Guselkumab treatment outcomes and persistence were assessed in a real-world cohort of Finnish patients with difficult-to-treat plaque psoriasis over a median follow-up of 1 year. Data on 181 patients who initiated guselkumab at the 15 study centres were collected retrospectively from the patient charts. Prior exposure to biologic therapies was common, with 56% and 35% having used at least 1 and 2 biologics, respectively. Median guselkumab treatment duration was 11 months with 21 patients (12%) discontinuing treatment during follow-up. Of 85 patients with a follow-up duration of at least 1 year, 73 (86%) were still on guselkumab at 1 year. Significant improvements during follow-up were seen in the absolute Psoriasis Area and Severity Index (PASI) scores with 32 patients (80%) having absolute PASI ≤ 2 after a 9-14-month treatment. Guselkumab treatment was effective and treatment persistence was high in the nationwide Finnish real-life setting.

RESUMEN

A Bangladeshi man had suffered for four years from dermatitis that turned out to be caused by a dermatophyte infection masked by the use of strong corticosteroid ointments. After starting internal antifungal medication he developed a painful kerion reaction in the bearded region. Two systemic corticosteroid courses were required to calm it down. After discontinuation of corticosteroids he developed extensive psoriasis.

Laser Doppler flux (LDF) is a product of the concentration of moving blood cells (CMBC) and the blood cell velocity (BCV). In an attempt to obtain more information about the cutaneous microcirculation in legs with venous ulcers and in healthy legs, the dynamics of the curves of the LDF, the CMBC, and the BCV were analyzed in 8 patients with venous leg ulcers and in 10 subjects with healthy legs. The curves of the CMBC and of the BCV were found to be in opposite phases both in the ulcerous legs and in the healthy legs. The maximal amplitude of the curves of the LDF and of the CMBC was greater in the legs with ulcers than in the healthy legs (p = 0.021 and p = 0.0085, respectively). The finding that the curves of the CMBC and the BCV were in opposite phase can be interpreted to reflect the capillary blood flow by fits and starts both in legs with venous ulcers and in healthy legs. The greater amplitude of the LDF and the CMBC in legs with venous ulcers reflects the blood flow in anatomically altered capillaries in those legs. It is useful to record the curves of the CMBC and the BCV together with the LDF because this gives additional information about the microcirculation of the skin.

Dicaprylyl maleate (DCM) has been reported rarely as a cause of allergic contact dermatitis. The objectives of this study were to identify patients from multiple centres with allergy to DCM in cosmetic products confirmed by patch testing and, in addition, to investigate the effect of testing with aged DCM. This is an international multicentre study of 22 patients with 26 reactions to products containing DCM. Patch testing was carried out to ingredients including DCM obtained from the manufacturer. Further testing was carried out with deliberately aged DCM in a sample of patients. 22 patients had clinical and positive patch test reactions at 4 days to a total of 26 cosmetic products containing DCM. 5 patients did not react to DCM prepared by the manufacturer from concurrent factory stock but did have positive reactions to a deliberately aged batch of DCM. DCM is an emerging cosmetic allergen. Testing with aged material yields a greater number of positive results. Co-operation between cosmetics manufacturers and clinicians is important in the identification of new allergens.
