ABSTRACT

Recurrent aphthous stomatitis [RAS] is the most common oral ulcerative disease that affects 20% of general population. The etiology of RAS is unknown and it is due several different mechanisms. The purpose of study was to assess the epidemiology and related factors in its occurrence. In this descriptive -cross sectional study, 1105 persons attending to health centers in Zahedan, selected in a multi-stage random manner, were assessed for abundance and effective factors in development of RAS. The data were gathered as single stage and by interview, completion of a questionnaire and clinical examination. The data were analyzed by Chi-squared test using SPSS software. Among 1105 persons [476 males and 629 females] examined, 199 persons [18%] were affected by aphthea and 168 [84.4%] case had minor, 26 case [13.1%] had major and 5 case [2.5%] had herpetiform aphthea. In this research, there were significant correlations between the abundance of RAS and age [P<0.001], education [P<0.001], job [P=0.002], allergy [P=0.02], kind of allergy [P =0.004], predisposing disease [P =0.005], family history [P< 0.001] and brushing [P<0.001]. No significant differences were found between the abundance of RAS and gender, using snuf, tobacco or mouth wash. ABSTRACT

La úlcera aftosa recurrente (RAU), es una lesión muy dolorosa de la mucosa oral, que afecta entre el 6 y el 20 por ciento de la población mundial. En personas inmunocompetentes es una enfermedad complicada de tratamiento muy difícil. En inmunodeficiencia humana (VIH), RAU es más persistente y severamente debilitante. Las últimas comunicaciones están interesadas en estudios inmunológicos. Objetivos: 1) Investigar en pacientes con úlceras RAU+: la expresión de moléculas de adhesión (MA), en especial los marcadores de activación endotelial y sus ligandos, subpoblaciones leucocitarias, gen anti-apoptosis, células Natural Killer (NK), y macrófagos. 2) Establecer las diferencias entre poblaciones con úlceras, RAU+ (VIH+/VIH-) y sin úlceras, RAU- (VIH+/VIH-). 3) Establecer las diferencias entre las subpoblaciones RAU+, VIH+ y VIH-. Población y métodos: pacientes mayores de 15 años de edad, VIH- y VIH+ (estadío III según CDC). Se obtuvieron biopsias de pacientes RAU+: 8 VIH+ y 15 VIH-, y de pacientes RAU: 2 VIH+ y 8 VIH- (grupo control). La tinción por inmunoperoxidasa fue realizada usando anticuerpos monoclonales primarios para los siguientes antígenos: ICAM-1 (CD54), CD11-CD18, CD56 (NK), CD15 (PMN), CD62 E-selectina, CD31, BM-1 (Lewys Y), BCL-2, CD8, CD68. La intensidad de la inmunotinción fue evaluada semicuantitativamente: Pd: positiva débil, P: positivo (marrón moderado), PP: positiva (marrón intenso), PPP: positiva (marrón muy intenso), E/P: positiva intraepitelial, N: negativa, L: leucocitos, V: vasos, E: epitelio. ABSTRACT

Realizamos estudo microbiológico, coletando amostras de ulceraçöes aftosas recorrentes (UAR) bucais, em 30 pacientes, 18 mulheres e 12 homens, através de "swabs", buscando verificar a participaçäo de bactérias gram-negativas nessas lesöes. Outros 30 pacientes pareados por idade, sexo e estado de saúde bucal, mas com história negativa de UAR foram utilizados como populaçäo controle. Apenas três pacientes do grupo estudo mostraram culturas positivas para gram-negativos (Klebsiella e Enterobacter), enquanto seis pacientes do grupo controle abrigavam enterobactérias. Todas as amostras, tanto do grupo estudo quanto do controle apresentaram crescimento de estreptococos alfa-hemolíticos. Conclui-se que as bactérias gram-negativas näo desempenham papel relevante na expressäo das UAR, uma vez que näo foi possível estabelecer qualquer relaçäo entre as culturas positivas e as características clínicas dos pacientes ou das lesöes em particular. Os estudos devem se aprofundar quanto aos aspectos de identificaçäo, aderência e virulência dos estreptococos alfa-hemolíticos isolados
