RESUMEN

We present improved constraints on the coupling of ultralight bosonic dark matter to photons based on long-term measurements of two optical frequency ratios. In these optical clock comparisons, we relate the frequency of the ^{2}S_{1/2}(F=0)↔^{2}F_{7/2}(F=3) electric-octupole (E3) transition in ^{171}Yb^{+} to that of the ^{2}S_{1/2}(F=0)↔^{2}D_{3/2}(F=2) electric-quadrupole (E2) transition of the same ion, and to that of the ^{1}S_{0}↔^{3}P_{0} transition in ^{87}Sr. Measurements of the first frequency ratio ν_{E3}/ν_{E2} are performed via interleaved interrogation of both transitions in a single ion. The comparison of the single-ion clock based on the E3 transition with a strontium optical lattice clock yields the second frequency ratio ν_{E3}/ν_{Sr}. By constraining oscillations of the fine-structure constant α with these measurement results, we improve existing bounds on the scalar coupling d_{e} of ultralight dark matter to photons for dark matter masses in the range of about (10^{-24}-10^{-17}) eV/c^{2}. These results constitute an improvement by more than an order of magnitude over previous investigations for most of this range. We also use the repeated measurements of ν_{E3}/ν_{E2} to improve existing limits on a linear temporal drift of α and its coupling to gravity.

RESUMEN

Russula subgenus Compactae is a group of ectomycorrhizal basidiomycetes, usually with large pileate fruitbodies. European members of the group are characterised by the absence of bright colours on the surfaces of their pilei, the context turning grey to black after cutting, the abundance of short lamellulae in the hymenophore, and spores with an inamyloid suprahilar spot and with low reticulate ornamentation. Our multi-locus phylogenetic study confirmed that this morphological delimitation corresponds to a well-supported clade. Within this clade, 16 species are recognised in Europe, of which five belong to the R. albonigra lineage and were described in a previous study, while eleven are fully described in this study. The application of the names R. acrifolia, R. adusta, R. anthracina, R. atramentosa, R. densissima, R. nigricans and R. roseonigra is based on the position of sequences retrieved from types or authentic material. Based on type sequences, R. fuliginosa is synonymised with R. anthracina and two varieties of R. anthracina are considered synonyms of R. atramentosa. The application of the name R. densifolia is based on a morphological match with the traditional species interpretation and the neotype specimen. Three species are described as new, R. marxmuelleriana sp. nov., R. picrophylla sp. nov. and R. thuringiaca sp. nov. This study recognises three major lineages and two species with isolated positions within the European Compactae and a morphological barcode was assigned to the species using an analysis of 23 selected characters. A search of publicly available sequences from the UNITE database revealed that the majority of species are host tree generalists and widely distributed in temperate and Mediterranean areas of Europe. Russula adusta is the only species so far proven to form ectomycorrhiza exclusively with conifers. Citation: De Lange R, Kleine J, Hampe F, et al. 2023. We study the coherent excitation of the ^{2}S_{1/2}→^{2}F_{7/2} electric octupole (E3) transition by twisted light modes with a single ^{171}Yb^{+} ion in the dark center of a vortex beam. The intensity distribution of the beam is mapped as a function of the ion's position by measuring the light shift on an auxiliary electric quadrupole transition. In the center of the vortex beam, we observe excitation of the E3 transition with a fivefold reduced light shift in comparison to excitation by plane wave radiation for the same Rabi frequency. We measure the excitation probabilities for Laguerre-Gaussian twisted light modes of first and second order for different polarization patterns at various orientations of the ion quantization axis with respect to the beam propagation vector. We compare the experimental results with theoretical predictions and find good qualitative agreement.

We report a measurement of the radiative lifetime of the ^{2}F_{7/2} level of ^{171}Yb^{+} that is coupled to the ^{2}S_{1/2} ground state via an electric octupole transition. The radiative lifetime is determined to be 4.98(25)×10^{7} s, corresponding to 1.58(8) yr. The result reduces the relative uncertainty in this exceptionally long excited state lifetime by 1 order of magnitude with respect to previous experimental estimates. Our method is based on the coherent excitation of the corresponding transition and avoids limitations through competing decay processes. The explicit dependence on the laser intensity is eliminated by simultaneously measuring the resonant Rabi frequency and the induced quadratic Stark shift. Combining the result with information on the dynamic differential polarizability permits a calculation of the transition matrix element to infer the radiative lifetime.

