely together. Objective: Report the multidisciplinary work in the surgical orthodontic treatment of a complex dentomaxillofacial disharmony. Case presentation: Male patient, 19 years old, mestizo, severe skeletal class III, hyperdivergent with open bite and asymmetry of the lower third, who received presurgical treatment decompensating orthodontics. Bimaxillary orthognathic surgery was performed with ascent genioplasty. Conclusions: The treatment of dentomaxillofacial disharmonies is complex and requires the concurrence of several specialties. For the resolution of this case, the coordinated and simultaneous work of specialists in orthodontics, dental prostheses, maxillofacial surgery, periodontics, psychology and otolaryngology was necessary. Aesthetic and functional results were achieved according to the proposed treatment objectives and there was stability one year after surgery(AU)

Introducción: la asimetría facial es una condición notoria en el tercio inferior de la facie del sujeto y entre los tercios medio y superior, en este último será menos evidente esta condición, de tal manera que podría considerarse como una de las molestias de mayor incidencia en pacientes con necesidades o no de terapia or- todóncica. Objetivo: evaluar mediante una revisión de la literatura los tratamientos ortodóncico-quirúrgicos de pacientes con asimetría facial. Material y métodos: la literatura se seleccionó mediante una búsqueda en las bases de datos electrónicas: PubMed, Scopus, Web of Science. Las palabras clave utilizadas fueron: facial asymmetry, asymmetry, surgical treatment, surgical orthodontic treatment. La búsqueda se restringió a artículos en inglés publicados del año 2011 al 2021. Resultados: después de aplicar los criterios de inclusión y exclusión en total se obtuvieron y revisaron 27 artículos. Se realizó la revisión de literatura del tratamiento ortodóncico-quirúrgico en pacientes con asimetría facial. Conclusión: es preciso el abordaje ortodóntico quirúrgico para la corrección de la asimetría facial, del protocolo dependerá la situación clínica y la elección de tratamiento ortodóntico quirúrgico, lo que brindará mejoras significativas en la simetría facial (AU)


Introduction: facial asymmetry, is a notorious condition in the lower third of the subject's facie and between the middle and upper thirds, in the latter this condition will be less evident; in such a way, it could be considered as one of the discomforts of greater incidence in patients with needs or not of orthodontic therapy. Objective: to evaluate by means of a literature review the orthodontic-surgical treatment of patients with facial asymmetry. Material and methods: the literature was selected through a search in the following electronic databases: PubMed, Scopus, Web of Science. The keywords used were: facial asymmetry, asymmetry, surgical treatment, surgical orthodontic treatment. The search was restricted to articles in English published from 2011 to 2021. Results: after applying the inclusion and exclusion criteria, a total of 27 articles were obtained and reviewed. The literature review of orthodontic-surgical treatment in patients with facial asymmetry was performed. Conclusion: surgical orthodontic approach is necessary for the correction of facial asymmetry, the protocol will depend on the clinical situation, the choice of surgical orthodontic treatment, which will give significant improvements in facial symmetry (AU)

Introducción: La corrección del exceso vertical del maxilar traerá no solo cambios verticales en el perfil, sino también sagitales y transversales. La ausencia clínica de mordida cruzada posterior puede enmascarar deficiencias del ancho y provocar planes de tratamiento incorrectos. Objetivo: Describir los aspectos más importantes en la atención de un caso con exceso vertical y deficiencia transversal del maxilar sin mordida cruzada posterior. Presentación de caso: Paciente femenina de 26 años de edad acude a consulta por "dientes hacia adelante". El examen físico y el estudio de los medios diagnósticos permitieron concluir la existencia de exceso vertical y deficiencia transversal del maxilar. Clínicamente no se observó mordida cruzada posterior. El tratamiento por etapas incluyó ortodoncia, expansión e impactación quirúrgica del maxilar. Conclusiones: El ascenso quirúrgico del maxilar debe planificarse a partir de un análisis holístico. Incluye los cambios sagitales del perfil y de la relación transversal entre las arcadas, provocados por el efecto de rotación mandibular. La ausencia de mordida cruzada posterior no excluye la existencia de deficiencia transversal del maxilar. Asimismo, la dimensión definitiva de la expansión debe establecerse una vez que la arcada inferior alcance sus dimensiones definitivas(AU)


Introduction: The correction of the vertical excess of the maxilla will bring not only vertical changes in the profile, but also sagittal and transverse ones. The clinical absence of posterior crossbite can mask width deficiencies and lead to incorrect treatment plans. Objective: Describe the most important aspects in the care of a case with vertical excess and transverse deficiency of the maxilla without posterior cross bite. Case presentation: A 26-year-old female patient comes to the consultation for "teeth forward". The physical examination and the study of the diagnostic means allowed to conclude the existence of vertical excess and transverse deficiency of the maxilla. Clinically, no posterior crossbite was observed. Staged treatment included orthodontics, expansion and surgical impaction of the maxilla. Conclusions: The surgical ascent of the maxilla should be planned based on a holistic analysis. It includes the sagittal changes of the profile and the transverse relationship between the arcades, caused by the effect of mandibular rotation. The absence of posterior crossbite does not exclude the existence of transverse maxillary deficiency. Likewise, the definitive dimension of the expansion must be established once the lower arcade reaches its definitive dimensions(AU)

