RESUMEN

This study aimed to understand the relationship between Nurse's production of subjectivity and the decision-making in the process of Nursing care. A qualitative design of research was conducted. The investigation was carried out with twelve nurses who work at the Associação de Caridade Santa Casa do Rio Grande, a hospital located in Rio Grande, RS, Brazil. For data collection, focus group technique was used three meetings were conducted in december 2011. The results were presented in semantic categories: Capitalist System: maintenance of employment bond; Submission System: institutionalized culture and vision of society; Nursing Hierarchical System; and Values System: feeling of guilt and lack of professional recognition. The capitalist system mediates, mainly, the behavior that prevails in the decision-making process in Nursing care.

RESUMEN

Constantly experiencing limiting situations that hinder a professional practice coherent with its principles - of autonomy and advocacy of users' interests -, and often conditioned to experience moral distress, the nursing profession plays a prominent role in the current health model because it has the characteristic of managing the care rendered to users in a perspective of social inclusion, both in the basic health network and in hospitals. Aiming at carrying out a reflection on the nursing practice and the difficulties present in its work routine, and considering its characteristics as a profession, this article sought to make a reflection between the practice of nursing and the numerous moral challenges imposed by the routine, resulting, in many cases, in a value crisis that can reverberate directly on the quality of the service rendered, and in abandonment of the ideals of advocacy for users.

RESUMEN

Qualitative study, developed in a Federal University in southern Brazil aiming to know the motives for choosing and evading a Bachelor's degree programme in Nursing, in the evaded students 'perception. The data were collected through a questionnaire sent by e-mail to 19 evaded students. The 9 questionnaires that returned were submitted to Qualitative Textual Analysis, and two categories emerged motives to opt for the nursing programme and motives to evade from the nursing programme. The results showed that the option for the programme is associated to personal vocation, perception of Nursing as a profession of care, and its closeness to the health area. Evasion seems to be related to passing the first option of undergraduate programme, ignorance about the profession, financial difficulties, and professional depreciation. We demonstrated that greater emphasis should be put on promoting knowledge about the nursing work, areas of activity and attributions.

RESUMEN

The objective of this study was to learn about the production of nursing knowledge in Brazil associated with the clinic theme. This is a qualitative study performed by means of an integrative review. Data collection was performed on the SciELO database using the keywords nursing and clinic, present in the abstracts of articles. It was found that the clinic is seen as an instrument used to establish connections between research and nursing care, having a constant movement of constructing and deconstructing knowledge and practices. The study results may contribute with the production of research and knowledge in nursing, providing elements to subsidize improvements in nursing care, in which there is an interaction between practice and biological and non-biological knowledge.

RESUMEN

This study aims to know the reactions and feelings of nursing professionals facing their patients' death. This qualitative research was developed at the Medical Clinic Unit of a university hospital in Southern Brazil. The population of the study was composed of four nurses and five nursing technicians that work at this unit. Data were collected in the second semester of 2006 through semi-structured interviews and analyzed with the use of thematic analysis. The analysisproduced three categories reactions of nursing professionals facing death in daily work, feelings towards facing death in daily work; and nursing team members facing the care of the body after death. Results indicate that there is a need for discussing this issue in the workplace in order to prepare these healthcare workers to deal with their patients' death.

RESUMEN

This is an integrative review that aims to identify the contribution of nursing care for woman's participation in the decision process of the pregnancy and puerperal cycle, as described in Brazilian scientific publications. The scientific productions were retrieved in May, 2010, from the Virtual Library of Health (Biblioteca Virtual em Saúde) database. From the eight articles reviewed, two themes stood out: Contributions of nursing care to the woman's participation in the decision process of the pregnancy and puerperal cycle; and Limitations of nursing care to the woman's participation in the decision process of the pregnancy and puerperal cycle. The following review supports the production of knowledge in nursing, by identifying a gap in what nurses know and do about this issue, as shown by the lack of nursing researches that concern, specifically, the participation of the woman in the decision process during the pregnancy and puerperal cycle and the possible contributions of nursing care to ensure women of this right.

RESUMEN

Etnonursing, as a research methodology, is applied to approach the cultural context and of care in health of one specific culture. Aiming at demonstrating the process of application of this methodology in nursing, from what is proposed by Leininger, four steps of observation are presented, one of interview and for steps of analysis of data, covering specific and congruent criteria with the qualitative paradigm. These steps are not isolated, they relate to one another in an intricate way. It is considered that the knowledge produced can contribute largely for the subjects of the research as well as for the practice of the health workers, developing nursing as a science and providing new paths for the health care, for performing their jobs, through ethics, diversity of workers; complementing the theory of Leininger.

