INTRODUCTION: Air pollution by particulate matter (PM) is a major public health problem. In Chile, the study has focused on outdoor air and PM10, rather than indoor air and PM2.5. Because newborns and infants spend most of their time at home, it is necessary to evaluate the exposure to indoor air pollution in this susceptible population. OBJECTIVE: To determine concentration of PM2.5 in the homes of newborns and identify the emission sources of the pollutants. PATIENTS AND METHOD: The PM2.5 concentration ([PM2.5]) was collected over a 24hour period in 207 households. Baseline sociodemographic information and environmental factors (heating, ventilation, smoking and house cleaning), were collected. RESULTS: The median [PM2.5] was 107.5µg/m3. Family history of asthma was associated with lower [PM2.5] (P=.0495). Homes without heating showed a lower median [PM2.5], 58.6µg/m3, while those using firewood, kerosene, and electricity ranged between 112.5 and 114.9, and coal users' homes reached 162.9µg/m3. Wood using homes had significant differences (P=.0164) in median [PM2.5] whether the stove had complete combustion (98.2µg/m3) vs. incomplete (112.6µg/m3), or a salamander stove (140.6µg/m3). Cigarette smoking was reported in 8.7% of the households, but was not associated with the [PM2.5]. Ventilation was associated with a higher median [PM2.5] (120.6 vs. 99.1µg/m3, P=.0039). CONCLUSION: We found homes with high [PM2.5]. Residential wood consumption was almost universal, and it is associated with the [PM2.5]. Natural ventilation increased MP2.5, probably due to infiltration from outside.

RESUMEN

The environment is a strong determinant of cardiovascular health. Environmental cardiology studies the contribution of environmental exposures with the aim of minimizing the harmful influences of pollution and promoting cardiovascular health through specific preventive or therapeutic strategies. The present review focuses on particulate matter and metals, which are the pollutants with the strongest level of scientific evidence, and includes possible interventions. Legislation, mitigation and control of pollutants in air, water and food, as well as environmental policies for heart-healthy spaces, are key measures for cardiovascular health. Individual strategies include the chelation of divalent metals such as lead and cadmium, metals that can only be removed from the body via chelation. The TACT (Trial to Assess Chelation Therapy, NCT00044213) clinical trial demonstrated cardiovascular benefit in patients with a previous myocardial infarction, especially in those with diabetes. Currently, the TACT2 trial (NCT02733185) is replicating the TACT results in people with diabetes. Data from the United States and Argentina have also shown the potential usefulness of chelation in severe peripheral arterial disease. More research and action in environmental cardiology could substantially help to improve the prevention and treatment of cardiovascular disease.

INTRODUCTION: Burnt sugarcane harvesting requires intense physical exertion in an environment of high temperature and exposure to particulate matter. OBJECTIVE: To evaluate the effects of burnt sugarcane harvesting on rhinitis symptoms and inflammatory markers in sugarcane workers. METHODS: A total of 32 male sugarcane workers were evaluated with questionnaire for rhinitis symptoms, and for inflammatory markers on peripheral blood and nasal lavage, in the non-harvesting, and 3 and 6 months into the sugarcane harvesting period. Weather data and particulate matter fine concentrations were measured in the same day. RESULTS: The particulate matter concentrations in sugarcane harvesting were 27 (23-33µg/m3), 112 (96-122µg/m3), and 63 (17-263µg/m3); 24h temperatures were 32.6 (25.4-37.4°C), 32.3 (26.7-36.7°C) and 29.7 (24.1-34.0°C) and relative humidities were 45.4 (35.0-59.7%), 47.9 (39.1-63.0%), and 59.9 (34.7-63.2%) in the non-harvesting period, three and 6 months of the harvesting period. The age was 37.4±10.9 years. The prevalence of rhinitis symptoms was significantly higher at 3 months of the harvesting period (53.4%), compared to non-harvesting period (26.7%; p=0.039) and at 6 months into the harvesting period (20%; p=0.006). Concentrations of interleukin 6 (IL-6) in nasal lavage increased after 3 months of the harvesting period compared to the non-harvesting period (p=0.012). The presence of rhinitis symptoms, after 3 months of the harvesting period, was directly associated with blood eosinophils and inversely associated with neutrophils. CONCLUSIONS: After 3 months of work in burnt sugarcane harvesting the prevalence of rhinitis symptoms and IL-6 in nasal lavage increased. Furthermore, eosinophil counts were directly associated with the rhinitis symptoms in the period of higher concentration of particulate matter.

INTRODUCTION AND OBJECTIVES: The role of the environment on cardiovascular health is becoming more prominent in the context of global change. The aim of this study was to analyze the relationship between apparent temperature (AT) and air pollutants and acute myocardial infarction (AMI) and to study the temporal pattern of this disease and its associated mortality. METHODS: We performed a time-series study of admissions for AMI in Cantabria between 2001 and 2015. The association between environmental variables (including a biometeorological index, AT) and AMI was analyzed using a quasi-Poisson regression model. To assess potential delayed and non-linear effects of these variables on AMI, a lag non-linear model was fitted in a generalized additive model. RESULTS: The incidence rate and the mortality followed a downward trend during the study period (CC=-0.714; P=.0002). An annual pattern was found in hospital admissions (P=.005), with the highest values being registered in winter; a weekly trend was also identified, reaching a minimum during the weekends (P=.000005). There was an inverse association between AT and the number of hospital admissions due to AMI and a direct association with particulate matter with a diameter smaller than 10 µm. CONCLUSIONS: Hospital admissions for AMI followed a downward trend between 2007 and 2015. Mortality associated with admissions due to this diagnosis has decreased. Predictive factors for this disease were AT and particulate matter with a diameter smaller than 10 µm.

