RESUMEN

Digestive tract diseases, especially digestive tract tumors, including liver cancer, pancreatic cancer, and colorectal cancer, have high incidence in China. Digestive tract tumor is one of the top 10 cancers in terms of the number of new cases and deaths in the world, and the incidence and mortality of tumor diseases have been increasing year by year. Therefore, the prevention and treatment of tumors is particularly important. With the application and promotion of traditional Chinese medicine in the medical field and the rapid development of molecular biology and pharmacology, more and more potential active components of Chinese medicinal materials have been extracted and studied. These active components can inhibit tumor cells in a multi-target and multi-pathway manner. Cinobufotalin is an effective component extracted from the skin of Bufo bufo gargarizans. It has been prepared into a variety of agents with anti-tumor, immunomodulatory, cardiac boosting, pain-easing, anti-inflammatory, and swelling-relieving activities. In clinical practice, cinobufotalin is mainly used to assist the treatment of liver cancer, lung cancer, colorectal cancer, gastric cancer and other malignant tumors, which can reduce the adverse reactions of patients in the middle and late stages and improve the quality of life and five-year survival rate of patients. The available studies of molecular mechanism have demonstrated that cinobufotalin can play a therapeutic role by inducing cell apoptosis, regulating cell cycle, inhibiting cell proliferation and angiogenesis, modulating immune response, reversing multidrug resistance, enhancing radiochemotherapy sensitivity, inhibiting tumor inflammation, invasion, and metastasis, etc. This review focuses on the clinical application and mechanism of cinobufotalin against digestive tract tumors in recent years, aiming to provide a theoretical basis for the anti-tumor research of cinobufotalin, promote the application of cinobufotalin in tumor treatment, and facilitate the further research and development of this compound.

RESUMEN

Traditional Chinese Medicine (TCM) external therapy for sleep disorder of chronic fatigue syndrome (CFS) has good anti-fatigue effect and can improve sleep quality of patients. The treatment for sleep disorders of CFS with TCM external treatment mainly adopts acupuncture, moxibustion, massage, TCM bath, transcutaneous acupoint electrical stimulation and auricular point sticking, etc., or alone, or comprehensive application, or combined with oral Chinese materia medica. The appropriate treatment method can be selected according to the patients' condition and compliance, which reflects the unique advantages of TCM syndrome differentiation and treatment and the treatment according to people and time. The existing research still needs to further form a standardized and recognized diagnosis and treatment system, so as to better guide clinical popularization and application.

AIM: To explore the mechanism of fructus lycii in treating dry eye based on network pharmacology and experimental verification.METHODS: Taking “fructus lycii” as key words, the active ingredients and target of fructus lycii were searched by using Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP). Gene targets related to dry eye(DE)were searched by GeneCards and OMIM databases. The target genes of fructus lycii and DE were imported into Venn software to obtain the intersection target map of them. After that, the data were imported into the String database to obtain the PPI protein-protein interaction network diagram. Using Cytoscape3.7.2 software, the PPI protein-protein interaction network diagram was constructed for active ingredients, target sites and related diseases of fructus lycii. The Bioconductor platform and R language were used for gene ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analysis. And the key targets in the pathogenesis of DE were verified by experiments.RESULTS: Through TCMSP, 45 types of effective chemical components of fructus lycii, 174 target genes corresponding to active components and 131 common target genes with DE were screenedout. In accordance with the network topology of “drug-composition-disease-target”, 27 main effective components of fructus lycii were found in the treatment of DE. The PPI network was analyzed according to the high degree value, which is the key targets of fructus lycii for DE treatment, mainly including AKT1, VEGFA, CASP3, IL1B, JUN, PTGS2, CXCL8, etc. According to GO enrichment analysis, 166 biological functions and processes of fructus lycii for DE treatment were obtained. KEGG enrichment analysis showed that 31 signaling pathways were involved. Additionally, experimental verification displayed that the protein expressions of AKT1, interleukin-6(IL-6), tumor necrosis factor(TNF-α)and IL-17 in conjunctiva tissue of the DE model group were significantly increased.CONCLUSIONS: Through network pharmacology, this study confirmed that the treatment of DE by fructus lycii is a complex process involving multi-components, multi-targets and multi-pathways, and that the treatment of DE by fructus lycii is mainly regulated by anti-inflammatory and apoptosis-related molecules.