We compare two optical clocks based on the ^{2}S_{1/2}(F=0)→^{2}D_{3/2}(F=2) electric quadrupole (E2) and the ^{2}S_{1/2}(F=0)→^{2}F_{7/2}(F=3) electric octupole (E3) transition of ^{171}Yb^{+} and measure the frequency ratio ν_{E3}/ν_{E2}=0.932829404530965376(32), improving upon previous measurements by an order of magnitude. Using two caesium fountain clocks, we find ν_{E3}=642121496772645.10(8) Hz, the most accurate determination of an optical transition frequency to date. Repeated measurements of both quantities over several years are analyzed for potential violations of local position invariance. We improve by factors of about 20 and 2 the limits for fractional temporal variations of the fine structure constant α to 1.0(1.1)×10^{-18}/yr and of the proton-to-electron mass ratio µ to -8(36)×10^{-18}/yr. Using the annual variation of the Sun's gravitational potential at Earth Φ, we improve limits for a potential coupling of both constants to gravity, (c^{2}/α)(dα/dΦ)=14(11)×10^{-9} and (c^{2}/µ)(dµ/dΦ)=7(45)×10^{-8}.

RESUMEN

We introduce a scheme to coherently suppress second-rank tensor frequency shifts in atomic clocks, relying on the continuous rotation of an external magnetic field during the free atomic state evolution in a Ramsey sequence. The method retrieves the unperturbed frequency within a single interrogation cycle and is readily applicable to various atomic clock systems. For the frequency shift due to the electric quadrupole interaction, we experimentally demonstrate suppression by more than two orders of magnitude for the ^{2}S_{1/2}→^{2}D_{3/2} transition of a single trapped ^{171}Yb^{+} ion. The scheme provides particular advantages in the case of the ^{171}Yb^{+} ^{2}S_{1/2}→^{2}F_{7/2} electric octupole (E3) transition. For an improved estimate of the residual quadrupole shift for this transition, we measure the excited state electric quadrupole moments Θ(^{2}D_{3/2})=1.95(1)ea_{0}^{2} and Θ(^{2}F_{7/2})=-0.0297(5)ea_{0}^{2} with e the elementary charge and a_{0} the Bohr radius, improving the measurement uncertainties by one order of magnitude.

We report on the first coherent excitation of the highly forbidden ^{2}S_{1/2}→^{2}F_{7/2} electric octupole (E3) transition in a single trapped ^{172}Yb^{+} ion, an isotope without nuclear spin. Using the transition in ^{171}Yb^{+} as a reference, we determine the transition frequency to be 642 116 784 950 887.6(2.4) Hz. We map out the magnetic field environment using the forbidden ^{2}S_{1/2}→^{2}D_{5/2} electric quadrupole (E2) transition and determine its frequency to be 729 476 867 027 206.8(4.4) Hz. Our results are a factor of 1×10^{5} (3×10^{5}) more accurate for the E2 (E3) transition compared to previous measurements. The results open up the way to search for new physics via precise isotope shift measurements and improved tests of local Lorentz invariance using the metastable ^{2}F_{7/2} state of Yb^{+}.

Functional tricuspid valve (TV) regurgitation secondary to left heart disease (e.g. mitral insufficiency and stenosis) is observed in 75% of the patients with TV regurgitation and is thus the most common etiology; therefore, the majority of patients who require TV surgery, undergo concomitant mitral and/or aortic valve surgery. Uncorrected moderate and severe TV regurgitation may persist or even worsen after mitral valve surgery, leading to progressive heart failure and death. Patients with moderate to severe TV regurgitation show a 3-year survival rate of 40%. Surgery is indicated in patients with severe TV regurgitation undergoing left-sided valve surgery and in patients with severe isolated primary regurgitation without severe right ventricular (RV) dysfunction. For patients requiring mitral valve surgery, tricuspid valve annuloplasty should be considered even in the absence of significant regurgitation, when severe annular dilatation (≥40 mm or >21 mm/m2) is present. Functional TV regurgitation is primarily treated with valve reconstruction which carries a lower perioperative risk than valve replacement. Valve replacement is rarely required. Tricuspid valve repair with ring annuloplasty is associated with better survival and a lower reoperation rate than suture annuloplasty. Long-term results are not available. The severity of the heart insufficiency and comorbidities (e.g. renal failure and liver dysfunction) are the essential determinants of operative mortality and long-term survival. Tricuspid valve reoperations are rarely necessary and associated with a considerable mortality.

