Purpose: To document whether the residency training in management of retinal diseases has improved in 2000s to meet the increasing demand of retina care in India. Methods: A survey, using a prevalidated questionnaire, was conducted by Academic and Research Committee (ARC) of the All India Ophthalmological Society (AIOS) in 2014–2016 among ophthalmologists to document teaching of retina-related clinical and surgical skills in the postgraduate residency program. Results: The 144-item questionnaire was mailed to 4512 practicing ophthalmologists with residency training in two different periods, between 1967 and 2000 (group 1; 20th-century trained) and between 2003 and 2012 (group 2; 21st-century trained). Response was received from 320 (19.1%) of group 1 ophthalmologists and 531 (18.7%) of group 2 ophthalmologists. The average age was 49.2 ± 8.7 and 32.6 ± 4 years, respectively. Group 2 residents had received superior training in indirect ophthalmoscopy, slit lamp biomicroscopy using + 78 and + 90D lens, optical coherence tomography, fundus photography, and fluorescein angiography (all P < 0.001), but there was large variation between the training institutions. The residents were not taught vitreous and retinal detachment surgeries in either period of training. Conclusion: Teaching of retina-related clinical skills have improved in Indian residency program, but there are variations across programs. This information might help redesign the ophthalmology residency programs to meet the demands of comprehensive eye care and universal health coverage of increasing retinal diseases in India.

We were intrigued to analyze donor eyes of two individuals without retinopathy even after 40 years of type 2 diabetes mellitus. Targeted molecular factors associated with angiogenesis and the key antioxidant enzymes in retinal tissue were analyzed. Accordingly PEDF, Adiponectin and Paraoxonase 2 showed augmented mRNA expression in both the retina with no significant change in VEGF expression. Vitreous showed increased PEDF protein in donor 1 and Adiponectin in donor 2 with no change in VEGF protein. This study highlights the profile of specific molecular factors that contribute to the non-development of diabetic retinopathy changes in these individuals.

A 42-year-old man with the complaints of left knee swelling, severe pain, difficult to stand was reportedto Siddha Varmam division after a road accident. He was diagnosed as acute avulsion of Posterior cruciateligament (PCL). It was diagnosed based on the history of trauma, knee pain and swelling after trauma,positive posterior drawer test and avulsion fracture shown by radiograph. He was treated with SiddhaVarmam therapy and traditional bone setting. After a month of treatment, the PCL avulsion fracture gothealed without any surgical interventions and patient able to walk normally. Aim: The aim of the study was to correlate the histological changes in cotyledon and embryonic axis in aged seeds of soybean genotypes with the physiological changes and storability. Methodology: Ten genotypes with differing, storability as predicted by accelerated ageing test, were selected and various physiological parameters viz., germination per cent, seedling length and dry weight, seedling vigour, mobilization efficiency were studied over a period of ten months at bi-monthly interval. These were correlated with histological changes like cell arrangement and cell membrane integrity in cotyledon and embryonic axis of seed. The findings were further supported by electrical conductivity test. Results: Good storer/performer (DSB 21) maintained comparatively better cell membrane integrity in both cotyledon and embryonic axis at the end of ten months of storage when compared to poor storer/performer (MAUS 61). Interpretation: Maintenance of cell membrane integrity in good storer genotype, as revealed by scanning electron microscopy, as result of better tolerance mechanism for biochemical degradation, is the main reason for higher physiological performance of all the good storer/performers genotypes during storage.

Background: In India, about 1 million people are bitten by snakes more in the rural area and they get admitted to hospitals. About 50,000, people die of snake bites every year in India. The patients admitted with poisonous snake bites were treated with polyvalent anti snake venom serum. Aim: To determine the significant difference and outcome between High Dose and Low dose Polyvalent Anti Snake Venom Serum (PVASVS), with regards to age gender and severity of coagulation abnormalities. The cost of ASV has an economic impact and if low dose ASV saves the scarce resource. Materials and Methods: 45 patients bitten by snake admitted and treated which included both males and females were taken up for this study based on Poisson model of sampling. All the patients had cellulitis, renal failure and varying degree of coagulation abnormalities. They were classified into LDG and HDG depending on the number of PVASVS they received which was ≤25 vials or >25 vials respectively. This was decided based on the treatment of ASV analyzed using chi-square contingency statistically for its significance. Results: The age and clotting time as such do not have association to distinguish HDG or LDG. It was revealed that Gender has an effect that female HDG are 26.3% (5 out of 19) as compared to male 73.7% (14 out of 19). In each group, 2 patients died out of 18, LDG (11%) 27 in HDG (7.4%) 62.9% of patients in LDG and 38.8% in HDG required hospital stay more than 10 days. Conclusions: The female gender falls in low dose conserving ASV. The LD ASV demands higher duration of stay (more than 10 days) in the hospital. The age, coagulation abnormality did not establish the significant results in the LDG and HDG.