RESUMEN

Nurses face moral problems, moral dilemmas and moral distress in their professional daily. The moral distress manifests itself when nurses demonstrate difficulties to face situations morally appropriate following their conscious. The study aims to focus about the nurses' moral distress and their relationship with moral problems and moral dilemmas presenting concepts, moral distress experiences and possible strategies to be adopted to face it. To focus on the moral distress provoked by facing the moral problems and dilemmas in nursery work seems to be primordial not just to reveal a phenomenon still a little known even experienced almost every day but, also to headline the need of problematization and valorization of the ethical dimension of work in health.

RESUMEN

Fathers have increasingly participated in child care. The objective of this study was to identify the perceptions of the nursing staff as the presence of the father as a caregiver in pediatric units. A qualitative, descriptive, and exploratory study was conducted during the first semester of 2005 in a pediatric unit of a university hospital in Brazil. Four nurses and fifteen orderlies were interviewed using semi-structured interviews, and data were submitted to thematic analysis. The results showed that the team believed that children need both the mother and the father, but the lack of space does not allow the presence of both. The nursing team also believes that mothers take better care of the children than fathers. Fathers are only accepted as caregivers in the hospital under special situations. It was concluded that the nursing team needs to think in how to help fathers to experience and to practice participatory fatherhood in pediatric wards.

This exploratory-descriptive survey was carried out with a sample consisting of 73 nurses from two hospitals in order to know the perception they have of their work via-a-vis patients' rights and develop an instrument capable of measuring such perception. Four constructs referring to the nurses' performance via-a-vis patients' rights were identified and validated, based on questions the nurses answered: preservation of individuality; respect for the patient's autonomy; personal identification; and information for decision-taking. Preservation of individuality was the construct with the best performance. Respect for the patient's autonomy and personal identification appear in an intermediate level, whereas the construct information for decision-taking was the least used. In general, preservation of individuality and respect for the patient's autonomy are the constructs that influenced the most nurses' perception on patients' rights.

RESUMEN

Difficulties faced in the nursing routine, mainly in hospitals, have been reported without the resulting ethical implications to workers and especially to clients, been sufficiently questioned. The work organization can be the main source of suffering to nursing workers, related to the exercise of power of different actors involved in the health institutions, which can potentially cause multiple problems and distress of ethical order. This study aims to make a critical reflection about some relations between the nursing work organization, power relations and its ethical dimension. Strategies for an ethical performance of nurses and other nursing professionals in the organization of work in the healthcare institutions point to the need of these professionals exercise power in an ethical way.

RESUMEN

This study derived from the observation of the difficulties in breastfeeding experienced by mothers dismissed from a university hospital. Nurses were interviewed, and observations were made to identify problems and possible strategies in order to make the proposal of becoming a children-friendly hospital feasible. The identified problems were insufficient staff excessive tasks performed by nurses, lack of continuity in the health team work, out-of-context guidelines, lack of commitment with the proposal. Health professionals suggested the following strategies: health team meetings, better articulation of tasks, systematic assessments, and higher commitment of all workers of the maternity ward.

RESUMEN

We need to take a critical look at our practices and the relationships we construct in our personal and professional life, with a view to leading a life guided by ethics. In this text, we discuss governability practices that are built in the relations between health and especially nursing professionals, with the health institutions, with the clients and with the nursing profession. We conclude that, for persons to deliver care and guide others, like nurses do in their practice, they first need to demonstrate that they can guide themselves, that they know the limits of their practice and can respect the other as someone different from themselves.

The present study is concerning a practice of assistance built and implemented with the health team members of an Oncology and Radiotherapy Service in a large general hospital in the east part of the state of Rio Grande do Sul, attempting to make the assistance more humane. It was a possibility of reflecting with the work team about the assistance provided to the patients in light of the proposals of SUS (New Public Health System) and with the perspective of the humanistic approach and its methodological proposals.

RESUMEN

Reflection about the way of distributing public resources in the area of health and the ensuing consequences. The fairness of distributive justice is questioned. We propose a contextual reading of the official figures through interrelations that make it possible to understand the meaning of government figures. AIDS and Diabetes, which have been chosen as paradigms, represent, respectively, the groups of Transmitted Diseases and Non-Transmitted Diseases. The official figures relating to the two diseases are correlated in some precise aspects such as expenses on drugs, prevalence, and evolution of mortality rates. We alert to the remarkable differences in official attention between these two diseases and to the consequences suffered by people affected with Diabetes and other nontransmitted diseases, arising from the insufficiency of public resources allotted for their health care.