Introducción: El SARS-CoV-2 se transmite principalmente por vía aérea, en la cual influyen factores ambientales como el material particulado. Objetivos: Determinar la concentración media de material particulado 10 en cinco estaciones de monitoreo ambiental en Santa Marta (Colombia) para evaluar el riesgo incremental o predisposición de morbimortalidad por COVID-19, previo inicio de la pandemia. Métodos: Estudio descriptivo de corte transversal en una muestra de cinco estaciones de monitoreo ambiental sobre la concentración de material particulado. La información se obtuvo de la base de datos "Calidad de Aire del Departamento del Magdalena" del sitio web de datos abiertos del Gobierno de Colombia. Resultados: En las cinco estaciones ambientales la concentración media anual de material particulado superó o se acercó a 50 µg/m3, el límite permitido por año en Colombia, y casi triplicó lo establecido por la Organización Mundial de la Salud. Entre el 13-30 % de las concentraciones diarias de material particulado 10 superaron los 75 µg/m3: umbral en el país. Entretanto, el 50-75 % de las concentraciones excedieron la dosis diaria de referencia en organismos internacionales. Los mayores niveles de contaminación se observaron en el sector sur. Conclusiones: En todas las estaciones gran parte de las concentraciones diarias y la media anual sobrepasaron el límite de material particulado 10. Esto aumentó el riesgo establecido de COVID-19, y se presume la predisposición para morbimortalidad por esta enfermedad, en la ciudad de Santa Marta, antes del inicio de la pandemia.


Introduction: SARS-CoV-2 is transmitted mainly by the airborne route, which is influenced by environmental factors such as particulate matter. Objectives: To determine the mean concentration of particulate matter 10 in five environmental monitoring stations in Santa Marta (Colombia) to evaluate the incremental risk or predisposition of morbidity and mortality due to COVID-19, prior to the onset of the pandemic. Methods: Descriptive cross-sectional study in a sample of five environmental monitoring stations on the concentration of particulate matter. The information was obtained from the database "Calidad de Aire del Departamento del Magdalena" of the open data website of the Colombian government. Results: In the five environmental stations the annual average concentration of particulate matter exceeded or approached 50 µg/m3, the limit allowed per year in Colombia, and almost tripled that established by the WHO. Between 13-30% of the daily concentrations of particulate matter exceeded 75 µg/m3: the threshold in the country. Meanwhile, 50-75 % of the concentrations exceeded the daily reference dose in international organizations. The highest pollution levels were observed in the southern sector. Conclusions: In all stations a large part of the daily concentrations and the annual mean exceeded the limit for particulate matter 10. This increased the established risk of COVID-19, and the predisposition for morbimortality due to this disease in the city of Santa Marta, before the beginning of the pandemic, is presumed.

The aim of the present study was to find out concentration of PM in ambient air and its associated health risk in Haripur city, Pakistan. Twenty-three samples were taken at various educational institutes, hospitals, recreational areas and industries in Haripur city. Concentration of PM2.5 (µg/m3) and PM10 (µg/m3) was measured with Youngteng YT-HPC 3000A portable PM counter. The results revealed that values of both PM2.5 and PM10 were above the permissible limits (35 µg/m3 for PM2.5 and 150 µg/m3 for PM10) set by Environmental Protection Agency Pakistan (Pak-EPA) in all the educational institutes, hospitals, recreational areas and industries investigated. Furthermore, significant (p<0.05) variation was found in the concentration of both PM2.5 and PM10 in all the educational institutes, hospitals, recreational areas, and industries studied. The concentration of PM2.5 was positively correlated with the concentration of PM10 in all the sampling sites. Therefore, from 1-14 scale standard of health index, the values of PM2.5 and PM10 exhibited that the ambient air quality of Haripur city Pakistan is under high risk. If the regulatory authorities such as Environmental Protection Agency, Health Department and Local Government monitor PM pollution in different settings of Haripur city, then a decrease can be possible in the pollution level. The remedies that can be taken to overcome the problem of ambient air pollution such as PM are plantation of trees at the sites where there are higher levels of air pollutants and use of masks on personal protection basis along with implementation of pollution control system in industries of Hattar Industrial Estate Haripur city, Pakistan.


O material particulado (MP) é um importante poluente do ar que causa sérios problemas de saúde. O objetivo do presente estudo foi descobrir a concentração de MP no ar ambiente e sua associação com o risco à saúde na cidade de Haripur, Paquistão. Vinte e três amostras foram coletadas em várias instituições de ensino, hospitais, áreas recreativas e indústrias na cidade de Haripur. A concentração de MP2,5 (µg/m3) e MP10 (µg/m3) foi medida por meio do contador de MP portátil Youngteng YT-HPC 3000A. Os resultados revelaram que os valores de MP2,5 e MP10 estavam acima dos limites permitidos (35 µg/m3 para MP2,5 e 150 µg/m3 para MP10) estabelecidos pela Agência de Proteção Ambiental do Paquistão (Pak-EPA) em todas as instituições de ensino, hospitais, áreas recreativas e indústrias investigadas. Além disso, foi encontrada variação significativa (p < 0,05) na concentração de MP2,5 e MP10 em todos os locais estudados. A concentração de MP2,5 correlacionou-se positivamente com a concentração de MP10 em todos os locais de amostragem. Portanto, a partir da escala padrão 1-14 do índice de saúde, os valores de MP2,5 e MP10 mostraram que a qualidade do ar ambiente na cidade de Haripur, Paquistão, está sob alto risco. Se as autoridades reguladoras, como a Pak-EPA, o Departamento de Saúde e o governo local, monitorarem a poluição por MP em diferentes configurações da cidade de Haripur, pode ser que haja uma diminuição no nível de poluição. As medidas que podem ser tomadas para superar o problema da poluição do ar ambiente, como o MP, são o plantio de árvores nos locais onde há maiores níveis de poluentes atmosféricos, o uso de máscaras e a implantação de sistema de controle de poluição nas propriedades industriais de Hattar, na cidade Haripur, Paquistão.

