PURPOSE: To investigate the difference in apparent diffusion coefficient (ADC) measurements of differently shaped regions of interest (ROIs) in ovarian tumors, and to investigate the diagnostic ability of differently shaped ROIs in differentiating benign from malignant ovarian tumors. MATERIALS AND METHODS: Fifty-four patients with ovarian tumors with a solid component were evaluated. The patients underwent magnetic resonance (MR) examinations including diffusion-weighted imaging using a 3.0T MR system. Two readers measured ADCs using four ROI methods: freehand, square, round, and five small round ROIs. The interclass correlation coefficient (ICC) and repeated-measures analysis of variance were used to assess their measurement reliability and to compare ADCs for each ROI method. Receiver operating characteristic curve analysis and unpaired t-test on each ROI were used to differentiate benign and malignant ovarian tumors and assess the diagnostic ability. RESULTS: All ROI methods except the square ROI (0.56) showed good or excellent correlations (0.70-0.91). Minimum and mean ADC values differed significantly between the ROIs (P < 0.05). When using the freehand ROI, the minimum and mean ADC values were the lowest and highest, respectively. The optimal cutoff minimum and mean ADC values of each ROI for differentiating benign and malignant tumors were 0.81-1.06 × 10(-3) mm(2) /s and 1.15-1.52 × 10(-3) mm(2) /s, respectively. The areas under the curve showed no significant differences among the ADCs in the different ROI methods (P > 0.05). Minimum and mean ADCs from all ROIs showed significant differences between benign and malignant tumors (P < 0.05). CONCLUSION: The ROI shape influences ADC values and the optimal cutoff ADC values for differentiating benign from malignant ovarian tumors.

PURPOSE: To investigate type II endometrial carcinoma characterization using dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) and evaluate the diagnostic accuracy of semiquantitative DCE-MRI in differentiating type II from type I tumors. MATERIALS AND METHODS: Seventy-seven patients with endometrial carcinoma were retrospectively evaluated using 3T DCE-MRI. The maximum absolute enhancement of signal intensity (SImax), maximum relative enhancement (SIrel), wash-in rate (WIR), and the SImax/SI (piriformis) ratio were analyzed. To differentiate type I from type II tumors, optimal threshold criteria were established. The Mann-Whitney U-test was used for statistical comparison and receiver operating characteristic curve analysis was used to determine optimal cutoff values. RESULTS: The SIrel (P < 0.001), WIR (P < 0.0001), and SImax/SI (piriformis) ratio (P < 0.0001), but not SImax, differed significantly between type I and type II carcinomas. Cutoff values of SIrel ≥58.8, WIR ≥37.0, and SImax/SI (piriformis) ratio ≥1.55 had sensitivities of 93%, 93%, and 67%, specificities of 60%, 60%, and 79%, accuracies of 66%, 66%, and 67%, respectively, for predicting type II endometrial carcinoma. CONCLUSION: Endometrial carcinoma with strong (high level) enhancement on DCE-MRI is suggestive of type II endometrial carcinoma. Semiquantitative evaluation of DCE-MRI may be useful for differentiating type II from type I tumors.

PURPOSE: To correlate the apparent diffusion coefficient (ADC) of endometrioid carcinoma with histological tumor grade and degree of myometrial invasion. MATERIALS AND METHODS: 3T diffusion-weighted (DW) magnetic resonance (MR) images of 63 patients were retrospectively reviewed. Two readers measured tumor ADC according to a freehand region of interest (ROI) and a round ROI. Mean and minimum ADCs were correlated with prognostic parameters. RESULTS: The minimum ADC was 0.64 × 10(-3) mm(2)/s for grade 1 (G1, n = 42), 0.62 for grade 2 (G2, n = 14), 0.46 for grade 3 (G3, n = 7) on freehand ROI. There were significant differences between G1 and G3 (P = 0.007), and G2 and G3 (P = 0.038). No significant correlation was found between tumor grade and mean ADC (0.85 for G1, 0.82 for G2, and 0.72 for G3, P = 0.166). The minimum ADC was significantly lower for patients with deep (n = 21, 0.54) than for those with superficial (n = 39, 0.65) myometrial invasion. Conversely, mean ADC did not differ significantly (0.84 for superficial and 0.78 for deep myometrial invasion, P = 0.081). The same tendency was shown on round ROI. CONCLUSION: The minimum ADC correlates with prognostic parameters of endometrial carcinoma more strongly than mean ADC. Lower minimum ADC is associated with higher histological tumor grade and higher degree of myometrial invasion.