This study aimed to analyze the nurse's ways of production of subjectivity in decision-making processes that involve nursing care. We adopted qualitative research design. The investigative scenario was a philanthropic hospital in the State of Rio Grande do Sul. The study participants were twelve nurses working in this institution. For data collection, it was used the technique of focus group, with three focal meetings conducted in December 2011. From discussions among nurses, it was found that the nurse's ways of production of subjectivity oscillate between two extremes: a relationship of alienation and oppression, resulting in a self-protection, redundancy and domination behavior, or an expression and creation ratio, resulting in proactive behavior of nurses. Nurse's ways of production of subjectivity can condition and define behaviors that prevail in the decision making in care practice.

OBJECTIVE: to identify the perceptions of professionals working in a facility connected with the Brazilian Unified Health System - SUS in regard to what they know, think and talk about public health policy. METHOD: this exploratory-descriptive study with a qualitative nature was conducted with 28 professionals working in a facility connected with the SUS. Data were collected through interviews with guiding questions and analyzed through the thematic content analysis technique. RESULTS: coded and interpreted data resulted in three thematic axes: The SUS - perfect web that does not work in practice; The recurrent habit of complaining about the SUS; The need to rethink the way of thinking about, acting in and managing the SUS. CONCLUSION: the professionals working for the SUS are aware of the principles and guidelines that govern the Brazilian health system, however, they reproduce a dichotomous and linear model of conception and practice strongly linked to the thinking of society in general.

O objetivo desse estudo foi conhecer os facilidades e limitações dos enfermeiros para desenvolver o raciocínio clínico. Estudo quantitativo, descritivo, de corte transversal, que procurou averiguar as "facilidades e limitações dos enfermeiros no processo do raciocínio clínico" de 163 enfermeiros dos sete hospitais do Município de Dourados/MS, no primeiro semestre de 2016, sendo 79,8% são do sexo feminino, 65,4% são especialistas e 85,9% são do regime de Consolidação das Leis trabalhistas. Os participantes apontaram "educação permanente" (62%) como o principal facilitador e "pouco tempo para estudar" (45,39%) como a maior limitação para desenvolverem o raciocínio clínico. Conclui-se ser preciso criar estratégias na rede hospitalar que possibilitem aos enfermeiros se atualizarem dentro da própria instituição que trabalha e averiguar os fatores que contribuem para essa falta de tempo que o grande grupo optou como processo que limita o raciocínio clínico.(AU)


The aim of this study was to know the facilities and limitations of nurses to develop clinical reasoning. A quantitative, descriptive, cross-sectional study that sought to ascertain the "facilities and limitations of nurses in the clinical reasoning process" of 163 nurses from the seven hospitals in the City of Dourados/MS, in the first half of 2016, of which 79.8 % are female, 65.4% are specialists and 85.9% are from the Consolidation of Labor Laws. Participants pointed to "permanent education" (62%) as the main facilitator and "little time to study" (45.39%) as the greatest limitation to develop clinical reasoning. It's concluded that is necessary to create strategies in the hospital network that allow nurses to update themselves within the institution that works and to investigate the factors that contribute to this lack of time that the large group has chosen as a process that limits the clinical reasoning.(AU)


El objetivo de este estudio fue conocer las facilidades y limitaciones de los enfermeros para desarrollar el raciocinio clínico. Estudio cuantitativo, descriptivo, de corte transversal, que buscó averiguar las "facilidades y limitaciones de los enfermeros en el proceso del raciocinio clínico" de 163 enfermeros de los siete hospitales del municipio de Dourados/MS, en el primer semestre de 2016, siendo 79,8 % son del sexo femenino, 65,4% son especialistas y 85,9% son del régimen de Consolidación de las Leyes laborales. Los participantes apuntaron "educación permanente" (62%) como el principal facilitador y "poco tiempo para estudiar" (45,39%) como la mayor limitación para desarrollar el raciocinio clínico. RESUMO Objetivo: conhecer a percepção dos Agentes Comunitários de Saúde acerca das cargas de trabalho presentes na sua dinâmica laboral e realizar prática assistencial relacionada às cargas de trabalho junto ao grupo. Método: estudo qualitativo, fundamentado na Pesquisa Convergente-Assistencial, realizado em duas unidades de atenção primária à saúde, com três equipes de Estratégia de Saúde da Família, em um município ao Sul do Brasil. Os participantes foram 14 Agentes Comunitários de Saúde atuantes nesses cenários. Os dados foram produzidos por meio de entrevistas semiestruturadas, grupos focais e grupos de convergência. Os dados foram analisados, conforme a análise do discurso do sujeito coletivo. Resultados: os Agentes Comunitários de Saúde convivem com intensa carga física (expressa pela exposição física na comunidade, estrutura precária na unidade e risco de violência); cognitiva (evidenciada pela necessidade de acúmulo de informações e conhecimentos técnicos para a orientação da comunidade e mudanças nos sistemas de informação); e psíquica (manifesta na fragilidade das relações interpessoais na equipe, na sobrecarga de trabalho e frustrações frente à falta de resolutividade do sistema e na falta de reconhecimento). A prática assistencial consistiu na mediação de um espaço de reflexão/ação/reflexão no qual os participantes puderam identificar estratégias para a minimização das cargas. Conclusão: a partir da prática assistencial os participantes realizaram uma reflexão crítica, problematizando a sua realidade laboral, envolvendo-se no desafio da transformação da realidade através da construção de estratégias para minimizar as cargas a que estavam expostos.