Regular sewer cleaning in North Rhine-Westphalia (Germany) generates annual costs of around 50 million Euros. This leads to the question of whether and to what extent sewer cleaning is necessary. To determine the effect of roughness, sewer surface condition and discharge, experiments with real wastewater were performed, using a sewer test track with acrylic glass tubes (DN 300) prepared with abrasive paper and nature stone tiles at the wastewater treatment plant (WWTP) Bochum-Ölbachtal (Ruhrverband, Germany). A logarithmic relationship between deposit height and time was found to lead to maximum deposit heights of 5 to 60 mm. Surface structure analysis by texture measuring indicated that deposits within the first 28 days after cleaning are highly influenced by the surface condition of the sewer and not necessarily by roughness. Furthermore, under dry weather conditions deposit heights are nearly stable after this time, indicating the limiting effect of sewer cleaning. Deposit formation amounted to 1.75-1.80 mm/d at a roughness of ks = 0.10 mm (fine but catchy microstructure) and 0-0.1 mm/d at ks 1.25 mm (wavy microstructure) at steady state and transient discharge within the first 28 days after sewer cleaning.

BACKGROUND: Transcatheter aortic valve implantation (TAVI) is a treatment strategy for patients with severe aortic stenosis. Although general anaesthesia (TAVI-GA) and sedation (TAVI-S) have previously been described for TAVI, the difference in safety and efficacy of both methods has not been studied in a randomized trial. METHODS: The INSERT trial was a single centre, controlled parallel-group trial with balanced randomization. Sixty-six patients (68-94 yr) with acquired aortic stenosis undergoing transfemoral CoreValve™ were assigned to TAVI-GA or TAVI-S. Comparable operative risk was determined from risk-scores (EUROscore, STS-Score). Monitoring and anaesthetic drugs were standardized. Near-Infrared-Spectroscopy was used to monitor cerebral-oxymetry blinded. Primary outcome was the perioperative cumulative cerebral desaturation. As secondary outcomes, changes in neurocognitive function and respiratory and haemodynamic adverse events were evaluated. RESULTS: Of 66 included patients, 62 (TAVI-GA: n=31, TAVI-S: n=31) were finally analysed. Baseline characteristics were comparable. In 24 patients (39%) cerebral desaturation was observed. Cumulative cerebral desaturation was comparable (TAVI-GA:(median [IQR]) (0[0/1308] s%) vs. TAVI-S:(0[0/276] s%); P=0.505) between the groups. Neurocognitive function did not change within and between groups. Adverse events were more frequently observed in TAVI-S patients (P<0.001). Bradypnoea (n=16, 52%) and the need for airway manoeuvres (n=11, 36%) or bag-mask-ventilation (n=6, 19%) were the most common respiratory adverse events. CONCLUSIONS: Cerebral desaturation occurred in both patient groups, but there was no significant difference between the two groups. Based on primary outcome, both methods were shown to be comparable. Neurocognitive outcome was similar. The higher incidence of adverse events in the sedation group suggests a potential advantage of general anaesthesia. CLINICAL TRIAL REGISTRATION: NCT 01251328.

WHAT IS KNOWN AND OBJECTIVE: Metamizole was withdrawn from the market in the United States and several European countries following reports of fatal agranulocytosis among users, but is still available in many countries in Europe, South America and Asia. Over the past several decades, a number of epidemiologic studies have been conducted to quantify the risk of agranulocytosis and other adverse effects associated with metamizole and other non-narcotic analgesics. The objective of this study was to perform a systematic review of the safety of metamizole. METHODS: Epidemiologic studies published between 1 January 1980 and 15 December 2014 were identified through systematic searches of PubMed and Google Scholar; the reference sections of selected articles were also reviewed to identify potentially relevant studies. Studies included in this review focused on the safety of metamizole, that is on outcomes such as haematologic abnormalities, gastrointestinal bleeding, anaphylaxis and hepatotoxicity. Two study investigators independently reviewed the abstracts and articles to determine relevant studies according to prespecified criteria. RESULTS AND DISCUSSION: A total of 22 articles met the criteria for evaluation. The majority of studies that evaluated agranulocytosis indicated an increased risk associated with metamizole, with relative risk (RR) estimates ranging from 1·5 (95% CI, 0·8-2·7) to 40·2 (95% CI, 14·7-113·3). Findings of three case-control studies do not suggest an association between metamizole and aplastic anaemia. Of the five case-control studies that evaluated the risk of upper gastrointestinal bleeding, four found a statistically significant increased risk associated with metamizole (RR estimates ranging from 1·4 to 2·7). There is insufficient evidence to determine whether metamizole increases the risk of other outcomes (e.g. hepatic effects, anaphylaxis, congenital anomalies). Few studies evaluated the effects of dose, route of administration or duration of therapy. WHAT IS NEW AND CONCLUSION: Published studies reported differences in the magnitude of risk of adverse outcomes associated with metamizole use and often had small sample sizes and a number of other limitations that may have biased the results. Further research is needed to better quantify the potential risks associated with metamizole compared to other non-narcotic analgesics.