Objective:To explore the correlation of serum cystatin C (CysC), serum and glucocorticoid-regulated kinase 1 (SGK1) and homocysteine ??(Hcy) with postoperative lymph node metastasis in patients with lung cancer and their predictive values.Methods:One hundred and thirty-one patients with stage Ⅰ-Ⅲ A non-small cell lung cancer (NSCLC) who underwent tumor resection and systematic lymph node dissection in Meishan Hospital of Traditional Chinese Medicine from November 2016 to June 2018 were prospectively selected. Patients received a 3-year follow-up after surgery, and were classified into metastasis group (42 cases) and non-metastasis group (89 cases) according to the presence or absence of lymph node metastasis during the follow-up period. Serum CysC, SGK1 and Hcy levels were detected at the 1st day after surgery, and the levels of the three indicators were compared among patients with different TNM stages, differentiation degrees and histological types. Meantime, the clinicopathological characteristics and levels of the three indicators were also compared between metastasis group and non-metastasis group. Spearman analysis was conducted to discuss the correlation between the three indicators and clinicopathological characteristics of patients. Multivariate logistic regression analysis was performed to screen the factors affecting postoperative lymph node metastasis (the median levels of CysC, SGK1 and Hcy were used as the cut-off values, > the median level was a high level). Taking the pathological examination results as the gold standard, receiver operating characteristic (ROC) curve was applied to evaluate the predictive value of level of the three indicators alone or in combination for postoperative lymph node metastasis. Results:The serum levels of CysC, SGK1 and Hcy in patients with TNM stage Ⅲ A were higher than those in patients with stageⅠ-Ⅱ; the serum levels of CysC, SGK1 and Hcy in patients with poorly differentiated tumors were higher than those in patients with medium and well-differentiated tumors; the serum levels of CysC, SGK1 and Hcy in patients with non-squamous cell carcinoma were higher than those in patients with squamous cell carcinoma; the differences were statistically significant (all P < 0.05). Spearman correlation analysis showed that serum CysC, SGK1 and Hcy levels were correlated with TNM stage ( r values were 0.454, 0.672 and 0.645), differentiation degree ( r values were -0.399, -0.403 and -0.451), histological type ( r values were 0.528, 0.760 and 0.611) (all P < 0.001). Compared with non-metastasis group, an elevation was found in serum levels of CysC, SGK1 and Hcy in metastasis group [(1.37±0.30) mg/L vs. (1.16±0.25) mg/L, (53±4) pg/ml vs. (41±3) pg/ml, (18.3±2.3) mol/L vs. (15.4±1.8) mol/L] (all P < 0.001). Multivariate logistic regression analysis showed that TNM stage Ⅲ A ( OR = 2.944, 95% CI 1.556-6.847, P = 0.004) and high level of CysC (> 1.23 mg/L, OR = 2.431, 95% CI 1.402-5.226, P = 0.008), high level of SGK1 (>50 pg/ml, OR = 4.010, 95% CI 1.815-11.748, P = 0.004), high level of Hcy (> 16.8 μmol/L, OR = 3.742, 95% CI 1.747-9.142, P = 0.001) were independent risk factors for postoperative lymph node metastasis. ROC curve analysis showed that for predicting postoperative lymph node metastasis, the area under the curve (AUC) of serum CysC, SGK1 or Hcy level alone was 0.769, 0.808 and 0.816, the AUC of CysC+Hcy, CysC+SGK1 and Hcy+SGK1 was 0.889, 0.890 and 0.910, and the AUC of the three indicators was 0.936. Conclusions:Levels of serum CysC, SGK1 and Hcy in NSCLC patients with postoperative metastasis are higher than those in patients without metastasis, and the levels of the three are positively correlated with the TNM stage and histological type, and negatively correlated with the differentiation degree. The combined detection of the three has good predictive value for postoperative lymph node metastases in NSCLC patients.