PURPOSE: To investigate the influence of different-shaped regions of interest (ROIs) on tumor apparent diffusion coefficient (ADC) measurements and interobserver variability in endometrial carcinoma. MATERIALS AND METHODS: Sixty-nine patients (age range, 32-92 years; mean, 61 years) were evaluated in this retrospective study. Patients had undergone magnetic resonance (MR) examinations including diffusion-weighted imaging (DWI) using a 3.0-T MR system. Two readers measured tumor ADCs using four ROI methods: freehand ROI; square ROI; round ROI; and five small, round ROIs. Minimum and mean ADCs were obtained. The interclass correlation coefficient (ICC) was statistically analyzed to assess measurement reliability. Repeated-measures analysis of variance was used for comparisons of ADCs measured with each ROI method. RESULTS: ICCs were 0.93 (minimum ADC) / 0.93 (mean ADC) for freehand ROIs, 0.94/0.95 for square ROIs, 0.94/0.95 for round ROIs, and 0.95/0.96 for five small, round ROIs. All ROI methods indicated excellent correlations. Each minimum ADC was significantly different except between square ROI and round ROI (P < 0.001). Mean ADCs showed significant differences only between freehand ROI and the other ROI methods (P < 0.001). CONCLUSION: ROI shape has no marked influence on ICC in endometrial carcinoma. Compared with minimum ADCs, mean ADCs are suggested to provide more stable results regardless of the ROI method.

RESUMEN

Massive perivillous fibrin deposition (MPFD) of the placenta is characterized by the obliteration of the villous trophoblast with extensive deposition of fibrinoid material in the intervillous space. Here, we describe the MRI findings of a case of MPFD. The placenta demonstrates linear and geographical hypointensity on T2-weighted imaging, which is suggested to mainly reflect fibrin deposition. This finding should be noted, particularly in patients with miscarriage in their past history.

OBJECTIVE: The purpose of this study was to investigate the efficacy of 3.0T MR imaging in the assessment of depth of myometrial invasion by endometrial carcinoma. METHODS: Fifty women with histopathologically confirmed endometrial carcinoma underwent preoperative MR imaging at 3.0T. MR imaging findings were compared with microscopic pathologic findings in all cases. On evaluation of MR images and histopathological findings, myometrial invasion was classified as absent (tumor confined to the endometrium), superficial (less than 50% of myometrial thickness), or deep (50% or more of myometrial thickness) by two radiologists. RESULTS: The sensitivity, specificity, and accuracy of the MR imaging in distinguishing no myometrial invasion from myometrial invasion were 95/95, 60/70, and 88/90%, respectively, and no and superficial myometrial invasion from deep myometrial invasion were 88/94, 97/94, and 94/92%, respectively. CONCLUSIONS: In evaluation of the depth of myometrial invasion by endometrial carcinoma, 3.0T MR imaging has a high diagnostic accuracy that is equivalent to that of previously reported 1.5T MR imaging.

PURPOSE: This study aimed to examine MRI features and staging of neuroendocrine carcinoma (NEC) of the endometrium and evaluate survival. METHODS: Clinical data, pathological, and preoperative pelvic MRI findings in 22 patients with histologically surgery-proven endometrial NEC were retrospectively reviewed. Tumors were pure NEC (n = 10) or mixed histotype (n = 12), with 13 large and nine small cell type. RESULTS: International Federation of Gynecology and Obstetrics (FIGO) staging was I, II, III, and IV in 6, 2, 12, and 2 patients, respectively. In 13 (76.4%) of 17 patients with pathological deep myometrial invasion, MRI showed abnormal diffusely infiltrative high T2 signal intensity throughout the myometrium with loss of normal uterine architecture. All tumors had restricted diffusion (apparent diffusion coefficient map low signal intensity, diffusion weighted imaging high signal intensity). Accuracy of T staging by MRI for all cases was 81.8%, with reference to pathology staging, while patient-based sensitivity, specificity, and accuracy for detecting metastatic pelvic lymph nodes was 60.0%, 100%, and 77.8%, respectively. Two intrapelvic peritoneal dissemination cases were detected by MRI. During follow-up (mean 30.4, range 3.3-138.4 months), 16 patients (72.7%) experienced recurrence and 12 (54.5%) died of disease. Two-year disease-free and overall survival rates for FIGO I, II, III, and IV were 66.7% and 83.3%, 50% and 100%, 10% and 33.3%, and 0% and 0%, respectively. CONCLUSION: Abnormal diffusely infiltrative high T2 signal intensity throughout the myometrium with normal uterine architecture loss and obvious restricted diffusion throughout the tumor are suggestive features of endometrial NEC. Pelvic MRI is reliable for intrapelvic staging of affected patients.