P-gp (ABCB1) belongs to the group of export transporters that is expressed in various species at biological barriers. Inhibition of P-gp can lead to changes in pharmacokinetics of drugs (drug-drug interactions), which can lead to toxicity and adverse side effects. This study aimed to establish a functional assay to measure the inhibitory potential of veterinary drugs on feline P-gp by means of fluorescence-associated flow cytometry of feline lymphoma cells. In this model, PSC833 and ivermectin potently inhibited P-gp function; cyclosporine and verapamil moderately inhibited P-gp function, whereas ketoconazole, itraconazole, diazepam, and its metabolites had no effect on P-gp function. This model can be used for testing the inhibitory potency of (new) drugs on feline P-gp.

The number of patients treated in hospital emergency departments in Germany has risen in recent years to approximately 20 million. This escalation also applies to the increasing numbers of patients presenting with neurological symptoms and diseases, which occur in approximately 20 % of emergency patients. In addition to patients with stroke, inflammatory or degenerative central nervous system (CNS) and peripheral nervous system (PNS) disorders who need urgent treatment, more and more patients with nonspecific complaints or conditions attend emergency departments for elective treatment, not least because timely appointments with specialist neurologists in practices could not be obtained. Neurological expertise and presence in emergency departments at the level of specialist standard are therefore indispensable for providing a professional level of treatment, which also corresponds to current legal requirements. The implementation of a generalist emergency physician in Germany, as introduced in some European countries, would mean a retrograde step for neurological expertise in emergency admission management. The discipline of neurology must work together with other emergency disciplines to improve the financing of emergency departments and provide neurologists working there with a substantive curriculum of further and continuing education in emergency-related aspects of neurology. The discipline of neurology has a responsibility to emergency patients within its range of competencies and must, therefore, strengthen and improve its role in healthcare politics and concerning organizational and personnel aspects of neurological emergencies.

INTRODUCTION: Repetitive transcranial magnetic stimulation (rTMS) is used to increase regional excitability to improve motor function in combination with training after neurological diseases or events such as stroke. We investigated whether a daily application of intermittent theta burst stimulation (iTBS; a short-duration rTMS that increases regional excitability) improves the training effect compared with sham stimulation in association with a four-day hand training program using a mirror (mirror training, MT). The right dorsal premotor cortex (dPMC right) was chosen as the target region for iTBS because this region has recently been emphasized as a node within a network related to MT. METHODS: Healthy subjects were randomized into the iTBS group or sham group (control group CG). In the iTBS group, iTBS was applied daily over dPMC right, which was functionally determined in an initial fMRI session prior to starting MT. MT involved 20 min of hand training daily in a mirror over four days. The hand tests, the intracortical excitability and fMRI were evaluated prior to and at the end of MT. RESULTS: The results of the hand training tests of the iTBS group were surprisingly significantly poorer compared with those from the CG group. Both groups showed a different course of excitability in both M1 and a different course of fMRI activation within the supplementary motor area and M1 left. CONCLUSION: We suggest the inter-regional functional balance was affected by daily iTBS over dPMC right. Maybe an inter-regional connectivity within a network is differentially balanced. An excitability increase within an inhibitory-balanced network would therefore disturb the underlying network.

The use of students' "consumer feedback" to assess faculty behavior and improve the process of medical education is a significant challenge. We used quantitative Rasch measurement to analyze pre-categorized student comments listed by 385 graduating medical students. We found that students differed little with respect to the number of comments they provided and that their comments indeed form a probabilistic Rasch hierarchy. However, different hierarchies were found across medical departments and faculty. An analysis of these interactions provides valuable, detailed, and quantitative information that can augment qualitative research approaches. In addition, we suggest how the Rasch scaling of student comments can assist researchers in the design and implementation of new faculty evaluation instruments. Finally, the interactions between student and department identified a subset of behaviors that appear to guide and possibly elicit students' comments.

