RESUMEN

To understand the transmissibility and spread of infectious diseases, epidemiologists turn to estimates of the instantaneous reproduction number. While many estimation approaches exist, their utility may be limited. Challenges of surveillance data collection, model assumptions that are unverifiable with data alone, and computationally inefficient frameworks are critical limitations for many existing approaches. We propose a discrete spline-based approach that solves a convex optimization problem-Poisson trend filtering-using the proximal Newton method. It produces a locally adaptive estimator for instantaneous reproduction number estimation with heterogeneous smoothness. Our methodology remains accurate even under some process misspecifications and is computationally efficient, even for large-scale data. The implementation is easily accessible in a lightweight R package rtestim.

RESUMEN

Acute pancreatitis-induced splanchnic vein thrombosis (APISVT) is an important sequela complication of acute pancreatitis, which may cause poor prognosis, such as severe gastrointestinal hemorrhage, bowel ischemic necrosis and liver failure. However, its mechanism remains uncertain, and there is not a general consensus on the management. In this study, we reviewed the latest academic publications in APISVT, and discussed its pathogenesis, clinical presentation, adverse outcome and treatment, especially focused on the role of anticoagulant therapy. It was indicated that anticoagulation therapy can significantly elevate thrombus recanalization and reduce the incidence of complications and mortality with no increase of bleeding. Actually, as most of these studies were retrospective analyses and prospective studies included small samples, the conclusion remains controversial. Thus, well-designed randomized controlled trials are urged to verify the effectiveness and safety of anticoagulation therapy for APISVT.

RESUMEN

The monomer emission property of the tetraphenylethene (TPE) derivative is rarely reported, and its photoluminescence (PL) mechanism related to supramolecular self-assembly needs further in-depth research. Two long alkyl chain modified derivatives, the TPE derivative (TPE-C10) and pyrene derivative (Pyrene-C10), are designed and synthesized, which possess similar supramolecular assembly behavior but exhibit different PL properties. TPE-C10 not only forms self-assembly morphologies with monomer emission but also emits aggregation-induced emission (AIE). Moreover, the polymer microspheres containing TPE-C10 and Pyrene-C10 are prepared, which can dissolve or swell in different organic solvents. The changed binding effect of polymer chains achieves the luminescence transformation of TPE-C10 from AIE to monomer emission. This work hopefully can enrich luminescent materials based on the monomer emission of the TPE derivative and provide a new method for mechanism studies about supramolecular self-assembly and luminescence.

This field study aimed to investigate naturally ventilated bedroom environment and its effects on subjective perception and sleep quality. Totally, 104 healthy subjects living in urban areas of Beijing participated in the study for one night during transition seasons. Bedroom environment parameters, including temperature, relative humidity, and CO2 concentration, were recorded before and during sleep. Objective sleep quality was measured by Fitbit Alta 2, a wrist-type actigraphy sensor. Subjective assessments were collected by paper-based questionnaires on sleep quality and environmental perceptions. The results showed that neutral temperature for waking state (before sleep) was estimated to be 23.8°C while for sleep state it was 26.5°C. Furthermore, pre-sleep thermal sensation vote was found to be positively correlated with deep sleep percentage. Indoor air quality was correlated with sleep quality as indicated by statistically significant correlations between odor intensity assessment, air quality acceptability, average nightly CO2 concentration, and measures of sleep quality. For naturally ventilated bedrooms during transition seasons with a mild outdoor climate, present findings suggest that a bedroom with slightly warm pre-sleep environment than neutral, and with high ventilation as indicated by low indoor CO2 concentration, could be beneficial for sleep quality of residents.

To investigate the need of allergic population for indoor environment quality, exposure effects of poor air quality on subjects with respiratory allergies were compared with those on healthy people, including perceptual responses, health symptoms, mental performance, and physiological responses. The experimental intervention was with and without ventilation at thermally neutral rooms, creating two exposure conditions indicated by CO2 concentration ranges of 502 to 3297 ppm (2438 ± 1527 ppm) and 517 to 5687 ppm (3615 ± 1527 ppm). 63 subjects (32 allergic subjects and 31 non-allergic subjects) were exposed to both conditions for 3 hours. The main results suggested that, compared with healthy people, people with respiratory allergy seem to be more sensitive or less acceptable to reduced air quality polluted by occupants during instantaneous exposure. Besides, the allergic group performed worse in cognitive tests than non-allergic group. After 3 hours of continuous exposure, people with respiratory allergy reported stronger intensity of respiratory irritations and seemed to suffer more inflammation indicated by a higher level of interleukin 1L-1ß.

