RESUMEN

Reticulocyte indices are used to characterize anemia, including the identification of regeneration. In people, the immature reticulocyte fraction (IRF), percentage of hypochromic red blood cells (%HYPO-RBC), and other reticulocyte indices have been used as earlier indicators of erythropoiesis and as valuable monitoring tools in the assessment of various therapies. The reference intervals (RI) of the IRF and %HYPO-RBC have not been reported in dogs. The objective of this study was to establish RIs for novel variables (IRF, %HYPO-RBC, and CH-delta) and assess RIs for more commonly reported reticulocyte indices in healthy dogs. RIs were calculated from blood results retrospectively collected from 106 client-owned healthy dogs at the time of induction into a blood donor program using the ADVIA 2120 hematology analyzer (Siemens Healthcare Diagnostics). For the calculation of RIs, appropriate tests were applied for outlier detection and normality assessment. For variables normally distributed, RIs and their respective 90% confidence intervals (CIs) were calculated using parametric methods, while for variables not normally distributed, robust methods were used and bootstrapping for calculating the 90% CIs. The following RIs were established: reticulocyte hemoglobin content (CHr) 24.5-28 pg, mean reticulocyte volume (MCVr) 85.9-99.3 fL, mean corpuscular hemoglobin concentration of reticulocytes (CHCMr) 271.0-306.3 g/L, IRF 10.4%-43.5%, CH-delta 0.5-4.3 pg, and percentage of hypochromic red blood cells (%HYPO-RBC) 0.10%-0.80%. The results of this study provide RIs for novel reticulocyte variables. Further studies are required to determine the clinical utility of IRF, %HYPO-RBC, and CH delta as early indicators of erythropoietic activity in canine patients.

RESUMEN

A 2-year-old male neutered domestic shorthair cat underwent investigations for acute onset of lethargy, hyporexia, and cough. Computed tomography of the thorax identified a large mass-like lesion in the left cranial lung lobe and bilateral pleural effusion. Thoracotomy and left cranial lung lobectomy were performed. Histopathology of the pulmonary mass was consistent with a localized Toxoplasma gondii pneumonia, confirmed by positive polymerase chain reaction on the affected lung lobe. After adjunctive medical management with a 28-day course of clindamycin (12.5 mg/kg PO q12h), clinical signs resolved and repeat thoracic radiographs documented no abnormalities. The cat remains clinically well 1 year after surgery.

RESUMEN

A 3-year-old male neutered Shih Tzu cross was presented for investigation of a three-week history of weight loss, seborrhoea, vomiting and diarrhoea. Initial clinicopathological findings included pancytopenia, mild hypercalcaemia and marked hyperglobulinaemia. Subsequent bone marrow and skin biopsies revealed the presence of Leishmania amastigotes. Quantitative serology was positive for Leishmania species and PCR on the bone marrow sample confirmed a Leishmania infantum infection. The patient had been in the owner's possession since a puppy, had no travel history outside of the UK and had never received a blood transfusion or been used for breeding. However, another dog in the household that had been imported from Spain had been euthanased six months previously due to severe leishmaniosis. To the authors' knowledge, this is the first reported case of canine leishmaniosis in the UK without a history of travel to an endemic area, and most likely represents a case of dog-to-dog transmission.

RESUMEN

An 11-year-old cat was presented with an approximately 2-month history of dysphagia, intermittent regurgitation and weight loss. An oesophageal foreign body was identified on plain radiographs, and an oesophagotomy was performed to remove a large V-shaped bone from the caudal cervical oesophagus. A gastrostomy feeding tube was placed to allow nutritional support postoperatively. Medical treatment for oesophagitis was initiated after surgery. No complications were encountered and the cat was discharged 4 days after surgery.