Objective:To investigate the distribution of respiratory pathogens and risk factors of death in patients with pulmonary infection in neurosurgical intensive care unit (NICU).Methods:A total of 87 patients with pulmonary infection in the NICU of the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine from January 2018 to December 2019 were collected, and the pathogens of their respiratory tract were analyzed to understand the types and distribution of bacteria in the lung infection. Univariate statistical analysis was used to analyze the relationship between the patient′s clinical outcome with age, diabetes, hypertension, renal insufficiency, hypoproteinemia, anemia, chronic respiratory disease, surgery, tracheotomy, and bacterial multi-resistance. Binary logistic regression analysis was used to analyze the influencing factors of death in NICU patients with pulmonary infection.Results:A total of 112 pathogenic bacteria were isolated in this research group, including 83 Gram-negative bacteria (74.11%), 22 Gram-positive bacteria (19.64%), and 7 Fungi (5.25%). Imipenem was highly sensitive to Gram-negative bacteria, vancomycin was highly sensitive to Gram-positive bacteria, and other drugs were highly resistant. 41 patients died (47.13%). Age≥60 ( OR=3.501, 95% CI: 1.152-10.638), renal insufficiency ( OR=3.872, 95% CI: 1.336-11.224), tracheotomy ( OR=0.317, 95% CI: 0.114-0.882), bacteria multi-drug resistance ( OR=3.480, 95% CI: 1.162-10.422) were independent risk factors for death in NICU patients with pulmonary infection. Conclusions:Patients with severe neurological diseases are in critical condition, and there are many patients with pulmonary infection, with poor prognosis and high mortality. Gram-negative bacteria are the most common respiratory pathogens. Carbapenems account for the highest proportion of antibiotics in clinic. Advanced age, renal insufficiency and bacterial multidrug resistance increase the mortality of patients, while early tracheotomy can reduce the mortality of patients.

Diabetic nephropathy (DN) is one of the serious microvascular complications of diabetes mellitus (DM),and is the main cause of end-stage renal disease(ESRD) worldwide. Although lowering blood glucose and,lowering blood pressure and blocking the renin-angiotensin-aldosterone system(RAAS) can reduce blood glucose,blood pressure and urinary protein to a certain extent,it is still difficult to prevent the progression of DN sometimes. The curative effect of traditional Chinese medicine on DN has been confirmed,but its mechanism is has not been fully clarified. Autophagy is a highly conserved lysosomal degradation pathway in mammals that removes protein aggregates and damaged organelles to maintain cell homeostasis. Podocyte,also known as glomerular epithelial cells,is an important component in maintaining the homeostasis of glomerular filtration barrier,and podocyte injury is considered to be a central link in the occurrence and development of DN. As a highly differentiated cell,podocyte maintains a high level of autophagy to maintain its homeostasis under physiological conditions. In DN state,mammalian target of rapamycin (mTOR),AMP-activated protein kinase (AMPK),silent information regulator 1(SIRT1) and other nutritional signaling pathways as well as intracellular stress response signaling pathways such as oxidative stress,endoplasmic reticulum stress ,and hypoxia stress,etc.,affect podocyte autophagy of podocytes, and ultimately leading to podocyte injury and the progression of DN. In recent years,regulation of podocyte autophagy has become one of the hot spots in DN research,and has also received extensive attention in the field of Chinese medicine. A review and summary of the domestic and international literature in this field reveals that Chinese medicine can affect podocyte autophagy in multiple pathways and targets. Nevertheless, those studiesbut mainly focuses on two nutrient-sensing signaling pathways,mTOR and AMPK,and there lacks more comprehensive and in-depth mechanism studies. In addition,the current research mainly concentrates focuses on the field of Chinese medicine monomers and Chinese medicine compounds,and rarely studies multi-component Chinese medicinelacking research on Chinese medicine component drugs and single drugs,and the research still lacks there is a lack of hierarchy. The regulatory mechanism of Chinese medicine on podocyte autophagy of podocytes in DN state in Chinese medicine still needs to be further studied in depth and systematically.