A design scheme of multi-element sensor which included electrical resistivity probes, multiple Cl- selective electrodes, and a steel corrosion monitoring system was proposed in this work. Embedding this multi-element sensor in concrete enables the real-time and non-destructive monitoring of internal electrical resistivity, free Cl- (Clf) contents in the concrete pore solution at different depths, and steel corrosion parameters. Based on the monitoring data obtained by the multi-element sensor, the freezing-thawing (F-T) damage degree, the Clf diffusion coefficient, the quantitative relation between F-T damage degree and Clf diffusion coefficient, the initiation period of steel corrosion, and the critical content related to steel corrosion are determined. To conclude, the multi-element sensor provides key durability parameters for the establishment of the Clf diffusion model, the assessment of health condition, and the prediction of service life of concrete under the coexistence of the F-T cycle and Cl-.

The effect of the thermal environment on sleep quality has attracted considerable attention, as sleep forms one-third of human lifetime and the occupied space is largely constrained during sleep. With an increasing development of partial space regulation and task air conditioning systems and devices, thermal comfort demand concerning local thermal conditions has attracted more and more attention. In the present study, experiment was conducted and data mining technologies were performed to investigate correlations between local thermal conditions and whole body thermal comfort in sleeping state. The identification of local thermal condition included two steps: the first step was to clarify thermal sensation links between local and covered body, and the second step was to identify local thermal sensation inclination towards different thermal comfort levels. Thermal sensation correlations among local body parts and covered body were obtained. Back, face, and thigh were identified as three dominant linear-correlated local parts with weighting factors 0.488, 0.388, and 0.152, respectively; in addition, chest, arm, leg and foot were found as non-negligible local parts in the estimation of covered body thermal sensation. By dividing the sleeping human body into three parts as head, trunk and extremity, the proper local thermal sensations and their coupling relationships for whole body sleeping thermal comfort have been elaborated by three rules. The present study provides implications in sleeping thermal environment regulation in neutral to cold indoor conditions.

This work investigates the change in conformations and the entropic free energy barrier in the "head-on" adsorption process of a flexible polymer chain towards a flat surface in the framework of the Gaussian chain model. Analytical expressions are reported for the distribution of chain end, the mean square end-to-end distance, the entropic free energy barrier and the adsorption equilibrium constant at low surface coverage (the "mushroom" regime). Theoretical results are discussed in the context of polyoxyethylene-phosphonate-type polymer dispersants, possessing a non-adsorbing polyoxyethylene chain and an adsorbing head-group.

Background: Combined small cell lung cancer (C-SCLC) is a rare type of small cell lung cancer (SCLC), and it is controversial whether to choose the same treatment regimen as SCLC due to its multiple histologic components. Study methods and results: Records of patients with small cell lung cancer diagnosed between 2010 and 2020 were extracted using the SEER database. The OS of patients with different histological types under the same staging and treatment regimen was analyzed. It was found that early-stage (stage IA-IIA) surgical treatment, systemic chemotherapy alone, and chemoradiotherapy were more efficacious than C-SCLC and P-SCLC in patients with limited-stage (P = 0.054, P = 0.001, P = 0.019). In patients with extensive staging, the OS of patients with systemic chemotherapy regimens differed (P = 0.045) and was better in C-SCLC than in P-SCLC. We further explored the treatment strategy for patients with C-SCLC, which was shown by a COX regression model based on prognostic factors screened by Random Forest and LASSO regression models. Surgery, radiotherapy, and chemotherapy would be beneficial for survival. In a subgroup analysis based on stage and treatment regimen, it was shown that patients with early staging (stage IA-IIA) had a better prognosis with surgery (P < 0.001); in patients with extensive staging, chemoradiotherapy was favorable to the patient's prognosis (P = 0.022). Conclusion: Both limited-stage and extensive-stage C-SCLC patients are more sensitive to chemotherapy than P-SCLC patients. Patients with C-SCLC who have access to surgery should undergo surgery as early as possible, while chemoradiotherapy is recommended for patients with extensive staging. Patient age, gender, tumor size, surgery, chemotherapy, radiotherapy, and metastasis may individually affect patient prognosis.

Nucleic acid drugs are one of the hot spots in the field of biomedicine in recent years, and play a crucial role in the treatment of many diseases. However, its low stability and difficulty in target drug delivery are the bottlenecks restricting its application. Hydrogels are proven to be promising for improving the stability of nucleic acid drugs, reducing the adverse effects of rapid degradation, sudden release, and unnecessary diffusion of nucleic acid drugs. In this review, the strategies of loading nucleic acid drugs in hydrogels are summarized for various biomedical research, and classify the mechanism principles of these strategies, including electrostatic binding, hydrogen bond based binding, hydrophobic binding, covalent bond based binding and indirect binding using various carriers. In addition, this review also describes the release strategies of nucleic acid drugs, including photostimulation-based release, enzyme-responsive release, pH-responsive release, and temperature-responsive release. Finally, the applications and future research directions of hydrogels for delivering nucleic acid drugs in the field of medicine are discussed.

