ABSTRACT

El objetivo de este estudio fue el análisis retrospectivo de 81 pacientes sometidos a corrección total de tetralogía de Fallot, realizada en el Instituto Nacional de Cardiología Ignacio Chávez de 1974 a 1989, para analizar los trastornos de conducción y las arritmias que presentaron después del tratamiento quirúrgico. Un total de 64 pacientes (78.9 por ciento) tuvieron bloqueo de rama derecha del haz de His de grado avanzado o intermedio, en 10 pacientes (12.3 por ciento) se asoció a bloqueo auriculoventricular avanzado, que requirió implantación de marcapaso definitivo. Se observaron arritmias ventriculares malignas en 11 pacientes (13.5 por ciento), predomimando a las extrasístoles ventriculares frecuentes, polimórficas, pareadas y, en un caso, taquicardia ventricular monomórfica sostenida. Su origen, por estudio electrofisiológico, era en la cicatrización del tracto de salida del ventrículo derecho y está actualmente controlada con medicación antiarrítmica

ABSTRACT

A retrospective study of 18 consecutive case of SLE was carried out and their clinical and laboratory manifestations evaluated. Ninety four percent of patients were female in the reproductive age group with the mean duration of symptoms at the time of presentation being twenty months. Polyarthritis prolonged fever and haematological disturbances were the three most common presenting features. Six patients had an associated infection complicating SLE. The diagnosis of SLE was confirmed in fifteen patients on the basis of positive anti ds DNA antibody test and in three a renal biopsy substantiated the diagnosis. SLE is underdiagnosed in Pakistan and should specifically be looked for in young females presenting not only with arthritis and skin manifestations, but also when presenting with prolonged fever undiagnosed anaemia, and significant proteinuria. Renal biopsy is a useful diagnostic tool when the anti ds DNA antibody test is negative