Introducción: La discrepancia transversal de las arcadas es un problema esquelético poco estudiado en personas con necesidad de tratamiento ortodóncico quirúrgico. Objetivo: Caracterizar a pacientes con discrepancia negativa transversal del maxilar y necesidad de tratamiento por cirugía ortognática. Métodos: Se realizó un estudio descriptivo de corte transversal en 25 expedientes clínicos de pacientes que acudieron al servicio de cirugía maxilofacial del Hospital Clínico Quirúrgico Hermanos Ameijeiras, con discrepancia negativa transversal del maxilar y necesidad de cirugía ortognática entre enero de 2016 y enero de 2020. Se estudiaron los casos por medio de variables cualitativas divididas en epidemiológicas, características faciales y mediciones dentarias. Para el análisis se confeccionaron cuadros estadísticos y como medidas de resumen se utilizaron frecuencias absolutas y relativas. Resultados: La frecuencia de la discrepancia negativa del maxilar fue de 13,3 por ciento. Los pacientes de piel blanca fueron los más afectados (80 por ciento) y el principal motivo de consulta fue estético (68 por ciento). La asimetría del tercio inferior y el perfil cóncavo estuvieron presentes en el 44 por ciento y 36 por ciento de los casos respectivamente. Se observó alta frecuencia de pacientes con el tercio inferior aumentado (76 por ciento), mordida cruzada posterior (48 por ciento), mordida abierta anterior (75 por ciento), apiñamiento en la arcada inferior (63,3 por ciento) y lingualización del incisivo inferior (72 por ciento). Conclusiones: La discrepancia negativa transversal del maxilar es frecuente en personas con necesidad de cirugía ortognática y con gran afectación estética. Se observó mayor predisposición a padecerla en pacientes de piel blanca(AU)


Introduction: Transverse discrepancy of the arches is a skeletal problem not sufficiently studied in persons requiring surgical orthodontic treatment. Objective: Characterize patients with negative transverse maxillary discrepancy requiring surgical orthognathic surgery. Methods: A descriptive cross-sectional study was conducted of 25 medical records of patients attending the maxillofacial surgery service at Hermanos Ameijeiras Clinical Surgical Hospital with negative transverse maxillary discrepancy requiring orthognathic surgery from January 2016 to January 2020. The cases were studied with qualitative variables divided into epidemiological, facial features and dental measurements. Analysis was based on statistical charts. The summary measurements used were absolute and relative frequencies. Results: The frequency of negative maxillary discrepancy was 13.3 percent. White skin patients were more commonly affected (80 percent). The main concern was aesthetic (68 percent). Lower third asymmetry and a concave profile were present in 44 percent and 36 percent of the cases, respectively. A high frequency was observed of patients with an augmented lower third (76 percent), posterior crossbite (48 percent), anterior open bite (75 percent), lower arch crowding (63.3 percent) and lower incisor lingualization (72 percent). Conclusions: Negative transverse maxillary discrepancy is common among patients requiring orthognathic surgery for large aesthetic alterations. White skin patients were found to be more prone to present this disorder(AU)

Introduction: Negative maxillary transverse discrepancy is not exclusive to narrow upper arch. The transverse discrepancy due to maxillary deficiency is well described, but this is not the case when the maxilla displays adequate dimensions and the origin of the problem is an excess of mandibular width. Objective: To describe the cephalometric characteristics of negative maxillary transverse discrepancy present in narrow upper arches with those of normal or increased width. Methods: An observational and cross-sectional study was carried out in records of patients with negative maxillary transverse discrepancy. The cases referred to the maxillofacial surgery department at Hospital Clínico Quirúrgico "Hermanos Ameijeiras" between January 2016 and January 2020 were selected. The initial study models were measured and two groups were formed: those with a distance between central fossae of 16 to 26, less than 47 mm and the other with equal or greater values. Lateral cephalometric analysis of Ricketts, McNamara, Björk & Jarabak, Burstone and soft profile were performed on the initial teleradiographs. Using the Mann Whitney U test of independent samples, measurements with statistically significant differences between both groups were selected. (p <0.05). Results: Significant differences were found regarding the cranial-mandibular relationship, mandibular anatomy, maxillo-mandibular relationship and occlusal relationship. The most important differences observed in each group were respectively: S-Ar / Ar-Go ratio (p = 0.033), mandibular body length (p = 0.01), mandibular-maxillary difference (p = 0.003) and overjet (p = 0.043). Conclusions: In the studied patients, negative maxillary transverse discrepancy has the same probability of presenting a wide or in norm upper arch, as well as a narrow arch. In the first case, it is more likely to find jaws with increased body length, a skeletal class III pattern and a concave profile; in the second case, the tendency is to present small jaws with increased articular angle and dental overjet(AU)