Objective:To investigate the efficacy of infliximab in the treatment of small intestinal Crohn's disease and its effects on simple endoscopic score for Crohn's disease (SES-CD), routine blood test and nutritional indexes.Methods:Eighty-four patients with small intestinal Crohn's disease who received treatment in Zhejiang Xin'an International Hospital between November 2019 and March 2021 were included in this study. They were randomly divided into control and observation groups with 42 patients each. The control group was treated with azathioprine. The observation group was treated with azathioprine and infliximab. Clinical efficacy, SES-CD score, hemoglobin (Hb) level, white blood cell (WBC) count, platelet (PLT), albumin (ALB), pre-albumin (PA) and total protein (TP) levels pre- and post-treatment were compared between the two groups.Results:Response rate in the observation group was significantly higher than that in the control group [92.8% (39/42) vs. 73.8% (31/42), χ2 = 5.48, P < 0.05]. There was no significant difference in SES-CD score pre-treatment between the control and observation groups [(7.24 ± 1.30) points vs. (7.33 ± 1.27) points, t = -0.34, P > 0.05]. After treatment, SES-CD scores in the control and observation groups were (5.12 ± 1.17) points and (4.22 ± 0.98) points, respectively, which were significantly decreased compared with those before treatment ( t = 11.01, 14.66, both P < 0.001). After treatment, SES-CD score in the observation group was significantly lower than that in the control group ( t = 4.19, P < 0.001). Before treatment, there were no significant differences in Hb level [(110.23 ± 16.19) g/L vs. (112.27 ± 15.98) g/L], WBC count [(7.61 ± 2.54) × 10 9 g/L vs. (7.47 ± 2.61) × 10 9 g/L] and PLT level [(420.14 ± 130.27) × 10 9 g/L vs. (419.23 ± 131.15) × 10 9 g/L] between the control and observation groups ( t = -0.58, 0.25, 0.03, all P > 0.05). After treatment, Hb level [(120.25 ± 14.36) g/L and (130.17 ± 12.24) g/L ], WBC count [(6.01 ± 1.88)× 10 9 g/L, (5.13 ± 1.96) × 10 9 g/L) and PLT level [(321.79 ± 110.21) × 10 9 g/L, (267.25 ± 100.23) × 10 9 g/L] in the control and observation groups were significantly decreased compared with those before treatment ( tcontrol group = -4.70, 6.60, 8.02, tobservaton group = -7.91, 3.50, 4.99, all P < 0.05). Hb level post-treatment in the observation group was significantly higher than that in control group ( t = -3.41, P < 0.05) . WBC count and PLT level in the observation group were significantly lower than those in the control group ( t = 2.10, 2.37, both P < 0.05). Before treatment, there were no significant differences in ALB level [(33.14 ± 5.66) g/L vs. (32.98 ± 5.73) g/L], PA level [(220.17 ± 71.14) mg/L vs. (219.89 ± 70.26) mg/L], TP level [(61.23 ± 8.21) g/L vs. (60.95 ± 8.17) g/L] between control and observation groups ( t = 0.12, 0.01, 0.15, all P > 0.05). After treatment, ALB level [(38.29 ± 6.13) g/L, (44.23 ± 6.07) g/L], PA level [(281.14 ± 85.23) mg/L, (320.27 ± 82.01) mg/L], TP level [(67.23 ± 7.22) g/L, (73.28 ± 6.97) g/L] in the control and observation groups were significantly increased compared with those before treatment ( tcontrol group = -7.90, -4.30, -5.08, tobservation group = -13.76, -7.52, -11.45, all P < 0.05). After treatment, ALB, PA and TP levels in the observation group were significantly higher than those in the control group ( t = -4.46, -2.14, -3.89, all P < 0.05). Conclusion:Infliximab is highly effective on small intestinal Crohn's disease. It can effectively decrease SES-CD score, improve routine blood parameters and nutritional indexes, alleviate azathioprine-induced myelosuppression, and reduce gastrointestinal reactions.

Malignant tumor, an important factor threatening human life and health, brings huge economic burden to patients. At present, chemoradiotherapy is still the main treatment method for tumor diseases, but there are also great side effects when it plays a therapeutic role. Traditional Chinese medicine in the prevention and treatment of tumor diseases has many advantages such as few side effects, improving the physiological state of patients, and slowing down the side effects of radiotherapy and chemotherapy. Berberine is an effective component of rhizoma coptidis, with a very good antitumor effect. It can inhibit tumor cell proliferation, promote tumor cell apoptosis, inhibit tumor metastasis and angiogenesis, regulate tumor autophagy, reverse multi-drug resistance of tumor, regulate the body immunity, and affect tumor metabolic reprogramming to play its role. Compared with chemical preparations, berberine has a wide range of sources, with high safety and easy access, and has great potential in the prevention and treatment of malignant tumors. In this article, we would mainly review the research progress on the antitumor mechanism of berberine in recent years.

