ABSTRACT

Rabdomiólise é uma síndrome que afeta a musculatura estriada esquelética que apresenta um amplo espectro de sintomas clínicos e achados laboratoriais. Decorre, basicamente, da lesão das células musculares, com liberação do conteúdo citoplasmático para a corrente sanguínea e devido a essa situação gerar complicações com alto potencial de morbi-mortalidade, como insuficiência renal aguda. O diagnóstico, baseado em suspeita clínica e em achados laboratoriais, deve ser feito o mais precocemente possível, para buscar a redução das complicações inerentes à síndrome. É relatado caso de paciente com rabdomiólise secundária a exercício físico intenso, que apresentou resolução, sem repercussões de maior morbidade com o tratamento adequado.


Rhabdomyolysis is a syndrome that affects the skeletal striated musculature and presents a wide range of clinical symptoms and laboratory findings. It is primarily a result of injury to muscle cells with release of the cytoplasmic content into the blood flow and thereby generating complications with a high risk of morbidity and mortality including acute kidney failure. The diagnosis, based on clinical suspicion and laboratory findings, must be made as early as possible in order to reduce the complications inherent in the syndrome. Here we report the case of a patient with rhabdomyolysis secondary to intense physical effort, which was successfully resolved with proper treatment without any repercussions of major morbidity.

ABSTRACT

The rosuvastatin inducing rhabdomyolysis in McArdle disease (MD) has not been reported to date. A 35-years-old man had exercise intolerance, muscular fatigue and cramps during physical activity since infancy. He presented severe rhabdomyolysis episode with seizure and coma after use of rosuvastatin. The investigation showed increased serum creatinekinase levels and the forearm ischemic exercise did not increased venous lactate. The muscle biopsy showed subsarcolemmal and central acummulation of glycogen and absence of the myophosphorylase enzyme. The statin induced myopathy is discussed and the danger of its use in MD is emphasized.


Rosuvastatina induzindo rabdomiólise na doença de McArdle (MD) não foi relatada até o momento. Descrevemos o caso de um homem de 35 anos que desde a infância apresentava sintomas de intolerância aos exercícios, fadiga muscular e cãibras durante o esforço físico, porém após o uso de rosuvastatina apresentou episódio de rabdomiólise com crises convulsivas e coma. A investigação mostrou creatinoquinase sérica elevada e teste do esforço isquêmico sem aumento no lactato venoso. A biópsia muscular revelou acúmulo central e subsarcolemal de glicogênio nas fibras e ausência da enzima miofosforilase. Discutimos as estatinas induzindo miopatia, enfatizando o risco do seu uso na MD.

ABSTRACT

La raabdomiólisis es un sindrome causado por la lesión en el músculo esquelético y la resultante liberación del contenido de las células musculares (mioglobina, potasio, fosfato, etc.) dentro del plasma. 5...El diagnóstico de rabdomiólisis queda establecido por una marcada elevación de la CPK sérica mayor a 10.000UI/L. El tratamiento tiene como objetivo prevenir la insuficiencia renal aguda, la cual es la complicación más grave de este sindrome.

ABSTRACT

La rabdomiolisis además de complicar muchas enfermedades críticas, puede requerir manejo intensivo debido a su frecuente asociación a complicaciones que amenazan la vida. La insuficiencia renal aguda (IRA) es una complicación frecuente de rabdomiolisis y puede presentarse después de daño muscular de origen traumático o no traumático y es una complicación frecuente del uso de cocaína. Nosotros presentamos dos casos de rabdomiolisis no traumática con IRA, uno de ellos secundaria a la ingestión compulsiva de alcohol y cocaína dentro de las 12 horas antes de su admisión a la UCI, el otro fue presumiblemente secundaria a una infección viral tipo influenza. Ambos casos sobrevivieron y requirieron diálisis

ABSTRACT

Trauma and non-traumatic insults can cause muscle damage to such an extent that serious sequelae to other organs may result. Myoglobinuria and subsequent acute renal failure [ARF] is a well known and widely studied fact of such sequelae. Twelve cases of ARF [between 1990-1993] who have developed renal dysfunction after prolonged muscular exercise e.g., squat jumping, sit-ups and blunt trauma from sticks or leather belts mainly given by law enforcing personnel for certain issues were studied. None of them had previous history of myopathy, neuropathy or renal disease. All were critically ill on presentation and required renal support in the form of dialysis. Although morbidity was high in all, eleven of them recovered and one expired due to sepsis

ABSTRACT

A hipocalemia severa é causa incomum de rabdomiólise. Descreve-se ocaso de uma paciente de 28 anos com acidose tubular renal distal, que desenvolveu hipocalemia severa com consequente rabdomiólise. O estudo histológico do músculo evidenciou áreas focais de necrose muscular predominando em fibras do tipo II, com discreta reaçäo macrofágica. A melhota clínica e laboratorial apresentada pela paciente ocorreu após a normalizaçäo do potássio sérico, tendo sido fundamental, neste caso, a correçäo da acidose metabólica