Background: Acetaminophen (APAP) is commonly used as an antipyretic analgesic. However, acetaminophen overdose may contribute to liver injury and even liver failure. Acetaminophen-induced liver injury (AILI) is closely related to mitochondrial oxidative stress and dysfunction, which play critical roles in cuproptosis. Here, we explored the potential role of cuproptosis-related genes (CRGs) in AILI. Methods: The gene expression profiles were obtained from the Gene Expression Omnibus database. The differential expression of CRGs was determined between the AILI and control samples. Protein protein interaction, correlation, and functional enrichment analyses were performed. Machine learning was used to identify hub genes. Immune infiltration was evaluated. The AILI mouse model was established by intraperitoneal injection of APAP solution. Quantitative real-time PCR and western blotting were used to validate hub gene expression in the AILI mouse model. The copper content in the mouse liver samples and AML12 cells were quantified using a colorimetric assay kit. Ammonium tetrathiomolybdate (ATTM), was administered to mouse models and AML12 cells in order to investigate the effects of copper chelator on AILI. Results: The analysis identified 7,809 differentially expressed genes, 4,245 of which were downregulated and 3,564 of which were upregulated. Four optimal feature genes (OFGs; SDHB, PDHA1, NDUFB2, and NDUFB6) were identified through the intersection of two machine learning algorithms. Further nomogram, decision curve, and calibration curve analyses confirmed the diagnostic predictive efficacy of the four OFGs. Enrichment analysis indicated that the OFGs were involved in multiple pathways, such as IL-17 pathway and chemokine signaling pathway, that are related to AILI progression. Immune infiltration analysis revealed that macrophages were more abundant in AILI than in control samples, whereas eosinophils and endothelial cells were less abundant. Subsequently, the AILI mouse model was successfully established, and histopathological analysis using hematoxylin-eosin staining along with liver function tests revealed a significant induction of liver injury in the APAP group. Consistent with expectations, both mRNA and protein levels of the four OFGs exhibited a substantial decrease. The administration of ATTAM effectively mitigates copper elevation induced by APAP in both mouse model and AML12 cells. However, systemic administration of ATTM did not significantly alleviate AILI in the mouse model. Conclusion: This study first revealed the potential role of CRGs in the pathological process of AILI and offered novel insights into its underlying pathogenesis.

The flow-field characteristics of high-temperature annular buoyant jets as well as the development laws influenced by ventilation system were studied using numerical methods to eliminate the pollutants effectively in this paper. The development laws of high-temperature annular buoyant jets were analyzed and compared with previous studies, including radial velocity distribution, axial velocity and temperature decay, reattachment position, cross-section diameter, volumetric flow rate, and velocity field characteristics with different pressures at the exhaust hood inlet. The results showed that when the ratio of outer diameter to inner diameter of the annulus was smaller than 5/2, the flow-field characteristics had significant difference compared to circular buoyant jets with the same outer diameter. For similar diameter ratios, reattachment in this paper occurred further downstream in contrast to previous study. Besides, the development laws of volumetric flow rate and cross-section diameter were given with different initial parameters. In addition, through analyzing air distribution characteristics under the coupling effect of high-temperature annular buoyant jets and ventilation system, it could be found that the position where maximum axial velocity occurred was changing gradually when the pressure at the exhaust hood inlet changed from 0 Pa to -5 Pa.

The pollutants emitted by traveling vehicles are prone to accumulation inside urban highway tunnels, which poses a serious threat to the driving safety and health of passengers. This study employed the dynamic mesh method to simulate a traveling vehicle and investigated the coupling effect of vehicle wake and jet flow on the dispersion characteristics of pollutants in urban highway tunnels. To ensure the accuracy of the numerical simulation results, the turbulence model (realizable k-ε model) and dynamic mesh model were validated through field tests. The results revealed that jet flow can disrupt the large-scale longitudinal vortices pattern in the wake region, whereas vehicle wake can simultaneously weaken the entrainment strength of jet flow. The jet flow was found to be decisive in the space with a height greater than 4 m, whereas the vehicle wake intensity was considerably stronger at the bottom space of the tunnel, leading to the accumulation of pollutants in the passenger breathing zone. To evaluate the effect of jet fans on pollutants in the breathing zone, an innovative dilution efficiency was proposed. The dilution efficiency can be significantly affected by the intensity of vehicle wake and turbulence. Moreover, the dilution efficiency of alternative jet fans was better than that of traditional jet fans.