Objective To evaluate the clinical effect of acupuncture on Parkison disease (PD) with systematic review in view of evidence-based medicine (EBM).Methods Articles on clinical effect of acupuncture on Parkinson disease published from the database-established year to May of 2017 we searched from China National Knowledge Internet (CNKI),Wanfang,VIP,Chinese Biomedical Literature Database (CBM),PubMed,and Cochrane Library databases without language limitation.Quality evaluation and data extraction were carried out according to the tool for assessing risk of bias provided by the Cochrane Handbook for Systematic Reviews of Interventions (Version 5.1.0).All statistical analyses were performed with Reviewer Manager Software (Version 5.3;Cochrane Collaboration,Oxford,UK).Assessments were performed with the total effective rate,the scores of each scale (the unified Parkinson disease rating scales-UPDRS and the Webster scale),and the improvement of different clinical manifestation.Results In all,12 randomized controlled trials (RCTs) met our inclusion criterion,a total of 892 patients,including 468 cases in the experimental group (acupunctuer with or without medicine) and 424 cases in the control group (medicine only).Meta-analysis showed favorable results for the experimental group compared to control group in the total effective rate,the total scales of UPDRS and the modified Webster scale [OR =2.16,95% CI (1.57,2.97),P<0.01;OR =7.20,95% CI(4.02,10.37),P<0.01;OR=3.33,95% CI(2.13,4.53),P <0.01].The experimental group was effective in relieving partial symptoms of PD such as rigidity,postural,gait,bradykinesia compared to the control group,while there was no significant difference in tremor at rest and sit-stand up movements (P > 0.05).Conclusions Acupuncture had certain clinical effect on Parkinson disease,it can relieve the clinical symptoms of Parkinson disease to some extent,and postpone the progression of PD,which improves the quality of life of PD patients.Acupuncture can be recommended as a combination treatment for Parkinson disease.

Objective To evaluate the curative effect of traditional Chinese medicine fumigation combined with conventional therapy for the diabetic foot. Methods A total of 90 patients with type 2 diabetes who were treated in the hospital from March 2015 to August 2017 were enrolled in this study. On the basis of conventional treatment, the treatment group received the traditional Chinese medicine fumigation, while the control group received warm water fumigation. The total limb popliteal artery, femoral artery, posterior tibial artery and dorsalis pedis artery and a score of symptoms and signs were compared between two groups after 4 weeks. Results The total effective rate of the treatment group was 73.3% (33/45), while the control group was 53.3% (24/45), and the difference was statistically significant (χ2=3.876, P=0.049). After treatment, total limb femoral artery(58.38 ± 12.53 cm/s vs.51.94 ± 11.62 cm/s,t=2.528),popliteal artery(43.87 ± 9.46 cm/s vs.39.87 ± 8.28 cm/s,t=2.134),posterior tibial artery(35.72 ± 8.29 cm/s vs.30.89 ± 8.51 cm/s,t=2.725),dorsalis pedis artery (26.82 ± 8.10 cm/s vs.22.87 ± 7.39 cm/s,t=2.413)of the treatment group were significantly higher than those of the control group(P<0.05).The main symptom score(2.0 ± 1.5 vs.4.1 ± 2.1,t=5.419),sub syndrome score(3.1 ± 1.8 vs.5.1 ± 2.3,t=4.692),tongue and pulse condition score(1.8 ± 0.8 vs.2.4 ± 0.6,t=4.277)of the treatment group were significantly lower than those of the control group (P<0.01). Conclusions The traditional Chinese medicine fumigation combined with conventional therapy could improve the lower extremity blood flow status in diabetic foot patients and clinical symptoms.

Objective To observe the effect of Q-Switching Nd:YAG treatment in pigmented dermatoses and its influence for melanin metabolism. Methods 114 patients with pigmented dermatoses in our hospital from October 2015 to August 2016 were selected as the study object,and were randomly divided into the control group (57 cases) and the observation group (57 cases).The control group were treated with intense pulsed light therapy while the observation group were treated with Q-Switching Nd:YAG treatment on the treatment of control group. Then the total effective rates,recurrence rates,adverse reaction rates,skin lesion scores and serum melanin metabolism related indexes before and after the treatment of two groups were analyzed and compared. Results Clinically, the total effective rates of observation group were higher than those of control group ( <0.05)and the recurrence rate of observation group was lower than that of control group ( <0.05) .The adverse reaction rates of two groups were compared and the differences were not statistically significant ( >0.05) .The skin lesion scores and serum melanin metabolism related indexes of two groups before the treatment were compared and the differences were not statistically significant (>0.05) .The serum melanin metabolism related indexes of two groups after the treatment were all better than those of control group ( <0.05) . Conclusion The clinical effect of Q-Switching Nd:YAG treatment in the patients with pigmented dermatoses is better, and it has the active influence for the melanin metabolism,so it has higher clinical value in the treatment of pigmented dermatoses.