Introducción. El material particulado (PM) es uno de los contaminantes del aire que mayor implicación tienen en la aparición o exacerbación de cuadros respiratorios en niños. Objetivo. Describir las características de las consultas por enfermedades respiratorias agudas en menores de 15 años, los niveles de PM en el aire, y analizar la asociación existente entre ellos en un sector de Bahía Blanca entre abril de 2019 y marzo de 2020. Población y métodos. Estudio ecológico de series temporales y grupos múltiples. Análisis descriptivo de consultas totales, por área, diagnóstico, y del PM. Modelo de correlación y regresión lineal generalizado para determinar la relación entre las variables. Se utilizó el programa SPSS®. Resultados. Se recopilaron 4787 consultas. Un 38,6 % (1846) correspondieron a rinitis y un 21,1 % (1011) a broncoespasmo. El PM de 10 nm (PM10) superó su valor límite el 31 % (115) de los días de estudio y el de 2,5 nm (PM2,5) un 3 % (8). Un aumento del 10 % del PM2,5 demostró incrementos de 1,3 % en las consultas totales; el incremento llegó al 2,1 % en el área más cercana al sector industrial (p <0,05). En esta última, el aumento del 10 % de los valores de PM10 se asoció al aumento del 1,8 % de las consultas (p <0,05). Conclusión. Se demostró asociación positiva entre las consultas por enfermedad respiratoria aguda y los niveles de PM del aire, sobre todo con el PM2,5 y en el área más cercana al sector industrial.


Introduction. Particulate matter (PM) is one of the air pollutants most involved in the onset or exacerbation of respiratory conditions in children. Objective. To describe the characteristics of consultations for acute respiratory diseases in children younger than 15 years and the levels of PM in the air and to analyze their association in a sector of Bahía Blanca between April 2019 and March 2020. Population and methods. Ecological, time-series study with multiple groups. Descriptive analysis of total number of consultations, by area, diagnosis, and PM. Generalized linear correlation and regression model to determine the relationship among variables. The SPSS® software was used. Results. Data from 4787 consultations were collected. Of these, 38.6% (1846) were related to rhinitis and 21.1% (1011), to bronchospasm. PM of 10 nm (PM10) exceeded its limit value on 31% (115) of the study days, and PM of 2.5 nm (PM2.5), on 3% (8). A 10% increase in PM2.5 showed increases of 1.3% in total consultations; the increase reached 2.1% in the area closest to the industrial sector (p < 0.05). In the latter, a 10% increase in PM10 was associated with an increase of 1.8% in consultations (p < 0.05). Conclusion. A positive association was evidenced between consultations for acute respiratory diseases and PM levels in the air, especially with PM2.5 and in the area closest to the industrial sector.

Abstract: In this study, we propose an indicator of air pollution exposure to identify potential hazardous areas for human health in the Amazon and Central-West Regions of Brazil from 2010 to 2019. This indicator aggregates both concentrations and time of exposure to fine particulate matter (PM2.5), according to the current limit recommended by the World Health Organization (WHO). We used daily PM2.5 averages obtained from the Brazilian Health Integrated Environmental Information System (SISAM) to calculate the percentages of days with PM2.5 concentrations exceeding the limit of 15µg/m³ per year and per month. From 2010 to 2019, the months from August to October presented the largest areas and the highest percentages of days with unacceptable pollution concentration values, harmful to human health. These areas were concentrated in the Arc of Deforestation. Therefore, 60% of the residents of the Amazon and Central-West regions were subjected to inadequate air quality for approximately six months per year. The proposed indicator is reproducible and appropriate to monitor areas of exposure and risk for human health.


Resumo: Este estudo propõe um indicador de exposição à poluição do ar para identificar potenciais áreas de risco para a saúde humana na região amazônica e no Centro-oeste do Brasil de 2010 a 2019. Esse indicador agrega as concentrações e o tempo de exposição à partículas finas de poluição (PM2.5), de acordo com o limite atual recomendado pela Organização Mundial da Saúde (OMS). Foram utilizadas médias diárias de PM2.5 obtidas do Sistema de Informações Ambientais Integrado a Saúde (SISAM) para o cálculo dos percentuais de dias cujas concentrações ultrapassaram o limite de 15μg/m³ por ano e por mês. De 2010 a 2019, os meses de agosto a outubro apresentaram as maiores áreas e os maiores percentuais de dias com valores de concentração inaceitáveis para a saúde humana. Tais áreas estavam concentradas na região do arco do desmatamento. Além disso, 60% dos moradores da região amazônica e do Centro-oeste eram expostos a uma qualidade inadequada do ar por aproximadamente seis meses por ano. O indicador proposto é reprodutível e adequado para monitorizar as áreas de exposição e de risco para a saúde humana.