Introducción: La discrepancia negativa transversal del maxilar no es exclusiva de una arcada superior angosta. La discrepancia transversal por deficiencia maxilar está bien descrita, pero no ocurre así cuando el maxilar se presenta con dimensiones adecuadas y el origen del problema es un exceso del ancho mandibular. Objetivo: Describir las diferencias cefalométricas de la discrepancia negativa transversal del maxilar que presentan arcadas superiores estrechas con aquellas de ancho normal o incrementado. Métodos: Se realizó un estudio observacional, de corte transversal, en expedientes de pacientes con discrepancia negativa transversal del maxilar. Se seleccionaron los casos que acudieron entre enero del 2016 y enero del 2020 a la consulta de cirugía maxilofacial del Hospital Clínico Quirúrgico "Hermanos Ameijeiras". Se midieron los modelos de estudio iniciales y se conformaron dos grupos: aquellos con distancia entre fosas centrales de 16-26, menor de 47 mm y el otro con valores iguales o superiores. Se realizaron análisis cefalométricos laterales en las telerradiografías iniciales de Ricketts, McNamara, Björk & Jarabak, Burstone y perfil blando. Por medio de la prueba U de Mann Whitney de muestras independientes, se seleccionaron las mediciones con diferencias estadísticas significativas entre ambos grupos (p < 0.05). Resultados: Se encontraron diferencias significativas concernientes a la relación cráneo mandibular, anatomía mandibular, relación máxilo mandibular y relación oclusal. Las diferencias observadas más importantes de cada grupo fueron respectivamente: relación S-Ar/Ar-Go (p = 0,033), longitud del cuerpo mandibular (p = 0,01), diferencia mandibular-maxilar (p = 0,003) y resalte (p = 0,043). Conclusiones: En los pacientes estudiados, las discrepancias negativas transversales tienen la misma probabilidad de presentarse con la arcada superior ancha o en norma, que angosta. En los primeros es más probable encontrar mandíbulas con longitud del cuerpo incrementada, patrón de clase III esquelética y perfil cóncavo; en los segundos, la tendencia es a presentar mandíbulas pequeñas con ángulo articular y resalte dentario aumentados(AU)

Introdução: A má oclusão Classe II pode influenciar negativamente na qualidade de vida dos pacientes, tanto na aparência facial, quanto função oral ou até ambas. Atualmente, o tratamento ortodôntico-cirúrgico é comumente utilizado em pacientes com discrepâncias esqueléticas graves. Objetivo: Este estudo tem como objetivo analisar os resultados da cirurgia ortognática associada ao tratamento ortodôntico nos pacientes que possuem DTM e má oclusão de Classe II por retrognatismo mandibular. Metodologia: Foi realizada uma pesquisa bibliográfica nas plataformas de buscas científicas Google Acadêmico, CAPES/MEC, PubMed/Medline, Scielo, Elsevier, e nas revistas AJO-DO (American Journal of Orthodontics and Dentofacial Orthopedics) e The Angle Orthodontist (An International Journal of Orthodontics and Dentofacial Orthopedics), utilizando as seguintes palavras chave: retrognatismo, cirurgia ortognática e transtornos da ATM. Conclusão: A maioria dos pacientes que apresentam sinais e sintomas de DTM pré-operatórios melhoram a disfunção e diminuem os níveis de dor com o tratamento ortognático. Além disso, o tratamento ortodôntico é de suma importância para se obter o sucesso do procedimento cirúrgico, assim como na estabilidade pós-cirúrgica.(AU)


Introduction: Class II malocclusion can negatively influence patients in quality of life, in the facial and oral appearance or both. Currently, orthodontic-surgical treatment is commonly used in patients with severe skeletal discrepancies. Objective: The objective of this study was to analyze the results of orthognathic surgery associated with orthodontic treatment in patients who have TMD and Class II malocclusion due to mandibular retrognathism. Methodology: A bibliographic search was performed in the scientific search platforms Google Scholar, CAPES/MEC, PubMed/Medline, Scielo, Elsevier, AJO-DO (American Journal of Orthodontics and Dentofacial Orthopedics) and in The Angle Orthodontist (An International Journal of Orthodontics and Dentofacial Orthopedics), using the keywords: retrognathism, orthognathic surgery and TMJ disorders. Conclusion: With orthognathic treatment, most patients who had preoperative DTM signs and symptoms showed an improvement and a decrease in pain levels. In addition, orthodontic treatment is important for the success of the surgical procedure, as well as for post-surgical stability.(AU)

Resumen Introducción: La incorporación de tecnologías en la práctica quirúrgica, ha cambiado la forma de enfrentar el proceso quirúrgico. Objetivo: Describir la experiencia de los últimos 9 años, en cirugía ortognática, con la incorporación de la cefalometría 3D. Materiales y Método: Se realizó una revisión retrospectiva de pacientes operados de cirugía ortognática durante el período enero de 2011 a agosto de 2018. Se registraron datos demográficos, quirúrgicos, tipo de planificación quirúrgica, complicaciones y resultados a largo plazo. Resultados: 21 pacientes requirieron cirugías ortognáticas. Se realizaron 16 cirugías bimaxilares (76%), 3 cirugías de avance maxilar superior (14%) y 2 cirugías de osteotomía sagital de rama (10%). Las principales etiologías fueron: 67% maloclusión clase III (n = 14), 28% maloclusión clase II (n = 6) y 5% desviación mandibular (n = 1). La planificación prequirúrgica virtual fue utilizada en 11 pacientes (52%). La tasa de complicaciones Clavien-Dindo > III fue 4,8% (n = 1). Conclusiones: En nuestra experiencia, las técnicas de cirugía ortognática son seguras. Las complicaciones de la serie fueron escasas y bien toleradas por los pacientes. Durante los últimos años y, de acuerdo a los avances tecnológicos, la incorporación de la planificación quirúrgica virtual 3D favoreció el desarrollo de la cirugía ortognática en nuestro centro.


