ABSTRACT

La tuberculosis extra pulmonar osteoarticular es una entidad infrecuente (16%), y aquellas de compromiso extravertebral son a su vez muy infrecuentes. Presentamos el caso clínico de un paciente masculino de 68 años de edad, con hipertensión arterial sistémica y enfermedad renal crónica, que presenta aproximadamente 1 año de evolución tórpida con dolor en retropié izquierdo, empeorando luego de infiltración con esteroide, con aumento de volumen, limitación funcional, y lesión ulcerosa con exudado purulento en cara medial. Siendo evaluado de forma clínica y con estudios de imagen por diferentes facultativos, hasta realizar toma de biopsia y cultivo óseo con diagnóstico de Tuberculosis ósea en calcáneo izquierdo. Tratado de manera quirúrgica, y actualmente cumpliendo tratamiento antituberculoso, con evolución satisfactoria recuperando movilidad articular, sin dolor y disminución del volumen de retropié(AU)


Osteoarticular extrapulmonary tuberculosis is a rare entity (16%), and those with extravertebral involvement are in turn very rare. We present the clinical case of a 68-years-old male patient with systemic arterial hypertension and chronic kidney disease, who presents approximately 1 year of torpid evolution with pain in the left hindfoot, worsening after steroid infiltration, with increased volume, limitation functional, and ulcerative lesion with purulent exudate on the medial side. He was being evaluated clinically and with imaging studies by different doctors, until taking a biopsy and bone culture with a diagnosis of Bone Tuberculosis in the left calcaneus. Surgically treated, and currently undergoing antituberculous treatment, with satisfactory evolution recovering joint mobility, without pain and decrease in rearfoot volume(AU)

ABSTRACT

La osteomielitis hematógena es frecuente en la metáfisis de los huesos largos en la infancia; siendo esta misma afectación rara en otras localizaciones y más aun, extremadamente rara en el astrágalo, tanto así que se reportan solo unos pocos casos y de manera esporádica en la literatura mundial. En este trabajo presentamos un caso clínico, de una paciente preescolar que presento osteomielitis hematógena de astrágalo cuyo tratamiento médico y quirúrgico representaron y aun representan todo un reto para el ejercicio clínico debido a la dificultad del diagnostico etiológico y tórpida evolución.


Hematogenous osteomyelitis is frequent in the metaphysis of long bones in childhood, being the same affection rare in other locations and even more, extremely rare in the talus, so much so that only a few reported sporadic cases and literature world. We present a clinical case of a patient presenting preschool hematogenous osteomyelitis of the talus which medical and surgical treatment represented and still represent a challenge for clinical practice because of the difficulty of diagnostic and torpid evolution.

ABSTRACT

Introducción: La tuberculosis osteoarticular se observa en la columna, la rodilla y la cadera y, menos a menudo,en las vainas tenosinoviales, y en los huesos de la mano y la muñeca. El objetivo del presente trabajo es analizar esta patología y su tratamiento, y valorar la técnica de reconstrucción simultánea a largo plazo. Materiales y métodos: El estudio abarca a 19 pacientes tratados en 45 años, correspondientes a: 3 espinas ventosas, 10 tenosinovitis de flexores, 4 tenosinovitis de extensores y 2 artritis de muñeca. Empleamos una metodología diagnóstica moderna y el estudio retrospectivo de los pacientes, sus tratamientos y resultados. Resultados: El tratamiento farmacológico correcto es eficaz en 99 por ciento de los casos para la infección tuberculosa. El quirúrgico acelera la curación y consiste en la resección de los tejidos necróticos y la reconstrucción funcional, primaria o secundaria. La curación de la infección se logra siempre; la recuperación funcional estará en relación con el tipo, la. gravedad y la extensión de la lesión. Conclusiones: Se demuestra la frecuencia de estas patologías en los trabajadores de la carne o que están en contacto diario con hacienda. Se destacan los procedimientos modernos de diagnóstico. Se enfatiza en un tratamiento médico completo en calidad, duración y tipo de fármacos. Se destacan las bondades de los tratamientos quirúrgicos y de la reconstrucción funcional simultánea que propusimos en 1965.