Resumen: Este estudio propone un indicador de exposición a la contaminación del aire para identificar posibles áreas de riesgo para la salud humana en la región amazónica y el Medio Oeste de Brasil de 2010 a 2019. Este indicador agrega las concentraciones y el tiempo de exposición a partículas finas de contaminación (PM2.5), de acuerdo con el límite actual recomendado por la Organización Mundial de la Salud (OMS). Se utilizaron los promedios diarios de PM2.5 obtenidos del Sistema Integrado de Información Ambiental en Salud (SISAM) para calcular el porcentaje de días cuyas concentraciones superaron el límite de 15μg/m³ por año y por mes. En la década de 2010 a 2019, los meses de agosto a octubre tuvieron las áreas más grandes y los porcentajes más altos de días con valores de concentración inaceptables para la salud humana. Tales áreas se concentraron en la región del arco de la deforestación. Además, el 60% de los residentes de la región amazónica y el Medio Oeste estuvieron sujetos a una calidad del aire inadecuada durante aproximadamente seis meses al año. El indicador propuesto es reproducible y adecuado para monitorizar las áreas de exposición y de riesgo para la salud humana.

Abstract: Exposure to ambient air pollution increases mortality and morbidity, leading disabilities, and premature deaths. Air pollution has been identified as a leading cause of global disease burden, especially in low- and middle-income countries in 2015 (Global Burden of Diseases, Injuries and Risk Factors Study, 2015). This study explores the relation between mortality rates and particulate matter (PM) concentrations in the 50 Spanish regions for the period 2002-2017. Moreover, we estimated the premature deaths due to PM in Spain according to welfare and production losses in 2017. Random-effects models were developed to evaluate the relation between mortality rates and PM concentrations. The economic cost of premature deaths was assessed using the Willingness to Pay approach to quantify welfare losses and the Human Capital method to estimate production losses. PM10 concentrations are positively related to mortality due to respiratory diseases and stroke. Based on 10,342 premature deaths in 2017, losses in welfare amount to EUR 36,227 million (3.1% of Spanish GDP). The economic value of current and future production losses reached EUR 229 million (0.02% of GDP). From a social perspective, air pollution is a public health concern that greatly impacts health and quality of life. Results highlight the need to implement or strengthen regulatory, fiscal, and health public policies to substantially benefit the population's health by reducing their exposure to air pollution.


Resumen: La exposición a la contaminación atmosférica aumenta la mortalidad y la morbilidad, lo que conduce a la discapacidad y a la muerte prematura. La contaminación del aire se identificó como una de las principales causas de la carga mundial de enfermedades, sobre todo en países de ingresos bajos y medianos en el 2015 (Global Burden of Diseases, Injuries and Risk Factors Study, 2015). Este artículo explora la relación entre las tasas de mortalidad y la concentración de material particulado (PM) en las 50 regiones españolas desde el 2002 hasta el 2017. Además, se realizó una estimación de las muertes prematuras provocadas por PM en España en términos de bienestar y pérdidas de producción en el 2017. Se desarrollaron modelos de efectos aleatorios para estudiar la relación entre las tasas de mortalidad y las concentraciones de PM. El costo económico de las muertes prematuras se evaluó usando el enfoque "disposición a pagar" para monetizar las pérdidas de bienestar y el método del capital humano para estimar las pérdidas de producción. Las concentraciones de PM10 están positivamente asociadas con la mortalidad por enfermedades respiratorias y accidente cerebrovascular. Con base en 10.342 muertes prematuras en el 2017, las pérdidas en el bienestar social ascendieron a EUR 36.227 millones (3,1% del PIB español). El valor económico de las pérdidas de producción presentes y futuras llegó a EUR 229 millones (0,02% del PIB). Desde un punto de vista social, la contaminación del aire es un problema de salud pública que tiene un gran impacto en la salud y en la calidad de vida. Los resultados ponen de manifiesto la necesidad de implementar o de fortalecer políticas públicas regulatorias, fiscales y de salud para obtener beneficios sustanciales para la salud con la reducción de la exposición.


Resumo: A exposição à poluição do ar ambiente aumenta a mortalidade e a morbidade, levando a incapacidades e mortes prematuras. A poluição do ar foi identificada como uma das principais causas da carga global de doenças, principalmente em países de baixa e média renda em 2015 (Global Burden of Diseases, Injuries and Risk Factors Study, 2015). Este artigo explora a relação entre as taxas de mortalidade e a concentração de material particulado (PM) nas 50 regiões espanholas de 2002 a 2017. Além disso, foi realizada uma estimativa das mortes prematuras causadas por PM na Espanha em termos de bem-estar e perdas de produção em 2017. Modelos de efeitos aleatórios foram desenvolvidos para estudar a relação entre as taxas de mortalidade e as concentrações de PMP. O custo econômico das mortes prematuras foi avaliado usando a abordagem "disposição a pagar" para monetizar as perdas de bem-estar e o método do capital humano para estimar as perdas de produção. As concentrações de PM10 estão positivamente associadas à mortalidade por doenças respiratórias e acidente vascular cerebral. Com base em 10.342 mortes prematuras em 2017, as perdas no bem-estar social subiram para EUR 36,227 bilhões (3,1% do PIB espanhol). O valor econômico das perdas de produção presentes e futuras atingiu os EUR 229 milhões (0,02% do PIB). Do ponto de vista social, a poluição do ar é um problema de saúde pública que tem grande impacto na saúde e na qualidade de vida. Objetivo. Determinar la existencia de asociación entre defectos congénitos y la exposición prenatal de mujeres gestantes a PM10 en una población colombiana. Métodos. Estudio retrospectivo de casos y controles. Se incluyeron casos de madres de recién nacidos con defectos congénitos confirmados posnatalmente y controles sin defectos congénitos. La exposición a emisiones se determinó mediante disgregación temporo-espacial mediante ArcGIS® y georreferenciación mediante gvSIG, Google Earth y Google Street View®, usando estimaciones previamente publicadas y validadas para la ciudad. El análisis estadístico se realizó utilizando Jamovi-Stats Open now. Resultados . Se incluyeron un total de 101 pacientes, correspondiendo a 31 casos y 70 controles. Existió un aumento del riesgo de desarrollar defectos congénitos tras la exposición a emisiones de PM10 superiores a 2,23 Ton/año/250 m2 (OR: 8,17; IC 95%: 1.61 a 41.46; p = 0,011). Conclusiones. Existió relación entre la exposición a niveles elevados de PM10 y aumento en el riesgo de defectos congénitos en la población estudiada. Se sugiere la realización de futuras investigaciones sobre la relación entre contaminación medioambiental y eventos obstétricos adversos.