RESUMEN Objetivo: conocer la percepción de los Agentes Comunitarios de Salud acerca de las cargas de trabajo presentes en su dinámica laboral y realizar práctica asistencial relacionada a las cargas de trabajo junto al grupo. Método: estudio cualitativo, fundamentado en la Investigación Convergente-Asistencial, realizado en dos unidades de atención primaria a la salud, con tres equipos de Estrategia de Salud de la Familia, en un municipio al sur de Brasil. Los participantes fueron 14 Agentes Comunitarios de Salud actuantes en esos escenarios. Los datos fueron producidos por medio de entrevistas semiestructuradas, grupos focales y grupos de convergencia. Los datos fueron analizados, según el análisis del discurso del sujeto colectivo. Resultados: los agentes comunitarios de salud conviven con intensa carga física (expresada por la exposición física en la comunidad, estructura precaria en la unidad y riesgo de violencia); cognitiva (evidenciada por la necesidad de acumulación de informaciones y conocimientos técnicos para la orientación de la comunidad y cambios en los sistemas de información); y psíquica (manifiesta en la fragilidad de las relaciones interpersonales en el equipo, en la sobrecarga de trabajo y frustraciones frente a la falta de resolutividad del sistema y en la falta de reconocimiento). La práctica asistencial consistió en la mediación de un espacio de reflexión/acción/reflexión en el cual los participantes pudieron identificar estrategias para la minimización de las cargas. Conclusión: a partir de la práctica asistencial los participantes realizaron una reflexión crítica, problematizando su realidad laboral, involucrándose en el desafío de la transformación de la realidad a través de la construcción de estrategias para minimizar las cargas a las que estaban expuestos.


ABSTRACT Objective: to understand the perception of Community Health Agents regarding their workloads and to perform care practices with this group related to workloads. Method: a qualitative study, based on the Convergent-Care Research, carried out in two primary health care units, with three Family Health Strategy teams, in a municipality in the South of Brazil. The participants were 14 Community Health Agents working in these scenarios. Data were produced through semi-structured interviews, focus groups and convergence groups. The data were analyzed, according to the discourse analysis of the collective subject. Results: community health agents coexist with intense physical burden (caused by physical exposure in the community, precarious structure in the unit and the risk of violence); cognitive burdens (evidenced by the need to gather information and technical knowledge to orientate the community and changes in the information systems); and mental burdens (resulting from the poor interpersonal relationships within the team, in the excessive workload and frustrations due to the incapabilities of the health system and lack of recognition). The care practice consisted in the mediation of a space for reflection/action/reflection in which the participants could identify strategies that could minimize the burdens. Conclusion: regarding to the care practice, participants performed a critical reflection, problematizing their work reality, engaging in the challenge of transforming reality through the construction of strategies in order to minimize the burdens they were exposed to.

This review study aimed to verify how studies conducted in Brazil have related infant mortality to prenatal care and to present contributions of the clinic in the light of Canguilhem and Foucault for qualification of the care. An integrative literature review was conducted from searches in the databases SciELO, LILACS, MEDLINE and BDENF for the period 2000 to 2009. The relationship between infant mortality and prenatal care is related to the insufficient number of consultations or to the quality of the care provided. Even when the number of and routine consultations in the prenatal care were adequate, avoidable deaths were present. For the qualification of prenatal care, it is suggested that the clinical knowledge and other elements that comprise the process of human living are considered, in order that the clinical view is enlarged and articulated to the technologies available in the health system and, together, they are able to contribute to the reduction of infant mortality in Brazil.