RESUMEN

Ovarian solid tumors have variable histological types including benign and malignant tumors. In addition, non-neoplastic lesions sometimes show a tumor-like appearance. It is important to differentiate benign from malignant tumors. In general, low signal intensity (SI) on T2-weighted imaging (T2WI), low SI on diffusion-weighted imaging (DWI), and gradual increased pattern on dynamic contrast-enhanced magnetic resonance (MR) imaging are known to be suggestive of a benign tumor. Conversely, there are some cases in which these rules do not apply. We should, therefore, strive for a greater understanding of these exceptional cases. Several tumors show characteristic findings on MR imaging reflecting pathologic features, which leads to the correct diagnosis. Additionally, MR imaging provides important information other than the nature of tumors, such as secondary uterine changes. Furthermore, clinical findings and laboratory examination data also help in determining the correct diagnosis.

OBJECTIVES: To investigate neuroendocrine carcinoma (NEC) of the uterine cervix cases for MRI features and staging, as well as pathological correlations and survival. RESULTS: FIGO was I in 42, II in 14, III in 1, and IV in 5 patients. T2-weighted MRI showed homogeneous slightly high signal intensity and obvious restricted diffusion (ADC map, low intensity; DWI, high intensity) throughout the tumor in most cases, and mild enhancement in two-thirds. In 50 patients who underwent a radical hysterectomy and lymphadenectomy without neoadjuvant chemotherapy (NAC), intrapelvic T staging by MRI overall accuracy was 88.0% with reference to pathology staging, while patient-based sensitivity, specificity, and accuracy for metastatic pelvic lymph node detection was 38.5%, 100%, and 83.3%, respectively. During a mean follow-up period of 45.6 months (range 4.3-151.0 months), 28 patients (45.2%) experienced recurrence and 24 (38.7%) died. Three-year progression-free and overall survival rates for FIGO I, II, III, and IV were 64.3% and 80.9%, 50% and 64.3%, 0% and 0%, and 0% and 0%, respectively. MATERIALS AND METHODS: Sixty-two patients with histologically surgery-proven uterine cervical NEC were enrolled. Twelve received NAC. Clinical data, pathological findings, and pretreatment pelvic MRI findings were retrospectively reviewed. Thirty-two tumors were pure NEC and 30 mixed with other histotypes. The NECs were small cell type (41), large cell type (18), or a mixture of both (3). CONCLUSIONS: Homogeneous lesion texture with obvious restricted diffusion throughout the tumor are features suggestive of cervical NEC. Our findings show that MRI is reliable for T staging of cervical NEC.

BACKGROUND: We investigated the distinguishing pathological features of bilateral ovarian tumors using magnetic resonance (MR) imaging. METHODS: Eighty-six patients with bilateral ovarian tumors on MR imaging were evaluated. The pathological diagnosis was investigated, and the results were subjected to statistical analysis using Mann-Whitney U test, Fisher's exact test, Chi-squared test and receiver operating characteristic (ROC) curve to determine the features useful for the differentiation of distinct types of lesions. RESULTS: The diagnosis of bilateral ovarian tumors was confirmed in eighty-one patients and the majority of the lesions were further classified into serous carcinoma (n = 36), mature teratoma (n = 20) and metastasis (n = 12). We assessed the existence of factors useful for the MR imaging differentiation between metastasis and serous carcinoma or primary malignant ovarian tumors. Cancer antigen (CA) 125 serum level and maximum tumor diameter were significantly different between metastasis and serous carcinoma and similarly, between metastasis and primary malignant ovarian tumors. MR imaging morphology, ascites and peritoneal implants did not show any significant difference between the different types of lesions. CONCLUSION: Within our patient cohort, most bilateral ovarian tumor lesions were determined to be serous carcinoma, mature teratoma or metastasis. CA 125 serum level and maximum tumor diameter are useful markers for the differentiation between metastasis and serous carcinoma or primary malignant ovarian tumors.

BACKGROUND: The aim of this paper was to evaluate the validity of tumor volume measurement using diffusion-weighted (DW) imaging in cervical cancer. METHODS: In this retrospective study, 22 patients, who underwent preoperative 3.0 T MR examinations with DW imaging were evaluated. Tumor volume measurement by oblique axial (short axis to the uterine cervix) T2-weighted imaging was performed by manually outlining the tumor on the monitor. The area of tumor in each slice was multiplied by the slice profile (slice thickness plus intersection gap), and the total tumor volume was calculated by summation of these obtained volumes. Meanwhile, one experienced radiological technologist generated three-dimensional DW images of cervical cancer using a volume-rendering algorithm at a computer workstation, and tumor volume was automatically calculated in the workstation. Analysis via the intraclass correlation coefficient (ICC) and Bland-Altman plots were used to assess the validity and reliability of these methods. RESULTS: Between tumor volumes measured by T2-weighted imaging methods and DW imaging methods, the ICC was excellent (0.962). The 95% limits of agreement of volume measurement were -52.7 and 35.7 mL (mean difference, -8.5 mL). In regards to intra-observer variability, the ICC was excellent (0.963). The 95% limits of agreement of volume measurement were -42.2 and 47.4 mL (mean difference, 2.6 mL). CONCLUSION: DW imaging can be used to measure cervical cancer volume.