To evaluate the effect of Chinese medicine of invigorating spleen and kidney detoxification on simian immunodeficiency virus-infected rhesus macaque. Eight SIV rhesus macaques of the same age were randomly divided into Chinese medicine of invigorating spleen and kidney detoxification group(hereinafter referred to as Chinese medicine group) and anti-virus drug(HAART) group. The traditional Chinese medicine and antiviral therapy were given for 8 weeks, and peripheral blood was collected for detection in every 4 weeks. The results showed that Chinese medicine of invigorating spleen and kidney detoxification could not obviously decrease plasma viral load as HAART, but it can increase CD4 number in peripheral blood, especially the CD4 naive cells, and increase the number of CD4 and CD8 cells, enhance the immune response to pathogens. Therefore, it delayed the occurrence and development of spleen deficiency to a certain extent, indicating that the medicine had immune regulation effect, with considerable clinical value and application prospects.

OBJECTIVE: To evaluate the effect of the clinical nursing path integrated with the holistic nursing on advanced schistosomiasis patients with ascites. METHODS: A total of 226 advanced schistosomiasis patients with ascites were randomly divided into a control group and an experimental group (113 cases each group). The subjects in the experimental group were nursed by the clinical nursing path integrated with the holistic nursing, while those in the control group were nursed only by the holistic nursing. Then the clinical relevant indexes of the two groups were observed, and the quality of life of the patients before and after hospital discharge was assessed. RESULTS: The improvement rate, satisfaction degree, and awareness rate of health knowledge of the patients in the experiment group were 93.8%, 100% and 97.4%, respectively, which were significantly higher than those of the control group (all P < 0.05). The mortality rate and the complication rate of the patients in the experimental group were 0 and 2.7%, respectively, which were significantly lower than those of the control group (both P < 0.05). In addition, the average hospitalization days and the hospitalization cost of the experiment group were (12.2 ± 0.7) d and (4 725.0 ± 310.1) Yuan respectively, which were less than those of the control group (both P < 0.01). When 6 months after the discharge from hospital, the quality of life of the patients in the experimental group in various fields was significantly better than that of the control group (all P < 0.05). CONCLUSIONS: The clinical nursing pathway integrated with holistic nursing can effectively improve the improvement rate and decrease the mortality of the advanced schistosomiasis patients with ascites; meanwhile, it can shorten the hospitalization time and save the hospitalization cost. Therefore, this nursing model is suitable for popularization and application in the treatment and nursing work of the advanced schistosomiasis assistance.

RESUMEN

<p><b>OBJECTIVE</b>To observe the curative effect of porous tantalum rod and Gugutou Huaisiyu Capsule (GHC) for steroid-induced osteonecrosis of femoral head (SONFH).</p><p><b>METHODS</b>A total 60 hips of 50 SONFH patients were randomly assigned to the treatment group and the control group according to grouping time, 25 in each group (30 hips). Patients in the control group were implanted with porous tantalum rod, while those in the treatment group additionally took GHC (5 pills each time, three time per day for 2 successive months; and then twice per day for 4 successive months). Then all patients were followed-up to observe Harris hip score. The curative effect and the femoral head survival time were assessed.</p><p><b>RESULTS</b>A total of 49 patients (59 hips) were followed-up. The Harris hip score of the two groups at the final follow-up was significantly improved after treatment, with statistical difference when compared with before treatment (P < 0.01). Besides, it was higher in the treatment group than in the control group. The curative effect and the survival time were superior in the treatment group, with statistical difference when compared with the control group (P < 0.05).</p><p><b>CONCLUSIONS</b>Porous tantalum rod combined GHC got better effect in treating SONFH. It could significantly improve the function of affected hips and prolong the survival time of femoral head.</p>