The inherent quasi-brittleness of cement-based materials, due to the disorder of their hydration products and pore structures, present significant challenges for directional matrix toughening. In this work, a rigid layered skeleton of cement slurry was prepared using a simplified ice-template method, and subsequently flexible polyvinyl alcohol hydrogel was introduced into the unidirectional pores between neighboring cement platelets, resulting in the formation of a multi-layered cement-based composite. A toughness improvement of over 175 times is achieved by the implantation of such hard-soft alternatively layered microstructure. The toughening mechanism is the stretching of hydrogels at the nano-scale and deflections of micro-cracks at the interfaces, which avoid stress concentration and dissipate huge energy. Furthermore, this cement-hydrogel composite also exhibits a low thermal conductivity (around 1/10 of normal cement) and density, high specific strength and self-healing properties, which can be used in thermal insulation, seismic high-rise buildings and long-span bridges.

Aim: The objective of this study is to examine the correlation between patient serum cholinesterase (SCHE) concentration and weaning failure in the context of invasive mechanical ventilation (IMV), as well as to identify predictors of ventilator weaning failure. Additionally, this study investigates the potential relationship between SCHE and nutritional risk for developing more effective weaning strategies. Method: A retrospective observational study was conducted. The sample was collected from 227 patients with IMV over 48 h who underwent SBT before weaning. Relevant experimental samples and data collection were analyzed at the time of patient admission and before the initiation of the SBT. The correlation between SCHE and weaning failure was determined by multifactorial logistic regression and propensity matching scores. Results: Weaning was successful in 127 patients and failed in 100 patients. Depending on the difficulty of weaning, 55 of these patients had difficulty in weaning and 45 had long-term weaning. In the crude cohort, experimental data collected on the day of SBT showed that SCHE concentrations were higher in patients with successful weaning than in those with failed weaning (4,514 u/l vs. 3,190 u/l p < 0.01). The critical value for predicting weaning failure was SCHE 3,228 u/l (p < 0.01). Ventilator weaning failure was predicted by multifactorial logistic regression analysis of SCHE, heart rate, and PaO2 before SBT, with SCHE predicting ventilator weaning failure (AUC 0.714; 95% CI 0.647-0.782) better than heart rate (AUC 0.618; 95% CI 0.545-0.690), PaO2 (AUC 0.59; 95% CI 0.515-0.664). After propensity-matched scores, SCHE remained an independent predictor of weaning failure (p = 0.05). And the SCHE concentration was strongly correlated with the patient's weaning difficulties (p < 0.01). The Nutrition Risk in Critically Ill (NUTRIC) score was also significantly correlated with SCHE according to Spearman's correlation analysis (p < 0.01). Conclusion: Our study revealed that the patients who experienced weaning failure exhibited lower SCHE values compared to those who successfully underwent weaning. Before spontaneous breathing trial (SBT), SCHE, heart rate, and PaO2 were identified as independent predictors of weaning failure. Following propensity score matching (PSM), SCHE and heart rate remained independent predictors. Patients with SCHE levels below 3,228 u/l should undergo careful evaluation before weaning. Our findings suggest that malnutrition may be a contributing factor to weaning failure in patients.

Background: Liver cancer due to hepatitis C (LCDHC) is one of the leading causes of cancer-related deaths worldwide, and the burden of LCDHC is increasing. We aimed to report the burden of LCDHC at the global, regional, and national levels in 204 countries from 1990 to 2019, stratified by etiology, sex, age, and Sociodemographic Index. Methods: Data on LCDHC were available from the Global Burden of Disease, Injuries, and Risk Factors (GBD) study 2019. Numbers and age-standardized mortality, incidence, and disability-adjusted life year (DALY) rates per 100,000 population were estimated through a systematic analysis of modeled data from the GBD 2019 study. The trends in the LCDHC burden were assessed using the annual percentage change. Results: Globally, in 2019, there were 152,225 new cases, 141,810 deaths, and 2,878,024 DALYs due to LCDHC. From 1990 to 2019, the number of incidences, mortality, and DALY cases increased by 80.68%, 67.50%, and 37.20%, respectively. However, the age-standardized incidence, mortality, and DALY rate had a decreasing trend during this period. In 2019, the highest age-standardized incidence rates (ASIRs) of LCDHC were found in high-income Asia Pacific, North Africa and the Middle East, and Central Asia. At the regional level, Mongolia, Egypt, and Japan had the three highest ASIRs in 2019. The incidence rates of LCDHC were higher in men and increased with age, with a peak incidence in the 95+ age group for women and the 85-89 age group for men in 2019. A nonlinear association was found between the age-standardized rates of LCDHC and sociodemographic index values at the regional and national levels. Conclusions: Although the age-standardized rates of LCDHC have decreased, the absolute numbers of incident cases, deaths, and DALYs have increased, indicating that LCDHC remains a significant global burden. In addition, the burden of LCDHC varies geographically. Male and older adult/s individuals have a higher burden of LCDHC. Our findings provide insight into the global burden trend of LCDHC. Policymakers should establish appropriate methods to achieve the HCV elimination target by 2030 and reducing the burden of LCDHC.