In actual jurisdiction the Federal Administrative Court has substantially reduced the standard of "health ability" for civil servants who shall be newly employed. That means for the regulated official pre-employment medical examination that in critical cases a recommendation for non-employment will be more rarely considered in the lack of valid, i. e., selective scientific principles. Thereby the formerly pursued selection effect based on civil service law mainly will not apply in the future. Even though the widely opened approach to the civil servant status might be a favour for interested handicapped persons, the following impacts on official-organisational as well as fiscal aspects (governmental aid, pension- burden) can be difficultly foreseen. Hence, in the future the official pre-employment medical -examination will develop an increasing focus on advices about restrictions of assignment possibly having to be considered for admitted candidates.

Médecins Sans Frontières-Operational Centre Amsterdam piloted the distribution of household disinfection kits (HDKs) and health promotion sessions for cholera prevention in households of patients admitted to their cholera treatment centres in Carrefour, Port au Prince, Haiti, between December 2010 and February 2011. We conducted a follow-up survey with 208 recipient households to determine the uptake and use of the kits and understanding of the health promotion messages. In 61% of surveyed households, a caregiver had been the recipient of the HDK and 57.7% of households had received the HDKs after the discharge of the patient. Among surveyed households, 97.6% stated they had used the contents of the HDK after receiving it, with 75% of these reporting using five or more items, with the two most popular items being chlorine and soap. A significant (p < 0.05) increase in self-reported use items in the HDK was observed in households that received kits after 24 January 2011 when the education messages were strengthened. To our knowledge, this is the first time it has been demonstrated that during a large-scale cholera outbreak, the distribution of simple kits, with readily available cleaning products and materials, combined with health promotion is easy, feasible, and valued by the target population.

Sediments in combined sewers may negatively affect the performance of sewer systems and significantly promote the formation of hydrogen sulfide. To prevent this, German sewer systems are cleaned regularly but at high costs. In order to optimize the cleaning intervals, this study investigates how deposits build up and change over a defined period under constant conditions. The experiments were performed using real wastewater at a pilot plant consisting of three 10 m long acrylic glass pipes with a diameter of 300 mm, a slope of I = 1‰, a discharge of Q = 2 L/s and a roughness of kS = 0.4 mm. Within the first 30 d, a slow increase of the deposit height (averaged over the pipe length) of 0.75 mm/d was observed. The deposits did not build up continuously, but in between times also decreased due to erosion. The daily changes in deposit amounts occurred partly as a function of the rainfall conditions and could go beyond the average growth many times. Within a day, the deposit height (averaged over pipe length) changed by up to 20 mm, at single measuring points even up to 100 mm. After about 50 d under constant test conditions, the deposit height came to a state of equilibrium between sedimentation and erosion.

Being a significant global biodiversity hotspot, the Atlantic Forest has been drastically reduced by human activities. Among the anthropic activities that most affect the biodiversity of this biome is the construction and operation of roads and highways. Between harmful effects of these infrastructures, wildlife roadkill is currently considered one of the biggest causes of mortality of wild vertebrates. This study evaluated patterns of vertebrates' roadkill on two roads in the coastal region of the largest continuous remnant of the Brazilian Atlantic Forest. For twelve months, we carry out weekly samples with a motor vehicle at a constant speed of 40 km/h to search for carcasses along the roads. All carcasses found were georeferenced and identified to the lowest possible taxonomic level. Then, using Siriema v.2.0 software, we analyzed roadkill aggregation and the spatial distribution of hotspots of wildlife roadkill. In 43 sampling days, 209 road-killed animals were registered (average roadkill rate of 0.105 and 0.111 animals/kilometer/day for PR-407 e PR-508, respectively). Extrapolating the rates found, we estimate that about 1,773 animals can be roadkill every year on these roads. The most affected groups were birds (33.01%) and amphibians (30.62%), followed by reptiles (19.13%) and mammals (17.33%). Warmer months had the highest roadkill rates. We found two critical roadkill hotspots for the PR-407 (kilometer 11.7 to 12.5 and kilometer 14.7 to 16.7). For the PR-508, we found a critical point of 5.2 km (kilometer 5 to 10.2). As a short-term measure, we recommend installing speed reducers in the identified stretches and implementing environmental education campaigns with residents and tourists, especially during the summer months, aiming to mitigate the roadkill on both roads. However, due to the importance and environmental fragility of the area, we emphasize the need for running periodic road ecology and local wildlife population viability studies in the medium and long term.