Introduction: Technological advances have been incorporated into cranio-maxillo-facial surgery changing the clinical practice of surgeons. Aim: The aim of this article is to describe our experience in orthognatic surgery in the last 9-years, with the incorporation of 3D cephalometry. Materials and Method: A retrospective chart review was performed from January 2011 to August 2018 on patients undergoing Orthognatic Surgery. Demographic and surgical data, type of surgical planning, complications and long-term results were recorded. Results: A total of 21 patients underwent orthognatic surgery. Average age was 28 years (DE 8.1), 11 men (52.3%) and 10 women (47.61%). The principal surgeries performed were: 16 (76%) bimaxillary, 3 maxillary advancement surgery 3 (14%) and bilateral sagital split osteotomy 2 (10%). Ethiologies were: 14 patients (this 67%) with malocclusion class III, 6 patients (28%) with malocussion class II, and 1 patient (5%) with mandibular deviation. Virtual 3D presurgical planning was used in 11 patients (52%). The total of complications Clavien-Dindo > III was 4.8% (n = 1). None patient required reoperation. Satisfaction rate with the procedure was high. Conclusions: Based on our experience, orthognatic surgery techniques are safe. The complication rate was lower and well tolerated by patients. The introduction of virtual planning provided a better scenario to develop maxillo-facial surgery.

ABSTRACT

Objetivos: analisar, por meio de revisão de literatura, o impacto da cirurgia ortognática na qualidade de vida de pacientes com diferentes deformidades orofaciais e identificar a concepção e a percepção dos pacientes em relação às correções de deformidades faciais de um quadro prévio ao procedimento até o momento da avaliação. Metodologia: foi realizado um levantamento da literatura nas bases de dados PubMed e Medline, utilizando os seguintes termos de busca: "ortognática", "distúrbios temporomandibulares", "qualidade de vida" e "psicossocial". Os artigos foram escolhidos nas línguas portuguesa e inglesa, dentre aqueles publicados entre os anos de 1990 a 2019. Considerações finais: a correção das deformidades esqueléticas faciais e das irregularidades dentárias associadas pela cirurgia ortognática apresenta um impacto positivo na qualidade de vida dos pacientes pela melhora harmônica dos ossos esqueléticos faciais, resultando em uma estética facial favorável e em um aprimoramento das funções do sistema estomatognático.(AU)


Objectives: to analyze, through literature review, the impact of orthognathic surgery on the quality of life of patients with different orofacial deformities and to identify the patients' conception and perception about corrections of facial deformities prior to the procedure until the moment of evaluation. Methods: a literature review was performed using the PubMed and Medline databases using the following search terms: "orthognathic", "temporomandibular disorders", "quality of life" and "psychosocial". The articles were chosen in Portuguese and English between 1990 and 2019. Final considerations: correction of facial skeletal deformities and associated dental irregularities, by orthognathic surgery, have a positive impact on patients' quality of life through the harmonious improvement of skeletal facial bones, resulting in a favorable facial esthetics and an improvement of the functions of the stomatognathic system.(AU)

RESUMEN Introducción: El síndrome de apnea obstructiva del sueño provoca somnolencia diurna; incide negativamente sobre el sistema cardiovascular y disminuye la calidad de vida. La cirugía ortognática provoca cambios en el espacio aéreo faríngeo, por lo que ha sido empleada en el tratamiento de dicho síndrome. Objetivo: Caracterizar las técnicas en cirugía ortognática más empleadas en el tratamiento del síndrome de apnea obstructiva del sueño, sus influencias sobre la vía aérea faríngea y efectividad terapéutica. Métodos: Se realizó una revisión de la literatura científica desde diciembre del 2016 a junio del 2017 a través de los buscadores de información y plataformas SciELO, Medline, Pubmed y Hinari. Los descriptores empleados para la búsqueda fueron cirugía ortognática, síndrome de apnea obstructiva del sueño, vía aérea faríngea, avance máxilo mandibular y sus combinaciones. Se obtuvieron 127 artículos de los que se seleccionaron 28 por su contenido, actualidad y objetividad. Análisis e integración de la información: El adelanto quirúrgico del maxilar, mandíbula, mentón o sus combinaciones minimiza los efectos del síndrome de apnea obstructiva del sueño. Es recomendable la cirugía bimaxilar en la corrección de las clases III esqueletales para atenuar posibles efectos perjudiciales sobre la vía aérea por la retroposición mandibular aislada. Conclusiones: La cirugía ortognática de avance, incrementa el espacio faríngeo y mejora la calidad del sueño. Los movimientos de retroposición pueden tener efecto inverso, aspecto importante en la corrección de las clases III esqueletales(AU)


ABSTRACT Introduction: Obstructive sleep apnea syndrome causes daytime sleepiness, affects the cardiovascular system and reduces the quality of life. Since orthognathic surgery brings about changes in the pharyngeal airway space, it has been used to treat this syndrome. Objective: Characterize the orthognathic surgery techniques most commonly used to treat obstructive sleep apnea syndrome, their impact on the pharyngeal airway and their therapeutic effectiveness. Methods: A bibliographic review was conducted from December 2016 to June 2017 using the search engines and platforms SciELO, Medline, Pubmed and Hinari. The search words used were orthognathic surgery, obstructive sleep apnea syndrome, pharyngeal airway, maxillomandibular advancement, and combinations thereof. Of the 127 papers obtained, 28 were selected based on their content, topicality and objectivity. Data analysis and integration: Surgical advancement of the maxilla, mandible, chin or combinations thereof minimizes the effects of obstructive sleep apnea syndrome. Bimaxillary surgery is recommended to correct skeletal class III malocclusion so as to lessen potential damaging effects on the airway caused by isolated mandibular retroposition. Conclusions: Advancement orthognathic surgery broadens the pharyngeal airway space and improves the quality of sleep. The fact that retroposition movements may have an opposite effect is an important aspect to be considered in the correction of skeletal class III malocclusions(AU)