OBJECTIVE: To determine whether dogs with ascites secondary to right-sided congestive heart failure (CHF) have bleeding disorders associated with hypofibrinogenemia and discordant plasma fibrin-fibrinogen degradation products (FDPs) and D-dimer assay results (ie, a circulating concentration of FDPs higher than the reference range and a circulating concentration of D-dimer within the reference range). DESIGN: Retrospective case-control study. ANIMALS: 80 client-owned dogs. PROCEDURES: Dogs with ascites secondary to right-sided CHF (group 1; n = 20), unhealthy dogs without cardiac disease (group 2; 40), and dogs with left-sided CHF (group 3; 20) were included in the study. Urine bile acids-to-creatinine concentration ratios were calculated as a marker of liver function. Differences among groups regarding coagulation profile analysis results and prevalence of discordant FDPs and D-dimer assay results were determined. RESULTS: No significant differences were detected among the 3 groups regarding urine bile acids-to-creatinine concentration ratios. Plasma fibrinogen concentration was significantly lower for group 1 versus groups 2 or 3. Prevalence of discordant FDPs and D-dimer assay results was significantly higher for group 1 versus groups 2 or 3. Eighteen group 1 dogs had discordant FDPs and D-dimer assay results. Ten of these dogs had concurrent hypofibrinogenemia, 2 of which had clinical signs of bleeding. Only 10 dogs in groups 2 or 3 had discordant FDPs and D-dimer assay results; none of these dogs had hypofibrinogenemia or clinical signs of bleeding. CONCLUSIONS AND CLINICAL RELEVANCE: Dogs with right-sided CHF and ascites may be at increased risk for primary hyperfibrinogenolysis (ie, hypofibrinogenemia and discordant FDPs and D-dimer assay results).

BACKGROUND: Complicated hypertension can be influenced by the characteristics of hypertensive patients. OBJECTIVE: To associate the condition of complicated hypertension with biosocial variables such as attitudes and beliefs about the disease and treatment and subjective well-being. METHODS: We studied 251 uncomplicated hypertensive patients (SBP > 140 mmHg and/or 90 < DBP < 110 mmHg for patients under no treatment and DBP <110mmHg for patients under treatment without target organ damage and other diseases) and 260 complicated hypertensive patients (DBP > 110 mmHg with or without treatment, with target organ damage or other diseases). RESULTS: Complicated hypertensive patients were significantly different from uncomplicated ones (p <0.05) in relation to: 1 - Prevalence of men, not white (53.0%), higher body mass index (29.5 ± 4.6 vs 28.5 ± 4.0 kg/m²), over 10 years of disease (54.0%), completion of previous treatment (53.0%) and reports of sadness about life as a whole (74.0%) 2 - Complicated hypertensive patients never bring the drugs when they travel (59.0%), nor do they buy them before running out the drugs (71.0%) and rarely follow eating guidelines (69.0%) 3 - Uncomplicated hypertensive patients showed no more migraines, joint pain and, among women, menopausal status and hormone replacement therapy, and 4 - Of those who had pressure control (< 140/90 mmHg), 61.9% were uncomplicated hypertensive patients; and 5 - Complicated hypertensive patients were not aware that treatment can prevent kidney problems and they thought that young people do not have high blood pressure. CONCLUSION: Complicated hypertensive patients showed more negative structural and psychosocial characteristics, more negative attitudes towards treatment and are unaware of the disease.

FUNDAMENTO: A hipertensão complicada pode ser influenciada pelas características dos pacientes hipertensos. OBJETIVO: Associar a condição de hipertensão complicada com variáveis biossociais, tais como as atitudes e as crenças sobre a doença e o tratamento e o bem-estar subjetivo. MÉTODOS: Foram estudados 251 hipertensos não complicados (PAS > 140 mmHg e/ou 90 < PAD < 110 mmHg para pacientes sem tratamento e PAD < 110 mmHg para pacientes com tratamento, sem lesões em órgãos-alvo e outras doenças) e 260 hipertensos complicados (PAD > 110 mmHg com ou sem tratamento, com lesões em órgãos-alvo ou outras doenças). RESULTADOS: Os hipertensos complicados foram significativamente diferentes dos não complicados (p < 0,05) em relação a: 1 - Predomínio de homens, não brancos (53,0 por cento), maior índice de massa corporal (29,5 ± 4,6 vs 28,5 ± 4,0 kg/m²), mais de 10 anos de doença (54,0 por cento), realização de tratamento anterior (53,0 por cento) e referência de tristeza em relação a sua vida como um todo (74,0 por cento); 2 - Os hipertensos complicados nunca levam os remédios quando viajam (59,0 por cento), nem os providenciam antes de acabarem (71,0 por cento) e raramente seguem as orientações sobre alimentação (69,0 por cento); 3 - Os hipertensos não complicados apontaram mais enxaqueca, dor articular e, entre as mulheres, presença de menopausa e tratamento de reposição hormonal; 4 - Dos que tinham a pressão controlada (< 140/90 mmHg), 61,9 por cento eram hipertensos não complicados; e 5 - Os hipertensos complicados desconheciam que o tratamento pode evitar problemas renais e desconheciam ainda que a hipertensão também pode acometer pessoas jovens. CONCLUSÃO: Hipertensos complicados apresentaram mais características estruturais e psicossociais desfavoráveis, mais atitudes negativas frente ao tratamento e desconhecem a doença.

