AIM: This study aimed to identify the effects of a web-based birth preparation programme based on the Health Belief Model and supported by motivational interviews on fear of childbirth, self-efficacy and the labour process in primigravida women. METHODS: The study was conducted in a single-blind, randomized controlled design with 73 pregnant women. The Birth Health Belief Scale, the Wijma Delivery Expectation/Experience Questionnaire A-B, the Childbirth Self-efficacy Scale and the Labour Process Assessment forms were used to collect data. Five sessions of motivational interviews were held with the pregnant women in the experimental group, while those in the control group were administered placebo education. RESULTS: Fear of birth was lower, and the birth self-efficacy total score was higher in the experimental group. None of the groups indicated significant differences in terms of their mode of delivery. CONCLUSION: Health Belief Model-based web-based birth preparation programme and motivational interviews were found to reduce the fear of birth, increase self-efficacy and positively affect the birth process in primigravida women.

RESUMEN

BACKGROUND: Accessory mitral valve tissue (AMVT) is an extremely rare causes left ventricular outflow tract (LVOT) obstruction and is usually incidentally detected in childhood. It is often associated with other cardiac and vascular congenital malformations. CASE PRESENTATION: In this case, we present a 15-year-old girl was diagnosed with AMVT by transesophageal echocardiography, resulting in LVOT obstruction during systole. Interestingly enough, the patient's accessory mitral valve remained undetected for years until he became symptomatic for wide ASD. Successful closure of the ASD with resection of the AMVT was performed with a transaortic approach. The patient was hemodynamically stable postoperatively. There were no abnormalities in the mitral valves and LVOT. CONCLUSION: It was also unusual to see AMVT with ASD instead of other frequently associated other congenital anomalies. Accessory mitral valve should be considered a rare but important cause of left ventricular outflow tract obstruction in childhood.

RESUMEN

Pacemaker-associated superior vena cava (SVC) syndrome is increasingly recognized as a nonmalignant type of SVC syndrome. Computed tomographic (CT) angiography is the first choice of imaging for diagnosis but in some cases there is discrepancy between the clinical findings and CT images. In this report we present the role of contrast study with transesophageal echocardiography in a patient with suspicion of SVC syndrome but inconclusive CT findings.

RESUMEN

Long-standing left-to-right shunting across a patent ductus arteriosus (PDA) can result in Eisenmenger syndrome. In this report, we present echocardiographic findings of a 27-year-old female patient with pulmonary hypertension. In diagnostic work-up especially Doppler findings of the pulmonary artery suggested the presence of a reversed PDA as a cause of pulmonary hypertension. The diagnosis was confirmed by contrast study and computed tomography.

RESUMEN

Coronary sinus atrial septal defect (ASD) is a rare congenital cardiac anomaly, which might be difficult to diagnose. In this report, we describe a patient with small secundum ASDs and an associated large coronary sinus ASD, which had been missed at initial evaluation. The diagnosis of coronary sinus ASD was established by using transesophageal echocardiography after percutaneous closure of a small secundum ASD at another center. Patient underwent corrective surgery.

BACKGROUND: Epicardial adipose tissue (EAT) is a cardiometabolic risk factor, and its possible relationship with hypertension has been reported previously. Fragmented QRS (fQRS) detected on electrocardiography (ECG) has been demonstrated to be a marker of myocardial fibrosis. In this study, we aimed to investigate the relationship between the thickness of EAT, and presence of fQRS in hypertensive patients. METHODS: Consecutive patients who were diagnosed with hypertension were included in the study. ECG and transthoracic echocardiography (TTE) were performed to all patients. fQRS was defined as additional R' wave or notching/splitting of S wave in two contiguous ECG leads. Thickness of EAT was measured by TTE. RESULTS: This study enrolled 69 hypertensive patients with fQRS on ECG and 45 hypertensive patients without fQRS as the control group. Age (P = .869), and gender distribution (P = .751) were similar in both groups. Left atrial diameter (P = .012), interventricular septal thickness (P < .001), posterior wall thickness (P < .001), left ventricular ejection fraction (P = .009), left ventricular mass (P = .006), left ventricular mass index (P = .014), left ventricular hypertrophy (P = .003), and EAT thickness (P < .001) were found to be significantly increased in patients with fQRS. In multivariate analysis, among these variables only EAT was observed to be an independent predictor of fQRS (odds ratio:3.306 [95% confidence interval, 0.030-0.118], P = .001). CONCLUSION: A significant association exists between the presence of fQRS and EAT thickness in hypertensive patients. The presence of fQRS, just as EAT thickness, may be used as a cardiometabolic risk factor in hypertensive patients.

This study was aimed at determining the quality of sexual life and changes occurring in the sexual life of women with high-risk pregnancy. The study was carried out at the obstetric clinic of a hospital in a province located in the Central Anatolia region of Turkey. The population of this cross-sectional study comprised 112 women diagnosed with high-risk pregnancy. When the period during which the women had a diagnosis of high-risk pregnancy was compared to the period during which they did not have such a diagnosis, it was determined that more than half of the women experienced adverse changes in their sexual lives. Also high-risk pregnancies involved such complaints as sexual dissatisfaction, sexual self-dislike, and feelings of pain during sexual intercourse.

