RESUMEN

Pediatric R-Tx patients are at high risk of developing EBV primary infection. Although high DNA replication is a risk factor for PTLD, some patients develop PTLD with low viral load. In this retrospective single-center study including all pediatric patients having received R-Tx (2003-2012 period), we aimed to identify risk factors for uncontrolled reactions to EBV (defined as the presence of a viral load >10 000 copies/mL or PTLD). A Cox proportional hazard model was performed. A total of 117 patients underwent R-Tx at a mean age of 9.7 ± 5.3 years, 46 of them being seronegative for EBV at the time of R-Tx. During follow-up, 54 patients displayed positive EBV viral load, 22 of whom presenting with primary infection. An uncontrolled reaction to EBV was observed in 24 patients, whilst 4 patients developed PTLD. Univariate and multivariate analyses suggested the following risk factors for an uncontrolled reaction: age below 5 years, graft from a deceased donor, ≥5 HLA mismatches, EBV-seronegative status at the time of R-Tx, and a secondary post-Tx loss of anti-EBNA. Monitoring anti-EBNA after R-Tx may contribute to the early identification of patients at risk for uncontrolled reaction.

OBJECTIVE: Providing health services involves a risk of medical events and adverse events. The transparency and quality of the healthcare system have a direct impact on patient's safety. One of the measures of the quality of health services is monitoring and reporting these irregularities, as well as analysing the causes of their occurrence. The aim of this study was to present the principles of the functioning of the Regional Commission for Evaluation of Medical Events in Szczecin and to analyse medical events in the West Pomeranian Voivodeship from 2012 to 2017. MATERIALS AND METHODS: The analysis included applications for evaluating medical events and documentation collected for the purpose of conducting cases by the Regional Commission for Evaluation of Medical Events in Szczecin. The study was retrospective. All applications for evaluating medical events that were received by the Regional Commission for Evaluation of Medical Events in Szczecin in 2012-2017 were analysed. The study was conducted from October 2017 to December 2018. RESULTS: The retrospective analysis of the years 2012-2017 revealed 42 medical events and 120 adverse events. The most common medical events were health disorders (33.3%) and bodily injuries (30.9%). Out of the 42 medical events, 34 (80.9%) were for surgical procedures and childbirth. The most common procedures were orthopedic (26.6%) and surgical (23.5%) procedures. CONCLUSIONS: Medical events and adverse events should be reported so that they can be analyzed, conclusions can be drawn, and remedial measures can be introduced.

RESUMEN

The electrocardiographic records were performed in the randomly chosen area of Cracow. The results were obtained from 512 persons (153 M and 359 F) aged 70-93 years. Normal electrocardiograms observed in 25.4% of the studied population. The prevalence of electrocardiographic abnormalities was as follows: ST segment depression--40.0%, patterns of infarction--17.2% and left ventricular hypertrophy--10.4%. On the contrary right axis deviation, bradycardia, sinus arrest, II and III of a-v blocks were rarely observed. The prevalence of electrocardiographic abnormalities in the aged in Cracow is in the accordance with the findings observed in the countries with high risk of cardiovascular diseases.

2.487 resting ECG were examined which is 61.3% of randomly chosen group of inhabitants of Cracow, aged from 20 to 93. There were: sinus tachycardia (HR 100)--4.2%, extrasystole--2.7%, fibrillatio atriorum 1.4%, blocks A-VI0--0.9%, intraventricular conduction defects--3.6% (most often LAH). Prevalence of disturbances of rhythm and conduction increases with age, mainly after 60.

The aim of the present epidemiological study was to recognize the distribution of blood pressure (BP) and the prevalence of arterial hypertension (AH) in the aged (> or = 70 years) population in Cracow. The data were obtained from the studied group of 512 persons (153 men and 359 women) which corresponds to 72.4% of the randomly chosen group. The mean systolic BP was 159.7 in males and 164.3 mmHg in females (p < 0.05). Diastolic BP was 86.3 mmHg and 87.3 mmHg (n.s.), respectively. We observed the single peaked distribution of BP. According to the WHO epidemiological criteria of AH (BP > or = 160 = 95 mmHg) hypertension was found in 60.6% (49.7% M and 64.8% F) with systolic AH 32.3%, systolic+diastolic in 25.4% and diastolic AH alone in 2.9% of this group.
