RESUMEN

Only a few cases of various glomerulonephropathies have been reported in patients with polycythemia vera. We report the case of a 72-year-old female with polycythemia vera in whom renal biopsy examination showed membranoproliferative glomerulonephritis (MPGN)-like lesion and glomerular expression of plasmalemmal vesicle-associated protein-1 (PV-1), a marker of glomerular capillary remodeling after injury. Prior to admission to our hospital for nephrotic syndrome, she had received hydroxyurea and phlebotomy. On admission, she was hypertensive with pretibial edema, hepatosplenomegaly, massive proteinuria (6.14 g/day), low serum albumin (2.9 g/dl), high fibrinogen, fibrin/fibrinogen degradation products and thrombomodulin levels, but with normal serum creatinine and complement levels. Microscopic examination of a renal biopsy demonstrated MPGN-like features with double contour and mesangial interposition. Electron microscopy showed subendothelial deposits, platelets attached to glomerular capillary walls and fibrin deposition. Immunofluorescence study identified IgM deposition along part of the capillary wall and mesangium. CD42b-positive platelets and megakaryocytes were detected in glomerular capillaries, accompanied with increased expression of platelet-derived growth factor receptor b and thrombomodulin in the mesangium and glomerular capillary, respectively. PV-1 was expressed along the glomerular capillary. Anti-platelet and anticoagulant combination therapy, together with the use of anti-hypertensive agents and hydroxyurea, resulted in improvement of the nephrotic syndrome. The findings suggested that activated platelets, enhanced coagulation state and endothelial damage may contribute to glomerulonephropathy associated with polycythemia vera.

RESUMEN

To analyze the permeation of Hg2+ in the epidermis as contact dermatitis allergen, the carrier substances from the HgCl2-treated epidermis were isolated. The peak amount of Hg2+ in the epidermis of mouse tail skin was observed at 12 h after the painting. Thereafter, it remained almost stationary. In the analysis of subcellular fractions, the nuclei fraction showed the highest Hg2+ incorporation, while the soluble protein fraction showed the lowest.

Eight healthy participants performed horizontal isometric jaw movements with maximum voluntary contractions of the jaw muscles until they could not tolerate the task. Electromyographic activities bilaterally recorded from the superior and inferior regions of the sternocleidomastoid muscle and the nuchal and shoulder regions of the trapezius muscle were observed. The results obtained were as follows: 1. In more than 94% of 32 trials, the amplitude of EMGs during the task were higher than those at rest, and in more than 79%, temporal changes in EMG burst were synchronized with the onset-offset of the exercise. 2. In more than 72% of all trials, small amplitude EMG lasted after cessation of the task. 3. Muscle activities of the lateral cervical region during the exercise were superior to the activity of the nuchal and shoulder regions, and amplitude of EMGs in the neck and shoulder regions tended to be the highest during isometric retrusion. 4. These results don't contradict with our hypotheses that occlusal habits might be one cause of the unpleasant sensation in the neck and shoulder regions and that sustained isometric retrusion might have heavier effects on it.

Ninety-five participants were epicutaneous patch tested in order to determine the prevalence of sensitivity to components of dental alloys. Seventeen individuals (17.9%) developed allergic reactions, which were caused by mercury (10.5%), copper (2.1%), nickel, cobalt, tin, gold and zinc (1.1%). Eight of 17 allergic responders had a history of dermatitis from metal contact. The results show that there is a need for careful immunological considerations during the selection of suitable alloys, particularly in the case of patients with lesions suspected of being caused by dental alloys.
