ABSTRACT
Lymphagiomas are uncommon benign tumours of tonsils. Lymphangiomas are benign lymphatic tumours typically composed of dialated lymphatic channels which often occur subcutaneously in the head and neck region. Tonsillar lymphangiomas have been reported rarely in the literature which presents as a mass lesion. Histological confirmation is necessary for diagnosis. We report a case of lymphangioma of the tonsil in an otherwise well 17 year old female patient and review the clinical and histological features of this tumour. These lesions are usually cured by simple surgical excision.

Hypertrophic cardiomyopathy with or without left ventricular outflow tract obstruction is characterized by asymmetric hypertrophy of the interventricular septum causing intermittent obstruction of the left ventricular outflow tract. Because Hypertrophic cardiomyopathy is the most common genetic cardiovascular disease, it may present to the anesthesiologist more often than anticipated, sometimes in undiagnosed form during routine preoperative visit. Surgery and anesthesia often complicate the perioperative outcome if adequate monitoring and proper care are not taken. Therefore, a complete understanding of the pathophysiology, hemodynamic changes and anesthetic implications is needed for successful perioperative outcome. We hereby describe the perioperative management of three patients with Hypertrophic cardiomyopathy for different surgical procedures.

A tonsilectomia é uma das cirurgias mais realizadas em todo o mundo. Apresenta uma grande variedade de indicações, seja em adultos ou crianças. É comum o envio do material retirado para exame histopatológico, seja para análise de material suspeito ou como documentação médico-legal de prova de remoção. OBJETIVO: Avaliar a necessidade e o custo do exame histopatológico de rotina para tonsilectomias. METODOLOGIA: Revisado o resultado histopatológico de todas as tonsilectomias no período de 1978 a 2004 num hospital universitário e analisado o prontuário dos casos encontrados de malignidade. RESULTADOS: Um total de 2.103 resultados de análise histopatológica foi analisado. Desta amostra, apenas quatro casos apresentaram algum tipo de malignidade, sendo todas elas linfomas do tipo não-Hodgkin e já suspeitados antes da cirurgia.DISCUSÃO: A literatura mundial encontra resultados semelhantes e cada vez mais se avalia o envio para análise de todos os casos de tonsilectomia. O custo do exame é alto e seu resultado, nos casos de malignidade, pôde ser previsto em todos os casos antes da cirurgia. CONCLUSÃO: Análise histopatológica de todas as tonsilectomias de rotina não está indicada. Os fatores de risco estabelecidos por Beaty deveriam guiar a solicitação de análise histopatológica, para assim conseguirmos diminuir os custos com exames desnecessários.

Neoplasms of follicular dendritic cells are uncommon and while majority of them occur in lymph nodes, they are increasingly recognized at varied sites such as abdominal viscera. Tonsil is the most common extra nodal site for occurrence of FDCT in the head and neck region. We describe three cases of follicular dendritic cell tumour occurring in the tonsil.

Avaliar o significado prognóstico dos níveis linfáticos acometidos por metástases nos carcinomas epidermóides da região tonsilar. OBJETIVO: Definir o tipo de esvaziamento cervical eletivo mais apropriado. FORMA DE ESTUDO: Série de casos. MATERIAL E MÉTODO: 51 pacientes com tumor de região tonsilar tratados entre 1992 e 2001. A incidência de metástases foi avaliada nos diferentes níveis linfonodais, bem como sua relacão com a extensão do tumor primário para os subsítios anatômicos adjacentes. RESULTADOS: Nos pacientes cN0 foram encontradas metástases apenas nos níveis I e II. Entre os pacientes pN+ com metástases no nível I, 6/7 apresentavam extensão da lesão para a cavidade oral. CONCLUSAO: O esvaziamento supraomohioideo (níveis I, II e III) mostrou ser o mais adequado para o tratamento eletivo do pescoco nos tumores da região tonsilar que se estendem para a cavidade oral. Nos casos N0 restritos à orofaringe, o esvaziamento apenas dos níveis II e III mostrou-se a melhor indicacão terapêutica.

We describe a six month old boy with a mass in the left tonsil. It was present since birth and was enlarging progressively. It was completely excised. Histopathology showed a choristoma of neural origin composed of glial cells - astrocytes in a fibrillary background, and a choroid plexus. Immunohistochemistry confirmed the findings

Pleomorphic lipoma is a benign adipocytic tumor, commonly located in the shoulder and posterior neck region. The presence of bizarre, hyperchromatic stromal cells along with multinucleated giant cells often poses difficulty in diagnosis, especially when it arises at an unusual site like tonsillar fossa.

Bone marrow aspiration and biopsy is a useful means of detecting systemic involvement in patients with non haematological malignancy. Metastases of malignant melanoma may be detected in the bone marrow in a small percentage of patients. Two cases of malignant melanoma, with metastasis to marrow at the time of presentation, are described. In one case, bone marrow was the first site where the malignancy was identified. Subsequent investigations revealed an anal melanoma. In the second case, the patient had widespread dissemination from a tonsillar melanoma to many organs of the body, including bone marrow.

Hematogenous spread from carcinoma of tonsil is an uncommon event and skin is an extremely rare site of metastasis. We encountered a 40-year-old male patient who initially presented with carcinoma of the tonsil with T3N2cMO disease and treated by curative radiotherapy. After about 2 years, he developed a skin lesion in the periorbital region which on cytological examination turned out to be metastasis from tonsillar carcinoma. The present paper describes this rare case report along with a brief review of the literature.

A rare benign polypoid tumor of the right palatine tonsil is described in a 23 year old male. It contained dilated lymphatic channels surrounded by fibrous tissue and foci of mature fat. The features of this lesion add support to the hypothesis that benign tumors of tonsil may be hamartomas of tonsil rather than true neoplasms. The differential diagnosis of polypoid lesions of the tonsil is discussed.
