RESUMEN

PURPOSE: To assess the frequency of stress and anxiety levels in infertile women, and to correlate these aspects with risk factors, in order to obtain data for specific psychological guidance. METHODS: The case-control study included a total of 302 women, 152 being infertile (case group: 30.3 +/- 5.4 years), and 150 non-fertile (control group: 25.7 +/- 7.9 years). The quantitative approach involved the application of Lipp's Stress Symptoms Inventory and State-Trait Anxiety Inventory. Response variables considered were: stress frequency and anxiety scores (State and Trait). Statistical analysis was performed by using the Chi-squared and Mann-Whitney tests, and logistical regression to test associations between response variables and considered risk factors. Statistical significance was defined as p < 0.05. RESULTS: The stress was more frequent in the case group than the control group (61.8 and 36.0%, respectively), however, no significant differences were observed between groups in relation to stress phases and predominant symptoms. With respect to anxiety, there were no significant differences between case and control groups as to median state scores (39.5 and 41.0; respectively) and anxiety trait scores (44.0 and 42.0; respectively). Factors significantly associated with greater risk for high anxiety scores in the case group were: primary infertility, unawareness of the causal factor, diagnostic phase investigation, religion, lack of children from other marriages. CONCLUSIONS: It can be concluded that infertile women are more vulnerable to stress; however, they are capable of adapting to stressful events without serious physical or psychological compromising.

BACKGROUND AND OBJECTIVES: Malignant hyperthermia (MH) is a pharmacogenetic disease that causes abnormal hypermetabolic reaction to halogenated anesthetics and/or depolarizing muscle relaxants. In Brazil, there is a hotline telephone service for MH since 1991, available 24 hours a day in São Paulo. This article analyzes the activity of the Brazilian hotline service for MH in 2009. METHODS: Prospective analysis of all phone calls made to the Brazilian hotline service for MH from January to December 2009. RESULTS: Twenty-two phone calls were received: 21 from the South/Southeast region of Brazil and one from the North region. Fifteen calls were requests for general information about MH. Seven were about suspected MH acute episodes, two of which were not considered as MH. In five episodes compatible with MH, all patients received halogenated volatile anesthetics (2, isoflurane; 3, sevoflurane) and one also used succinylcholine; there were four men and one woman, with a mean age of 18 years (2-27). The problems described in the five MH episodes were tachycardia (5), increased expired carbon dioxide (4), hyperthermia (3), acidemia (1), rhabdomyolysis (1), and myoglobinuria (1). One patient received dantrolene. All five patients with MH episodes were follow-up in the intensive care unit and recovered without sequelae. Susceptibility to MH was later confirmed in two patients by in vitro muscle contracture test. CONCLUSIONS: The number of calls per year in the Brazilian hotline service for MH is still low. The characteristics of MH episode were similar to those reported in other countries. The knowledge of MH in Brazil needs to be increased.

RESUMEN

O objetivo deste estudo é empreender uma análise aprofundada das motivações dos estudantes de Medicina da Universidade Federal do Rio Grande do Norte para a escolha profissional, como também compreender suas vivências no cotidiano acadêmico. A metodologia foi qualitativa, utilizando-se a entrevista semi-estruturada como procedimento metodológico. A casuística foi constituída por 30 estudantes do primeiro ao sexto ano, distribuídos eqüitativamente entre os diferentes níveis do curso. Os dados foram analisados por meio da técnica análise de conteúdo temática categorial. A análise permitiu identificar que a influência familiar, a identificação pessoal, a busca da independência financeira e de status profissional e o desejo de ajudar e de ser útil às pessoas foram fatores que influenciaram os estudantes na escolha do curso de Medicina. Em relação aos momentos vividos durante o curso, o reconhecimento da família e dos amigos, a experiência com o paciente e o contato com professores-modelo foram os mais gratificantes, enquanto provas extensas e pouco tempo para estudo, aulas monótonas com professores desatualizados, contato com pacientes terminais e com a morte e cansaço físico foram considerados pelos estudantes como os momentos mais angustiantes. Os dados apontam importantes aspectos do cotidiano dos estudantes de Medicina, que podem orientar estratégias de apoio psicopedagógico no contexto das novas diretrizes curriculares