RESUMEN

We report two cases with adnexal torsion to assess the mechanism of very strong signal intensity in ovarian lesions on DW imaging using MR imaging such as T2*-weighted imaging. MR imaging findings of the cases suggest that the very strong signal intensity on DW imaging is contributed to cytotoxic edema with or without vasogenic edema, as well as oxyhemoglobin or extracellular methemoglobin.

RESUMEN

Müllerian duct anomalies (MDAs) are the result of incomplete development, vertical or lateral fusion, or absorption of the müllerian ducts. The range of anomalies includes uterovaginal agenesis or hypoplasia, unicornuate uterus, uterus didelphys, bicornuate uterus, septate uterus, and arcuate uterus. Correct diagnosis and classification of these anomalies are essential because pregnancy outcomes and treatment options vary between the types of anomaly. Furthermore, early identification of MDAs helps to avoid prolonged symptomatic periods and the complications that may subsequently arise, such as infertility, endometriosis, and neoplasm. Although many of these abnormalities are initially diagnosed by ultrasound or hysterosalpingography, MR imaging is the most accurate noninvasive modality available for classification of the various anomalies because of its better anatomic assessment compared with other diagnostic modalities. Familiarity with the wide variety of MDA presentations can help in the planning of appropriate treatment.

PURPOSE: The characteristics of fetal thyroid on magnetic resonance (MR) imaging, including normal thyroid and disorders other than goiter have not been fully evaluated. Our aim was to assess fetal thyroid using three dimensional (3D) gradient echo (GRE) T1-weighted MR imaging and to examine the usefulness of this modality. MATERIALS AND METHODS: The study included 27 3D GRE T1-weighted images from 26 fetuses. The largest possible region of interest (ROI) within the thyroid at the slice level depicting the thyroid was manually defined and three circular ROIs on neck muscle were manually defined on the image slices showing the highest signal intensity (SI) of the thyroid. Maximum and mean thyroid-to-muscle SI ratios (SIRs) were then calculated as SIR = maximum or mean thyroid SI/muscle SI. RESULTS: The thyroid could not be identified in two cases. Fetal thyroid function was normal in 17 cases, and there were 7 cases of hypothyroidism (6 transient and 1 thyroid dysgenesis). There was no linear relationship between mean and maximum SIR and gestational age. The mean and maximum SIR in the cases of normal fetal thyroid were 1.85 ± 0.20 and 2.61 ± 0.39, and the mean and maximum SIR in fetal hypothyroidism were 1.58 ± 0.20 and 2.13 ± 0.37. Mean (P = 0.0088) and maximum (P = 0.0221) SIR values were significantly different between euthyroid and hypothyroid fetuses. CONCLUSION: Thyroid SIR measurement provided useful information regarding fetal thyroid function.

PURPOSE: To determine whether fluorodeoxyglucose positron emission tomography (FDG-PET) before and after palliative radiation therapy (RT) can predict long-term pain control in patients with painful bone metastases. METHODS AND MATERIALS: Thirty-one patients with bone metastases who received RT were prospectively included. Forty painful metastatic treatment fields were evaluated. All patients had undergone pre-RT and post-RT PET/CT scanning. We evaluated the relationships between the pre-RT, post-RT, and changes in maximum standardized uptake value (SUVmax) and the pain response, and between SUVmax and pain relapse of the bone metastases in the treatment field. In addition, we compared the SUVmax according to the length of time from the completion of RT to pain relapse of the bone metastases. RESULTS: Regarding the pain response at 4 weeks after the completion of RT, there were 36 lesions of 27 patients in the responder group and 4 lesions of 4 patients in the nonresponder group. Changes in the SUVmax differed significantly between the responder and nonresponder groups in both the early and delayed phases (P=.0292 and P=.0139, respectively), but no relationship was observed between the pre-RT and post-RT SUVmax relative to the pain response. The responder group was evaluated for the rate of relapse. Thirty-five lesions of 26 patients in the responder group were evaluated, because 1 patient died of acute renal failure at 2 months after RT. Twelve lesions (34%) showed pain relapse, and 23 lesions (66%) did not. There were significant differences between the relapse and nonrelapse patients in terms of the pre-RT (early/delayed phases: P<.0001/P<.0001), post-RT (P=.0199/P=.0261), and changes in SUVmax (P=.0004/P=.004).