OBJECTIVE: To characterize a group of hypertensive patients in relation to beliefs, knowledge, attitudes and factors that could affect treatment compliance. METHODS: The data were collected by interviewing hypertensive outpatients. RESULTS: A total of 511 hypertensive patients were studied: most were women, white, with elementary education, and 53±11 years old. The patients had high levels of knowledge about hypertension and treatments. However, they interrupted the treatment due to the expensive medicines and the lack of instructions. Furthermore, they believed they had to take medicines only when they felt unwell, and they did not attend their medical appointment usually due to forgetfulness and personal problems. Regarding the attitudes against the antihypertensive treatment, hypertensive patients forgot to take the medicines, took the medication at different hours, stopped taking the medication on their own account, did not follow instructions, and did not exercise regularly. CONCLUSION: The profile of the hypertensive patients identified aspects that can hamper treatment compliance.


OBJETIVO: Caracterizar um grupo de hipertensos em relação a crenças, conhecimentos, atitudes e fatores que podem interferir na adesão ao tratamento. MÉTODOS: Os dados foram coletados através de entrevista com hipertensos em seguimento ambulatorial. RESULTADOS: Foram estudados 511 hipertensos: a maioria mulheres, brancas, com escolaridade de nível fundamental, 53,0 ±11,0 anos. Foram verificados índices elevados de conhecimento sobre a doença e tratamento. Porém, o tratamento foi interrompido devido a remédios muito caros e falta de orientação e acreditavam que devem tomar os medicamentos somente quando se sentem mal, além de faltarem à consulta médica, principalmente por esquecimento e problemas particulares. Em relação às atitudes frente ao tratamento, observou-se que esquecem de tomar os remédios, não tomam no mesmo horário, deixam de tomar por conta própria, não seguem as orientações e não praticam exercícios físicos regularmente. CONCLUSÃO: A caracterização dos hipertensos identificou aspectos que podem dificultar a adesão ao tratamento.


OBJETIVO: Caracterizar a un grupo de hipertensos en relación a las creencias, conocimientos, actitudes y factores que pueden interferir en la adhesión al tratamiento. MÉTODOS: Los datos fueron recolectados a través de una entrevista a hipertensos con seguimiento en consulta externa. RESULTADOS: Participaron 511 hipertensos: la mayoría mujeres, blancas, con un nivel de escolaridad primario y edad de 53,0 ±11,0 años. Fueron verificados índices elevados de conocimiento sobre la enfermedad y el tratamiento. Sin embargo, el tratamiento fue interrumpido debido a medicamentos muy caros y falta de orientación, considerando que deben tomar los medicamentos solamente cuando se sienten mal, además se identificó falta a la consulta médica, principalmente por olvido y problemas particulares. En relación a las actitudes frente al tratamiento, se observó que olvidan de tomar los remedios, no lo hacen en el mismo horario, dejan de tomar por cuenta propia, no siguen las orientaciones y no practican ejercicios físicos regularmente. CONCLUSIÓN: la caracterización de los hipertensos permitió identificar aspectos que pueden dificultar la adhesión al tratamiento.