RESUMEN

Interrupted aorta is a very rare heart defect in which there is a gap between the ascending and the descending thoracic aorta. It is usually associated with other cardiac anomalies, including ventricular septal defect, ductus arteriosus, and truncus arteriosus. Severe cases present with serious complications such as hypertension, heart failure, or intracranial hemorrhage. Neurological complications are very rare form of presentation and commonly associated with intracranial aneurysms. We have reported a case of interrupted aorta who presented with transient ischemic attack due to thrombus formation in the interrupted segment of the aorta.

RESUMEN

We report here a case of left atrium inversion after implanting HeartMate III LVAD, which is known to be the first in literature. LVAD can be functional only if there is adequate inflow to the device. Parameters and filling of left ventricle can be assessed by TEE. In our case, initial examination with TEE showed thrombus like images. HeartMate III has a reliable algorithm that automatically reduces pump speed if 'suction effect' is detected. HeartMate III demonstrates clean flow properties and good surface wash. Despite these positive features of the HeartMate III, left atrium inversion can still be seen with it, so users should be alert in this regard.

RESUMEN

Myocardial clefts are defined as narrow, deep blood-filled invaginations within the left ventricular myocardium. They occur in hypertrophic cardiomyopathy patients with different frequency and represent a distinctive morphological expression of hypertrophic cardiomyopathy. Although two-dimensional transthoracic echocardiography is able to detect myocardial clefts in some cases, cardiovascular magnetic resonance imaging with its high spatial resolution can detect myocardial clefts not visualized with echocardiography. In this report, we represent multimodality imaging of a septal myocardial cleft in a hypertrophic cardiomyopathy patient.

This was a cross-sectional study to analyse the relationship between menopausal symptoms and sexual dysfunction among 229 married Turkish women in the 40-65 age group. The study was carried out at a menopause clinic of a state hospital between 1 October and 31 December 2010. Data were collected with Personal Characteristics Form, Menopause Rating Scale (MRS) and the Arizona Sexual Experience Scale (ASEX). The average age of the women was 52.33 (SD = 4.80) years. The average MRS total score was 20.13(SD = 9.20). The ASEX mean score was 19.97 (SD = 5.44). It was determined that there is a positive meaningful relationship between ASEX mean score, MRS total mean score and the sub-score of women. From the results obtained from this study, it can be said that women have differing levels of menopausal symptoms, and as the severity of menopausal symptoms increases, there is an increase in sexual dysfunction.

OBJECTIVES: Left ventricular (LV) synchronous contraction is impaired in patients with hypertension (HT). The deleterious effects of HT on cardiovascular system are more evident in patients with nondipper HT than dippers. In this study, we aimed to investigate the effect of nondipping HT on LV systolic synchronicity compared with dippers and controls. METHODS: One hundred patients with newly diagnosed essential HT and 50 normotensive subjects were enrolled in this study. The hypertensive patients were assigned 2 groups comprising 55 dippers and 45 nondippers. Each subject underwent a comprehensive transthoracic echocardiographic examination. The evaluation of systolic dyssynchrony was performed by tissue synchronization imaging, and the time to regional peak systolic tissue velocity (Ts) in LV was measured on the basis of 12 segmental models. The standard deviation (SD) of the 12 LV segments (Ts-SD-12) and maximal difference in Ts between any two of the 12 LV segments (Ts-12) were calculated. RESULTS: Compared with the control group, the synchronicity indexes were significantly prolonged in the hypertensive patients. Furthermore, Ts-SD-12 and Ts-12 values were found to be significantly impaired in patients with nondipper HT, compared with dippers: Ts-SD-12 (38.1 ± 18.7 vs. 31.8 ± 15.4, P ≤ 0.001); Ts-12 (123.0 ± 50.6 vs. 98.4 ± 42.3, P ≤ 0.001). Stepwise multivariate logistic regression analysis revealed a significant negative association between LV dyssynchrony indices and percentage decline in BP level from day to night. CONCLUSION: Synchronous systolic contraction of LV is found to be significantly impaired in patients with nondipping circadian pattern of HT compared with dippers and the controls.

AIM: This cross-sectional study was carried out to determine physical and emotional discomforts experienced before and after a gynecologic examination by women who presented to the outpatient clinic of the gynecology and obstetrics department at a university hospital. MATERIAL AND METHODS: The sample of study was composed of 248 women. Data were collected with a survey form developed by researchers. T-test and variance analysis were used in statistical analysis. RESULTS: Emotional discomfort before the examination was felt by 80.2% of the women, while 80.6% stated they felt emotional discomfort after the examination. Physical discomfort before the examination was experienced by 67.3% of the women, while 76.6% stated that they felt physical discomfort after the examination. The emotional discomfort mean score was 5.02 ± 3.24 before examination and 4.62 ± 3.23 after examination (P > 0.05). The physical discomfort mean score was 3.38 ± 3.12 before examination and 3.94 ± 3.02 after examination and the difference between mean scores was statistically significant (P < 0.05). CONCLUSION: The women felt more physical discomfort during the examination than they anticipated beforehand. The emotional discomfort in women who preferred a female physician was significantly higher than in those who preferred a male physician or who had no preference on the sex of their physician.