RESUMEN Introducción: Las anomalías dentofaciales son trastornos del crecimiento dental y facial que afectan tanto a niños como a adultos. Objetivo: Presentar un caso clínico, en que en la búsqueda de armonía, por una anomalía dentofacial, se aplicaron principios del manejo multidisciplinario de cirugía ortognática y de otoplastia. Caso clínico: Paciente masculino de 18 años de edad que refiere inconformidad estética y en el que, mediante el análisis de modelos de estudio, mediciones de las telerradiografías indicadas y los hallazgos del examen físico, se llegó al diagnóstico de prognatismo mandibular con exceso vertical del mentón. Se ejecuta tratamiento ortodóncico-quirúrgico, realizándose osteotomía sagital mandibular y mentonoplastia de reducción de altura. Se emplearon como medios de fijación interna los tornillos bicorticales en la zona de ángulo mandibular y miniplacas en la región del mentón. Tres meses después se realizó otoplastia, para corregir la presencia de orejas prominentes, con el logro de un resultado funcional y estético satisfactorio. Conclusiones: Con la aplicación de los principios del manejo multidisciplinario de cirugía ortognática, combinada con la realización de una otoplastia, los resultados fueron positivos. Se alcanzó el objetivo de brindar armonía facial, con mejoría funcional y estética, y una alta satisfacción del paciente(AU)


ABSTRACT Introduction: Dentofacial anomalies are dental and facial growth disorders affecting children and adults alike. Objective: Present a clinical case of dentofacial anomaly in which principles of the multidisciplinary management of orthognathic surgery and otoplasty were applied to achieve harmony. Case report: A male 18-year-old patient reports esthetic dissatisfaction. Analysis of study models, measurements taken by teleradiography and findings of the physical examination led to the diagnosis of mandibular prognathism with a vertically extended chin. Orthodontic-surgical treatment was performed, consisting in sagittal mandibular osteotomy and height reduction mentoplasty. Bicortical screws were used for internal fixation in the mandibular angle area and miniplates in the chin region. Otoplasty was performed three months later to correct the presence of protruding ears, achieving satisfactory functional and esthetic results. Conclusions: Application of the principles of the multidisciplinary management of orthognathic surgery, combined with the conduct of otoplasty, led to positive results. The objective of achieving facial harmony was fulfilled, alongside functional and esthetic improvement and high patient satisfaction(AU)

ABSTRACT

O propósito deste trabalho foi analisar as mudanças bi e tridimensionais na via área superior após procedimentos de cirurgia ortognática. Tomografias pré (T1) e pós-operatórias (T2) foram utilizadas, padronizando pontos craniométricos que serviram de referência para analisar mudanças lineares e volumétricas de diferentes áreas na via aérea superior. Vinte e dois pacientes foram incluídos no estudo, tendo sido submetidos a cirurgia bimaxilar com alteração do plano oclusal. Todos os casos foram planejados virtualmente, e o software Dolphin Imaging (versão 11.9) foi utilizado para análise das medidas lineares, bem como, o cálculo da área da via área superior total, e de suas 3 subdivisões e da região de maior constrição. A via aérea posterior e superior total, foi beneficiada com os movimentos de avanço das cirurgias bimaxilares. Houve correlação positiva entre o valor da constrição pré e pós e o valor total 3D da via aérea pré e pós. Observou-se diferença significativa quando se avalia a média Pré e Pós do seguimento de via aérea total (ST) bi e tridimensionalmente. Encontrou-se uma correlação positiva entre o giro anti-horário do plano oclusal e a ampliação da via aérea. Não foi possível determinar com precisão como cada ponto craniométrico individualmente influenciou e/ou contribui para as mudanças em cada segmento da via aérea analisada. Com bases nos dados da pesquisa a cirurgia ortognática bimaxilar demonstrou aumento das dimensões da via aérea superior, apresentando comportamentos diferentes a depender do segmento da via aérea superior avaliado, e da sua relação com os movimentos dos pontos craniométricos (AU).


The purpose of this work aimed to analyze the bi and three-dimensional changes in the upper airway after orthognathic surgery procedures. Pre (T1) and postoperative (T2) CT scans were used, standardizing craniometric points that served as a reference to analyze linear and volumetric changes in different areas in the upper airway. Twenty-two patients were included in the study, having undergone bimaxillary surgery with alteration of the occlusal plane. All cases were planned virtually, and the Dolphin Imaging software (version 11.9) was used to analyze the linear measurements, as well as the calculation of the area of the total upper area, and its 3 subdivisions and the region most significant. The posterior and upper total airways benefited from the advancing movements of bimaxillary surgeries. There was a positive correlation between the pre and post constriction value and the total 3D value of the pre and post airway. A significant difference was observed when evaluating the pre and post average of the total airway (ST) bi and three-dimensional follow-up. A positive correlation was found between the counterclockwise rotation of the occlusal plane airway's enlargement of the airway. It was not possible to accurately determine how each craniometric point individually influenced and / or contributed to the changes in each segment of the analyzed airway. Based on the research data, bimaxillary orthognathic surgery demonstrated an increase in the dimensions of the upper airway, showing different behaviors depending on the segment of the upper airway evaluated, and its relationship with the movements of the craniometric points (AU).