Objective: To determine the existence of an association between birth defects and prenatal exposure of pregnant women to PM10 in a Colombian population. Methods: Retrospective case-control study. Cases of mothers of newborns with postnatally confirmed congenital defects and controls without congenital defects were included. Emission exposure was determined by temporo-spatial disaggregation using ArcGIS® and georeferencing using gvSIG®, Google Earth® and Google Street View®, using previously published and validated estimates for the city. Statistical analysis was performed using Jamovi-Stats Open now®. Results: A total of 101 patients were included, corresponding to 31 cases and 70 controls. There was an increased risk of developing birth defects after exposure to PM10 emissions above 2.23 Ton/year/250m2 (OR: 8.17; 95% CI: 1.61 - 41.46; p = 0.011). Conclusions: There was a relationship between exposure to high levels of PM10 and increased risk of birth defects in the population studied. Future research on the relationship between environmental contamination and adverse obstetric events is suggested.

Introducción: El efecto deletéreo de material particulado fino exterior sobre la salud respiratoria de la población de niños y de adultos mayores, es de interés en salud pública. Objetivo: Establecer el efecto de la contaminación por Material Particulado de menos de 2,5 μm de diámetro (PM2,5), sobre la Enfermedad Respiratoria Aguda (ERA) en los menores de 5 y personas de mínimo 65 años, ajustado por variables meteorológicas y climáticas, en los municipios del Área Metropolitana del Valle de Aburrá (Colombia), 2008 a 2015. Materiales y métodos: Estudio ecológico con información de la red de vigilancia de calidad del aire y de registros de prestación de servicios de salud. Se construyeron Modelos Aditivos Generalizados con función de enlace Poisson y suavización spline. Para cada rezago distribuido se calculó la medida de la asociación e intervalo de confianza. Resultados: Los casos de ERA aumentaron significativamente en los menores de 5 años en Envigado y Caldas (43,3% vs 29,6%) y en los de 65 y más años, en Medellín (13,2%) por cada incremento de 10 µg/m3 en PM2,5 al día quince a partir de la exposición. Conclusiones: Los eventos diarios respiratorios tuvieron especial frecuencia en Medellín y en municipios de la zona sur.


Introduction: The harmful effect of fine particulate matter on the respiratory health of child and elderly populations is a concern for public health. Objective: To establish the effect of pollution by less than 2.5 μm in diameter (PM2.5) particulate matter on Acute Respiratory Disease (ARD) during 2008-2015 in children younger than 5 and adults older than 65 from the Metropolitan Area of the Aburrá Valley (Colombia), adjusting for meteorological and climate variables. Materials and methods: Ecological study with information from the air quality surveillance network and individual records of health providers. Generalized Additive Models were developed using smoothing spline Poisson models. The assessment of the association and confidence intervals were calculated for each distributed lag. Results: For each 10 µg/m3 increment in PM2,5 and the day 15 post-exposure, ARD cases increased significantly in populations who are younger than 5 and older than 65 in Envigado and Caldas (43.3% vs. 29.6%) and Medellín (13.2%), respectively. Conclusions: Daily respiratory events had a special frequency in Medellín and the municipalities of the southern region.

Abstract Objective: To evaluate air quality in the waiting room of a pediatric emergency service considering the serial concentrations of particulate matter (PM2.5), and to determine if the number of people present in the room can have an influence on the pollutant concentrations. Methods: Cross-sectional study, carried out in the waiting room of a reference pediatric hospital in the city of Porto Alegre, conducted in a one-year period, in a continuous-time sample including all of the four seasons of the year. The monitoring of PM2.5 was performed using a real-time aerosol monitor (DustTrak II). The number of people in the room was determined every hour and the climatic characteristics per daily mean. The concentration of PM2.5 and the number of people were expressed by mean and standard deviation. The means were compared by Analysis of Variance and Pearson's correlation coefficient. Results: There was a significant increase in the concentration of PM2.5 in the autumn, when compared to other seasons (p<0.001). The pollutant increase, in this season, was accompanied by the higher number of people in the emergency room (p=0.026). The association between PM2.5 and the number of people is confirmed by the positive correlation between these two variables (r=0.738; p<0.001). Conclusions: The pediatric emergency waiting room showed elevated PM2.5 in all seasons. The number of people in the room had a positive correlation with the concentration of the pollutant in the environment.