OBJECTIVES: This cross-sectional study was conducted with the aim of determining the quality of sexual life and related factors in women with gynaecological cancers. METHODS: The study was conducted in the gynaecological oncology clinic of a state hospital. Women who were diagnosed with gynaecological cancers constituted the study population. Eighty married women who were diagnosed with cancer and who agreed to participate were included in the study. Data were collected by a personal data form, the Sexual Quality of Life (SQOL) Questionnaire-Female and the Multidimensional Scale of Perceived Social Support (MSPSS). RESULTS: The mean age of the women was 52.16 (SD 10.04). Of the patients, 61.3% had ovarian cancer, 22.5% endometrial cancer, and 13.8% cervical cancer and 56.3% had been diagnosed with cancer during the previous year. Sixty percent of the women were receiving cancer therapy. The mean score of MSPSS was 67.60 (SD 14.03), and SQOL was 52.50 (SD 22.87). Although there was no significant difference between mean SQOL according to individual and disease characteristics, a positive statistically significant relationship was detected between total and subdimension (family, friend and significant other) scores of MSPSS (p < 0.05). CONCLUSIONS: According to the results obtained from the study, it can be stated that the sexual quality of life of women with gynaecological cancers is moderate and social support, especially support from a significant person, may affect quality of sexual life positively.

Kounis syndrome refers to the concurrence of acute coronary events and allergic or hypersensitivity reactions. In this report, we describe the case of a male patient, in whom acute ST-segment elevation and myocardial infarction developed immediately after injection of depot penicillin, and we discuss the Kounis syndrome. A 52-year-old male patient had chest pain, hypotension and ST-elevation on leads DI and aVL of electrocardiography 30 minutes after intramuscular penicillin injection due to cryptic tonsillitis. Kounis syndrome was considered as a possible diagnosis according to the presentation. Histamine and tryptase levels were not studied due to the delay on arrival to the emergency department. The patient promptly underwent coronary angiography, which revealed only diffuse plaques in all main coronary arteries without any obstructive lesion. We found only increased immunoglobulin (Ig) E, which is associated with the syndrome. With this report, we remind clinicians to consider Kounis syndrome in patients who are subjected to allergenic substances and demonstrate acute chest pain.

RESUMEN

In this clinical vignette, we describe a case of a patient with windsock-like posterior mitral annulus perforation at the site of annular calcification caused by infective endocarditis. Three-dimensional transesophageal images of the perforation resembling an "alien's mouth" are very striking for how 3-dimensional imaging can improve visualization of anatomy of the heart. OBJECTIVES: To determine the sexual experience and risky sexual behaviours of Turkish university students. MATERIALS AND METHODS: This cross-sectional questionnaire survey was carried out in a state university in central Turkey. The data were collected in the 2006-2007 academic year in this university. Participants were 949 convenience students from this university. The study group was determined by a stratified sampling method, with faculty and sex representing the strata. Data were collected with the questionnaire designed by researchers to determine the personal features, sexual experience and risky sexual behaviours of students. A chi-square test was applied in the statistical analysis of data. RESULTS: Of all the students in the research; 42.9% were female and 57.1% male. The percentage of students having had sexual experience was 33.8%. Of the students having had sexual experience, 17.1% were sexually active. The average age for first sexual experience was 17.47 ± 1.79. Among sexually active students, 51.9% indicated that they had unprotected sexual intercourse, 64.2% had sexual intercourse when they were intoxicated and 59.3% had more than one sexual partner. CONCLUSIONS: According to the results obtained from this research, it can be said that a considerable number of the university students had sexual experience and those who are sexually active are under the risk of important health problems, such as sexually transmitted diseases and unintended pregnancy.

Brucella infection may cause vascular complications such as deep venous thromboembolism. This is the first report on an entrapped thrombus in a patent foramen ovale (PFO) in a patient with Brucella infection. A 43-year-old woman was admitted with complaints of fever, dyspnea, malaise, myalgia, and pretibial edema. Clinical and laboratory findings were consistent with brucellosis. Transthoracic echocardiography demonstrated a mobile, hyperechoic worm-like thrombus entrapped in a PFO. Right ventricular enlargement and elevated systolic pulmonary artery pressure (77 mmHg) showed acute pulmonary embolism. Multidetector computed tomography revealed a huge thrombus, 11.7 cm in length, in the bifurcation of the main pulmonary artery. Considering the huge size of the right heart thrombus and hemodynamically significant acute pulmonary embolism, open heart surgery was performed, during which an 11-cm thrombus was found extending from the right atrium across the PFO into the left atrium. The interatrial septum was excised en bloc together with the thrombotic mass and the PFO was closed. Pulmonary thromboendarterectomy was also performed. After surgery, systolic pulmonary artery pressure decreased to 38 mmHg and the patient was discharged without complications.