RESUMEN Introducción: La cirugía ortognática y la rinoplastia son procedimientos consolidados con altas tasas de éxito. La sinergia entre ambos procedimientos ha sido presentada en diferentes investigaciones pero aún no es un procedimiento estándar para pacientes con deformidades faciales. Objetivo: Analizar una serie de casos tratados con cirugía ortognática y rinoplastia y discutir las opciones y fundamentos de la técnica combinada. Material y método: 14 pacientes operados de forma consecutiva fueron incluidos en el estudio. Se identificaron sujetos según el tipo de deformidad facial, el tipo de deformidad nasal y los tipos de cirugías realizadas. El seguimiento mínimo de cada sujeto fue de 12 meses. Resultados: El 50% de los sujetos presentaba deformidad facial de clase III, seguido de deformidades de clase II y I. Las deformidades nasales, de la punta y del puente nasal fueron más prevalentes, siendo el ancho nasal también una deformidad común. En la etapa intraoperatoria, después de realizar la cirugia ortognática, fue necesario realizar un análisis nuevo respecto de la morfologia nasal debido a los cambios que generaba el movimiento maxilar en la condición nasal. Conclusión: La cirugía combinada es sinérgica y viable de ser realizada con bajo número de complicaciones; protocolos y nuevos criterios de análisis son necesarios para obtener mayor predictibilidad en los resultados estéticos.


ABSTRACT Introduction: Orthognathic surgery and rhinoplasty are procedures with high success rate. The synergy between both surgeries has been showed in the past by researches but still they are not standardized procedures for the treatment of patients with facial deformities. Aim: To analyze a case series treated with orthognathic surgery and rinoplasty in the same surgical time, discussing surgical options and techniques in the combined procedure. Material and method: 14 patients were treated consecutively and were included in this research. Subjects were selected by facial deformity, nasal deformity and type of surgery. Minimal follow up was for 12 months to observe results. Results: 50% of subjects showed class III facial deformity, follow by class II and class I facial deformity. In nasal deformities, tip and nasal bridge were more prevalent, being the nasal width a common deformity; in the intraoperative time, after orthognathic surgery, it was necessary to make a new analysis and approach to nasal morphology because the new conditions related to movement of the maxilla. Conclusion: Orthognathic and nasal surgery are good complement and it is possible to do with few complications; surgical protocol and criteria for the analysis are necessaries to obtain more predictability in the esthetic results.

Objetivo: desenvolver, avaliar e correlacionar a aceitabilidade de um aplicativo educativo de celular para pacientes submetidos à cirurgia ortognática. Método: estudo metodológico baseado no design instrucional sistemático com conteúdo de aprendizagem ao paciente por aplicativo de celular. Usabilidade e satisfação do usuário foram avaliadas por 30 pacientes em perioperatório por meio de questionário eletrônico enviado por redes sociais, e-mail e cartão de visita, mensurados com o instrumento System Usability Scale validado em português, e satisfação do usuário com instrumento baseado em outro estudo, utilizado após uso do aplicativo. Os dados foram analisados com estatística descritiva e correlação de Spearman. Resultados: o aplicativo nomeado "OrtogApp" apresenta conteúdo validado em estudo prévio, incluiu cinco sessões de conteúdo de aprendizagem essenciais para gerenciar o cuidado perioperatório e foi disponibilizado nas plataformas IOS e Android. Usabilidade correspondeu a 79,8 + 15,4 pontos e o índice de satisfação 82,9%. A correlação da idade, escolaridade e usos do aplicativo com os instrumentos não foi significativa. Conclusão: OrtogApp é um aplicativo educativo com conteúdo validado por profissionais que resultou em alta satisfação do usuário e boa usabilidade. Os pacientes podem utilizar o aplicativo como material educativo de apoio complementar às orientações dadas por enfermeiras perioperatórias e/ou cirurgiões durante o cuidado perioperatório.


Objective: to develop, evaluate and correlate the acceptability of an educational mobile application to patients submitted to orthognathic surgery. Method: methodological study based on systematic instructional design with contents aimed at patient learning through a mobile application. Usability and user satisfaction were evaluated by 30 patients in the perioperative stage through an electronic questionnaire sent by social networks, e-mail and business card, measured using the System Usability Scale instrument validated in Portuguese and user satisfaction with an instrument based on another study, after its applications. Data were analyzed with descriptive statistics and Spearman correlation. Results: the application named "OrtogApp" features content validated in a previous study included five learning content sessions essential for managing perioperative care, and it is available on IOS and Android platforms. Usability corresponded to 79.8 + 15.4 points and the satisfaction index was 82.9%; correlation of age, schooling and uses of the application with the instruments was not significant. Conclusion: OrtogApp is an educational application with content validated by professionals, resulting in high user satisfaction and good usability. Patients may use the application as supportive educational material to supplement guidance provided by perioperative nurses and/or surgeons during perioperative care.

