RESUMEN

OBJECTIVE: The technical feasibility of virtual noncontrast (VNC) images from dual-energy computed tomography (DECT) for the detection of the hyperdense artery sign (HAS) in ischemic stroke patients was investigated. METHODS: True noncontrast (TNC) scans of 60 patients either with or without HAS (n = 30 each) were investigated. Clot presence and characteristics were assessed on VNC images from DECT angiography and compared with TNC images. Clot characterization included the level of confidence for diagnosing HAS, a qualitative clot burden score, and quantitative attenuation (Hounsfield unit [HU]) measurements. RESULTS: Sensitivity, specificity, and accuracy of VNC for diagnosing HAS were 97%, 90%, and 93%, respectively. No significant differences were found regarding the diagnostic confidence (P = 0.18) and clot burden score (P = 0.071). No significant HU differences were found among vessels with HAS in VNC (56 ± 7HU) and TNC (57 ± 8HU) (P = 0.691) images. CONCLUSIONS: Virtual noncontrast images derived from DECT enable an accurate detection and characterization of HAS.

OBJECTIVE: To compare 2D spoiled dual gradient-echo (SPGR-DE) and 3D SPGR-DE with fat and water separation for the assessment of focal and diffuse fatty infiltration of the liver. METHODS: A total of 227 consecutive patients (141 men; 56 ± 14 years) underwent clinically indicated liver MRI at 1.5 T including multiple-breath-hold 2D SPGR-DE and single-breath-hold 3D SPGR-DE with automatic reconstruction of fat-only images. Two readers assessed the image quality and number of fat-containing liver lesions on 2D and 3D in- and opposed-phase (IP/OP) images. Liver fat content (LFC) was quantified in 138 patients without chronic liver disease from 2D, 3D IP/OP, and 3D fat-only images. RESULTS: Mean durations of 3D and 2D SPGR-DE acquisitions were 23.7 ± 2.9 and 97.2 ± 9.1 s respectively. The quality of all 2D and 3D images was rated diagnostically. Three-dimensional SPGR-DE revealed significantly more breathing artefacts resulting in lower image quality (P < 0.001); 2D and 3D IP/OP showed a similar detection rate of fat-containing lesions (P = 0.334) and similar LFC estimations (mean: +0.4 %; P = 0.048). LFC estimations based on 3D fat-only images showed significantly higher values (mean: 2.7 % + 3.5 %) than those from 2D and 3D IP/OP images (P < 0.001). CONCLUSION: Three dimensional SPGR-DE performs as well as 2D SPGR-DE for the assessment of focal and diffuse fatty infiltration of liver parenchyma. The 3D SPGR-DE sequence used was quicker but more susceptible to breathing artefacts. Significantly higher LFC values are derived from 3D fat-only images than from 2D or 3D IP/OP images.

OBJECT: To assess the feasibility of magnetization transfer (MT) imaging of the bowel wall in patients with Crohn's disease (CD), and to evaluate its utility for the detection of intestinal fibrosis. MATERIALS AND METHODS: In this prospective study, 31 patients (age 39.0 ± 13.2 years) with CD were examined in a 1.5T MR scanner. To establish a standard of reference, two independent readers classified the patients in different disease states using standard MR enterography, available clinical data and histological findings. In addition to the standard protocol, a 2D gradient-echo sequence (TR/TE 32 ms/2.17 ms; flip angle 25°) with/without 1,100 Hz off-resonance prepulse was applied. MT ratios (MTR) of the small bowel wall were computed off-line on a pixel-by-pixel basis. RESULTS: The MT sequences acquired images of sufficient quality and spatial resolution for the evaluation of the small bowel wall without detrimental motion artefacts. In normal bowel wall segments, an intermediate MTR of 25.4 ± 3.4 % was measured. The MTR was significantly increased in bowel wall segments with fibrotic scarring (35.3 ± 4.0 %, p < 0.0001). In segments with acute inflammation, the mean MTR was slightly smaller (22.9 ± 2.2 %). CONCLUSION: MT imaging of the small bowel wall is feasible in humans with sufficient image quality and may help with the identification of fibrotic scarring in patients with CD.

BACKGROUND & AIMS: Hepatocellular carcinoma (HCC) recurs in approximately 70% of cases after resection. Vascular invasion by tumor cells can be classified as gross or microscopic (microvascular invasion [mVI]) and is a risk factor for recurrence. We examined a large cohort of patients with HCC who were treated by resection to identify features of mVI that correlated with recurrence and survival. METHODS: We reviewed the records of all HCC resections performed at the Mount Sinai School of Medicine between January 1990 and March 2006 to identify those with mVI, established by histologic analysis. The numbers and sizes of vessels invaded, invasion of a vessel with a muscular wall, distance from the tumor, and satellite nodules were recorded. RESULTS: Of the 384 patients who underwent resection for HCC, 131 (34.1%) met the entry criteria. The median follow-up period was 28.9 months. There were 68 recurrences and 54 deaths. In multivariate analysis, invasion of a vessel with a muscular wall predicted recurrence (hazard ratio, 1.8; P = .02), and invasion of a vessel with a muscular wall (hazard ratio, 2.2; P = .018) and invasion of a vessel that was more than 1 cm from the tumor (hazard ratio, 2.1; P = .015) predicted survival. A risk score that assigned points for the presence of each variable correlated with recurrence (P = .028) and survival (P < .0001). CONCLUSIONS: A novel classification system that includes invasion of a vessel with a muscular wall and invasion of a vessel that is more than 1 cm from the tumor can accurately predict risk of recurrence and survival of patients with mVI after resection of HCC.