Capecitabine is a widely-used antineoplastic drug, a prodrug to 5-fluorouracil which commonly induces gastrointestinal toxicity. Enterocolitis, as a rarely recognized gastrointestinal adverse effect (AE) of capecitabine, is potentially severe and usually results in antitumor treatment withdrawal. For the better management of severe AEs, pharmacogenetics is one promising field. Herein, we describe a case of capecitabine-induced enterocolitis presenting with severe diarrhea in order to improve recognition by clinicians. Moreover, we conduct a pharmacogenetic profile of the patient and review the current studies of gene polymorphisms of 5-fluorouracil-related diarrhea, hoping to offer a reference for further clinical pharmacogenetic practice in predicting capecitabine AEs showing diarrhea as the main symptom.

Temperature distribution in concrete is significant to the concrete structure's macro properties and different factors affect the heat transfer in concrete, and therefore influence the temperature distribution. This work established a three-dimensional transient heat transfer model coupled with various environmental factors, using the finite element method for calculating the results and real-measured data for testing accuracy. In addition, a sensitivity evaluation of various factors was conducted. Due to various environmental factors, the results revealed that the prediction of temperature distribution in concrete by the three-dimensional model had great accuracy with an error of less than 4%. A particular hysteresis effect of temperature response in the concrete existed. Considering heat transfer in different spatial directions, the model can predict the temperature change of each spatial point instead of the spatial surface in different depths, proving the shortcomings of a one-dimensional heat transfer model. A greater solar radiation intensity caused a more significant temperature difference on the concrete surface: the surface temperature difference in July was twice as significant as that in December. Wind speed had a cooling effect on the concrete surface, and stronger wind speed accompanied with a stronger cooling effect made the surface temperature closer to the ambient temperature. Material properties had different effects on the temperature distribution of the surface part and internal part: the specific heat capacity determined the speed of the outer layer temperature change while the thermal conductivity determined the speed of the inner layer temperature change.

A microbial transglutaminase-induced cross-linked sodium caseinate (MSC) was used to stabilize zein nanoparticles, and the study was to investigate whether zein-MSC nanoparticles (zein-MSC NPs) can be used as an encapsulation carrier for resveratrol. A group of resveratrol-loaded zein-MSC nanoparticles (Res-zein-MSC NPs) with varying zein to Res mass ratios was first prepared. The particle sizes and zeta-potentials were in the ranges from 215.00 to 225.00 nm and from -29.00 to -31.00 mV. The encapsulation efficiency (EE) of Res was also influenced by the zein to Res mass ratio, and the encapsulated Res existed in an amorphous form. The major interactions between Res and zein-MSC NPs were hydrogen bonding and hydrophobic interaction. Furthermore, compared with free Res, the photo-stability and bioaccessibility of Res-zein-MSC NPs were significantly improved. The cellular studies also showed that Res-zein-MSC NPs exhibited lower cytotoxicity and desirable anti-inflammatory activity.

Durability improvement is always important for steel-concrete structures exposed to chloride salt environment. The present research investigated the influence of a novel nano-precursor inhibiting material (NPI), organic carboxylic acid ammonium salt, on the mechanical and transport properties of concrete. The NPI caused a slight reduction in the strength of concrete at later ages. NPI significantly decreased water absorption and slowed down the speed of water absorption of concrete. In addition, the NPI decreased the charge passed and the chloride migration coefficient, and the results of the natural chloride diffusion showed that the NPI decreased the chloride concentration and the chloride diffusion coefficient. The NPI effectively improved the resistance of chloride penetration into testing concrete. The improvement in the impermeability of concrete was ascribed to the incorporation with the NPI, which resulted in increasing the contact angle of cement pastes. The contact angle went up from 17.8° to 85.8° for 0% and 1.2% NPI, respectively, and cement pastes became less hydrophilic. Some small pore throats were unconnected. Besides, the NPI also optimized the pore size distribution of hardened cement paste.