Resumo Objetivo: Avaliar a qualidade do ar na sala de espera de um serviço de emergência pediátrica pelas concentrações ambientais seriadas de material particulado (MP2,5) e determinar se o número de pessoas presentes no ambiente influencia as concentrações do poluente. Métodos: Estudo transversal, realizado na sala de espera de um hospital pediátrico de referência na cidade de Porto Alegre, conduzido ao longo de um ano, com um período de amostragem temporal contínua com duração de uma semana, em todas as quatro estações do ano. O monitoramento do MP2,5 foi realizado por meio de um monitor de aerossol em tempo real (DustTrak II). O número de pessoas no ambiente foi determinado a cada hora e as características climáticas por média diária. As concentrações de MP2,5 e o número de pessoas foram expressos por médias e desvio padrão. As médias foram comparadas por análise de variância e pelo coeficiente de correlação de Pearson. Resultados: Houve aumento significativo na concentração do MP2,5 no outono, quando comparado com as outras estações (p<0,001). O aumento desse poluente, nessa estação, esteve acompanhado de maior número de pessoas na emergência (p=0,026). A associação entre MP2,5 e número de pessoas é ratificado pela correlação positiva entre essas duas variáveis (r=0,738; p<0,001). Conclusões: A sala de espera da emergência pediátrica apresentou concentrações elevadas de MP2,5 durante todas as estações do ano. O número de pessoas no ambiente guardou correlação positiva com as concentrações do poluente no ambiente.

O aumento da prevalência de doenças respiratórias crônicas coincide com o da exposição aos poluentes atmosféricos pelo crescente processo de industrialização, aumento do tráfego veicular e migração da população para áreas urbanas. A poluição do ar é uma mistura complexa de poluentes e outros compostos químicos tóxicos e não tóxicos, e o efeito na saúde pode derivar dessa mistura e da interação com parâmetros meteorológicos. Apesar disso, busca-se estabelecer o papel de um poluente específico em separado e consideram-se os parâmetros meteorológicos como fatores de confusão. Há evidências de que a exposição aos poluentes contribui para maior morbidade e mortalidade por doenças respiratórias, especialmente nas crianças, mesmo em concentrações dentro dos padrões estabelecidos pela legislação. Identificar os efeitos dos poluentes no sistema respiratório, isoladamente e em associação, é um desafio, e os estudos têm limitações devido à variabilidade de resposta individual, a presença de doenças pré-existentes, aos fatores socioeconômicos, às exposições a poluentes intradomiciliares, ocupacionais e ao tabaco. A maioria das evidências sobre o efeito dos poluentes no sistema respiratório de crianças deriva de estudos que incluem desfechos de função pulmonar. Entretanto, esses estudos têm diferenças quanto ao desenho, ao método de avaliação de exposição aos poluentes, às medidas de função pulmonar, às covariáveis consideradas como capazes de alterar a resposta aos poluentes e aos tipos de modelos utilizados na análise dos dados. Considerar todas essas diferenças é fundamental na interpretação e comparação dos resultados dessas pesquisas com os dados já existentes na literatura.


The increase in the prevalence of chronic respiratory diseases coincides with that of exposure to air pollutants due to the growing industrialization process, increased vehicular traffic and population migration to urban areas. Air pollution is a complex mixture of pollutants and other toxic and non-toxic chemical compounds and its effect on health can derive from this mixture and the interaction with meteorological parameters. Despite this, it seeks to establish the role of a specific pollutant separately and considers the meteorological parameters as confounding factors. There is evidence that exposure to pollutants contributes to greater morbidity and mortality from respiratory diseases, especially in children, even at concentrations within the standards established by legislation. Identifying the effects of pollutants on the respiratory system, alone and in association, is a challenge and studies have limitations due to the variability of individual response, the presence of pre-existing diseases, socioeconomic factors, exposure to indoor, occupational and environmental pollutants as well tobacco. Most of the evidence on the effect of pollutants on the respiratory system of children comes from studies that include lung function outcomes. However, these studies differ in terms of design, method of assessing exposure to pollutants, measures of lung function, covariates considered capable of altering the response to pollutants, and types of models used in data analysis. Considering all these differences is fundamental in interpreting and comparing the results of these researches with data already existing in the literature.

Resumen OBJETIVO: Determinar la asociación entre la exposición a emisiones vehiculares de PM10 y monóxido de carbono y la preeclampsia en Manizales, Colombia. MATERIALES Y MÉTODOS: Estudio relacional, retrospectivo, de casos y controles efectuado entre julio de 2014 y julio de 2015 en pacientes con preeclampsia, residentes en la zona urbana de Manizales, Colombia. Se usó el sitio de residencia para la estimación de la exposición y la concentración de contaminantes a través de un instrumento de determinación de exposición a emisiones de PM10 y monóxido de carbono (toneladas por año por cada 250 metros cuadrados). Se utilizaron dos definiciones de exposición: cuartil superior (Q4) en comparación con los cuartiles restantes (Q1-Q3) y otra: comparación de los cuatro cuartiles tomando como referencia el primero (Q1). Se ajustaron modelos de regresión logística con el fin de explorar el efecto de la exposición. RESULTADOS: Se incluyeron 222 pacientes: 74 casos y 148 controles. No se observó relación entre la concentración de PM10 en el área de residencia de la madre y la probabilidad de preeclampsia con la primera definición de exposición (RM de 1.013 [IC95%: 0.35 a 2.97; p = 0.981] y la segunda [Q2; p = 0.562], [Q3; p = 0.347], [Q4; p=0.887]). Para el caso del monóxido de carbono tampoco se encontró relación estadística en las dos definiciones (RM: 0.829 [IC95%: 0.29 a 2.39] p = 0.729.). CONCLUSIONES: No se observó asociación entre las concentraciones de exposición a PM10 y monóxido de carbono y la aparición de preeclampsia durante los tiempos descritos.