A number of treatments targeting VEGF or mTOR pathways have been approved for metastatic clear cell Renal Cell Carcinoma (ccRCC), but the majority of patients show disease progression after first line therapy with a very low rate of complete or long-term responders. It has been shown that miRs may play a role in prediction of treatment response in various cancer types. The aim of our study was to identify a miR signature predictive for RCC patients' response to antiangiogenic tyrosine kinase inhibitor (TKI) treatment in the first line therapy. Sequencing of 40 paired normal/tumor formalin fixed and paraffin embedded ccRCC tissues revealed separate clustering via unsupervised dendrograms. With supervised analysis, the strongest differential expression was obtained with miR-99b-5p, which was significantly lower in patients with short progression free survival (<8 months) and TKI non-responders (progressive disease patients according to RECIST) (p<0.0001, each). Validation using RTqPCR and a second patient cohort compiled from three different hospitals (n=65) showed higher expression of miR-99b-5p in complete responders, but this trend did not reach statistical significance. It is concluded that low miR-99b-5p expression analyzed with sequencing methodology may correlate with tumor progression in TKI-treated ccRCC patients.

OBJECTIVES: The purpose of this study was to evaluate to which degree investment of acquisition time in more encoding directions leads to better image quality (IQ) and what influence the number of encoding directions and the choice of b-values have on renal diffusion tensor imaging (DTI) parameters. MATERIAL AND METHODS: Eight healthy volunteers (32.3 y ± 5.1 y) consented to an examination in a 1.5T whole-body MR scanner. Coronal DTI data sets of the kidneys were acquired with systematic variation of b-values (50, 150, 300, 500, and 700 s/mm(2)) and number of diffusion-encoding directions (6, 15, and 32) using a respiratory-triggered echo-planar sequence (TR/TE 1500 ms/67 ms, matrix size 128 × 128). Additionally, two data sets with more than two b-values were acquired (0, 150, and 300 s/mm(2) and all six b-values). Parametrical maps were calculated on a pixel-by-pixel basis. Image quality was determined with a reader score. RESULTS: Best IQ was visually assessed for images acquired with 15 and 32 encoding directions, whereas images acquired with six directions had significantly lower IQ ratings. Image quality, fractional anisotropy, and mean diffusivity only varied insignificantly for b-values between 300 and 500 s/mm(2). In the renal medulla fractional anisotropy (FA) values between 0.43 and 0.46 and mean diffusivity (MD) values between 1.8-2.1 × 10(-3) mm(2)/s were observed. In the renal cortex, the corresponding ranges were 0.24-0.25 (FA) and 2.2-2.8 × 10(-3) mm(2)/s (MD). Including b-values below 300 s/mm(2), notably higher MD values were observed, while FA remained constant. Susceptibility artifacts were more prominent in FA maps than in MD maps. CONCLUSION: In DTI of the kidneys at 1.5T, the best compromise between acquisition time and resulting image quality seems the application of 15 encoding directions with b-values between 300 and 500 s/mm(2). Including lower b-values allows for assessment of fast diffusing spin components.

RATIONALE AND OBJECTIVES: To evaluate prospectively, in patients with suspected or known urinary stone disease, the image quality and diagnostic confidence of nonenhanced abdominal low-dose computed tomography (CT) with iterative reconstruction (IR) compared to filtered back-projection (FBP). MATERIALS AND METHODS: Fifty consecutive patients with suspected (n = 33) or known (n = 17) urinary stone disease underwent nonenhanced abdominal low-dose CT (120 kVp, 30 effective mAs, 1.6 ± 0.5 mSv). Reconstructions were performed with sinogram-affirmed IR and with FBP. Attenuation (in Hounsfield units) was measured in subcutaneous fat and urinary bladder; image noise was determined. Two readers assessed image quality, number and location of urinary calculi were recorded, and diagnostic confidence was assessed. Statistical analyses included Mann-Whitney, Friedman's two-way, Wilcoxon signed rank, Pearson's, and Spearman's rank order correction tests. RESULTS: Attenuation of urinary bladder (P = .208, reader 1; P = .123, reader 2) and fat (P = .568, reader 1; P = .834, reader 2) was similar among FBP and IR datasets. Image noise was reduced in IR datasets by 40.1% (P < .001). IR improved image quality (P < .01), and obesity as factor impairing image quality was noted in FBP but not in IR images (P < .05). There was no significant difference in number of calculi in datasets reconstructed with IR and FBP (P = .102, reader 1; P = .059, reader 2). Diagnostic confidence regarding identification of urinary calculi improved with IR (P < .05, reader 1; P < .01, reader 2). CONCLUSION: IR improves image quality and confidence for diagnosing urinary stone disease in abdominal low-dose CT.