Facilidad de uso y la satisfacción del usuario se evaluaron para 30 pacientes en el perioperatorio mediante un cuestionario electrónico enviado por redes, correo electrónico y tarjeta de visita social. La medida del instrumento Escala usabilidad Sistema validado en el instrumento portugués y la satisfacción del usuario basado en otro estudio fueron aplicados después del uso de la aplicación. Los datos fueron analizados con estadística descriptiva y correlación de Spearman. Resultados: la aplicación denominada "OrtogApp" presenta contenido validado en estudio previo, incluye cinco sesiones de contenido de aprendizaje esenciales para administrar el cuidado perioperatorio. Era disponible en las plataformas IOS y Android. La facilidad de uso correspondió a 79,8 + 15,4 puntos y el índice de satisfacción fue 82,9%, la correlación de la edad, escolaridad y usos de la aplicación con los instrumentos no fue significativa. Conclusión: OrtogApp es una aplicación educativa con contenido validado por profesionales, que resultó en alta satisfacción del usuario y buena facilidad de uso. Los pacientes pueden utilizar la aplicación como material educativo de apoyo complementario a las orientaciones dadas por enfermeras perioperatorias y/o cirujanos durante el cuidado perioperatorio.

Abstract Facial asymmetry is a condition that compromises function and social interactions and, consequently, the quality of life. Orthodontic-surgical treatment may be indicated to achieve a stable occlusion and significant improvement in facial aesthetics. The virtual planning of the maxillary, mandibular and chin movements can be done prior to surgery. These movements can be successfully performed with the use of prototyped guides obtained from virtual planning. The aim of this article is to show the state of the art of treatments of facial asymmetries, and emphasize how important is the multi-disciplinary approach to achieve predictable aesthetic and functionally stable results in a patient with facial asymmetry and chin protrusion.


Resumo A assimetria facial é uma condição capaz de comprometer a função oclusal e as interações sociais e, consequentemente, a qualidade de vida dos indivíduos. Nessas condições, para se obter oclusão estável e melhora significativa na estética facial, o tratamento ortodôntico-cirúrgico pode estar indicado. A simulação virtual da cirurgia permite planejar de forma adequada, e antecipada, os movimentos cirúrgicos a serem efetuados na maxila, mandíbula e mento. Esses movimentos são, então, realizados com sucesso graças ao uso de guias prototipados obtidos a partir do planejamento virtual. Assim, os objetivos do presente artigo consistem em relatar o estado da arte no planejamento virtual do tratamento de um paciente com assimetria facial e protrusão do mento, e enfatizar a importância da abordagem multidisciplinar para se atingir resultados estéticos previsíveis e funcionalmente estáveis.

Introdução: As deformidades dentofaciais estão associadas às alterações de oclusões dentárias, que podem causar modificações no sistema miofuncional de acordo com o tipo de desproporção. Essas deformidades, podem provocar alterações e/ou adaptações no sistema estomatognático. O objetivo desse estudo foi identificar, com base na literatura arbitrada, a relação entre a Fonoaudiologia e a Cirurgia Ortognática. Método: Foi realizado levantamento bibliográfico sem período específico, nas bases de dados PubMed, SciELO e BVS. Resultados: Foram avaliados 15 artigos de forma crítica, quanto ao tipo de estudo, seus objetivos, número e gênero dos participantes, metodologia, resultados e conclusões. Os artigos foram agrupados com base nos seus objetivos em 1) Grupo Fonoaudiologia (GF); 2) Grupo Mudanças (GM); 3) Grupo Técnicas (GT) e 4) Grupo Outros (GO). Conclusão: Pouco se encontrou sobre a intervenção fonoaudiológica direta, sendo a maioria dos estudos direcionada às mudanças estéticas e funcionais do tecido duro e mole de pacientes submetidos à cirurgia ortognática. Houve também estudos que abordaram métodos e/ou exames específicos para verificar as mudanças nos tecidos moles e duros do perfil facial dos pacientes submetidos à cirurgia ortognática e estudo que detalhou o perfil dos indivíduos que realizaram tratamento para deformidade dentofacial e síndrome da Apneia e Hipopneia do Sono.


Introduction: Dentofacial deformities are associated with changes in dental occlusion, which may cause changes in the myofunctional system according to the type of disproportion. These deformities can cause changes and/or adaptations in the stomatognathic system. The objective of this study was to identify the relationship between speech therapy and orthognathic surgery based on refereed publications. Method: A bibliographic survey was conducted without considering a specific publication period in the databases, PubMed, SciELO and BVS. Results: Fifteen articles were critically evaluated with respect to the type of study, objectives, number and sex of participants, methodology, results and conclusions. Based on their aims, the articles were grouped into 1) speech-therapy group (STG); 2) changes group (CG); 3) techniques group (GT) and 4) others group (OG). Conclusion: There is little information regarding direct speech therapy intervention, and the majority of the studies were directed to the aesthetic and functional changes in the hard and soft tissue in patients who underwent orthognathic surgery. Some studies addressed methods and/or specific examination procedures to verify the changes in the soft and hard tissues of the facial profile of patients who underwent orthognathic surgery, and a study detailed the profile of individuals who underwent treatment for dentofacial deformity and apnea-hypopnea syndrome.