Abstract OBJECTIVE: To determine the association between exposure to PM10 and carbon monoxide vehicle emissions and preeclampsia in Manizales, Colombia. MATERIALS AND METHODS: This was a relational, retrospective, case-control study in patients with pregnancy complicated by preeclampsia, residents of Manizales-Colombia between July 2014 and July 2015. The place of residence was used to estimate exposure and the concentration of pollutants through an instrument for determining exposure to PM10 and CO emissions (tons per year for every 250 square meters). Two definitions of exposure were used: upper quartile (Q4) vs the remaining quartiles (Q1-Q3) and another: comparison of the four quartiles taking the first quartile (Q1) as a reference. Logistic regression models were fitted in order to explore the effect of exposure. RESULTS: 222 patients were included, 74 cases and 148 controls. No relationship was observed between the concentration of PM10 in the mother's area of residence and the probability of presenting preeclampsia with both the first definition of exposure (OR of 1.013 (95%CI: 0.35 to 2.97) p = 0.981) as with the second (Q2 (p=0.562), Q3 (p = 0.347), Q4 (p = 0.887)). In the case of OC, no statistical relationship was found in the two definitions (OR: 0.829 (95%CI: 0.29 to 2.39) p = 0.729). CONCLUSIONS: No association was observed between the levels of exposure to PM10 and CO and the appearance of preeclampsia in pregnant women during the times described.

El material particulado (PM) es un grupo de elementos sólidos y líquidos producidos por diferentes actividades antropogénicas y naturales, que son emitidos directamente al aire. Tiene diferentes propiedades fisicoquímicas y termodinámicas y según su diámetro aerodinámico se puede clasificar en PM10 (<10 µm), PM2.5 (<2.5 µm) y PM0.1 (<0.1 µm); pueden inhalarse y llegar desde los pulmones a otros órganos, causando enfermedades respiratorias, cardiovasculares y neurológicas, entre otras. Considerando que el feto es susceptible a contaminantes atmosféricos a través de la exposición gestacional y que las perturbaciones en la vida temprana son cruciales para el desarrollo, el PM puede causar complicaciones, incluyendo parto prematuro, bajo peso al nacer y daños neuropsicológicos, entre otras. Esta revisión resume evidencia epidemiológica que demuestra la relación entre la exposición materna a PM y los resultados adversos en el desarrollo del feto, el nacimiento y la infancia, así como algunos mecanismos moleculares que podrían explicar estas asociaciones.


Summary Particulate matter (PM) is a group of solid and liquid elements produced by different anthropogenic and natural activities, which are emitted directly into the air. It has different physicochemical and thermodynamic properties and according to its aerodynamic diameter, it can be classified as PM10 (<10 µm), PM2.5 (<2.5 µm) and PM0.1 (<0.1 µm). It can be inhaled and reach other organs from the lungs, causing respiratory, cardiovascular and neurological diseases, among others. Considering that the foetus is susceptible to air pollutants such as PM through gestational exposure, and that early life disturbances are crucial for development, PM can cause various disorders or complications including premature birth, low birth weight and neuropsychological damage, among others. This review summarizes the epidemiological evidence that demonstrates the relationship between maternal exposure to PM and adverse outcomes in foetal development, birth, and childhood, as well as some molecular mechanisms that could explain these associations.

BACKGROUND AND OBJECTIVE: Parkinson's disease (PD) is the second most common neurodegenerative disease, and the etiology of its sporadic form is unknown. The present study analyzes the temporal and spatial variations of mortality by PD in Spain over a period of 14 years and its relationship with lead concentration levels in the atmosphere. METHOD: An ecological study was performed, in which deaths by PD and age group in 50 Spanish provinces between 2000 and 2013 were analyzed. The annual trend of PD mortality was assessed using the non-parametric Spearman's Rho test. Finally, the relationship between lead concentration levels in the air and mortality by PD was evaluated. RESULTS: Between 2000 and 2013, 36,180 patients with PD died in Spain. There is an increasing trend in mortality through PD over the study period (P<.0001). La Rioja, Asturias, Basque Country and the Lower Ebro valley were the regions with the highest values of PD mortality. Those regions with the highest lead concentrations also showed higher mortality by this disease in people over 64 (P=.02). CONCLUSIONS: Over our period of study, there has been an increase in mortality through PD in Spain, with the northernmost half of the country registering the highest values. Mortality in men was higher than mortality in women. Moreover, a direct correlation was found between lead levels in the air and mortality through PD.

INTRODUCCIÓN: En los estudios espirométricos se recomienda que cada población cuente con sus propios valores de referencia debido a condiciones regionales y a características de tipo étnico, etario, social, geográfico y climático. Estudios previos muestran elevados niveles de material particulado, hidrocarburos policíclicos y compuestos volátiles en el aire de La Plata y alrededores, con correlación entre exposición crónica a contaminantes y efectos adversos sobre desarrollo y función pulmonar. El objetivo fue establecer valores de referencia regionales para parámetros espirométricos en jóvenes de La Plata, siguiendo las recomendaciones de la American Thoracic Society (ATS) y la European Respiratory Society (ERS). MÉTODOS: Estudio transversal, observacional y descriptivo. Se realizaron y analizaron 171 espirometrías a jóvenes sanos de 13-17 años. Se llevaron a cabo medidas de correlación, identificando las variables predictoras. Para las ecuaciones de predicción se ajustaron modelos de regresión en función del sexo. RESULTADOS: La mayor correlación se encontró con talla y peso. Se desarrollaron ecuaciones para capacidad vital forzada (FVC) y volumen espiratorio forzado en el primer segundo (FEV1) según género. DISCUSIÓN: El cálculo de nuevos parámetros con valores menores a los propuestos por un estudio en Barcelona y adaptados por la Sociedad Española de Neumología y Cirugía Torácica (SEPAR) confirmó la importancia de contar con valores de referencia regionales.

