RESUMEN

The effects of lymphatic drainage on ileal anastomotic healing using interrupted polyglycolic acid sutures were studied in rats after division and obstruction of celiac-mesenteric lymphatics, and the data compared with sham-operated controls. In 4 of 20 rats with lymph stasis, but not sham-controls, anastomotic leakage was associated with generalized peritonitis and death. Histologic examination of the anastomotic site at 7 and 14 days revealed prolonged exudation with acute inflammatory reaction and less prominent granuloma formation where lymphatics were interrupted. Whereas foreign body giant cell reaction predominated at 14 days with lymph stasis, histiocytic and fibroblastic proliferation dominated in sham-controls. The data suggest that an intact lymphatic system favors optimal intestinal healing and repair.

RESUMEN

The authors analyze vascular liver modifications after porta-caval shunt in the rat. The examination of the portal vascular bed obtained by neoprene latex injection allows the definition of different morphologic phases, throughout these liver reaches new structural balance, with an efficacious lobular perfusion. In the same periods histological findings show a cell damage, without necrosis, followed by a recovery after 8 months. In distance, in normal rat porta-caval shunts induce a reduction of the liver volume, both the lobular structures have normal functions and morphology.

RESUMEN

The importance of the papillary structure in the control of biliary and pancreatic outflow induced the authors to investigate the effect of papillary damage, chemical and physical, on the pancreatic parenchyma of the rat. The results show that mild papillary lesions produce acute interstitial flogosis of the pancreas; in 16,6% of the investigated animals this inflammation develops with the clear findings of a lethal acute haemorrhagic pancreatitis. This model is like the aspects of the post-operative acute pancreatitis.

RESUMEN

Ultrasound diagnosis of pyloric stenosis depends on the typical "target" and "cervix" pictures, respectively in transverse and longitudinal scan. Furthermore, pyloric dimensions can be considered pathologic when the muscular wall is greater than or equal to 4 mm. in width, having a maximum diameter greater than or equal to 5 mm. and the pyloric canal is greater than or equal to 18 mm in length. At the Department of Paediatric Surgery of Parma, from 1986 to 1988, 34 infants were studied by ultrasound, basing on clinical suspect of pyloric stenosis. 22 of them were subsequently operated on. Pyloric ultrasound was diagnostic in 20 cases, negative in 1 and doubtful in 1, confirming its reliability in 90-95% of cases. No false positive findings were recorded. Basing on these results, the Authors believe that ultrasound must be the first choice in case of clinical suspect of pyloric stenosis in infancy, because of its high diagnostic value and safety. X-ray studies should be indicated only in doubtful cases.

RESUMEN

The authors refer their experience in the treatment of distal hypospadias using the MAGPI technique. The authors discuss the indications and the details of the operation. Over a 12 month period 20 patients with subcoronal hypospadias underwent repair, with no complication and an excellent cosmetic result. The authors conclude that this technique, as recommended by several authors, is a simple, safe and effective procedure for distal hypospadias.

RESUMEN

A comparison between lymphangiovenous and lymphadeno-venous anastomoses has been realized in rats with mesenteric lymphostasis. The lymphangiovenous anastomoses have showed a better patency than lymphonode-venous shunt owing to technics and biologic problems. Their function was verified until 15th p.o. day, while all lymphonode venous shunt were closed at 10 th p.o.. The shunt occlusion may be related with lymphatic newborn vessels, which exclude the anastomoses from lymph flow.

RESUMEN

From January 1983 to December 1984 in the Division of Pediatric Surgery of Parma, 112 endoscopies of upper gastro-intestinal tract (UGT) were done. Besides the authors refer 49 endoscopies of the colon. Endoscopic indications of the UGT included evaluation of the esophagus in gastro-esophageal reflux, achalasia, lye ingestion, gastro-duodenal ulcer, bleedings of UGT, removal of foreign bodies. The indication of the colonoscopy included bleedings and all symptoms of colitis. The endoscopic procedures were done under sedation or in general anesthesia.

RESUMEN

The Authors studied the healing of the intestinal anastomoses in rats with mesenteric and celiac lymph-vessels coagulated and cut. The experiment was realized utilizing 104 rats, divided in fours groups: after the induction of the lymphostasis, in the first group an end-to-end ileal anastomosis was made by polyglycolic acid suture, 6/0 in diameter. The control groups consisted of rats with ileal anastomoses, with lymphostasis solely and sham operated. The macroscopic observation and the histological findings were studied 7 and 14 days after the anastomoses' performance. Only in the animals with lymphostasis was observed an anastomosal leakage of 19% at 7th day control. In the same period the histologic sections showed a modified anastomosal healing, related to the significant reduction of the granulomatous tissue formation in presence of the lymphostasis.
