RESUMO

Erythema multiforme is a cutaneous reaction pattern precipitated by varied agents, notably herpes simplex and drugs. It predominantly occurs in adolescents and young adults but may be seen at other ages also. While vaccination is rarely a precipitating factor for erythema multiforme, it may occasionally be seen in infants and children. We report here a case of a two month-old infant with lesions of erythema multiforme minor appearing after two weeks following vaccination for DPT, Hepatitis B and influenza.

OBJECTIVE: To report a case of simultaneous erythema nodosum and erythema multiforme after local lidocaine injection. CASE SUMMARY: A 33-year-old female experienced coexisting erythema nodosum and erythema multiforme after lidocaine spray was used for upper gastrointestinal endoscopy. The reaction was exacerbated after localized injection of 2% lidocaine for a skin biopsy. An objective causality assessment revealed that an adverse drug reaction was highly probable. DISCUSSION: Lidocaine is the most frequently used local anesthetic agent. The coexistence of erythema nodosum and erythema multiforme has been reported in lepromatous leprosy, Yersinia enterocolítica and Yersinia pseudotuberculosis gastrointestinal infection (both with diarrhea), histoplasmosis, hepatitis C, and milkers nodule, but not with lidocaine. CONCLUSIONS: If clinicians observe similar lesions after lidocaine exposure, they should suspect an adverse drug reaction and exercise caution in the administration of amide anesthetics to patients with a documented history of adverse reaction to lidocaine.

RESUMO

Two cases of tuberculoid leprosy who developed erythema multiforme bullosum (EMB) due to Dapsone (DDS) is reported. Burning and itching sensations were found to be the prominent prodromal symptoms. The patients gave history of urticaria and bronchial asthma. Salient clinical features and further management of the cases by desensitization with slow induction to DDS under cover of steroids and antihistamines have been discussed.

RESUMO

A case of Erythema Multiforme Bullosum in patient of lepromatous leprosy with pulmonary tuberculosis due to Rifampicin is described. It is stressed that ethambutol may act as a trigger factor to the toxic effects of Rifampicin.