Background and Objectives: Myoepithelial cells (ME) are known to contribute in the patterning of salivary gland neoplasms (SGN) and possess cytoplasmic smooth muscle actin (SMA) revealed by alpha SMA (?-SMA). The present study aimed to assess the expression of ?-SMA in selected benign and malignant SGN (pleomorphic adenoma printarticle.asp?issn=0377-4929;year=2018;volume=61;issue=4;spage=479;epage=484;aulast=Ravi, mucoepidermoid carcinoma (MEC), adenoid cystic carcinoma (ACC), and polymorphous low-grade adenocarcinoma (PLGA). Materials and Methods: The intensity and pattern of expression of ?-SMA were studied in 25 cases of SGN's ACC (n = 7), MEC (n = 8), PA (n = 8), and PLGA (n = 2), and correlated with the histological patterns. Results: Maximum expression of ?-SMA in the epithelial compartment was seen in ACC, followed by PA, whereas MEC and PLGA showed completely negative staining. The connective tissue expression was mild in ACC and MEC. The myxoid stroma of PA with “melting” pattern was weakly positive for ?-SMA. The stroma in PLGA showed complete negativity. In ACC, ?-SMA-positive cells were lining the cribriform spaces, small islands, and dispersed within large islands. Small nests showed complete positivity for ?-SMA. Interpretation and Conclusion: In ACC, ?-SMA expression supports the involvement of ME in epithelial organization explaining the histological patterns seen. In PA, the expression correlates with the predominantly secretory nature of ME. The absence of epithelial positivity in MEC and PLGA suggest that ME has less role to play in their histogenesis. The weak stromal positivity observed in MEC and ACC may be attributed to the positive immunoreactivity of myofibroblasts playing a role in modulating the course of SGN's.

Polypoidal choroidal vasculopathy (PCV) is increasingly recognized as an important cause of exudative maculopathy in Asians as against Wet age-related macular degeneration in Caucasians. A panel of retinal experts methodically evaluated pertinent updated literature on PCV with thorough PubMed/MEDLINE search. Based on this, the panel agreed upon and proposed the current consensus recommendations in the diagnosis (clinical and imaging), management and follow-up schedule of PCV. Diagnosis of PCV should be based on the gold standard indocyanine green angiography which demonstrates early nodular hyperfluorescence signifying the polyp with additional features such as abnormal vascular network (AVN). Optical coherence tomography is an excellent adjuvant for diagnosing PCV, monitoring disease activity, and decision-making regarding the treatment. Current treatment modalities for PCV include photodynamic therapy, anti-vascular endothelial growth factor agents, and thermal laser. Choice of specific treatment modality and prognosis depends on multiple factors such as the location and size of PCV lesion, presence or absence of polyp with residual AVN, amount of submacular hemorrhage, presence or absence of leakage on fundus fluorescein angiography, visual acuity, and so on. Current recommendations would be invaluable for the treating physician in diagnosing PCV and in formulating the best possible individualized treatment strategy for optimal outcomes in PCV management.

Purpose: To gauge the differences in ophthalmology residency training, academic, clinical and surgical, in the last three decades of the 20th century and the first decade of the 21st century. Methods: A survey was conducted by the Academic and Research Committee of the All India Ophthalmological Society, in 2014�16, using a prevalidated questionnaire, which was circulated to ophthalmologists to gauge the practicality of the teaching protocols of clinical and surgical skills during postgraduate residency program. Results: Of the 1005 respondents, 320 ophthalmologists who completed residency between 1967 and 2002 (20th century trained) and 531 who completed a residency in 2003�12 (21st century trained) fulfilled the inclusion criteria. The average age was 49.2 years (standard deviation [SD] 4) and 32.6 years (SD 4), respectively. Twenty-first century trained ophthalmologists rated their training significantly better than the 20th century trained ophthalmologists for slit lamp examination (P = 0.001), indirect ophthalmoscopy, gonioscopy, automated perimetry, optical coherence tomography, and fundus photography (all having P < 0.001), while the 20th century trained rated their teaching of refraction, synoptophore, diplopia charting better (all P < 0.001). The range of grading was 0� in all categories. The median number of surgeries performed independently by 20th century and 21st century trained (during their training period) were: intracapsular cataract extraction (ICCE) 10, 0; extracapsular cataract extraction (ECCE) 43, 18; small incision cataract surgery (SICS) 5, 55; phacoemulsification (Phaco) 0, 1; pterygium excision 20, 15; dacryocystectomy 11, 4; dacryocystorhinostomy 11, 2; chalazion 35, 30; trabeculectomies 5, 0; strabismus correction 0, 0; vitrectomy 0, 0; keratoplasty 0, 0; eyelid surgery 6, 2; and ocular emergencies 18, 20. Conclusion: Teaching of many clinical skills had improved over decades. Cataract surgery training has shifted from ICCE and ECCE to SICS and Phaco, but other surgeries were still taught sparingly. There was an enormous variation across the country in residency training which needs immediate attention.

A survey was carried out among the Kani tribals in Kaniyakumari district- Southern Western Ghats, of Tamil Nadu, India. Traditional uses of 62 plant species belonging to 34 families are described under this study. In this communication, the informations got from the rural inhabitant were compared with the already existing literature. The medicinal plants were mostly used to cure asthma, rheumatoid arthritis, skin and sex related diseases. The medicinal plants used by people are arranged alphabetically followed by Botanical name, family, Voucher specimen number, Local name, Part used, mode of preparation.
