RESUMO

Lichenoid tissue reaction or interface dermatitis embrace several clinical conditions, the prototype of which is lichen planus and its variants, drug induced lichenoid dermatitis, special forms of lichenoid dermatitis, lichenoid dermatitis in lupus erythematosus, and miscellaneous disorders showing lichenoid dermatitis, the salient clinical and histological features of which are described to facilitate their diagnosis. Background of lichenoid reaction pattern has been briefly outlined to enlighten those interested in this entity.

BACKGROUND: Current evidence suggests that lichen planus is an immunological disease. Cytotoxic CD8+ cells in the lesional epidermis recognize a unique antigen called lichen planus specific antigen. This antigen could be demonstrated by indirect immunofluorescence using the patient's serum and autologous lesional skin. AIM: To study indirect immunofluorescence pattern in lichen planus, among Indian patients. METHODS: Twenty-five consecutive patients with the clinical diagnosis of lichen planus were enrolled in the study. Direct immunofluorescence was done in all patients. Indirect immunofluorescence using lesional skin as substrate was done in all 25 patients and five patients with other dermatoses. RESULTS: A specific fluorescence pattern corresponding to the distribution of lichen planus specific antigen was observed in the stratum spinosum and granulosum in 22 (88%) patients. It was absent from other parts of the epidermis, dermis and in patients with other dermatoses. CONCLUSION: Indirect immunofluorescence is a useful adjuvant test in lichen planus, particularly in atypical cases.

RESUMO

Lichen planus of the eyelid alone is a rarely reported condition with just about 11 cases reported so far in the international literature. We report a case of lichen planus of single lower eyelid with characteristic histopathologic findings on eyelid biopsy. It is unusual to encounter a usually characteristic and often widespread entity like lichen planus occurring at only one site like an eyelid. Lichen planus of the eyelid should be included in the differential diagnosis of eyelid dermatitis.

RESUMO

Thalidomide has gained an infamous history due to severe birth defects observed in patients who had taken the drug to control nausea during pregnancy. The medication was withdrawn from the market because of its teratogenicity, but was approved by the FDA in 1998 for the treatment of erythema nodosum leprosum. However, thalidomide has been employed with success by dermatologists for a host of off-label uses including the treatment of lichen planus. Currently, no clinical trials or studies exist to evaluate the efficacy of using thalidomide to treat lichen planus, but case reports have been published in the medical literature supporting its therapeutic benefits. TNF-alpha is among the many cytokines that have been implicated in the pathogenicity of lichen planus. It is thought that thalidomide acts.

RESUMO

Immunoperoxidase techniques provide the pathologist with the capability for staining a wide range of antigens in tissue sections. More than 100 different antigens have been successfully demonstrated in fixed paraffin sections; other antigens can only be visualized in frozen sections. This latter group particularly includes lymphocyte surface antigens detectable by monoclonal antibodies. This review describes the current state of the art and provides several illustrations of the use of monoclonal antibodies for the identification of T-lymphocyte phenotypes in frozen section from cases of leprosy, mycosis fungoides, halo nevus, Kaposi's sarcoma, lichen planus and atopic dermatitis. Technical details and potential applications are discussed. The growing availability of commercial immunostaining kits makes these techniques more accessible to the surgical pathologist; indeed a whole new range of truly specific, special stains are available, as pathologists we must simply learn to use them.

RESUMO

O autor apresentou à Sociedade Paulista de Leprologia em sua sessão ordinária de 14 de Agosto de 1948, um caso de liquen plano que surgiu em uma doente de lepra em tratamento pelo promin. As lesões se assentavam na fronte, regiões malares, nariz, semi-mucosas dos lábios e nos antebraços. Explicou o seu aparecimento pelo fenômeno biotrópico de Milian, (lues). O quadro histológico correspondia ao classicamente verificado.
