RESUMO
Com o objetivo de restringir o número de diagnósticos diferenciais das neoplasias ovarianas sólidas, apresentamos uma análise racional e sistematizada, levando se em consideração fatores epidemiológicos, relevância clínica e aspectos radiológicos, através de revisão bibliográfica e análise retrospectiva de casos. Os critérios utilizados foram: a) faixa etária; b) quadro clínico e laboratorial; c) unilateralidade ou bilateralidade; d) homogeneidade/heterogeneidade; e) comportamento de realce; f) comportamento de sinal na ressonância magnética; g) sinais associados. Foram considerados , entre os diagnósticos diferenciais, os seguintes tunores de ovário: a) teratoma, b) tunor de Brenner; c) disgerminoma; d) fibroma/fibrotecoma; e) tumor de células da granulosa; f) tumor estromal esclerosante; g) tumores metastáticos. Ao se avaliar uma lesão sólida de ovário, é possível propor algumas alternativas diagnósticas pertinentes, fundamentadas em uma análise sistemática dos principais aspectos epidemiológicos e de imagem do tumor.
ABSTRACT
With the objective to substantially narrow the number of differential diagnosis of solid ovarian tumors, we present a rational and systematical analysis, based on epidemiological findings, clinical relevance and radiological features , guided by bibliographical revision and retrospective analysis of the cases. The criteria used have been: a) age; b) clinical and laboratorial findings; c) unilateral or bilateral; d) homogeneity/heterogeneity; e) enhancement; f) signal behavior in magnetic resonance imaging; g) associated findings. The following ovary tumors have been considered in the differential diagnosis: a) teratoma; b) Brenner tumor; c) dysgerminoma; d) fibroma/fibrothecoma; e) granulosa cell tumor; f) esclerosing estromal tumor; g) metastatic tumors. During the evaluation of a solid ovary tumor, it is possible to consider some important diagnostic alternatives, based on a systematic analysis of the main epidemiological and imaging findings.
