RESUMEN

BACKGROUND: Deep sternal wound infection (DSWI) is a severe complication after cardiac surgery, mostly caused by staphylococci. Little is known about the optimal antibiotic management. METHODS: A 10 year retrospective analysis of 100 patients with staphylococcal DSWI after cardiac surgery in a tertiary hospital. Treatment failure was defined as sternal wound dehiscence or fistula at the end of the prescribed antibiotic therapy, 12 months later, or DSWI-related death. RESULTS: Most patients were male (83%) and the median age was 72 years [interquartile range (IQR) 63-76]. Coronary artery bypass was the most frequent preceding procedure (93%). The median time to diagnosis of DSWI was 13 days (IQR 10-18) after surgery. Clinical presentation consisted of wound discharge in 77% of patients. Coagulase-negative staphylococci were isolated in 54 and Staphylococcus aureus in 46 patients. All patients received antibiotics and 95% underwent surgical debridement. The median duration of antibiotic treatment was 47 days (IQR 41-78). During follow-up, 21 out of 100 patients experienced treatment failure. Of these, 8/21 patients (38%) died from DSWI after a median of 12 days (IQR 8-30). In the multivariate analysis, a rifampicin-containing antibiotic regimen was the only factor associated with lower risk of treatment failure (hazard ratio 0.26, 95% confidence interval 0.10-0.64, P = 0.004). Prolonged treatment (12 weeks instead of 6 weeks) did not alter outcome (P = 0.716) in patients without prosthetic valve endocarditis. CONCLUSIONS: Treatment of rifampicin-susceptible staphylococcal DSWI with a rifampicin-containing antibiotic regimen may improve the outcome. After surgical debridement an antibiotic treatment of 6 weeks may be adequate for staphylococcal DSWI.

OBJECTIVE: One of the most important factors in bypass surgery is the preservation of endothelial function in the arterial graft. It was of interest, therefore, whether a slightly modified preparation procedure during surgery could contribute to improved endothelial function of the graft. We compared the functional activity of internal thoracic arteries (ITA) prepared according to the traditional harvesting method with occlusion by a clip, dissection at the distal end and storage of the artery in papaverine until its implantation (CA) with the functional activity of arteries which were also prepared and wrapped in papaverine, but were left perfused and dissected immediately before their anastomoses (PA). METHODS: Samples of ITA were obtained from a total number of 28 patients, undergoing bypass surgery, and randomly distributed into two groups. The arteries were cut into rings and suspended in organ baths, containing Krebs-Henseleit solution, for isometric tension recording. Cumulative concentration response curves were determined for the contractile agents endothelin-1 (ET-1), 5-hydroxytryptamine (5-HT), noradrenaline (NA) and potassium chloride (KCl) and the relaxant compounds acetylcholine (ACH) and sodium nitroprusside (SNP) during active tone induced by 30 mM KCl. RESULTS: ET-1 and 5-HT stimulated rings from both groups within the same concentration ranges but elicited significantly (P<0.05) higher contractile responses in CA compared to PA. By contrast, concentration response curves for KCl and NA where nearly superimposable. On the other hand, maximal endothelium-dependent relaxant responses to ACH proved to be significantly stronger in PA (0.84+/-0.20 g) as compared to CA (0.31+/-0.05 g, P<0.05) while endothelium independent relaxant responses to SNP where similar in both groups. CONCLUSION: These data suggest that leaving the ITA perfused during harvesting might improve considerably the endothelial function of the graft.

BACKGROUND: In patients with a small aortic root, the use of 19-mm valve prostheses for valve replacement is controversial because of the small orifice area of these valves. METHODS: To assess stress hemodynamics in patients with 19-mm valve prostheses, to find predictors of unfavorable hemodynamics, and to document the long-term follow-up, we examined 30 patients (age, 64 +/- 19 years; 27 women and 3 men; follow-up, 38 +/- 50 months) clinically and with the use of dobutamine stress echocardiography. A history was taken, and a physical examination was performed. At rest and during dobutamine stress, Doppler echocardiography was performed. RESULTS: At rest, transprosthetic gradients were moderately elevated with mean and peak gradients of 15 +/- 7 and 32 +/- 14 mmHg, and effective orifice areas were small (0.91 +/- 0.31 cm(2)). Gradients rose markedly during stress (mean, 37 +/- 14 mmHg; peak, 83 +/- 41 mmHg). Predictors of high transprosthetic gradients were larger body surface area, younger age, and valve type. Mean and peak gradients were lower with St. Jude Medical Hemodynamic Plus valves than with standard St. Jude Medical (P < 0.05) and other valves, and the effective orifice area was highest (1.07 +/- 0.29 cm(2); P < 0.05 versus standard St. Jude Medical) in this valve model. Sixty percent of patients developed significant dynamic subvalvular or intraventricular gradients (84 +/- 41 mmHg) during dobutamine stress. CONCLUSIONS: After aortic valve replacement with 19-mm prostheses in patients with a small aortic root, dobutamine stress leads to high transvalvular gradients, which are dependent on valve model, age, and body surface area. In addition, 60% of patients develop significant dynamic outflow obstructions. These findings and the persistence of some degree of exercise-induced symptoms in 70% of patients suggest that alternative surgical techniques should be considered if the size of the aortic annulus demands a 19-mm valve, especially if the patient seeks physical activity, is young, or is of larger body size.

AIM: To assess treatment effects of optimised medical therapy and PCI or CABG surgery on one-year outcome in patients 75 years old with chronic angina. METHODS AND RESULTS: On-treatment analysis of the TIME data: all re-vascularised patients (REVASC n=174: 112 randomised to revascularisation and 62 to drugs with late revascularisation) were compared to all patients on continued drug therapy (MED n=127: 86 randomised to drugs and 41 to revascularisation only). Baseline characteristics of both groups were similar (age 80 +/- 4 years). Risk of death at one year (adjusted hazard ratio (HR)=1.31; 95%-CI: 0.58-2.99; P=0.52) and of death/infarction (adjusted hazard RATIO=1.77; 95%-CI 0.91-3.41; P=0.09) were comparable between REVASC and MED patients. Furthermore, the risk of death within 30 days was even slightly lower among REVASC patients (unadjusted hazard RATIO=0.73; 95%-CI: 0.21-2.53; P=0.98). Overall, REVASC patients had greater improvements in symptoms and well-being than MED patients (P<0.01). Surgical patients had similar mortality rates as angioplasty patients, but they also had greater symptomatic improvements (P<0.01). CONCLUSION: Treated medically, elderly patients with chronic angina have a similarly high 30-day and one-year mortality as patients of the same age being re-vascularised; however, they can expect lower improvements in symptoms and well being.