On March 24, 2020, a partial lockdown was decreed in the state of São Paulo, Brazil, as a measure to hinder the spread of COVID-19, which consisted in prohibiting crowding and advising people to stay home, except for urgent or extremely necessary matters. Based on studies performed in other countries, this study aims to assess the impacts of the lockdown on the air quality of five cities in the state of São Paulo. Our study was conducted by using particulate matter and nitrogen dioxide as air quality indicators, and by correlating the contaminants concentrations with weather data. The results showed an increase in these contaminants in all cities within the first weeks after the lockdown compared with the weeks before the decree and with the same period in previous years. This result is inconsistent with the literature. Therefore, a secondary goal was set to investigate the possible cause (or causes) of such deterioration in air quality, which led to the increased number of wildfires. The anomalous dry weather favored the burning of vegetation in agricultural rural areas and in small, vegetated areas near the municipalities, and limited pollution scavenging by rainfall, both of which contributed to higher pollution concentration. We hypothesize the possible effects of worse air quality on the aggravation of COVID-19, but further research is necessary to obtain a complete assessment.


Em 24 de março de 2020, foi decretado confinamento parcial no Estado de São Paulo, Brasil, como medida para desacelerar a disseminação da COVID-19. O decreto consistia na proibição de aglomerações e na recomendação para as pessoas permanecerem em casa, exceto em situações urgentes ou de extrema necessidade. Na esteira de estudos realizados em outros países, o artigo busca avaliar os impactos do confinamento na qualidade do ar em cinco cidades no Estado de São Paulo. Nosso estudo foi realizado com o material particulado e dióxido de nitrogênio enquanto indicadores da qualidade do ar e pela correlação das concentrações dos contaminantes com dados meteorológicos. Os resultados mostraram um aumento desses contaminantes em todas as cinco cidades dentro das primeiras semanas depois do confinamento, comparado às semanas que antecederam o decreto e com o mesmo período em anos anteriores. O resultado é inconsistente com os achados usualmente relatados em outros estudos. Portanto, foi definido um objetivo secundário a fim de investigar a possível causa (ou causas) da piora na qualidade do ar, o que revelou um aumento no número de incêndios. O tempo anormalmente seco favoreceu a queima de vegetação nas áreas agrícolas rurais e em pequenas áreas de vegetação próximas às cidades, além do limitado escoamento da poluição pela chuva, o que contribuiu à maior concentração de poluentes. Os achados sugerem hipóteses sobre os possíveis efeitos dessa situação de pior qualidade do ar sobre o agravamento de casos de COVID-19, porém são necessários mais estudos para uma avaliação completa.


El 24 de marzo de 2020 se decretó un confinamiento parcial en el estado de São Paulo, Brasil, como medida para evitar la propagación de la COVID-19, que consistió en prohibir aglomeraciones de personas y avisar a la gente que permaneciera en casa, salvo para asuntos urgentes o extremadamente necesarios. Siguiendo algunos estudios realizados en otros países, el objetivo de este trabajo es evaluar los impactos del confinamiento en la calidad del aire de ciudades en el estado de São Paulo. Nuestro estudio fue realizado usando material particulado y dióxido de nitrógeno, como indicadores de la calidad del aire, y mediante la correlación de las concentraciones de contaminantes con los datos meteorológicos. Los resultados mostraron un incremento en estos contaminantes en todas las ciudades dentro de las primeras semanas tras el confinamiento, comparando las semanas antes del decreto y dentro del mismo periodo en los años previos. Este resultado es inconsistente con aquellos que han sido informados en otros estudios. Por ello, un objetivo secundario fue investigar la posible causa (o causas) de tal deterioro en la calidad del aire, que conduce a un incremento en el número de incendios. La climatología seca anómala favoreció la quema de vegetación en las áreas rurales agrícolas, y en áreas con poca vegetación cerca de las ciudades, y la eliminación limitada de la contaminación gracias a la lluvia, ambos contribuyeron a una mayor concentración de contaminación. Se especula que los posibles efectos de esta situación de una peor calidad del aire podrían afectar en el agravamiento de los casos de COVID-19, pero son quizás necesarias más investigaciones para conseguir una evaluación completa.

OBJECTIVE: To estimate the association between exposure to fine particulate matter with an aerodynamic diameter <2.5 microns (PM2.5) and hospitalizations for pneumonia and asthma in children. METHODS: An ecological study of time series was performed, with daily indicators of hospitalization for pneumonia and asthma in children up to 10 years of age, living in Taubaté (SP) and estimated concentrations of PM2.5, between August 2011 and July 2012. A generalized additive model of Poisson regression was used to estimate the relative risk, with lag zero up to five days after exposure; the single pollutant model was adjusted by the apparent temperature, as defined from the temperature and relative air humidity, seasonality and weekday. RESULTS: The values of the relative risks for hospitalization for pneumonia and asthma were significant for lag 0 (RR=1.051, 95%CI; 1.016 to 1.088); lag 2 (RR=1.066, 95%CI: 1.023 to 1.113); lag 3 (RR=1.053, 95%CI: 1.015 to 1.092); lag 4 (RR=1.043, 95%CI: 1.004 to 1.088) and lag 5 (RR=1.061, 95%CI: 1.018 to 1.106). The increase of 5mcg/m(3) in PM2.5 contributes to increase the relative risk for hospitalization from 20.3 to 38.4 percentage points; however, the reduction of 5µg/m(3) in PM2.5 concentration results in 38 fewer hospital admissions. CONCLUSIONS: Exposure to PM2.5 was associated with hospitalizations for pneumonia and asthma in children younger than 10 years of age, showing the role of fine particulate matter in child health and providing subsidies for the implementation of preventive measures to decrease these outcomes.