RESUMEN

Objective To optimize the concentrate process ofDanshen injection.Methods The concentration changes of Salvianic acid A sodium, Protocatechuic aldehyde and Salvianolic acid B were selected as indexes to compare mospheric concentrate, vacuum concentrate, nanofiltration concentrate and nanofiltration-vacuum coupling concentrate methods with HPLC. Results Phenolic acid composition changed when atmospheric and vacuum concentrate method were used. Retention rate of three phenolic acids were over 93% after nanofiltration concentration, but the concentration efficiency was limited by solution viscosity. Nanofiltration-vacuum coupling concentrate methods could solve the above problems, meanwhile, retention rate of three phenolic acids were over 91%.Conclusions Nanofiltration technology takes advantage in medicine extraction concentrate with heat sensitive ingredients, and its application prospect in traditional Chinese medicine pharmaceutical industry would be broad.

Objective To evaluate the effect of the clinical nursing path integrated with the holistic nursing on advanced schistosomiasis patients with ascites. Methods A total of 226 advanced schistosomiasis patients with ascites were randomly di?vided into a control group and an experimental group (113 cases each group). The subjects in the experimental group were nursed by the clinical nursing path integrated with the holistic nursing,while those in the control group were nursed only by the holistic nursing. Then the clinical relevant indexes of the two groups were observed,and the quality of life of the patients before and after hospital discharge was assessed. RESUMEN

Objective]To study the discipline of Guizhi Tang in clinical application, understand further indication of Guizhi Tang to improve clinical efficacy. [Method]After consulting medical cases about Guizhi Tang that are from different medical practitioners, analysis on the discipline of Guizhi Tang in clinical application with the help of six meridian syndrome differentiation, zang-fu differentiation and drug efficacy of Guizhi Tang. [Result]Guizhi Tang can be used to cure urticaria, coronary heart disease, nape carbuncle, high fever, half sweating and diarrhea. The cases of urticaria, diarrhea and nape carbuncle all contain symptoms of tai yang disease.The case of high fever is due to discord of ying-wei. The case of half sweating is due to discord of yin-yang. The case of coronary heart disease reflects Guizhi Tang can regulate spleen-stomach. [Conclusion] The medical cases about Guizhi Tang involve the departments of internal medicine, surgery, gynaecology and pediatrics,such as urticaria, coronary heart disease and so on. Using Guizhi Tang should get help from six meridian syndrome differentiation or zang-fu differentiation. When using six meridian syndrome differentiation, doctors should pay attention to difference of tai yang disease and other five meridian diseases. When using zang-fu differentiation, doctors should attach importance to the feature of Guizhi Tang that involves regulating yin-yang, spleen-stomach and ying-wei.

OBJECTIVE: To evaluated the effect of calycosin on left ventricular ejection fraction and angiogenesis. METHODS: Adult male Sprague-Dawley rats were randomly assigned into calycosin-treated groups (0.5, 1, 2, and 4 mg/kg qd), a dimethyl sulfoxide (DMSO), or a sham-operated control group. The myocardial ischaemia (MI) model was intraperitoneally administered calycosin for 28 days. The survival rates and left ventricular ejection fractions (LVEF) were compared between groups. The expression levels of vascular endothelial growth factor (VEGF) and cluster of differentiation 31 (CD31) in ischaemic myocardium were also measured and compared. RESULTS: The construction of MI model resulted in a LVEF reduction of 50% compared with the sham-control. After 28 days, the LVEF value was 10% higher when calycosin (4 mg/kg) was administered compared with the DMSO group. The expression of VEGF and CD31 showed a dose-dependent manner when calycosin was administrated. The calycosin-treated (4 mg/kg) group displayed a two-fold increase in VEGF expression at both the mRNA and protein levels compared with the DMSO group. In addition, CD31 expression in the microvascular increased 1.5-fold in the 4 mg/kg calycosin-treated group. CONCLUSION: Calycosin improved left ventricular ejection fraction in the MI rat models, induced VEGF expression in the ischaemic myocardium, increased CD31 expression and promoted angiogenesis.