ABSTRACT Objective: to verify the correlation between anxiety level and degree of knowledge in patients before they are subjected to orthognathic surgery. Method: Descriptive cross-sectional study with 40 patients in the preoperative period before orthognathic surgery of a private clinic in the city of São Paulo. Results: IDATE-trait anxiety levels feature prevalence of medium-level anxiety with 72.5% (n=29), followed by low-level anxiety with 72.5% (n=29) and high-level level anxiety with 10% (n=4). In the pre-operative period, transitory IDATE-state anxiety levels feature medium-level anxiety with 65% (n=26), followed by high-level anxiety with 22.5% (n=9) and low-level anxiety with 10% (n=4). Pearson's correlation coefficient resulted in negative r (−0.2) for anxiety-trait (p 0.197) and in negative r (−0.1) for anxiety-state (p 0.417). Conclusion: The data shows a weak correlation in which greater knowledge about the surgical procedure reduces levels of anxiety that may be related to the absence of appropriate guidance about the surgical procedure.


RESUMEN Objetivo: verificar la correlación entre nivel de ansiedad y grado de conocimiento en pacientes en el preoperatorio de cirugía ortognática. Método: Estudio transversal descriptivo con 40 pacientes en preoperatorio de cirugía ortognática de una clínica privada en la ciudad de São Paulo. Resultados: Los niveles de ansiedad IDATE-trazo presentan una prevalencia de ansiedad de medio grado con el 72,5% (n=29), seguido de bajo grado, con el 17,5% (n=7), y alto grado con el 10% (n=4). En el momento preoperatorio, los niveles de ansiedad transitoria IDATE-estado de medio grado con el 65% (n=26), seguido de alto grado, con el 22,5% (n=9), y bajo grado, con el 10% (n=4). La correlación de Pearson resultó en r negativo (−0,2), para ansiedad-trazo (p 0.197) y ansiedad-estado valor r negativo (−0,1) (p 0.417). Conclusión: Los datos demuestran una correlación débil en la que mayor conocimiento sobre el procedimiento quirúrgico reduzca los niveles de ansiedad, lo que puede estar relacionado ausencia de orientaciones adecuadas sobre el procedimiento quirúrgico.

Resultados: Os níveis de ansiedade IDATE-traço apresentam uma prevalência de ansiedade de médio grau com 72,5%(n=29), seguido de baixo grau 17,5% (n=7) e alto grau 10% (n=4). E apresentavam no momento pré-operatório níveis de ansiedade transitório IDATE-estado de médio grau com 65% (n=26), seguido de alto grau 22,5% (n=9) e baixo grau 10% (n=4). A correlação de Pearson resultou em r negativo (−0,2), para ansiedade-traço (p 0.197) e ansiedade-estado valor r negativo (−0,1) (p 0.417). Conclusão: Os dados demonstram uma correlação fraca na qual maior conhecimento sobre o procedimento cirúrgico reduza os níveis de ansiedade que pode estar relacionado ausência de orientações adequadas sobre o procedimento cirúrgico.

Introducción: la cirugía ortognática corrige las alteraciones del componente esquelético en las anomalías dentomaxilofaciales. Estas deformidades provocan deficiente relación de los huesos maxilares, afectando la oclusión dentaria y sus funciones. Objetivo: identificar el comportamiento de la oclusión dentaria en pacientes que fueron intervenidos por cirugía ortognática. Materiales y Métodos: se realizó un estudio observacional descriptivo con el objetivo de identificar características oclusales en pacientes intervenidos por cirugía ortognática. Se utilizaron las variables: relación de molares, guías de función lateral, tipo de masticación, interferencias oclusales en movimiento de protrusión y de lateralidad. Resultados: se observaron pacientes con relación de molares en mesioclusión, lo cual es un indicio de recidiva de la neutroclusión que había sido alcanzada. La intervención quirúrgica de mayor incidencia fue la retroposición mandibular acompañada de centralización. Existió una gran mejoría de la función masticatoria. La mayoría de los pacientes, se encontraban libres de interferencias oclusales. Conclusiones: los examinados se encontraron entre los grupos de edades de 24 a 38 años, y el sexo femenino predominó con un 60,5 %. El 13, 2 % de los sujetos presentaron mesioclusión, con mayor incidencia en los casos intervenidos con retroposición y centralización de la mandíbula. Predominó la función canina en los movimientos de lateralidad. La mayoría de los pacientes se encontraban libres de interferencias oclusales, representando el 71,1 % de los pacientes estudiados.


Introduction: the orthognatic surgery corrects the alterations of the skeletal component in dentomaxillafacial anomalies. These deformations cause deficient relations of the maxillary bones, affecting the dental occlusion and its functions. Objective: to identify dental occlusion behavior in patients undergoing orthognatic surgery. Materials and methods: an observational, descriptive study was carried out with the objective of identifying oclussal characteristics in patients who underwent orthognatic surgery. The used variables were: molars relation, guides of lateral functioning, kinds of chewing, occlusive interferences in protrusion movements and in movements of laterality. Results: patients were found with molars relation of mesiooclussion, which is an indication of recidivism in the achieved neutroclussion. The surgical intervention with higher incidence was mandibular retro positioning accompanied by centralization. There it was a great improvement of the chewing function. Most of the patients were free of oclussal interferences. Conclusions: the assessed patients were in the age-groups from 24 to 38 years, and female sex predominated with 60.5 %. 13.2 % of the subjects had mesiocclusion, with higher incidence in the cases surgically treated with jaw retroposition and centralization. The canine tooth function in the lateral movements predominated. Most of the patients were free from occlusive interferences, being 71.1 % of the studied patients.