ABSTRACT

Objetivo: Descrever um caso de infecção por Mycobacterium tuberculosis, comprometendo ombro esquerdo de paciente do sexo masculino, com 24 anos de idade, que referia dor constante, difusa, sem irradiações e fatores de melhora ou piora. A dor se iniciou espontaneamente há três meses, sem história traumática. Métodos: A radiografia de ombro esquerdo não revelou anormalidades e a ressonância magnética da articulação indicou imagem sugestiva de lesão do lábio superior da glenoide. Após tratamento inicial conservador com uso de anti-inflamatório não hormonal e fisioterapia, sem sucesso, foi realizada artroscopia da articulação acometida, desbridando os tecidos inflamados e realizando-se ressecção de material para exame anatomopatológico, o qual teve como resultado granulomas sugestivos de infecção por Mycobacterium tuberculosis. O diagnóstico foi confirmado por pesquisa de infecção por micobactérias. Foi realizada antibioticoterapia específica para tuberculose, havendo regressão completa do quadro após o término do tratamento. Conclusão: O trabalho destaca a importância do método artroscópico no diagnóstico e tratamento dessa lesão do ombro.

ABSTRACT

Epiphyseal extension of benign pathology is regarded as an infrequent occurrence. This observation has been attributed to anatomical and biochemical phenomenon unique to physeal cartilage. We report a retrospective series of six patients over a period of 4 years, diagnosed with benign pathologies that showed crossing of an open physeal plate by the disease. Four of these cases were infections and two were benign tumors. The patients were aged between 5 and 11 years, all of them were treated at a tertiary referral centre and followed up for a minimum period of 6 months to evaluate the progress of disease. The findings are more than just a pathological curiosity as they alter the management and surgical procedure that needs to be performed for these conditions. The recognition of the fact that benign tumors may occasionally present with transphyseal spread will prevent unjustified radical procedures that are best reserved for aggressive malignant conditions.

Spina Ventosa is a rare condition. A rare case of disseminated tuberculosis of bones and skin without primary foci is presented. Gross sclerosis of the short bones of hand and leg were noted.

ABSTRACT

Tuberculous osteitis of clivus is rare. Its diagnosis is difficult because of the rarity of the site and the non-specific nature of the disease. Management consists of confirmation of diagnosis by biopsy and chemotherapy with anti-tubercular drugs. Meningitis may complicate the clinical course, increase morbidity and mortality.

ABSTRACT

A study was conducted on 70 cases of tuberculous synovitis. Maximum number of cases were in the age group of 11-30 years (58.58%) with a male preponderance. Knee joint was found to be most commonly (57.12%) affected. Diagnosis of tuberculous synovitis was possible clinically in 75.72% and radiologically in 67.14% of cases only. Histopathological studies made on biopsy materials obtained from synovial membranes of the affected joints showed the presence of fibrosis, lymphocytic infiltration and epithelioid cells forming typical granuloma of tuberculous lesion. Proliferation of synovial membrane (92.85%), increased number of mast cells (82.95%) and caseation (78.57%) were observed. In all the cases the synovial membrane was of greyish white appearance.

ABSTRACT

Rib tuberculosis is an extremely rare condition with the incidence not exceeding 3 percent of all skeletal tuberculosis. The authors experienced a recalcitrant case of pulmonary tuberculosis accompanied by chest wall cold abscesses involving ribs recurring at a new site in approximately 10 months despite of medical and surgical treatment. The patient has twice taken thoracotomy for abscess drainage and during the second thoracotomy, a partial resection of involved ribs was performed

ABSTRACT

Se presenta un paciente de sexo masculino, de 62 años de edad, que comenzó su enfermedad con una lesión sobreelevada roja de 0,5x0,5 cm de diámetro en dorso de mano izquierda, de aspecto verrugoso que fue creciendo paulatinamente en forma centrífuga, con atrofia en la zona central y con desarrollo de múltiples bocas fistulosas. Simultáneamente se observó compromiso progresivo y destrucción de la articulación metacarpo falángica. Se efectuaron los correspondientes estudios de laboratorio Rx., biopsias y cultivos, encontrándose como resultado BAAR por lo que se diagnosticó una TBC ósteoarticular con compromiso cutáneo. Se realizó el tratamiento antituberculoso correspondiente, observándose la mejoria cutánea, pero el empeoramiento del cuadro ósteoarticular. Se realiza una revisión del tema, haciendo hincapié en el papel que juega el dermatólogo en el diagnóstico precoz y efectividad terapéutica futura en la TBC ósteoarticular, a partir de sus manifestaciones cutáneas. Se hace referencia, en esta variedad de TBC, a la dificultad que se tiene para confirmar el diagnóstico y la necesidad de instituir un tratamiento médico-quirúrgico temprano si se quieren evitar terapéuticas quirúrgicas invalidantes