RESUMEN

The purpose of this study was to establish a drug screening method for small molecules extracted from traditional Chinese medicines (TCM) that have neuronal differentiation promoting effects, using P19 embryonic carcinoma cell as a cell-based model. First, the constructed plasmid (pTα1-Luc) was transfected into P19 cells to establish a screening model. Second, several TCMs were screened using the established model and all-trans-retinoic acid as a positive control. Finally, the underlying molecular mechanism was explored using immunofluorescence staining, qT-PCR, and Western blot analysis. Our results indicated that the drug screen model was established successfully and that both honokiol and hyperoside induced P19 differentiation into neurons, with the possible molecular mechanism being modulating the Wnt signaling pathway. In conclusion, the drug screening model developed in the present study provides a rapid, cell-based screening platform for identifying natural compounds with neuronal differentiation effects.

<p><b>OBJECTIVE</b>To systematically evaluate acupuncture as a treatment for male infertility.</p><p><b>METHODS</b>We searched Chi na Biology Medical Database (CBM), Wan Fang Medical Information System, China National Knowledge Infrastructure (CNKI), VIP Information Resource System (VIP), and PubMed for published literature on acupuncture as a treatment for male infertility on May 1 2014. Based on the Standards for Reporting Interventions in Clinical Trials of Acupuncture (STRICTA), we evaluated the quality of the reports, conducted meta-analysis on the identified studies via RevMan5.2, and assessed the quality of the evidence in the literature by Grading of Recommendations Assessment, Development, and Evaluation (GRADE).</p><p><b>RESULTS</b>A total of 12 studies involving 2,177 patients were included, the quality of which was evaluated as mediocre. With regard to the cure rate, acupuncture was comparable to traditional Chinese medicine (TCM) (P > 0.05) but better than Western medicine (RR = 4.00, 95% CI 1.63 to 9.82, P < 0.01) while acupuncture + TCM was better than either TCM (RR = 1.77, 95% CI 1.20 to 2.60, P < 0.01) or Western medicine used alone (RR = 2.73, 95% CI 1.51 to 4.93, P < 0.01), and acupuncture + Western medicine was better than Western medicine alone (RR = 1.88, 95% CI 1.17 to 3.02, P = 0.01). The combined use of acupuncture, ear pressure, TCM, and Western medicine showed a higher cure rate than the combination of TCM and Western medicine (RR = 3.45, 95% CI 2.90 to 4.11, P < 0.01). In therapeutic effectiveness, acupuncture was comparable to TCM (P > 0.05) but superior to Western medicine (RR = 1.41, 95% CI 1.12 to 1.71, P < 0.01), acupuncture + TCM was superior to either TCM (RR = 1.14, 95% CI 1.05 to 1.23, P < 0.01) or Western medicine alone (RR = 1.43, 95% CI 1.22 to 1.67, P < 0.01), and acupuncture + Western medicine was superior to Western medicine alone (RR = 1.25, 95% CI 1.05 to 1.49, P = 0.01). In improving sperm concentration, acupuncture was as effective as TCM (P > 0.05) and sham acupuncture (P > 0.05) but outdid Western medicine (RR = 27.00, 95% CI 24.27 to 29.73, P < 0.01) and acupuncture + TCM outdid either TCM (RR = 14.65, 95% CI 7.58 to 21.72, P < 0.01) or Western medicine alone (RR = 1.04, 95% CI--1.43 to 3.51, P > 0.05). In improving grade a sperm, acupuncture exhibited a similar effect to TCM (P > 0.05) and sham acupuncture (P > 0.05), and acupuncture + TCM was more effective than TCM alone (RR = 7.78, 95% CI 3.51 to 12.23, P < 0.01) but equally effective as Western medicine (P > 0.05). In elevating the level of grade a + b sperm, acupuncture + TCM excelled either TCM (RR = 11.00, 95%, CI 3.17 to 18.82, P < 0.01) or Western medicine alone (RR = 12.22, 95% CI 6.87 to 17.57, P < 0. 01), while acupuncture produced a comparable effect with sham acupuncture (P > 0.05). As for the quality of the included studies, only 3 conclusions of the 23 meta-analyses were assessed to be of average quality, while the others of poor or extremely poor quality. Therefore, the recommendation grade of the conclusions was low.</p><p><b>CONCLUSION</b>For the treatment of male infertility, acupuncture is reported to be equally effective as TMC and more effective than Western medicine, and its effectiveness is enhanced when applied in combination with either TCM or Western medicine. Acupuncture is distinctively efficacious in improving sperm quality. Nevertheless, the overall quality of the included studies is low.</p>
